Somatic molecular changes and histo-pathological features of colorectal cancer in Tunisia

AIM:To determine correlations between family history,clinical features and mutational status of genes involved in the progression of colorectal cancer(CRC).METHODS:Histo-pathological features and molecular changes[KRAS,BRAF and CTNNB1 genes mutations,microsatellite instability(MSI)phenotype,expression of mismatch repair(MMR)and mucin(MUC)5AC proteins,mutation and expression analysis of TP53,MLH1promoter hypermethylation analysis]were examined in a series of 51 unselected Tunisian CRC patients,10 of them had a proven or probable hereditary disease,on the track of new tumoral markers for CRC susceptibility in Tunisian patients.RESULTS:As expected,MSI and MMR expression loss were associated to the presence of familial CRC(75%vs 9%,P<0.001).However,no significant associations have been detected between personal or familial cancer history and KRAS(codons 12 and 13)or TP53(exons 4-9)alterations.A significant inverse relationship has been observed between the presence of MSI and TP53 accumulation(10.0%vs 48.8%,P=0.0335)in CRC tumors,suggesting different molecular pathways to CRC that in turn may reflect different environmental exposures.Interestingly,MUC5AC expression was significantly associated to the presence of MSI(46.7%vs 8.3%,P=0.0039),MMR expression loss(46.7%vs8.3%,P=0.0039)and the presence of familial CRC(63%vs 23%,P=0.039).CONCLUSION:These findings suggest that MUC5AC expression analysis may be useful in the screening of Tunisian patients with high risk of CRC.

AIM:To investigate the polymorphic simple sequencerepeat in intron 1 of the epidermal growth factor receptorgene(EGFR)(CA-SSRⅠ),which is known to affect theefficiency of gene transcription as a putative target of themismatch repair(MMR)machinery in colorectal tumors.METHODS:The CA-SSRⅠgenotype was analyzed ina total of 86 primary colorectal tumors,selected upontheir microsatellite instability(MSI)status[42 with highfrequency MSI(MSI-H)and 44 microsatellite stable(MSS)]and their respective normal tissue.The effect of the CA-SSRⅠgenotype on the expression of the EGFR gene wasevaluated in 18 specimens using quantitative real-timereverse transcription PCR and immunohistochemistry.RESULTS:Mutations in CA-SSRⅠwere detected in 86%(36 of 42)of MSI-H colorectal tumors and 0%(0 of 44)ofMSS tumors,indicating the EGFR gene as a novel putativespecific target of the defective MMR system(P<0.001).Impaired expression of EGFR was detected in most ofthe colorectal tumors analyzed[6/12(50%)at the mRNAlevel and 15/18(83%)at the peptide level].However,noassociation was apparent between EGFR expression andCA-SSRⅠstatus in tumors or normal tissues.CONCLUSION:Our results suggest that CA-SSRⅠsequence does not contribute to the regulation of EGFRtranscription in colon,and should thus not be consideredas a promising predictive marker for response to EGFRinhibitors in patients with colorectal cancer.

Integrating Aster Images Processing and Fieldwork for Identification of Hydrothermal Alteration Zones at the Oumjrane-Boukerzia District, Moroccan Anti-Atlas

Mapping from remote sensing has become more effective in the field of geology, mainly in lithological discrimination and identification of hydrothermal alteration zones. The use of this technique consists in obtaining information about the rock mass and the main ones existing in the inaccessible areas. Satellite data from the ASTER (Advanced Spaceborne Thermal Emission and Reflection Radiometer) sensor represent a favorable potential for detecting the spectral signatures of mineral zones and identifying their nature. These data are more reliable in places where the climate is arid with less abundant vegetation, as at the Oumjrane-Boukerzia mining district. This region which is part of the Eastern Anti-Atlas, is composed of several mineralized veins which still require detailed studies and exploration by the technique of remote sensing. In this work we applied several processing techniques on ASTER imagery such as Colored Composition, Principal Component Analysis and Ratio Bands. The use of the reports of the specialized Bands makes it possible to identify some hydrothermal alteration minerals within the mining district of Oumjrane Boukerzia. These minerals are represented mainly by iron oxides and hydroxides (Hematite, jarosite, limonite and goethite), carbonate minerals (dolomite, calcite), clay minerals (Illite, kaolinite and chlorite) and quartz minerals. This work allows us to produce a map of hydrothermal alteration zones which can be used as a valuable reference in the strategy of mining exploration for the base metals (Cu, Pb, Zn and Ba), in the mining district of Oumjrane-Boukerzia and in the entire Eastern Anti-Atlas.

Total phenolic content and antioxidant activity of six wild Mentha species(Lamiaceae) from northeast of Algeria

Objective:To investigate the total phenolics,flavonoids and tannins content and the in vitro antioxidant activities of methanolic extracts of six wild Mentha species which are Mentha aquatica,Mentha arvensis,Mentha piperita,Mentha pulegium,Mentha rotundifolia and Mentha villosa.Methods:The Folin–Ciocalteu method was used to determine the total phenols content while flavonoids were estimated according to the aluminum chloride colorimetric method.To evaluate tannins content,vanillin and HCl were added to methanolic extracts.The antioxidant potential was measured by 1,1-diphenyl-2-picrylhydrazyl radical scavenging,ferrous ion chelating and the inhibition of b-carotene bleaching assays.Results:The methanol extracts of Algerian mints were rich in phenolic compounds and exhibited powerful antioxidant activity ranging from 7.5 mg/mL to 44.66 mg/mL,which varied significantly among species.Mentha aquatica stood out with efficient antioxidant ability which was correlated to the high total phenolics content,followed by Mentha arvensis and Mentha piperita with very close values,comparing to Mentha pulegium,Mentha rotundifolia and Mentha villosa with lowest values.Conclusions:These results show that methanolic extracts of Mentha species from Algeria have a great potential of polyphenols which can be used as a natural food preservative and antioxidant source.

Place of Partial Arytenoidectomy in the Management of Bilateral Vocal Cord Immobilitis

Surgical management of laryngeal paralysis varies depending on whether the vocal cords are in abduction, adduction or paramedian position. Various surgical techniques have been described including partial arytenoidectomy which is reported to give good surgical results that are stable over time. The objective of the study was to analyze the surgical therapeutic elements of bilateral paralysis, especially to assess partial arytenoidectomy, one of the most performed techniques. This was a descriptive retrospective study of cases of bilateral immobility admitted between January 1st 2008 and March 31st 2018 and treated surgically. Socio-demographic and therapeutic data were collected. The survey involved 46 patients, with an equal number of male and female (23) with 50% of male patients and 23 patients were female, or a sex ratio of 1. The average age of the patients was 56 ± 17 years ranging between 14 and 89 years. Posterior partial arytenoidectomy was the most widely performed surgical technique (26 patients or 56.5%), followed by cordopexia or lateral-fixing of a vocal cord (19.6%) and posterior cordectomy (17.4%). Patients who received a partial arytenoidectomy and cordopexia had their vocal cords either in adduction or in the paramedian position. Those who received a posterior cordectomy had their vocal cords in adduction. 18 patients (39.13%) were taken to the operating theatre in less than 6 hours, 28 (60.9%) had no post-operative complications, and 9 patients received a surgical enlargement resumption. In post-operative follow-up, 11 patients suffered pulmonary aspiration corrected after speech therapy;26 patients (56.5%) did not. Partial arytenoidectomy remains the most performed surgical procedure in the management of closed bilateral laryngeal paralysis at the Mont-Godinne University Hospital. It allows a reliable and durable breathing function over time with less impact on the voice.

To evaluate lead and cadmium contamination in Moringa oleifera leaves powders marketed in Cotonou and health risks associated with its consumption, cross-sectional and analytical study were done from October 23th, 2012 to June 20th, 2013. Samples of Moringa powders were purchased at different outlets (health centers, supermarkets, pharmacy, etc.). Assays were performed by atomic absorption spectrophotometry after digestion according calcination method. Pb and Cd levels were compared with Codex Alimentarius standards, using the Student t test. Daily Exposure Doses (DDE) and Hazard Quotients (HQ) specific to each contaminant were calculated using a conventional method. Results revealed that the mean concentration of lead for all samples (1.526 mg/kg) exceeded of about 5.08 times the maximum allowable limit whereas cadmium levels (0.246 mg/kg) was not statistically higher than the standard. The highest concentrations of Pb and Cd are respectively 4.263 mg/kg and 0.354 mg/kg. Children are 2.3 to 3.8 times in greater danger than adults although all QD calculated are less than 1, reflecting that there is a low risk to consumers in general. For both metals, DJE specifically linked to the consumption of Moringa was less than 2% compared to other kinds of food intake. In conclusion, the current level of contamination (with lead and cadmium) of Moringa leaves powders marketed in Cotonou is not yet an alarming threat to consumers’ health. However, care must be taken to reduce chemical pollution especially soils where these plants grow.
