Academic productivity correlates with industry earnings in foot and ankle fellowship programs in the United states:A retrospective analysis

Estimation of the Direct Cost of Management of Venous Thromboembolism in Three Reference Hospitals in the City of Yaoundé: A Retrospective study over a Three-Year period

Background: Venous thromboembolism (VTE) is a major public health problem due to its increasing frequency, mortality and management cost. This cost may require major financial efforts from patients, especially in developing countries like ours where less than 7% of the population has health insurance. This study aimed to estimate the direct cost of managing VTE in three reference hospitals in Yaoundé. Methods: This was a cross-sectional retrospective study over a three-year period (from January 1st 2018 to December 31 2020) carried out in the Cardiology departments of the Central and General Hospitals, and the Emergency Centre of the city of Yaoundé. All patients managed during the study period for deep vein thrombosis and pulmonary embolism confirmed by venous ultrasound coupled with Doppler and computed tomography pulmonary angiography respectively were included. For each patient, we collected sociodemographic and clinical data as well as data on the cost of consultation, hospital stay, workups and medications. These data were analysed using spss version 23.0. Results: A total of 92 patient’s records were analysed. The median age was 60 years [48 - 68] with a sex ratio of 0.53. The median direct cost of management of venous thromboembolism was 766,375 CFAF [536,455 - 1,029,745] or $1415 UsD. Management of pulmonary embolism associated with deep vein thrombosis was more costly than isolated pulmonary embolism or deep vein thrombosis. Factors influencing the direct cost of management of venous thromboembolism were: hospital structure (p = 0.015), health insurance (p 0.001), type of pulmonary embolism (p = 0.021), and length of hospital stay (p = 0.001). Conclusion: Management of VTE is a major financial burden for our patients and this burden is influenced by the hospital structure, health insurance, type of pulmonary embolism and length of hospital stay.

Adverse Events following AstraZeneca COVID-19 Vaccination: A Case study in Abidjan, Côte d’Ivoire

Introduction: pharmaceutical companies have boosted vaccine production following the global COVID-19 pandemic. In Côte d’Ivoire, the first vaccination campaign with the AstraZeneca vaccine began on March 1, 2021, as part of the Covax program. Despite the positive benefit/risk balance, the adverse effects of vaccination should not be minimized. Objective: To identify adverse events of AstraZeneca’s COVID-19 vaccination for better management. Materials and Methods: This is a case of a 57-year-old obese (BMI = 39 kg/m2) female health care worker who experienced adverse events in March 2021 after the second dose of AstraZeneca vaccine administered 4 weeks apart. These were subject to mandatory case reporting. Results: Major post-vaccination events occurred in a noisy systemic picture with parameters showing significant disturbances. Biological surveillance remains costly and makes the accountability of the vaccine complex. Conclusion: Vaccination remains the ultimate weapon in the fight against endemic diseases but should not overshadow the reporting of adverse events.

The Kandi basin is located in northeast Benin (West Africa). This study is focused on the estimation of water fluxes exchanged between the river Niger (and its tributaries) and the transboundary Iullemeden Aquifer system. In that framework, an innovative approach based on the application of the Bayesian Mixing Model (MixsIAR) analysis on water isotopes (oxygen-18, deuterium and tritium) was performed. Moreover, to assess the relevance of the model outputs, pearson’s correlation and principal Component Analysis (pCA) have been done. A complex relationship between surface water and groundwater has been found. sixty percent (60%) of groundwater samples are made of more than 70% river water and rainwater;while 31.25% of surface water samples are made of about 84% groundwater. To safeguard sustainable water resources for the well-being of the local communities, surface water and groundwater must be managed as a unique component in the Kandi basin.

This study conducts a comprehensive comparative analysis of machine learning models for customer churn prediction, focusing on the U.s. context. The research evaluates the performance of logistic regression, random forest, and neural networks using industry-specific datasets, considering the economic impact and practical implications of the findings. The exploratory data analysis reveals unique patterns and trends in the U.s. banking and finance industry, such as the age distribution of customers and the prevalence of dormant accounts. Correlation of Rainfall Anomalies in Rwanda from september to December (sOND) with Indian Ocean Dipole (IOD) and El Nino southern Oscillation (ENsO) Events

Understanding the relationship between rainfall anomalies and large-scale systems is critical for driving adaptation and mitigation strategies in socioeconomic sectors. This study therefore aims primarily to investigate the correlation between rainfall anomalies in Rwanda during the months of september to December (sOND) with the occurrences of Indian Ocean Dipole (IOD) and El Nino southern Oscillation (ENsO) events. The study is useful for early warning and forecasting of negative effects associated with extreme rainfall anomalies across the country, using Climate Hazards Group InfraRed precipitation with station (CHIRps), the National Centers for Environmental prediction (NCEp) National Center for Atmospheric Research (NCAR) reanalysis sea surface temperature and ERA5 reanalysis datasets, during the period of 1983-2021. Both empirical orthogonal function (EOF), correlation analysis and composite analysis were used to delineate variability, relationship and the related atmospheric circulation between Rwanda seasonal rainfall september to December (sOND) with Indian Ocean Dipole (IOD) and El-Nino southern Oscillation (ENsO). The results for Empirical Orthogonal Function (EOF) for the reconstructed rainfall data set showed three modes. EOF-1, EOF-2 and EOF-3 with their total variance of 63.6%, 16.5% and 4.8%, Indian ocean dipole (IOD) events resulted to a strong positive correlation of rainfall anomalies and Dipole model index (DMI) (r = 0.42, p value = 0.001, DF = 37) significant at 95% confidence level. The composite analysis for the reanalysis dataset was carried out to show the circulation patterns during four different events correlated with september to December seasonal rainfall in Rwanda using T-test at 95% confidence level. Blockchain-Based system for secure and Efficient Cross-Border Remittances: A potential Alternative to sWIFT

This paper proposes a blockchain-based system as a secure, efficient, and cost-effective alternative to sWIFT for cross-border remittances. The current sWIFT system faces challenges, including slow settlement times, high transaction costs, and vulnerability to fraud. Leveraging blockchain technology’s decentralized, transparent, and immutable nature, the proposed system aims to address these limitations. Key features include modular architecture, implementation of microservices, and advanced cryptographic protocols. The system incorporates proof of stake consensus with BLs signatures, smart contract execution with dynamic pricing, and a decentralized oracle network for currency conversion. A sophisticated risk-based authentication system utilizes Bayesian networks and machine learning for enhanced security. Mathematical models are presented for critical components, including transaction validation, currency conversion, and regulatory compliance. simulations demonstrate potential improvements in transaction speed and costs. However, challenges such as regulatory hurdles, user adoption, scalability, and integration with legacy systems must be addressed. The paper provides a comparative analysis between the proposed blockchain system and sWIFT, highlighting advantages in transaction speed, costs, and security. Mitigation strategies are proposed for key challenges. Recommendations are made for further research into scaling solutions, regulatory frameworks, and user-centric designs. The adoption of blockchain-based remittances could significantly impact the financial sector, potentially disrupting traditional models and promoting financial inclusion in underserved markets. However, successful implementation will require collaboration between blockchain innovators, financial institutions, and regulators to create an enabling environment for this transformative system.

Assessing the Impact of Gold Mining on the Quality of Water Resources in the Commune of Meguet, Burkina Faso

Despite its often illegal nature, artisanal gold mining in Burkina Faso contributes to the economic and social development of the country. However, the rudimentary techniques used in gold panning have a significant impact on the environment due to inappropriate practices and the use of various chemical substances. This study aims to assess the impact of artisanal gold mining on the quality of water resources in a rural community at Méguet, Burkina Faso. To this end, surface and groundwater samples were collected and analyzed at the BUMIGEB laboratory. Field results show that the waters are slightly alkaline (6.97 pH s/cm), with temperatures ranging from 24.6˚C to 31.6˚C. In addition, trace metals (TMEs) analyzed from surface and subsurface waters show very high levels, generally deviating from the levels recommended by WHO guidelines for Burkina Faso. Trace metals contamination of water resources in the commune of Méguet is mainly due to Fe (3.78 - 11.12 mg/kg), Hg (0.03 - 0.29 mg/kg), As (0.01- 6.31 mg/kg) and pb (0.01 - 3.8 mg/kg). This study can serve as a basis for guiding national environmental policies to protect the water resources of the Méguet mine.

Beta-HCG Levels and Ovarian Ultrasonography Results among Non-pregnant Women of Reproductive Age in port Harcourt, Nigeria: A Cross-sectional study

Background: Certain ovarian cancers that were previously common in postmenopausal women are now increasingly observed in women of reproductive age. The research on using β-HCG as a diagnostic biomarker for ovarian cancer in women of reproductive age is ongoing. Aim: This study assessed the level of serum β-HCG in non-pregnant women of reproductive age and determined its potential association with suspicious ovarian ultrasonography results. Methods: The study was conducted in port Harcourt, Nigeria. This study adopted a cross-sectional design on a quota sample of 224 case notes of women aged 18 - 40 years obtained from eight diagnostic centres. A data extraction form was used for data collection. Data analysis employed descriptive statistics, Chi-square, Fisher’s exact test, and Odds Ratio at 95% confidence and 5% significance levels. Results: About 5.8% of the participants exhibited detectable levels of serum β-HCG above 5 IU/L (World Health Organization reference) at a mean concentration of 5.87 (±1.75) IU/L. About 4.0% of the participants had suspicious ovarian lesions identified through ultrasonography. participants with elevated serum β-HCG levels above the WHO reference were 59 times more likely to have suspicious ovarian lesions (Odds ratio: 59.4, 95%CI: 12.3 - 287.8, p β-HCG level and age (p = 0.041) as well as parity (p Conclusion: serum β-HCG levels above the WHO reference in non-pregnant women were associated with suspicious ovarian lesions. More rigorous primary research, systematic reviews, and meta-analyses are needed to confirm the findings of this study.
