Role of illness perception and self-efficacy in lifestyle modification among non-alcoholic fatty liver disease patients

To describe the relationships between non-alcoholic fatty-liver disease (NAFLd) patient’s disease consequences and treatment perceptions, self-efficacy, and healthy lifestyle *** cross-sectional study among 146 ultrasound diagnosed NAFLd patients who visited the fatty liver clinic at the Tel-Aviv Medical Center. Eighty-seven of these individuals, participated in a clinical trial of physical activity and underwent fasting blood tests, analyzed at the same lab. Exclusion criteria included positivity for serum HBsAg or anti-HCV antibodies; fatty liver suspected to be secondary to hepatotoxic drugs; excessive alcohol consumption (≥ 30 g/d in men or ≥ 20 g/d in women) and positive markers of genetic or immune-mediated liver diseases. Patients were asked to complete a self-report structured questionnaire, assembled by the Israeli Center for disease Control. Nutrition habits were measured using six yes/no questions (0 = no, 1 = yes) adopted from the national survey questionnaire. Participants in the clinical trial completed a detailed semi-quantitative food frequency questionnaire (FFQ) reporting their habitual nutritional intake during the past year. Self-efficacy was assessed by the Self-Efficacy Scale questionnaire, emotional representation, degree of illness understanding, timeline perception, treatment perception and symptoms were measured by the Brief Illness Perception questionnaire. Illness consequences were measured by the Personal Models of diabetes Interview questionnaire. A path analysis was performed to describe the interrelationships between the patients’ illness perceptions, and assess the extent to which the data fit a prediction of nutritional *** study sample included 54.1% men, with a mean age of 47.76 ± 11.68 years (range: 20-60) and mean body mass index of 31.56 ± 4.6. Total hip arthroplasty for posttraumatic osteoarthritis following acetabular fracture： A systematic review of characteristics, outcomes, and complications

Purpose： Posttraumatic arthritis （PTA） may develop years after acetabular fracture, hindering joint function and causing significant chronic musculoskeletal pain. Given the delayed onset of PTA, few studies have assessed outcomes of delayed total hip arthroplasty （THA） in acetabular fracture patients. This study systematically reviewed the literature for outcomes of THA in patients with PTA and prior acetabular fracture. Methods： Pubmed, EMBASE, SCOPUS, and Cochrane library were searched for articles containing the keywords ＂acetabular＂, ＂fracture＂, ＂arthroplasty＂, and ＂post traumatic arthritis＂ published between 1995 and August 2017. Studies with less than 10 patients, less than 2 years of follow-up, conference abstracts, and non-English language articles were excluded. data on patient demographics, surgical characteristics, and outcomes of delayed THA, including implant survival, complications, need for revision, and functional scores, was collected from eligible studies. Results： With 1830 studies were screened and data from 10 studies with 448 patients were included in this review, The median patient age on date of THA was 51.5 years, ranging from 19 to 90 years. The median time from fracture to THA was 37 months, with a range of 27-74 months. Mean follow-up times ranged from 4 to 20 years, The mean Harris hip scores （HHS） improved from 41.5 pre-operatively, to 87.6 post-operatively, The most prevalent postoperative complications were heterotopic ossification （28% -63%）, implant loosening （1%-24%）, and infection （0%-16%）. The minimum 5-year survival of implants ranged from 70% to 100%. Revision rates ranged from 2% to 32%. Conclusion： despite the difficulties associated with performing THA in patients with PTA from previous acetabular fracture （including soft tissue scarring, existing hardware, and acetabular bone loss） and the relatively high complication rates, THA in patients with VIA following prior acetabular fracture leads to significant improvement in pai

Best Bounds on Measures of Risk and Probability of Ruin for Alpha Unimodal Random Variables When There Is Limited Moment Information

This paper presents explicit formulae giving tight upper and lower bounds on the expectations of alpha-unimodal random variables having a known range and given set of moments. Such bounds can be useful in ordering of random variables in terms of risk and in PERT analysis where there is only incomplete stochastic information concerning the variables under investigation. Explicit closed form solutions are also given involving alpha-unimodal random variables having a known mean for two particularly important measures of risk—the squared distance or variance, and the absolute deviation. In addition, optimal tight bounds are given for the probability of ruin in the collective risk model when the severity distribution has an alpha-unimodal distribution with known moments.

Review on the fully mulched ridge–furrow system for sustainable maize production on the semi-arid Loess Plateau

The fully mulched ridge–furrow(FMRF) system has been widely used on the semi-arid Loess Plateau of China due to its high maize(Zea mays L.) productivity and rainfall use efficiency. However, high outputs under this system led to a depletion of soil moisture and soil nutrients, which reduces its sustainability in the long run. Therefore, it is necessary to optimize the system for the sustainable development of agriculture. The development, yield-increasing mechanisms,negative impacts, optimization, and their relations in the FMRF system are reviewed in this paper. We suggest using grain and forage maize varieties instead of regular maize;mulching plastic film in autumn or leaving the mulch after maize harvesting until the next spring, and then removing the old film and mulching new film;combining reduced/notillage with straw return;utilizing crop rotation or intercropping with winter canola(Brassica campestris L.), millet(Setaria italica), or oilseed flax(Linum usitatissimum L.);reducing nitrogen fertilizer and partially replacing chemical fertilizer with organic fertilizer;using biodegradable or weather-resistant film;and implementing mechanized production. These integrations help to establish an environmentally friendly, high quality, and sustainable agricultural system, promote highquality development of dryland farming, and create new opportunities for agricultural development in the semi-arid Loess Plateau.

Global food demand requires that soils be used intensively for agriculture, but how these soils are managed greatly impacts soil fluxes of carbon dioxide (CO2). Soil management practices can cause carbon to be either sequestered or emitted, with corresponding uncertain influence on atmospheric CO2 concentrations. The situation is further complicated by the lack of CO2 flux measurements for African subsistence farms. For widespread application in remote areas, a simple experimental methodology is desired. As a first step, the present study investigated the use of Bowen Ratio Energy Balance (BREB) instrumentation to measure the energy balance and CO2 fluxes of two contrasting crop management systems, till and no-till, in the lowlands within the mountains of Lesotho. Two BREB micrometeorological systems were established on 100-m by 100-m sites, both planted with maize (Zea mays) but under either conventional (plow, disk-disk) or no-till soil mangement systems. The results demonstrate that with careful maintenance of the instruments by appropriately trained local personnel, the BREB approach offers substantial benefits in measuring real time changes in agroecosystem CO2 flux. The periods where the two treatments could be compared indicated greater CO2 sequestration over the no-till treatments during both the growing and non-growing seasons.
