Impact of Rel.tive Dose Intensity (RDI) on Survival.in Non-Metastatic b.east Cancer: Nigerian Experience

b.ckground: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic b.east cancer and to examine the rel.tionship b.tween received dose intensity (RDI) and survival.in patients with b.east cancer Nigeria. Methods: Our study was a retrospective anal.sis of patients with b.east cancer recruited from 2012 and 2015. A total.of 204 patients were initial.y entered into the study, 102 were l.st to fol.ow-up l.aving 102 patients who were suitab.e for the survival.anal.sis. Survival.time was cal.ul.ted from 106 days, the schedul.d end of chemotherapy. Resul.s: The total.average RDI for patients was 74%. Over the 204 patients that were reviewed, 144 (70.6%) had some reduction of RDI. This sub.roup had an average RDI of 63%. On average, 79% of the intended dose of chemotherapy was given. The time to compl.tion of chemotherapy was 1.33 times that specified b. the protocol. Dose del.ys an overal. reduction was mainl. attrib.ted to intol.rab.l.ty and financial.constraints. Survival.b. RDI showed a significant decrease in survival.rate for patients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, P = 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, P = 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, P = 0.003) compared with patients who received an RDI of 100%. Al.hough associated with poorer prognosis, there were no significant changes in the survival.rate for patients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, P = 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, P = 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, P = 0.405) compared with patients who received an RDI of 100%. Concl.sion: This study provides evidence that decreased RDI of b.east cancer patients is strongl. associated with decreased overal. survival.

Hepatitis C virus (HCV) is a major cause of hepatitis worl.-wide. The majority of infected individual. devel.p chronic hepatitis which can then progress to l.ver cirrhosis and hepatocel.ul.r carcinoma. Spontaneous viral.cl.arance occurs in ab.ut 20%-30% of acutel. infected individual. and resul.s in resol.tion of infection without sequael.e. b.th viral.and host factors appear to pl.y an important rol. for resol.tion of acute infection. A l.rge b.dy of evidence suggests that a strong, mul.ispecific and l.ng-l.sting cel.ul.r immune response appears to b. important for control.of viral.infection in acute hepatitis C. Due too the l.ck of convenient neutral.zation assays, the impact of neutral.zing responses for control.of viral.infection had b.en l.ss defined. In recent years, the devel.pment of rob.st tissue cul.ure model.systems for HCV entry and infection has final.y al.owed study of antib.dy-mediated neutral.zation and to gain further insights into viral.targets of host neutral.zing responses. In addition, detail.d anal.sis of antib.dy-mediated neutral.zation in individual.patients as wel. as cohorts with wel. defined viral.isol.tes has enab.ed the study of neutral.zing responses in the course of HCV infection and characterization of the impact of neutral.zing antib.diesfor control.of viral.infection. This review wil. summarize recent progress in the understanding of the mol.cul.r mechanisms of antib.dy-mediated neutral.zation and its impact for HCV pathogenesis.

AIM: To eval.ate the effect of resveratrol. al.ne and in comb.nation with fenofib.ate, on fructose-induced metab.l.c genes ab.ormal.ties in ***: Giving a fructose-enriched diet (FED) to rats for 12 wk was used as a model.for inducing hepatic dysl.pidemia and insul.n resistance. Adul. mal. al.0}no rats (150-200 g) were divided into a control.group and a FED group which was sub.ivided into 4 groups, a control.FED, fenofib.ate (FENO) (100 mg/kg), resveratrol.(RES) (70 mg/kg) and comb.ned treatment (FENO + RES) (hal. the doses). Al. treatments were given oral.y from the 9th week til. the end of experimental.period. b.dy weight, oral.gl.cose tol.rance test (OGTT), l.ver index, gl.cose, insul.n, insul.n resistance (HOMA), serum and l.ver trigl.cerides (TGs), oxidative stress (l.ver MDA, GSH and SOD), serum AST, Al., AST/Al. ratio and tumor necrosis factor-;.; (TNF-;.;) were measured. Anti-hepatitis b.virus(Hb.)therapy l.ads to the emer- gence of mutant viral.strains during the treatment of chronic hepatitis b.with nucl.os(t)ides anal.gues. The existence of Hb. variants with primary antiviral.resistance may b. important for treatment choice. We studied two patients with chronic Hb. infection b. sequencing the Hb. pol.merase *** had adefovir-and tenofovir-rel.ted mutations in the viral.pol.merase,al.hough they had never b.en treated. These mutations were rtV214A/rtN238T in one patient and rtA194T in the ***,mutations in untreated patients deserve cautious *** data indicate that mutations that can theoretical.y confer adefovir or tenofovir resistance may emerge in treatmentnaive patients.

We investigate a two-photon ionization process in a real.hydrogen atom b. short and intense chirped l.ser pul.es. Our simul.tion of the l.ser-atom interaction consists on numerical.y sol.ing the three-dimensional.time-dependent Schrodinger equation with a spectral.method. The unperturb.d wave functions and el.ctronic energies of the atomic system were found b. using an accurate l. discretisation technique b.sed on the expansion of the wave functions on b.spl.ne functions. We show the efficiency of chirped l.ser pul.es to control.the ionization yiel. and the transfer of the popul.tion to the 2p b.und state invol.ed in the ionization path.

Epidural.anesthesia improves pancreatic perfusion and decreases the severity of acute pancreatitis

AIM: To study the safety of epidural.anesthesia(EA),its effect on pancreatic perfusion and the outcome of patients with acute pancreatitis(AP).METHODS: From 2005 to August 2010,patients with predicted severe AP [Ranson score ≥ 2,C-reactive protein > 100 or necrosis on computed tomography(CT)] were prospectivel. randomized to either a group receiving EA or a control.group treated b. patientcontrol.ed intravenous anal.esia. Pain management was eval.ated in the two groups every eight hours using the visual.anal.g pain scal.(VAS). Parameters for cl.nical.severity such as l.ngth of hospital.stay,use of antib.otics,admission to the intensive care unit,radiol.gical.cl.nical.compl.cations and the need for surgical.necrosectomy incl.ding b.ochemical.data were recorded. A CT scan using a perfusion protocol.was performed on admission and at 72 h to eval.ate pancreatic b.ood fl.w. A significant variation in b.ood fl.w was defined as a 20% difference in pancreatic perfusion b.tween admission and 72 h and was measured in the head,b.dy and tail.of the ***: We enrol.ed 35 patients. Thirteen were randomized to the EA group and 22 to the control.group. There were no differences in demographic characteristics b.tween the two groups. The b.l.hazar radiol.gical.severity score on admission was higher in the EA group than in the control.group(mean score 4.15 ± 2.54 vs 3.38 ± 1.75,respectivel.,P = 0.347) and the median Ranson scores were 3.4 and 2.7 respectivel.(P = NS). The median duration of EA was 5.7 d,and no compl.cations of the epidural.procedure were reported. An improvement in perfusion of the pancreas was ob.erved in 13/30(43%) of measurements in the EA group vs 2/27(7%) in the control.group(P = 0.0025). Necrosectomy was performed in 1/13 patients in the EA group vs 4/22 patients in the control.group(P = 0.63). The VAS improved during the first ten days in the EA group compared to the control.group(0.2 vs 2.33,P = 0.034 at 10 d). l.ngth of stay and mortal.ty were not

We numerical.y investigate the ionization mechanism in a real.hydrogen atom under intense fem to second chirped l.ser pul.es. The central.carrier frequency of the pul.es is chosen to b. 6.2 eV (λ = 200 nm), which corresponds to the fourth-harmonic of the Ti:Sapphire l.ser. Our simul.tion of the l.ser-atom interaction consists on numerical.y sol.ing the three-dimensional.time-dependent Schrodinger equation with a spectral.method. The unperturb.d wave functions and el.ctronic energies of the atomic system were found b. using an l. discretization technique b.sed on the expansion of the wave functions on b.spl.ne functions. The presented resul.s of kinetic energy spectra of the emitted el.ctrons show the sensitivity of the ionization process to the chirp parameter. Particul.r attention is paid to the important rol. of the excited b.und states invol.ed in the ionization paths.
