Anticoagulation in Pregnant Women with Mechanical Heart Valve Prostheses: 25-Year Experience at a Tertiary Care Hospital in a Developing Country

The aim of this retrospective study was to compare the different anticoagulation regimens used in pregnant women with prosthetic heart valves. We reviewed 86 pregnancies in 57 women from 1987 to 2011. The patients were divided into two groups: group A (39 pregnancies) had oral acenocoumarol throughout pregnancy;and in group B (47 pregnancies), acenocoumarol was replaced by subcutaneous heparin during the first trimester. Both groups received heparin at the time of delivery. The valves replaced were mitral (59.65%), aortic (12.28%), or both (28.07%). 74 pregnancies (86.04%) resulted in live births, 9 (10.46%) had stillbirths, 1 (1.16%) had spontaneous abortion and 2 (2.32%) underwent therapeutic abortions. The live birth rate was higher in women on heparin (87.23%) compared with those on acenocoumarol (84.61%). No malformations appeared in the 74 newborns, except for one case of hydrocephalus. There was one maternal death due to acute mitral valvular thrombosis while on heparin in the first trimester. Hemorrhagic complications occurred in 13 patients in the postpartum period, 4 of whom required transfusion. No anticoagulant regimen can be said to be entirely safe for use during pregnancy as there is a degree of risk with each regimen. Heparin does not offer a clear advantage over oral anticoagulation in the pregnancy outcome.

经过170年的历史发展，发表于1848年的《共产党宣言》至今仍具有现实意义。尽管发表于170年前，但其整个表述甚至更契合于当代现实。马克思和恩格斯比任何人都更加了解资本主义的本质特征，他们从自己所处的时代尚未明确的前提出发得出的结论，在170年的历史发展中得到了充分的巩固。马克思和恩格斯强调指出，资本主义只是人类历史上的一段小插曲，它的历史作用是创造出向共产主义这一更高的文明阶段过渡的条件。而晚年马克思更是意识到，由于全球积累所造成的不平等发展，社会主义革命有可能从世界体系的边缘开始。基于马克思和恩格斯的上述思想，本文探讨了无产阶级从自在阶级向自为阶级的转变、资本主义全球化的条件和影响、农民在向社会主义过渡的过程中的地位、反资本主义的思想和运动在表达和实践上的多样性等问题。作者最后指出，我们的时代不再是资产阶级革命的时代，而是社会主义革命的时代。与其屈从于资本主义文明衰落的致命变迁，不如通过社会主义革命的进程改变世界。

Paired arcuate and modified circular keratotomy in keratoconus

AIM: To reduce astigmatism, increase corneal volume and improve visual acuity. METHODS: A retrospective, single-surgeon, singlecenter, clinic-based study of a surgical procedure on twenty-four eyes of fourteen patients diagnosed with stage Ⅲ or stage Ⅳ keratoconus. Paired arcuate keratotomy coupled with modified circular keratotomy was performed at a single center by a single surgeon as an outpatient procedure with local anaesthetic in a minor surgery room. Modified circular keratotomy was performed 7 mm from the pupillary center with depth of incision ranging between 70% and 90% of corneal thickness. Arcuate keratotomy was performed 2.5 mm from the pupillary center with the depth of incision at 90% of corneal thickness. Angular length of the arcs ranged between 60° and 120° depending on the astigmatic power of the cornea. RESULTS: Astigmatism decreased in 87.5% of the 24 treated eyes, increased in 8.33% and did not change in 4.17%. Corneal volume increased in 91.66% of the 24 eyes and decreased in 8.34%. Visual acuity improved in 100% of the eyes; there was a mean improvement of 59% from preoperative visual acuity, 8.34% of the treated eyes reaching a visual acuity of 1.0(20/20) with correction. No complications occurred during orafter surgery. No suturing was performed and there was no rupturing at incision sites. There was statistical significance difference between *** against ***(P = 0.001). Also between *** against ***(P = 0.005), there was no significance difference between *** against ***(P = 0.05).CONCLUSION: Paired arcuate keratotomy coupled with modified circular keratotomy should be considered as an intervention before performing keratoplasty.

Investigation of the Effect of Film Thickness and Heat Treatment on the Optical Properties of TeSeSn Thin Films

Glassy substrates TeSeSn thin films were thermally evaporated onto chemically cleaned glass. The as-deposited (as-prepared) and annealed thin films were characterized by scanning electron microscopy (SEM), X-ray diffraction (XRD) and optical transmission. The optical absorption of the as-prepared and annealed TeSeSn thin films is studied in the wavelength range of 300 nm - 900 nm. The direct optical energy gap (Eg) increases from 1.989 to 2.143 eV with increasing the thickness of the as-prepared films from 100 to 200 nm. The annealed TeSeSn films showed a decrease in the optical energy gap with increasing the annealing temperature. The effect of heat treatment on the lattice dielectric constant (εL) and carrier concentration (N) are also studied.

Differences Among Self-Esteem in a Nationally Representative Sample of 15-17-Year-Old Omani Adolescents

This study aimed to understand the association between self-esteem(SE)and gender,age,body mass index(BMI)and rural-urban dichotomy among Omani *** regression analyses were conducted using generalized linear models to study the impact of gender,age,BMI,and place of residence on the SE score after adjusting for other *** the 4,432 teenagers studied,47.4%were boys and 52.6%*** majority(64.4%)were from rural areas and 35.6%from urban areas of *** 23%were physically underweight,while 51%were normal and 26.2%were overweight or *** results of this study revealed that Omani girls have higher SE than *** also showed that no relationship between SE and ***,the results showed that children residing in rural areas had higher SE than those in city dweller.

Undeniably,Deep Learning(DL)has rapidly eroded traditional machine learning in Remote Sensing(RS)and geoscience domains with applications such as scene understanding,material identification,extreme weather detection,oil spill identification,among many *** machine learning algorithms are given less and less attention in the era of big ***,a substantial amount of work aimed at developing image classification approaches based on the DL model’s success in computer *** number of relevant articles has nearly doubled every year since *** in remote sensing technology,as well as the rapidly expanding volume of publicly available satellite imagery on a worldwide scale,have opened up the possibilities for a wide range of modern ***,there are some challenges related to the availability of annotated data,the complex nature of data,and model parameterization,which strongly impact *** this article,a comprehensive review of the literature encompassing a broad spectrum of pioneer work in remote sensing image classification is presented including network architectures(vintage Convolutional Neural Network,CNN;Fully Convolutional Networks,FCN;encoder-decoder,recurrent networks;attention models,and generative adversarial models).The characteristics,capabilities,and limitations of current DL models were examined,and potential research directions were discussed.

Decellularization of xenogeneic heart valves might lead to excellent regenerative implants,from which many patients could ***,this material carries various xenogeneic epitopes and thus bears a considerable inherent immunological ***,we investigated the regenerative and immunogenic potential of xenogeneic decellularized heart valve implants using pigs deficient for the galactosyltransferase gene(GGTA1-KO)as novel large animal *** aortic and pulmonary heart valves obtained from sheep,wild-type pigs or GGTA1-KO pigs were implanted into GGTA1-KO pigs for 3,or 6 months,*** were analyzed histologically,immunhistologically(CD3,CD21 and CD172a)and anti-aGal antibody serum titers were determined by *** sheep derived implants exhibited a strong immune reaction upon implantation into GGTA1-KO pigs,characterized by massive inflammatory cells infiltrates,presence of foreign body giant cells,a dramatic increase of anti-aGal antibody titers and ultimately destruction of the graft,whereas wild-type porcine grafts induced only a mild reaction in GGTA1-KO *** implants,wild-type/wild-type and GGTA1-KO/GGTA1-KO valves did not induce a measurable immune ***,GGTA1-KO pigs developed a‘human-like’immune response toward decellularized xenogeneic implants showing that immunogenicity of xenogeneic implants is not sufficiently reduced by decellularization,which detracts from their regenerative potential.

Association between IL-37 and VEGF Gene Expression in Psoriasis Pathogenesis in Egyptian Population

Background: Psoriasis is considered a common skin disease, marked by the production and elevation of inflammatory plaques that regularly shed scales resulting from extensive skin epithelial cell proliferation. T lymphocytes, neutrophils, and other leukocytes enter the irritated skin, resulting in epidermal keratinocyte hyperplasia, vascular hyperplasia, ectasia, and T lymphocyte, neutrophil other forms of leukocyte infiltration. Aim of the Work: the study aimed to investigate the role of IL-37 and VEGF gene expression in the pathogenesis of psoriasis in Egyptian patients. Methodology: Polymerase chain reaction techniques (PCR) were applied to detect VEGF in the skin homogenates of psoriasis patients. In addition, the ELIZA technique was applied to investigate IL-37 in skin homogenates. One hundred cases have been divided into two groups: 50 healthy volunteers as the group I healthy control;50 psoriasis patients as group II. PCR real-time technology was assessed through extracted DNA samples for VEGF amplification in skin homogenate. Result: Our results revealed that psoriasis patients significantly had a substantial reduction in IL-37 compared to the control group (p Conclusion: There is a significant inverse association between IL-37 and VEGF in psoriasis patients. Study findings revealed that IL-37 gene expression decreases while VEGF gene expression increases in psoriatic individuals. Such a measurement may be beneficial in determining the severity of the condition, as well as taking into consideration of the disease’s diagnosis.
