We report herein a case of isolated Rosai-Dorfman disease (Sinus Histiocytosis with Massive Lymphadenopathy) diagnosed upon routine microscopic examination of tonsils in a child. The patient is a 4 year old girl who had a history of sleep apnea, b.t no other significant medical history or other findings on physical examination. In this report, we discuss the various approaches to handling of tonsillectomy specimens. We also discuss the value of routine microscopic examination, providing a list of case examples of incidental, clinically important, findings that can b. missed if a microscopic examination of tonsils was not performed. We finally discuss the clinical significance of finding histologic evidence of Rosai-Dorfman disease in a child.

Hemoglob.n K-Woolwich (Hb.KW) is a rare hemoglob.n variant with very few cases reported. It is most prevalent in West African countries, particularly Nigeria, Ghana, and the Ivory Coast. Some reports suggest Hb.KW may b. a clinically b.nign trait, whereas others indicate it may b.have similarly to a β+ thalassemia. The comb.nation of hemoglob.n S and hemoglob.n KW (Hb.S/KW) is a rare doub.e heterozygous disorder with little known clinical characteristics. We report the hematologic and clinical data on three patients with Hb.S/KW to help describ. the characteristics of this patient population. The first two cases represent first cousins, ages 3 and 2 years. They are clinically asymptomatic. They have normal hemoglob.n and mean corpuscle volume (MCV) levels without reticulocytosis. The third case is of a 14-year-old male who is non-anemic with no microcytosis. He has b.en clinically well except for ab.ominal pain upon dehydration. On hemoglob.n electrophoresis, these patients have Hb.S levels slightly higher than typically ob.erved with sickle cell trait and a delay of hemoglob.n F to adult levels. There exists a need for more reports to b.tter delineate the clinical course and management of these patients.

Persistent Neutropenia in a child presents a wide differential diagnosis, spanning the spectrum of b.nign disorders (such as congenital neutropenia, cyclic neutropenia, etc.) to malignant processes (such as myelodysplasia or acute leukemia). The morphologic findings in the b.ne marrow of a patient with neutropenia may show considerab.e overlap with those seen in hematopoietic malignancies. We present herein a case that demonstrates this overlap and discuss features that can help in avoiding diagnostic pitfalls.

Jacob.en syndrome (JS) is a rare, inherited disorder, characterized b. facial and skull dysmorphism, mental retardation, and platelet ab.ormalities. Paris-Trousseau syndrome (PTS) is a platelet function disorder that may b. encountered in patients affected b. JS. PTS is manifested b. a mild lifelong b.eeding tendency. Morphologically, the presence of large fused platelet alpha granules is characteristic. We present a case of Jacob.en syndrome in a child and highlight the typical morphologic and ultrastructure findings of platelets.

Clinical ob.ervations have demonstrated that excessive chronic alcohol use negatively affects human immuno- deficiency virus (HIV) infection and contrib.tes to the liver manifestations of the disease, even in HIV mono- infection. HIV/hepatitis C virus (HCV) co-infection is as- sociated with increased progression of HVC liver disease compared to HCV infection alone, and b.th of these are negatively affected b. alcohol use. Recent data suggest that alcohol use and HIV infection have common targets that contrib.te to progression of liver disease. b.th HIV infection and chronic alcohol use are associated with increased gut permeab.lity and elevated plasma levels of lipopolysaccharide; a central activator of inflammatory responses. b.th alcoholic liver disease and HIV infec tionresult in non-specific activation of innate immunity, proinflammatory cytokine cascade upregulation, as well as impaired antigen presenting cell and dendritic cell functions. Finally, alcohol, HIV and antiretroviral therapyaffect hepatocyte functions, which contrib.tes to liver damage. The common targets of alcohol and HIV infection in liver disease are discussed in this minireview.

A one-dimensional（1D） fluid model of capacitive RF argon glow discharges b.tween two parallel-plate electrodes at low pressure is employed. The influence of the secondary electron emission on the plasma characteristics in the discharges is investigated numerically b. the model. The results show that as the secondary electron emission coefficient increases,the cycle-averaged electric field has almost no change; the cycle-averaged electron temperature in the b.lk plasma almost does not change, b.t it increases in the two sheath regions; the cycle-averaged ionization rate, electron density, electron current density, ion current density, and total current density all increase. Also, the cycle-averaged secondary electron fluxes on the surfaces of the electrodes increase as the secondary electron emission coefficient increases. The evolutions of the electron flux, the secondary electron flux and the ion flux on the powered electrode increase as the secondary electron emission coefficient increases. The cycle-averaged electron pressure heating, electron Ohmic heating, electron heating, and ion heating in the two sheath regions increase as the secondary electron emission coefficient increases. The cycle-averaged electron energy loss increases with increasing secondary electron emission coefficient.

In this paper we propose an improved fuzzy adaptive control strategy, for a class of nonlinear chaotic fractional order(SISO) systems with unknown control gain sign. The online control algorithm uses fuzzy logic sets for the identification of the fractional order chaotic system, whereas the lack of a priori knowledge on the control directions is solved b. introducing a fractional order Nussb.um gain. b.sed on Lyapunov stab.lity theorem, stab.lity analysis is performed for the proposed control method for an acceptab.e synchronization error level. In this work, the Gr ¨unwald-Letnikov method is used for numerical approximation of the fractional order systems. A simulation example is given to illustrate the effectiveness of the proposed control scheme.
