In this research, a content-based image retrieval （CBIR） system for high resolution satellite images has been developed by using texture features. The proposed approach uses the local binary pattern （LBP） texture feature and a block based scheme. The query and database images are divided into equally sized blocks, from which LBP histograms are extracted. The block histograms are then compared by using the Chi-square distance. Experimental results show that the LBP representation provides a powerful tool for high resolution satellite images （HRSI） retrieval.

Impact of Postoperative Radiotherapy and Chemotherapy on Survival for Patients with Node Positive Oral Cancer

Purpose: The aim of the present study is to compare between the use of post-operative concurrent chemotherapy and radiotherapy alone on survival for patients with high risk oral cavity tumors at the National cancer Institute of Egypt, undergoing surgery and receiving adjuvant treatment. Patients and Methods: This is a retrospective study, which was carried out at the National Cancer Institute (Cairo University) on patients with node positive oral cavity cancer diagnosed between the year 2000 and 2008. The study included 60 patients (45 males and 15 females) with median age 57 years old. The patients underwent surgery, followed by postoperative radiotherapy 60 Gy/6 weeks versus postoperative radiotherapy 60 Gy/6 weeks with concurrent cisplatin 100 mg/m2 at day 1, day 22 and day 43. Results: Regarding use of concurrent chemotherapy, there was a significant difference in overall survival rate and locoregional control favoring patients who received concurrent chemotherapy and radiotherapy, 3 years and 5 years overall survival rates respectively were 53.8% and 40.4% compared to 37.5% and 26.3% for patients who didn’t receive any chemotherapy with (p 0.038) for 5 years. Regarding age, there was a significant difference in overall survival rate favoring patients ≤57 years in both arm groups, 3 years and 5 years overall survival rates respectively were 51.6% and 38.9% compared to 28.3% and 18.9% for patients >57 years with (p 0.028) for 5 years. Conclusion: We recommend for oral cavity tumor patients at the NCI of Egypt who have positive neck nodes to be treated with concurrent chemo-radiotherapy rather than radiotherapy alone especially by using the new techniques as intensity modulated radiotherapy (IMRT) and image guided radiotherapy (IGRT).

The World Health Organization (WHO) defines chordoma as a malignant tumor of intermediate or low grade, developing at the expense of embryonic remnants of the notochord. These tumors represent 8.4% of all malignant bone tumors. We report 2 cases collected in the medical oncology department at the CHU Hassan II in Fes. In whom the clinical examination revealed a mass and/or signs of loco-regional compression. The tumor was located in the sacrum in both patients. The diagnosis was based on the data of the anatomopathological examination showing the association of a lobulated architecture with classic physaliphore cells and a mucoid intercellular substance. Immunohistochemistry was performed for both patients. The principle of therapeutic management was based on radiotherapy and targeted therapy since surgery was difficult given the local tumor extension.

Introduction :-The purpose of this study was to assess the prevalence of orthodontic treatment need and the orthodontic awareness level in multi-ethnic population and understand the correlation if any between the the two and also in relation to the socio-economic status of subjects. Methods: - A sample of male and female university graduate students were taken (n = 400, age 18–22 years, Chinese =180(Chinese male CM =*** female CF=82), Indian=120 (Indian male IM=72, Indian female IF=58), Nepal=88(Nepal male NM=47, Nepal female =42) with no history of medical or any other illness, participated in the study on a voluntary basis with informed consent . The subjects participated on their will .The subjects were made to answer a simple questionnaire on orthodontic awareness. Objective treatment need was assessed based on clinical examination by a post graduate student using the(DHC-Dental Health Component ) Index of Orthodontic Treatment Need (IOTN).The socio-economic status and sex variations were also calculated. All the questionnaire were checked for completeness. The socio-economic conditions in this sample was found to be highest in order of descending Indian>Nepalese >Chinese(table1). So to prevent the country barrier in assessing income, we used Latest G.N.I from world bank Survey information and assessed the family income as low, middle and high in their respective countries and used it for assessment.Results: Out of total 400 students who participated in the study,only ( n = 388, Chinese =180(CM =98. CF=82), Indian=120 (IM=62, IF=58), Nepal=88(NM=46, NF=42) gave the completely filled questionnaires. So we considered only these 388 subjects for final analyses Socio-economic conditions was statistically insignificant . Introduction: Primary extra-gastrointestinal stromal tumors (E-GIST) of the liver are rare. The clinical presentation can range from asymptomatic to bleeding or manifestations of mass effect. Cancer surgery followed by adjuvant treatment with imatinib is the standard of care. However, in special circumstances, a cytoreductive approach may represent a therapeutic option. We describe here the case of two patients who presented with a painful and protruding epigastric mass, the first case was a localized E-GIST candidate for neoadjuvant treatment with imatinib with complete resection surgery thereafter with very good evolution, and the second is that of an E-GIST of the liver at an advanced stage with an evolution very quick to death. Conclusion: Primary liver E-GIST is a rare condition. Treatment is based on systemic therapy and total resection surgery. However, cytoreductive surgery will be required when complete resection is not possible.

研究了高斯光束在外加空间调制电场的光折变晶体中的演化,结果表明,在不同的外加调制电场作用下高斯光束的光场分布会随着外电场变化而被调制。重点研究了同相位和反相位以及相位差为π/2的高斯光束在外加空间调制电场的光折变晶体中的相互作用,在外加空间分布线性增加电场作用下出现了许多新的实用特性,通过调整外加电场分布,可以让两同相位相干光靠得更近而不发生融合,也可以让两靠得很近的反相位光束可控偏转,还可以控制相位差为π/2的两相干光束进行能量转移。这种连续变化的电场可以用阶梯分布的电场替代,为工程实现提供了可能。

Sintering and Mechanical Properties of Magnesium and Fluorine Co-Substituted Hydroxyapatites

Biological apatites contain several elements as traces. In this work, Magnesium and fluorine co-substituted hydroxyapatites with the general formula Ca9Mg(PO4)6(OH)2-yFy, where y = 0, 0.5, 1, 1.5 and 2 were synthesized by the hydrothermal method. After calcination at 500℃, the samples were pressureless sintered between 950℃ and 1250℃. The substitution of F- for OH- had a strong influence on the densification behavior and mechanical properties of the materials. Below 1200℃, the density steeply decreased for y = 0.5 sample. XRD analysis revealed that compared to hydroxylfluorapatite containing no magnesium, the substituted hydroxyfluorapatites decomposed, and the nature of the decomposition products is tightly dependent on the fluorine content. The hardness, elastic modulus and fracture toughness of these materials were investigated by Vickers’s hardness testing. The highest values were 622 ± 4 GPa, 181 ± 1 GPa and 1.85 ± 0.06 MPa.m1/2, respectively.

巴基斯坦的医疗保健体系受到诸多问题的困扰:组织体系失灵,性别敏感性下降,资源不足以及缺乏结构性责任制。面对外部债务沉重、效率低下等不安全经济形势,巴基斯坦的医改方案毫无疑问是有限的。显然,医疗改革中的不公平现象是多方面的,包括地理、社会、经济甚至总体政治等方面。但是最近出台的医疗改革法案提供了一个机会之窗,引发了医疗保健和社会部门的广泛变革。公共医疗服务鼓励公私合作伙伴关系,划分基本医疗基础费用的成本。通过适当的培训和服务提供（例如WHO,WORLD BANK,UNDP）以及社区参与决策过程,巴基斯坦必须建立对性别问题更加敏感和践行性别平权的医疗体系。论文基于公共领域的二手分析数据、UHS（健康科学大学）的研究论文以及其他国家和地区的医疗改革经验,重点分析了医疗改革对医生满意度和医患纠纷的主要原因的影响,并在第四章详细论述了此项研究对巴基斯坦医疗改革的理论和现实方面的启示。

Today, the ultrasound guidance (USG) in regional anesthesia is gold standard more and more often used for medial or paramedian approaches around to the spine, such as the paravertebral (PV) block. Local anatomical changes may greatly handicap the performance of this type of block. We present clinical, sonographic, and radiological data on successful PV block and catheter placement in four patients with vertebral diseases, targeting thoracotomy or lumbotomy postoperative pain after stabilization of the involved vertebral body and preliminary arthrodesis with laminectomy by the posterior approach. We emphasize the importance of USG in this special context involving local anatomical disturbance.
