Interventional Cardiology Management of a Cardiogenic Shock Induced by Takotsubo Cardiomyopathy at the Hospital Center of Montlucon: A Case Report

Background: Takotsubo cardiomyopathy is frequently considered as a benign disorder. We present an atypical form with cardiogenic shock that was managed by interventional cardiology measures. Case presentation: A 58 years old female patient with a past history of hypertension, obesity and multiple sclerosis was admitted at the Hospital Center of Montlucon for septic shock of urinary origin. During hospitalization in intensive care unit, the patient presented a markedly increasing of troponin levels with a diffused ST-segment elevation. Transthoracic Echocardiography showed an altered left ventricular ejection fraction at 35% with hypokinesia of apex and lateral ventricular segments in conjunction with compensatory hyperkinesis of the base;these findings were strongly suggestive of a diagnosis of Takotsubo cardiomyopathy. Despite concomitant anemia, renal failure and sepsis, all adequately treated with complete remission, the patient developed frequent episodes of ventricular tachycardia that prompted an emergency coronarography. During this procedure, the patient presented a cardiogenic shock and bradyasystole that were successfully managed by intra-aortic balloon pumping and temporary transvenous pacing. Finally, there were no coronary lesions and ventriculography confirmed a Takotsubo cardiomyopathy. Acute Stent Thrombosis: A Case at the Montlucon Hospital Center

Background and objective: Coronary angioplasty is one of the techniques introduced in 1976 by Andreas Grüntzig in Zurich. It is a revolutionary procedure that allows coronary circulation to be restored by inserting a stent. This new technique has considerably evolved over time, but sometimes has limitations, such as the development of neo-pathologies like stent thrombosis. The aim of our case report is to highlight one of the limitations of coronary angioplasty, although rare, and to encourage greater clinical and electrical monitoring after each procedure. Case report: We report the case of a patient who presented with early stent thrombosis barely an hour after placement of a pharmacoactive stent. Chest pain reported by the patient after the procedure and electrical changes prompted an urgent repeat procedure. Aetiologies of stent thrombosis are multifactorial, including patient-, procedure- and stent-dependent factors. Conclusion: Although rare, there is a risk of stent thrombosis after coronary angioplasty. Careful monitoring and rigorous follow-up of patients after coronary angioplasty are therefore required, as the prognosis for stent thrombosis is fairly poor.

Determining driver perceptions about distractions and modeling their effects on driving behavior at different age groups

This study was aimed at determining the driving distractions which are perceived most hazardous and determining the effects of these distractions and age on speed and headway. A questionnaire survey was done to find out the opinion of drivers related to the most hazardous distraction. 639 responses were collected in the survey which were used to determine the top-rated distractions for drivers in Bahrain. Roadside observations were taken to observe the speed, headway, age and type of distraction for the driver. Speed was observed for 48 drivers while headway was observed for 36 drivers along with other parameters. The most hazardous distractions, according to the participants of the questionnaire, are using mobile phones, handling children, and accidents or incidents on the road. Further, the results of the two-way analysis of variance(ANOVA) test and regression analysis demonstrated that using mobile phones and age have a significant effect on both speed and headway. Speed tends to decrease with distraction for all age groups while decreasing the headway for young and middle-aged drivers. The effect of distraction is higher than the effect of age on speed, as well as headway. Texting has the most significant effect among distractions on headway. It is hereby recommended that policymakers should focus on increasing awareness and stringent law enforcement related to handling mobile phones and children, especially for young and middle-aged drivers.

Pore structure of cementitious material enhanced by graphitic nanomaterial： a critical review

Carbon nano tubes （CNT） has been introduced as an efficient nanomaterial in order to improve the mechanical and durability properties of concrete. The effect of CNT on the microstructures of cementitious materials has been widely reported. This paper combines a critical review on the effect of CNT on the pore and microstructure of cement composite with a discussion on the porosity measurement of pastes containing CNT using mercury intrusion porosimetry techniques （MIP）. It was found that, surface treatment by H2SO4 and HNO3 solution forms carboxyl acid groups on CNTs＇ surfaces that lead to the improvement of reinforcement. In this scope, this review paper involves analyzing the effect of CNT on the microstructure and the pore structure of cementitious materials. The existing methods of measuring the porosity of cementitious material are reviewed, in particular, the contact angle measurement is discussed in detail in which the most effective parameters and possible errors of calculation is presented.

Controlled growth of Bi2WO6 nanorods with exposed[001]facets and the fabrication of an Fe2O4-Bi2WO6 magnetic composite by a microwave-assisted polyol process,were achieved in this *** adsorp-tivity and photocatalytic performance of the composite toward sunset yellow dye degradation were greatly enhanced by the β-cyclodextrin cavities on its surface,firmly anchored through a cetyltrimethylammonium bro-mide linkage.A series of examinations and characteriza-tions were carried out to determine the influence of various factors on the morphological modulation-photocatalytic behavior of the pure Bi2WO6 prior to final *** the pH of the precursor solution impacted the formation of 0D,2D,and 3D structures;however,the presence of hexamethylenetetramine surfactant induced the development of 1D nanorod structure.A reasonable crystal growth mechanism was proposed to elucidate the formation ***,the mechanism of the activity enhancement of β-cyclodextrin functionalized Fe2O4-Bi2WO6,compared to that of the non-functiona-lized samples,could be realized with the assistance of chemical trapping experiments on sunset yellow,and was confirmed on the colorless antibiotic(sulfamethoxazole).The high performance and durability of this composite can be attributed to the facet-dependent activity,large adsorp-tion capacity due to inclusion interactions,enhanced visible light absorption,and efficient charge separation.

The aim of this work is to find exact solutions of the Dirac equation in(1+1) space-time beyond the already known *** consider exact spin(and pseudo-spin) symmetric Dirac equations where the scalar potential is equal to plus(and minus) the vector *** also include pseudo-scalar potentials in the *** spinor wavefunction is written as a bounded sum in a complete set of square integrable basis,which is chosen such that the matrix representation of the Dirac wave operator is tridiagonal and *** makes the matrix wave equation a symmetric three-term recursion relation for the expansion coefficients of the *** solve the recursion relation exactly in terms of orthogonal polynomials and obtain the state functions and corresponding relativistic energy spectrum and phase shift.
