Management of cytomegalovirus infection after liver transplantation

Cytomegalovirus(CMV)infection is one of the primary causes of morbidity and mortality following liver transplantation(Lt).Based on current worldwide guidelines,the most effective strategies for avoiding post-transplant CMV infection are antiviral prophylaxis and pre-emptive ***-IgG serology is the established technique for pretransplant screening of both donors and *** clinical presentation of CMV infection and disease exhibits variability,prompting clinicians to consistently consider this possibility,partic-ularly within the first year post-transplantation or subsequent to heightened *** annual symposia to discuss CMV prevention and how treatment outcomes can be improved,evidence on the incorporation of immune functional tests into clinical practice is presented,and the results of studies with new antiviral treatments are *** there are ongoing studies on the use of letermovir and maribavir in solid organ transplantation,a consensus reflected in the guidelines has not been *** the most appro-priate strategy at the individual level appears to be the key to enhancing *** prevention strategies reduce the risk of CMV disease,the disease can still occur in up to 50%of high-risk patients.A balance between the risk of infection and disease development and the use of immunosuppressants must be considered when talking about the proper management of CMV in solid organ transplant *** objective of this study was to establish a compre-hensive framework for the management of CMV in patients who have had Lt.

Spatio-temporal variability and dynamics in Sahelian agro-pastoral zones make each local situation a special case. these specificities must be considered to guide the dissemination of agricultural options with a view to sustainable development. the territorial scale of municipalities is not sufficient for this necessary contextualization;the scale of the “village terroir” seems to be a better option. this is the hypothesis we put forward in the framework of the Global Collaboration for Resilient Food Systems program (CRFS), i.e. local context is spatially defined by village terroir. the study is based on data collected through participatory mapping and surveys in “village terroirs” in three regions of Niger (Maradi, Dosso and tillabéri). then the links between farm managers and their cultivated land, as well as the spatio-temporal dynamics of local context are analyzed. this study provides evidence of the existence and functional usefulness of the village terroir for farmers, their land management and their activities. It demonstrates the usefulness of contextualizing agricultural options at this scale. their analysis elucidates the links between “terroirs village” and the specific functioning of the agrosocio-ecosystems acting on each of them, thus laying the systemic and geographical foundations for a model of the spatio- temporal dynamics of “village terroirs”. this initial work has opened up new perspectives in modeling and sustainable development.

An Eigenspace Method for Detecting Space-time Disease Clusters with Unknown Population-Data

Space-time disease cluster detection assists in conducting disease surveillance and implementing control *** state-of-the-art method for this kind of problem is the Space-time Scan Statistics(SatScan)which has limitations for non-traditional/non-clinical data sources due to its parametric model assumptions such as Poisson orGaussian *** this problem,an Eigenspace-based method called Multi-EigenSpot has recently been proposed as a nonparametric ***,it is based on the population counts data which are not always available in the least developed *** addition,the population counts are difficult to approximate for some surveillance data such as emergency department visits and over-the-counter drug sales,where the catchment area for each hospital/pharmacy is *** extend the population-based Multi-EigenSpot method to approximate the potential disease clusters from the observed/reported disease counts only with no need for the population *** proposed adaptation uses an estimator of expected disease count that does not depend on the population *** proposed method was evaluated on the real-world dataset and the results were compared with the population-based methods:Multi-EigenSpot and *** result shows that the proposed adaptation is effective in approximating the important outputs of the population-based methods.

Fatigue Life Estimation of High Strength 2090-t83 Aluminum Alloy under Pure torsion Loading Using Various Machine Learning techniques

the ongoing effort to create methods for detecting and quantifying fatigue damage is motivated by the high levels of uncertainty in present fatigue-life prediction approaches and the frequently catastrophic nature of fatigue *** fatigue life of high strength aluminum alloy 2090-t83 is predicted in this study using a variety of artificial intelligence and machine learning techniques for constant amplitude and negative stress ratios(R?1).Artificial neural networks(ANN),adaptive neuro-fuzzy inference systems(ANFIS),support-vector machines(SVM),a random forest model(RF),and an extreme-gradient tree-boosting model(XGB)are trained using numerical and experimental input data obtained from fatigue tests based on a relatively low number of stress *** particular,the coefficients of the traditional force law formula are found using relevant numerical *** is shown that,in comparison to traditional approaches,the neural network and neuro-fuzzy models produce better results,with the neural network models trained using the boosting iterations technique providing the best *** strong models from weak models,XGB helps to predict fatigue life by reducing model partiality and variation in supervised *** neural models can be used to predict the fatigue life of alloys more accurately than neural networks and traditional methods.

Excessive fertilization has led to nutrient use inefficiency and serious environmental consequences for radish cultivation in North *** Nutrient Expert(NE)system is a science-based,site-specific fertilization decision support system,but the updated NE system for radish has rarely been *** study aims to validate the feasibility of NE for radish fertilization management from agronomic,economic,and environmental perspectives.A total of 46 field experiments were conducted over four seasons from April 2018 to November 2019 across the major radish growing regions in North *** results indicated that NE significantly reduced N,P_(2)O_(5),and K_(2)O application rates by 98,110,and 47 kg ha^(-1) relative to those in the farmers’practice(FP),respectively,and reduced N and P_(2)O_(5) inputs by 48 and 44 kg ha^(-1),respectively,while maintaining the same K_(2)O rate as soil testing(St).Relative to FP and St,NE significantly increased radish yield by 2.7 and 2.6 t ha^(-1)(4.2 and 4.0%)and net returns by 837 and 432 USD ha^(-1),*** average,NE significantly improved the agronomic efficiency(AE)of N,P,and K(relative to FP and St)by 42.4 and 31.0,67.4 kg kg^(-1) and 50.9,and 20.3 and 12.3 kg kg^(-1);enhanced the recovery efficiency(RE)of N,P,and K by 11.4 and 7.0,14.1 and 7.5,and 11.3 and 6.3 percentage points;and increased the partial factor productivity(PFP)of N,P,and K by 162.9 and 96.8,488.0 and 327.3,and 86.9 and 22.4 kg kg^(-1),***,NE substantially reduced N and P_(2)O_(5) surpluses by 105.1 and 115.1 kg ha^(-1),respectively,and decreased apparent N loss by 110.8 kg ha^(-1) compared to *** results indicated that the NE system is an effective and feasible approach for improving NUE and promoting cleaner radish production in North China.

the rare metals of Abu Dabbab area in the Central Eastern Desert of Egypt have been investigated for their mineralogy and conditions of precipitation using combination of EMPA and fluid inclusions studies,in order to delineate the source,mechanism of formation and evolutionary model for these economic ***(ta-Nb-Sn)-bearing minerals at the Abu Dabbab area include columbite group minerals(CGMs),wodginite and *** both granitic intrusion and its enclosed quartz veins,most of zoned CGMs and cassiterite grains are commonly characterized by a well-developed twostage ***,columbite-(Mn)(CGM-Ⅰ)represents the early formed phase of CGMs that is characterized by high Mn#values(0.64-0.92)with quite low ta#values(0.13-0.49).It was invaded by ta-rich phases including tantalite-(Mn)(CGM-Ⅱ;ta#=0.13-0.49)and wodginite,which contain high ta_(2)O_(5)and SnO_(2)(17.91 wt.%).In regard to cassiterite,there are distinct compositional differences between the early-phase cassiterite(Cst-Ⅰ)and the late-phase one(Cst-Ⅱ),where the latter is enriched in ta_(2)O_(5),Nb_(2)O_(5)and *** chemistry and textural criteria of the early stage CGM-Ⅰand Cst-Ⅰ,all are indicative of magmatic ***,the latter CGM-Ⅱ,wodginite and Cst-Ⅱwere influenced by the late magmatic ta-rich *** inclusions microthermometry shows criteria of phase separation represented by both boiling and fluid *** initial fluid was supposed to be of magmatic origin(magmatic CH4),that was consequently influenced by fluid mixing/dilution with post-hydrothermal/meteoric water with respect to the decompression process during *** construction gave rise to an estimate P-t conditions(t=330-370℃,P=22-50 MPa).the fluid inclusions’microthermometry supports a transition between magmatic and late to post-hydrothermal activities in addition to surface-derived fluid(meteoric fluid?)in a part as main source for the polymetallic deposits.

BACKGROUND Diabetes mellitus is a worldwide public health problem associated with significant complications. there is lack of data on the quality of care of patients with diabetes, specifically among the non-Western countries. Efforts have been made in Lebanon to better study the characteristics of patients with diabetes mellitus in order to improve glycemic control and prevent late-term *** to investigate control and therapeutic management of patients with diabetes mellitus in the current medical practice in *** Wave 6 of the International Diabetes Management Practice Study in Lebanon is an international and multicenter study involving selected *** Only 1 patient with type 1 diabetes and 595 patients with type 2 diabetes were included in Wave 6. Average age was around 60 years, with a mean body mass index of 30. the mean fasting serum glucose was 159.42 mg/dL, and the mean glycosylated hemoglobin(HbA1 c) level was 7.98 with around 30% achieving an HbA1 c target of tients were on oral anti-diabetic *** of diabetic complications has improved over the years. A large percentage is diagnosed with hypertension and dyslipidemia, the majority ofwhom were treated but only a small percentage were *** Diabetes, with its associated dyslipidemia and hypertension, is still not very well controlled. Screening for diabetes complications has improved over the *** need to have more proper care, and physicians need to follow diabetes guidelines, and to have a larger number of patients who have appropriate treatment of diabetes, hypertension and lipids.

Space-time Cluster Analysis of Accidental Oil Spills in Rivers State, Nigeria, 2011–2019

Oil spills cause environmental pollution with a serious threat to local communities and sustainable *** oil spills can be modelled as a stochastic process where each oil spill event is described by its spatial locations and incidence-time and hence allow for space-time cluster *** cluster analysis can detect space-time pattern distribution of oil spills which can be useful for implementing preventive measures and evidence-based decision *** study aims to detect the space-time clusters of accidental oil spills in Rivers state,Nigeria through the Space-time Scan *** Space-time Scan Statistic was applied under the permutation model to the oil spill data(each for sabotage and operational oil spills)collected at Local Government Area(LGA)-level during the period from 2011 to *** results show that the sabotage oil spill clusters have covered most of the LGAs in the southern part of the state at the start of the study period and then in 2018–2019,it moved to the west covering a single *** operational oil spill clusters covered two neighboring LGAs in the *** addition,the temporal cluster of sabotage oil spills was seen in 2019 and operational oil spills in 2011–*** sabotage oil spills show an increasing trend with the maximum in 2019 while the operational oil spills show a decreasing trend with the minimum in *** findings assist in more effective decision-making for combating the environmental problems and controlling the future spill incidence in the cluster-regions.
