AIM:To assess the outcomes of laparoscopic colorectal cancer resection in the octogenarian population at our institution.METHODS:Retrospective analysis of registry data accumulated prospectively were used in conjunction with the data obtained from patient notes to identify outcome data for octogenarians who had undergone elective laparoscopic colorectal cancer resection.RESULTS:Laparoscopic colorectal cancer resections were performed in 68 octogenarians between 2003 and 2011 at our institution.Four operations(6%) were converted to an open technique.There were twelve cases of morbidity(18%) and two cases of mortality(3%).The overall median hospital stay was 8 d.The median time for a patient to be deemed surgically fit for discharge was 5 d reflecting a delay in provision of social care or stoma education.CONCLUSION:Our results support the view that laparoscopic surgery in octogenarians is safe,feasible and with a reduced length of stay.This is well reflected in our results which are compatible with United Kingdom national figures.

A tool was developed to assist the cooling systems designer in designing and installing the microsprinklers and fan cooling system. The tool was developed by integrating a mathematical model into an electronic spark map in order to use the mathematical model practically. The mathematical model was developed using the designs, parameters, variables, and constant values of the microsprinklers and fans cooling system. Subsequently, an electronic spark map (decision tree) was developed, and then the mathematical model was integrated into the electronic spark map. Afterwards, C# (C Sharp) programming language was used to develop a computer system via the electronic spark map, and to make the user interface. The developed computer system assists the designer in making decisions to specify and to calculate the required discharge of cooling system pump, length and diameter of cooling system pipelines, number of cooling fans, and number of microsprinklers. Moreover, this tool calculates the capital investment and the fixed, variable, and total costs of the cooling system. However, the mathematical model of the spark map requires some input data such as: pressure and discharge of microsprinklers, and some other engineering parameters. Data of 4 cooling systems were used to carry out the model validation. The differences between actual and calculated values were determined, and the standard deviations were calculated. The coefficients of variation were between 2.25% and 4.13%.

The genetic basis underlying liver fibrosis remains largely unknown.We conducted a study to identify genetic alleles and underlying pathways associated with hepatic fibrogenesis and fibrosis at the genome-wide level in 121 human livers.By accepting a liberal significance level of P<1e-4,we identified 73 and 71 candidate loci respectively affecting the variability in alpha-smooth muscle actin(a-SMA)levels(fibrogenesis)and total collagen content(fibrosis).The top genetic loci associated with the two markers were BAZA1 and NOL10 for a-SMA expression and FAM46A for total collagen content(P<1e-6).We further investigated the relationship between the candidate loci and the nearby gene transcription levels(cis-expression quantitative trait loci)in the same liver samples.We found that 44 candidate loci for a-SMA expression and 44 for total collagen content were also associated with the transcription of the nearby genes(P<0.05).Pathway analyses of these genes indicated that macrophage migration inhibitory factor(MIF)related pathway is significantly associated with fibrogenesis and fibrosis,though different genes were enriched for each marker.The association between the single nucleotide polymorphisms,MIF and a-SMA showed that decreased MIF expression is correlated with increased a-SMA expression,suggesting that variations in MIF locus might affect the susceptibility of fibrogenesis through controlling MIF gene expression.In summary,our study identified candidate alleles and pathways underlying both fibrogenesis and fibrosis in human livers.Our bioinformatics analyses suggested MIF pathway as a strong candidate involved in liver fibrosis,thus further investigation for the role of the MIF pathway in liver fibrosis is warranted.The study was reviewed and approved by the Institutional Review Board(IRB)of Wayne State University(approval No.201842)on May 17,2018.

In this study,we have proposed an artificial neural network(ANN)model to estimate and forecast the number of confirmed and recovered cases of COVID-19 in the upcoming days until September 17,2020.The proposed model is based on the existing data(training data)published in the Saudi Arabia Coronavirus disease(COVID-19)situation—Demographics.The Prey-Predator algorithm is employed for the training.Multilayer perceptron neural network(MLPNN)is used in this study.To improve the performance of MLPNN,we determined the parameters of MLPNN using the prey-predator algorithm(PPA).The proposed model is called the MLPNN–PPA.The performance of the proposed model has been analyzed by the root mean squared error(RMSE)function,and correlation coefficient(R).Furthermore,we tested the proposed model using other existing data recorded in Saudi Arabia(testing data).It is demonstrated that the MLPNN-PPA model has the highest performance in predicting the number of infected and recovering in Saudi Arabia.The results reveal that the number of infected persons will increase in the coming days and become a minimum of 9789.The number of recoveries will be 2000 to 4000 per day.

Effect of Cowshed Design and Cooling Strategy on Welfare and Productivity of Dairy Cows

Nine cowsheds were used, the differences between each cowshed are orientation (east-west or north-south), cooling (with or without), and height (3 m, 5 m or 8 m height). Dry-bulb temperature, relative humidity, dew point, shaded area, air velocity, and maximum temperature were measured beneath each shed. The temperature-humidity index (THI) and the shading efficiency were calculated for each cowshed. The trials were conducted using 180 lactating Holstein Friesian cows; twenty cows housed beneath each shade structure. Measurements performed on cows were respiration rate, skin temperature, and rectal temperature; moreover, the milk produced by each cow was recorded, and the feed intake. High air velocities were recorded under high shade structures which enhance the aeration; consequently, maximum temperatures, and THI recorded beneath high sheds were less than those recorded beneath low sheds; also, skin temperatures, and respiration rates for cows housed under high sheds were less than those measured for animals housed under low sheds(differences were found to be significant). Thereby, increasing shed height enhances dairy cows’ microclimate under Egyptian conditions (hot climate); consequently, the milk production increases. The best orientation is the east-west orientation, which is preferable for hot climates. The exploitation of cooling has significant effect on microclimate, cooling enhances the ambient environment and provides comfortable zone for dairy cows within which cows reach high production levels.

Objective:Prostate cancer is the most frequent cancer in men and radical retropubic prostatectomy(RRP)is one of the first-line treatment.However,RRP has some side effects and can lead to chronic perineal pain.The objective of the study was to determine in patients suffering from perineal pain after RRP the possibility of a neurogenic damage by means of a specific questionnaire dedicated to track down neuropathic pain.Methods:Forty patients were explored by a specific and validated questionnaire,the Neuropathic Pain Symptom Inventory(NPSI).Patients were divided into two groups:Group A with an NSPI score≥4 was considered as suffering from neuropathic pain,and Group B was considered as a control group without neuropathic pain(NSPI score<4).All patients had a perineal electrophysiological testing to confirm the possibility of a neurogenic damage.Results:Group A was composed by 13 men and Group B by 27 men,with mean age 72.45 years and mean duration of pain 2.7 years.In Group A,the most frequent symptoms were burning sensation,electrical shock and numbness.Location of the pain was global perineal area(8/13),anus(10/13),penis(5/13)and glans penis(2/13).Electromyography(EMG)findings confirmed the presence of denervation and neurogenic damages compared with controls(p<0.001).Conclusion:One third of the patients consulting for chronic pain following RRP had probably a neuropathic lesion leading to a chronic perineal pain as suggested by an NSPI score≥4 and EMG alterations.

Theranostics are one of the practical aspects of personalized medicine.This concept was designed to describe a material combining diagnosis,treatment and follow up of a disease.It evolved and included molecular targeting and nanotechnologies that incorporate both diagnosis and therapeutics.In this editorial,we are presenting briefly the concept and evolution of theranostics,highlighting many applications of theranostics in daily practice and discussing future perspectives and aspects of this model in gastrointestincal cancers.

Prioritization of W. Alarab Sub-Watersheds (North Jordan) for Conservation Measures Using RS, GIS, and Multi-Criteria Analysis

Soil erosion and high sediment flow are of eminent environmental concern in Wadi Alarab catchment, northern Jordan. The objective of this research is to conduct a prioritization scheme using RS, GIS, and multi-criteria analysis approach based on morphometric analysis, land use/land cover (LULC) change analysis, and soil loss modeling based on RUSLE model factors. ASTER GDEM and Arc GIS were utilized to delineate watersheds and extract the drainage networks using the Arc Hydro tool. Five basic morphometric parameters, five linear and five shape parameters, six LULC classes, and five soil erosion risk classes are applied to prioritize 13 sub-watersheds connected to W. Alarab basin. LANDSAT images were subjected to supervised classification (the Maximum Likelihood Method) to determine land use/cover changes and to establish the LULC map/layer. Soil erosion risk classes were estimated using the RULSE model. RULSE factors (R, K, L, S, C, and P) were calculated in a GIS environment, then multiplied together so as to estimate soil loss (ton·ha-1·yr-1) and to establish a soil erosion risk map for the entire watershed and the thirteen sub-watersheds. A GIS-based integration of the three layers compiled for each criterion reveals that six sub-watersheds (1, 5, 8, 9, 10, and 11) are categorized under low priority. Further, three sub-basins (4, 12, and 13) are fall under moderate priority, and four sub-basins (2, 3, 6, and 7) are designated as of high priority. It is obvious that 53.8% of these sub-basins must be prioritized immediately for soil and conservation measures. The validity of the achieved priority classes was tested statistically using Discriminant Analysis (DA), and the results showed that morphometric parameters, LULC analysis, and soil loss are accepted criteria for prioritization. These results are intended to help decision-makers to prepare reliable soil erosion management plans.

AIM:To assess levels of serum resistin upon hospital admission as a predictor of acute pancreatitis(AP) severity.METHODS:AP is both a common and serious disease,with severe cases resulting in a high mortality rate.Several predictive inflammatory markers have been used clinically to assess severity.This prospective study collected data from 102 patients who were diagnosed with an initial acute biliary pancreatitis between March 2010 and February 2013.Measurements of body mass index(BMI) and waist circumference(WC) were obtained and serum resistin levels were analyzed at the time of hospital admission using enzyme-linked immunosorbent assay.Additionally,resistin levels were measured from a control group after matching gender,BMI and age.RESULTS:A total of 102 patients(60 females and 42 males) were diagnosed with acute gallstone-induced pancreatitis.The mean age was 45 years,and mean BMI value was 30.5 kg/m2(Obese,class?Ⅰ).Twentytwo patients(21.6%) had severe AP,while eighty-eight patients had mild pancreatitis(78.4%).Our results showed that BMI significantly correlated with pancreatitis severity(P = 0.007).Serum resistin did not correlate with BMI,weight or WC.Furthermore,serum resistin was significantly higher in patients with AP compared to control subjects(P < 0.0001).The mean resistin values upon admission were 17.5 ng/mL in the severe acute biliary pancreatitis group and 16.82 ng/mL in the mild AP group(P = 0.188),indicating that resistin is not an appropriate predictive marker of clinical severity.CONCLUSION:We demonstrate that obesity is a risk factor for developing severe AP.Further,although there is a correlation between serum resistin levels and AP at the time of hospital admission,resistin does not adequately serve as a predictive marker of clinical severity.

Simulation plays a pivotal role in neurosurgical training by allowing trainees to develop the requisite expertise to enhance patient safety.Several models have been used for simulation purposes.Non-living animal models offer a range of benefits,including affordability,availability,biological texture,and a comparable similarity to human anatomy.In this paper,we review the available literature on the use of non-living animals in neurosurgical simulation training.We aim to answer the following questions:(1)what animals have been used so far,(2)what neurosurgical approaches have been simulated,(3)what were the trainee tasks,and(4)what was the experience of the authors with these models.A search of the PubMed Medline database was performed to identify studies that examined the use of non-living animals in cranial neurosurgical simulation between 1990 and 2020.Our initial search yielded a total of 70 results.After careful screening,we included 22 articles for qualitative analysis.We compared the reports in terms of the(1)animal used,(2)type of surgery,and(3)trainee tasks.All articles were published between 2003 and 2019.These simulations were performed on three types of animals,namely sheep,cow,and swine.All authors designed specific,task-oriented approaches and concluded that the models used were adequate for replicating the surgical approaches.Simulation on non-living animal heads has recently gained popularity in the field of neurosurgical training.Non-living animal models are an increasingly attractive option for cranial neurosurgical simulation training.These models enable the acquisition and refinement of surgical skills,with the added benefits of accessibility and cost-effectiveness.To date,16 different microneurosurgical cranial approaches have been replicated on three non-living animal models,including sheep,cows,and swine.This review summarizes the experience reported with the use of non-living animal models as alternative laboratory tools for cranial neurosurgical training,with particular attention to the se
