Some pelvic pain syndromes are very resistant to medical treatment. Several studies have demonstrated that sacral neuromodulation, which has been successfully used for the treatment of bladder dysfunction, incontinence, urinary retention and urinary frequency [1]-[3], can be successfully used for the treatment of chronic pelvic pain [4]-[7]. Several studies have also demonstrated significant involvement of dorsal column pathways in the transmission of visceral pelvic pain [8] and the successful use of spinal cord stimulation for the treatment of chronic pelvic pain [9]. We report three cases of severe chronic pelvic pain that failed conservative treatment modalities. Placement of a combined sacral nerve roots stimulator and a low thoracic spinal cord stimulator resulted in a significant pain relief and improvement in daily life activities. We believe that this combination may help patients suffering from chronic pelvic pain resistant to medical management.

A 40-years-old female patient with severe right facial pain with a throbbing component along the mandibular division of the trigeminal nerve resistant to medical management. Continuous inferior alveolar nerve block with local anesthetics using an indwelling catheter provided a complete pain resolution for the patient for 2 weeks, after which the catheter got infected and was removed. A trial of balloon occlusion of the right internal maxillary artery provided complete resolution of the throbbing component of the patient’s pain. This was followed by permanent embolization with multiple coils.

Anaphylactoid Shock with Infusion of 5% Albumin in a Patient under General Anesthesia

A 61 year old male patient undergoing gastrectomy under general anesthesia developed severe anaphylactoid shock after 15 minutes from starting 5% albumin infusion. His blood pressure went from 101/67 down to 44/23 with his heart rate going up from 91 to 99. A total of 1600 mcg of phenylephrine were given without any improvement in the hemo-dynamics. We then gave 50 mcg of Epinephrine to which blood pressure rapidly responded going up to 141/60 (mean 88). Then 15 mg of Dexamethasone were given. Blood pressure remained stable with the mean between 75 and 90, without requiring anymore pressors. 5% albumin is considered among the safer colloids with a risk of anaphylactoid reactions less than gelatins and dextrans and comparable to starches. However, severe life threatening anaphylaxis has been reported with Albumin infusion. With the still ongoing crystalloid colloid debate, with many studies showing no survival benefit with colloid use for volume resuscitation, the risk of severe anaphylactic shock, even with the safer colloids like albumin should drive to a more conservative use of albumin for volume resuscitation, specially under general anesthesia, when recognizing the signs and symptoms of anaphylaxis and the offending agent maybe more challenging.

AIM: To assess the relation between central retinal artery(CRA) resistive index(RI) and retinal nerve fiber thickness measured by optical coherence tomography(OCT) in assessment of disease progress in cases of open angle glaucoma.METHODS: Twenty-three patients with diagnosed open angle glaucoma were included in this study. They were examined by colored duplex ultrasonography of CRA with estimation of RI of CRA and ophthalmic artery(OA)with estimation of CRA/OA RI ratio as well as OCT measurement of the average retinal nerve fiber layer(RNFL) thickness in order to assess the disease progress.RESULTS: There was strong inverse relation between the increased RI in CRA as well as the increased CRA/OA RI ratio and the decrease in average RNFL thickness in cases of open angle glaucoma.CONCLUSION: Assessment of CRA RI can indirectly assess the vascular changes associated with glaucoma and can assess the degree of retinal atrophy helping in evaluating prognosis thus guiding the choice of treatment.

Discharge Summaries Based Sentiment Detection Using Multi-Head Attention and CNN-BiGRU

Automatic extraction of the patient’s health information from the unstructured data concerning the discharge summary remains challenging.Discharge summary related documents contain various aspects of the patient health condition to examine the quality of treatment and thereby help improve decision-making in the medical field.Using a sentiment dictionary and feature engineering,the researchers primarily mine semantic text features.However,choosing and designing features requires a lot of manpower.The proposed approach is an unsupervised deep learning model that learns a set of clusters embedded in the latent space.A composite model including Active Learning(AL),Convolutional Neural Network(CNN),BiGRU,and Multi-Attention,called ACBMA in this research,is designed to measure the quality of treatment based on discharge summaries text sentiment detection.CNN is utilized for extracting the set of local features of text vectors.Then BiGRU network was utilized to extract the text’s global features to solve the issues that a single CNN cannot obtain global semantic information and the traditional Recurrent Neural Network(RNN)gradient disappearance.Experiments prove that the ACBMA method can demonstrate the effectiveness of the suggested method,achieve comparable results to state-of-arts methods in sentiment detection,and outperform them with accurate benchmarks.Finally,several algorithm studies ultimately determined that the ACBMA method is more precise for discharge summaries sentiment analysis.

In this paper,a generalized nonlinear dissipative and dispersive equation with time and space-dependent coefficients is considered.We show that the control of the higher order term is possible by using an adequate weight function to define the energy.The existence and uniqueness of solutions are obtained via a Picard iterative method.As an application to this general Theorem,we prove the well-posedness of the Camassa-Holm type equation.

Single-relay selection techniques based on themax-min criterion can achieve the highest bit error rate(BER)performance with full diversity gain as compared to the state-of-the-art single-relay selection techniques.Therefore,in this work,we propose a modified max-min criterion by considering the differences among the close value channels of all relays while selecting the best relay node.The proposed criterion not only enjoys full diversity gain but also offers a significant improvement in the achievable coding gain as compared to the conventional one.Basically,in this article,an improved bi-directional three-phase single-relay selection technique using the decodeand-forward protocol for wireless cooperative communication networks that enhances the overall network performance in terms of BER is proposed and its performance is proved analytically and through Monte-Carlo simulations.More specifically,the proposed criterion is first used to select the best relaynode.After that the selected relay-node forwards the information symbols of the communicating terminals after performing a digital network coding to minimize power consumptions.In our simulations,we show that our proposed technique outperforms the best-known single relay selection techniques.Furthermore,we prove that the BER results obtained from our conducted simulations perfectly match those obtained from the theoretical analysis.

Daraa City is widely known as the birthplace of the Syrian uprising.The uprising,which was initially motivated by high-minded ideas and opposition to the arbitrary violence of an authoritarian state,rapidly degenerated into a civil war orientated by external agendas and priorities.In this paper,I want to situate Daraa governorate at the centre of this development,with the intention of highlighting how the course of events in this small part of Syria had vital implications for the development of the Syrian Civil War.In seeking to develop an analysis of the interplay of internal dynamics and external influences,i seek to‘reconcile’the‘micro’and‘macro’dimensions of civil war,and also draw on contributions to the peacebuilding literature,and this enables me to reconceptualise the relationship between‘internal’and‘external’drivers of conflict.

Improving Association Rules Accuracy in Noisy Domains Using Instance Reduction Techniques

Association rules’learning is a machine learning method used in finding underlying associations in large datasets.Whether intentionally or unintentionally present,noise in training instances causes overfitting while building the classifier and negatively impacts classification accuracy.This paper uses instance reduction techniques for the datasets before mining the association rules and building the classifier.Instance reduction techniques were originally developed to reduce memory requirements in instance-based learning.This paper utilizes them to remove noise from the dataset before training the association rules classifier.Extensive experiments were conducted to assess the accuracy of association rules with different instance reduction techniques,namely:DecrementalReduction Optimization Procedure(DROP)3,DROP5,ALL K-Nearest Neighbors(ALLKNN),Edited Nearest Neighbor(ENN),and Repeated Edited Nearest Neighbor(RENN)in different noise ratios.Experiments show that instance reduction techniques substantially improved the average classification accuracy on three different noise levels:0%,5%,and 10%.The RENN algorithm achieved the highest levels of accuracy with a significant improvement on seven out of eight used datasets from the University of California Irvine(UCI)machine learning repository.The improvements were more apparent in the 5%and the 10%noise cases.When RENN was applied,the average classification accuracy for the eight datasets in the zero-noise test enhanced from 70.47%to 76.65%compared to the original test.The average accuracy was improved from 66.08%to 77.47%for the 5%-noise case and from 59.89%to 77.59%in the 10%-noise case.Higher confidence was also reported in building the association rules when RENN was used.The above results indicate that RENN is a good solution in removing noise and avoiding overfitting during the construction of the association rules classifier,especially in noisy domains.
