Bilateral Retractile Testis: A Possible Risk Factor for Male Infertility

Healthy youth is an asset and strength for economic and social growth of any society. Developmental changes during puberty of youngsters need proper guidance to avoid future physical, physiological and psychological ailments. Bilateral Retractile Testis (BRT) is a health problem if remains unnoticed till adolescence can lead to tribulations. BRT may affect testicular parenchyma and spermatogenesis. Cremasteric muscles (muscles attached with testes) pull the testes up into groin so it randomly moves up and down and do not reside in its natural pouch always. It has been a controversial issue in male infertility as retraction may affect infertility. Objective: To estimate the prevalence of male infertility among patients with Bilateral Retractile Testis. Methodology: A total of 200 male patients were enrolled after the power calculation, with different reproductive health problems between the age of 18 - 40 years during the year 2016 at Reproductive health clinic Jinnah Post Graduate Medical Center (JPMC), Karachi. 25 male patients were reported with infertility and BRT. The data was collected using self-developed questionnaire including outdoor patients’ physical check-up, manual testicular examination as size and shape were done. Development of a Bias Compensating Q-Learning Controller for a Multi-Zone HVAC Facility

We present the development of a bias compensating reinforcement learning(RL)algorithm that optimizes thermal comfort(by minimizing tracking error)and control utilization(by penalizing setpoint deviations)in a multi-zone heating,ventilation,and air-conditioning(HVAC)lab facility subject to unmeasurable disturbances and unknown *** is shown that the presence of unmeasurable disturbance results in an inconsistent learning equation in traditional RL controllers leading to parameter estimation bias(even with integral action support),and in the extreme case,the divergence of the learning *** demonstrate this issue by applying the popular Q-learning algorithm to linear quadratic regulation(LQR)of a multi-zone HVAC environment and showing that,even with integral support,the algorithm exhibits bias issue during the learning phase when the HVAC disturbance is unmeasurable due to unknown heat gains,occupancy variations,light sources,and outside weather *** address this difficulty,we present a bias compensating learning equation that learns a lumped bias term as a result of disturbances(and possibly other sources)in conjunction with the optimal control *** results show that the proposed scheme not only recovers the bias-free optimal control parameters but it does so without explicitly learning the dynamic model or estimating the disturbances,demonstrating the effectiveness of the algorithm in addressing the above challenges.

Autoimmune Hepatitis Induced after Treatment of Syphilitic Hepatitis

We present a unique case of biopsy-proven syphilitic hepa-titis which presented as severe acute liver injury with sig-nificant elevation in aminotransferases and bilirubin,and improved with antibiotic ***,the patient re-turned weeks after initial presentation with new-onset acute liver injury and had developed hypergammaglobulinemia,positive autoantibody titers,and repeat liver biopsy dem-onstrating interface hepatitis,supporting a diagnosis of autoimmune *** had an otherwise unrevealing etiologic workup,and responded to glucocorticoid *** believe that syphilitic hepatitis and its treatment sub-sequently triggered an immunogenic response,leading to autoimmune *** hepatitis is a chronic liver disease thought to manifest as a result of predisposing genetic factors in combination with environmental insults,especially hepatotropic *** is a sexually transmitted disease caused by Treponema pallidum that has been associated with autoimmunity and the develop-ment of *** propose that in the setting of syphilitic hepatitis,a molecular mimicry event resulting from structural similarities between *** and liver antigens,as well as impaired regulatory T-cell function,led to the breakdown of immune tolerance and the onset of au-toimmune *** support this hypothesis,further mo-lecular analyses and case series are necessary to determine if syphilitic hepatitis and its treatment are risk factors for the onset of autoimmune *** hepatitis should be considered early as the cause of acute liver injury in susceptible patients with risk factors for the disease,as prompt recognition and appropriate treatment may prevent progression of liver injury and result in improved outcomes.

In an Underwater Wireless Sensor Network(UWSN),extreme energy loss is carried out by the early expiration of sensor nodes and causes a reduction in efficiency in the submerged acoustic sensor *** based on clustering strategies,instead of each node sending information by itself,utilize cluster heads to collect information inside the clusters for forwarding collective information to *** can effectively minimize the total energy loss during *** environment of UWSN is 3D architecture-based and follows a complex hierarchical clustering strategy involving its most effecting unique parameters such as propagation delay and limited transmission *** base clustering strategy works in rounds,where each round comprises three fundamental stages:cluster head selection,grouping or node association,and data aggregation followed by forwarding data to the *** UWSN,the energy consumed during the formation of clusters has been considered casually or completely evaded in the previous *** this paper,the cluster head setup period has been considered the main contributor to extra energy utilizer.A numerical channel model is proposed to compute extra *** is performed by using a UWSN broad *** results have shown that extra maximum energy consumption is approximately 12.9 percent of the system total energy consumed in information transmissions.

The prevalence of both type 2 diabetes and metabolic syndrome is increasing exponentially all over the world because of global changes in obesity, sedentary lifestyle, and the aging of the population.1 The metabolic syndrome consists of a constellation of risk factors including obesity, glucose intolerance, hypertension, and dyslipidemia. Both metabolic syndrome and type 2 diabetes occur in the setting of insulin resistance and confer an increased risk of atherosclerosis and cardiovascular disease （CVD）. The possible role of inflanunatory cytokines and atherogenic markers in this process is still being elucidated.
