Pancreatic regenerating protein (regⅠ) and regⅠreceptor mRNA are upregulated in rat pancreas after induction of acute pancreatitis

AIM： Pancreatic regenerating protein （reg Ⅰ ） stimulates pancreatic regeneration after pancreatectomy and is mitogenic to ductal and 13-cells. This suggests that reg Ⅰand its receptor may play a role in recovery after pancreatic injury. We hypothesized that reg Ⅰ and its receptor are induced in acute pancreatitis. METHODS： Acute pancreatitis was induced in male Wistar rats by retrograde injection of 3% sodium taurocholate into the pancreatic duct. Pancreata and serum were collected 12, 24, and 36 hours after injection and from normal controls （4 rats/group）. Reg Ⅰ receptor mRNA, serum reg Ⅰ protein, and tissue reg Ⅰ protein levels were determined by Northern analysis, enzymelinked immunosorbent assay （ELISA）, and Western analysis, respectively. Immunohistochemistry was used to localize changes in reg Ⅰ and its receptor. RESULTS： Serum amylase levels and histology confirmed necrotizing pancreatitis in taurocholate treated rats. There was no statistically significant change in serum reg Ⅰ concentrations from controls. However, Western blot demonstrated increased tissue levels of reg Ⅰ at 24 and 36 h. This increase was localized primarily to the acinar cells and the ductal cells by immunohistochemistry. Northern blot demonstrated a significant increase in reg Ⅰ receptor mRNA expression with pancreatitis. Immunohistochemistry localized this increase to the ductal cells, islets, and acinar cells. CONCLUSION： Acute pancreatitis results in increased tissue reg Ⅰ protein levels localized to the acinar and ductal cells, and a parallel threefold induction of reg Ⅰ receptor in the ductal cells, islets, and acinar cells. These changes suggest that induction of reg Ⅰand its receptor may be important for recovery from acute pancreatitis.

Double segmental tibial fractures-- an unusual fracture pattern

A case of a 50-year-old pedestrian who was hit by a bike and suffered fractures of both bones of his right leg was presented. Complete clinical and radiographic assessment showed double segmental fractures of the tibia and multisegmental fractures of the fibula. Review of the literature revealed that this fracture pattern was unique and only a single case was reported so far. Moreover, we discussed the possible mechanisms which can lead to such an injury. We also discussed the management of segmental tibial fracture and the difficulties encountered with them. This case was managed by modern osteosynthesis technique with a pleasing outcome.

Epidemiology of pelvic fractures in adults: Our experience at a tertiary hospital

Purpose: Pelvic fractures are severe injuries and are often associated with multiple system injuries, exacerbating the overall outcome. In India, the incidence of pelvic fractures is on a rise due to suboptimal roads and traffics but related literature regarding the overall epidemiology of these injuries is scarce and scanty. Our aim was to study the epidemiology of patients admitted with pelvic fractures at a level 1 trauma centre in India. Methods: A 16-month (between September 2015 and December 2016) prospective observational study was carried out on trauma patients with pelvic fractures at a level 1 trauma centre of a tertiary care hospital. Demography of patients, mechanism of injuries and complications were recorded prospectively. Results: We observed 75 patients who presented with pelvic fractures, where 56 were males and 19 were females. Mean age of the study populationwas 37.57 years. Road traffic accidents were the most common mode of injuries. Lateral compression injuries were the most common pattern. Associated injuries frequently encountered were lower extremities and acetabulum fractures, blunt abdominal trauma, urogenital injuries and head injuries. Out of the 75 patients, 52 were treated surgically and 23 were managed by conservative methods. Associated injuries of the extremities, head, abdomen and urogenital system indicated a longer hospital stay. Conclusion: Pelvic fractures, although belong to a relatively rare trauma subset, cause a high morbidity and mortality with considerable burden on the economy. Proper road safety training and driving etiquettes along with its strict implementation in true sense and spirit are the need of the hour.

Malakoplakia: Rare Disorder with No Significant Racial Prevalence

Introduction: Malakoplakia is a rare acquired granulomatous inflammatory condition. Microscopically it is characterized by sheets of histiocytes with abundant eosinophilic granular cytoplasm (known as von Hansemann cells) with intracytoplasmic rounded concentric basophilic inclusions (Michelis-Guttmann bodies). We reviewed all cases of malakoplakia diagnosed at our institution in the past 15 years, including clinical, macroscopic, and microscopic features as well as treatment and prognosis. Methods: Gross and microscopic features of cases diagnosed as malakoplakia from 2005 to 2019 at our institution were reviewed. Clinical history, including sites of involvement, clinical presentation, imaging, past medical history, treatment, follow-up and prognosis, as well as demographic characteristics were obtained. Results: Eighteen surgical pathology cases of malakoplakia were identified. Most cases occurred in female (14/18, 77.8%) and all in adults (median age 48.5 years;range 31 - 87). Half of the patients were Caucasian, 44% African American and 6% were reported as “other” race. One third (6/18) of patients presented with a mass-like lesion on imaging or endoscopic studies. As to site of lesion, 61.1% (11/18) involved GU tract, 27.8% (5/18) gastrointestinal (GI) tract, 5.6% (1/18) liver, and 5.6% (1/18) vagina. 38.9% of patients (7/18) had antibiotics treatment upon diagnosis. Conclusion: Malakoplakia is a rare disorder occurring in a wide age range, more commonly in females, with no significant racial prevalence. While the GU and GI tracts are the most common sites, other anatomic locations can be involved. Accurate diagnosis and appropriate treatment are important to avoid recurrence.

Total hip arthroplasty for posttraumatic osteoarthritis following acetabular fracture： A systematic review of characteristics, outcomes, and complications

Purpose： Posttraumatic arthritis （PTA） may develop years after acetabular fracture, hindering joint function and causing significant chronic musculoskeletal pain. Given the delayed onset of PTA, few studies have assessed outcomes of delayed total hip arthroplasty （THA） in acetabular fracture patients. This study systematically reviewed the literature for outcomes of THA in patients with PTA and prior acetabular fracture. Methods： Pubmed, EMBASE, SCOPUS, and Cochrane library were searched for articles containing the keywords ＂acetabular＂, ＂fracture＂, ＂arthroplasty＂, and ＂post traumatic arthritis＂ published between 1995 and August 2017. Studies with less than 10 patients, less than 2 years of follow-up, conference abstracts, and non-English language articles were excluded. Data on patient demographics, surgical characteristics, and outcomes of delayed THA, including implant survival, complications, need for revision, and functional scores, was collected from eligible studies. Results： With 1830 studies were screened and data from 10 studies with 448 patients were included in this review, The median patient age on date of THA was 51.5 years, ranging from 19 to 90 years. The median time from fracture to THA was 37 months, with a range of 27-74 months. Mean follow-up times ranged from 4 to 20 years, The mean Harris hip scores （HHS） improved from 41.5 pre-operatively, to 87.6 post-operatively, The most prevalent postoperative complications were heterotopic ossification （28% -63%）, implant loosening （1%-24%）, and infection （0%-16%）. The minimum 5-year survival of implants ranged from 70% to 100%. Revision rates ranged from 2% to 32%. Conclusion： Despite the difficulties associated with performing THA in patients with PTA from previous acetabular fracture （including soft tissue scarring, existing hardware, and acetabular bone loss） and the relatively high complication rates, THA in patients with VIA following prior acetabular fracture leads to significant improvement in pai
