Risk Factors and Complications in 947 Tuberculosis Patients Hospitalized in the Pulmonology Department of Fann Chnu from 2017 to 2019

Introduction: Despite current progress, tuberculosis remains a major public health problem, given its still high incidence, prevalence, and mortality, particularly in sub-Saharan African countries, including Senegal. This risk is higher for immunocompromised people. Complications and comorbidities can also affect the course of the disease, affecting the prognosis. It is in this context that this study was undertaken with the objective of determining the risk factors and complications in patients hospitalized for tuberculosis. Materials and Methods: This was a retrospective and descriptive study carried out in 2021, from records of patients hospitalized for tuberculosis from January 1, 2017, to December 31, 2019, at the Pulmonology Department of Fann. Inclusion criteria were all patients on TB treatment after diagnosis of tuberculosis has been confirmed bacteriologically or clinically according to the World Health Organization’s TB case definition. Multidrug-resistant TB was excluded. Results: Out of 4516 hospitalized patients, 20.96% of patients were tuberculosis patients. The sex ratio was 2.18. 4/5 of the patients were between 18 and 39 years old. The main contributing factors of TB found were undernutrition (93.13%), active smoking (36.75%) and diabetes (35.97%). The time between hospitalization and onset of symptoms was greater than 2 months in 60.53% of cases. A complication was noted in 89.10% of patients, particularly bacterial/viral bronchopulmonary co-infection (31.15%). The trend was favorable in 88.49% of cases. It resulted in death in 10.98% of cases. Conclusion: Most integrated-care nutritional support programs focus on HIV. Undernutrition appears to play a much more important role than HIV in the extent of TB in poor countries. Surgical advances in the treatment of neuromuscular scoliosis

Neuromuscular disorders are a group of diseases affecting the neuro-musculo-skeletal system. Children with neuromuscular disorders frequently develop progressive spinal deformities with cardio-respiratory compromise in the most severe cases. The incidence of neuromuscular scoliosis is variable, inversely correlated with ambulatory abilities and with a reported risk ranging from 80% to 100% in non-ambulatory patients. As surgical and peri-operative techniques have improved, more severely affected children with complex neuromuscular deformities and considerable co-morbidities are now believed to be candidates for extensive surgery for spinal deformity. This article aimed to provide a comprehensive review of how neuromuscular spinal deformities can affect normal spine balance and how these deformities can be treated with segmental instrumentation and sub-laminar devices. Older concepts have been integrated with newer scientific data to provide the reader with a basis for better understanding of how treatment of neuromuscular scoliosis has evolvedover the past few decades. Recent advances, as well as challenges that remain to be overcome, in the surgical treatment of neuromuscular curves with sub-laminar devices and in the management of post-operative infections are outlined.

Left Atrium Emboligenic Myxoma: Case Report and Review of the Literature

Introduction: Cardiac myxomas represent the most frequent forms of primary tumors of the heart. The most frequent location is the interatrial septum. We report the clinical case of a myxoma of the left atrium and discuss its epidemiological and therapeutic aspects through a review of the literature. Observation: This was a 41-year-old female patient who presented with sudden rotational dizziness associated with vomiting. MRI revealed multiple punctiform bilateral supra and subtentorial strokes of different ages, recent and semi-recent, suggesting an embologenic etiology. Transesophageal echocardiography found a large pedunculated, homogeneous, avascular tumor hanging from the interatrial septum. The patient is operated on urgently under cardiopulmonary bypass for resection of a large tumor located in the left atrium. The pathological examination concluded with the diagnosis of myxoma of the left atrium. The postoperative follow-up was straightforward and the patient was discharged from the hospital via home hospitalization. Conclusion: The diagnosis of cardiac myxomas is suspected in the presence of symptoms associated with echocardiographic images of intracardiac masses and confirmed by histological study. Embolic accidents are one of the formidable complications of myxomas. Surgical management is urgent, especially in the presence of predictive morphological features of embolism on echocardiography.

Pelvic Multifocal Primitive Bone Epithelioid Angiosarcoma (Iliac Crest and Hip) about One Case and Literature Review

Epithelioid angiosarcoma is a rare vascular tumour with dark prognosis at short term. The primitive bone localization is exceptional and sometimes presents itself under a unifocal form. We report a clinical case of pelvic multifocal primitive bone epithelioid angiosarcoma (iliac crest and hip) diagnosed on a young male adult (45 years old). The radiological investigations put in evidence geographic osteolytic lesions (type IC) and, diagnostic was given thanks to the histological examination of the surgical biopsy pieces. The evolution was marked by the appearance of pulmonary and lymph node metastases with a death occurring in less than 4 months. Extremely rare tumour, bone epithelioid angiosarcoma is a variant of angiosarcoma characterised by an aggressive nature whose evolution is always fatal. Through this observation we report our experience on the diagnostic approach;and using a literature review we discuss the therapeutic modalities of this tumour with very bad prognosis.

A New Observation of Bone Marrow Involvement by Diffuse Large B-Cell Lymphoma Mimicking Myelofibrosis

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma, characterized by high clinical and biological heterogeneity. Patients typically present with progressive lymphadenopathy, extranodal disease and may also experience fever, night sweats and unexplained weight loss. We report here the case of a 16-year-old female with osteoarticular pain, dizziness, and dyspnea on exertion. Clinical examination showed no lymphadenopathy. Complete blood count (CBC) revealed pancytopenia and marrow smears found to be hypocellular. Initial diagnosis favored secondary myelofibrosis. Diagnosis of bone marrow involvement by DLBCL was retained on bone marrow histology and immunohistochemistry which showed infiltration of large B lymphoid cells. The patient was treated by immunochemotherapy R-CHOP regimen. This case highlights a very rare and atypical circumstance of discovery of DLBCL with myelofibrosis as an initial symptom. Prognosis value of this presentation and management difficulties are also discussed.

Analysis of the Relationship between Oral Diseases and Glycemic Control of Diabetes in the West African Context: Survey at the Centre Anti-Diabetique d’Abidjan (CADA),cote d’Ivoire

The relationship between diabetes and periodontitis is established and described as a bidirectional influence, whereas that with dental caries is still controversial. The objective of this study was to analyze the relationship between blood glucose control and oral diseases in a population of diabetics followed at the “Centre Anti Diabétique d’Abidjan” (CADA). The dental condition of the diabetics included in this cross-sectional study was assessed by the DMF index, periodontal condition by the CPITN and oral hygiene by the OHIS index. Diabetes data were extracted from CADA patient medical records. Blood glucose control has been defined by an average of the blood glucose values of the last 3 months less than or equal to 1.26 g/l. Regression models were constructed to estimate the risk of diabetes imbalance adjusted for all characteristics collected. The sample of 356 diabetics consisted of 53.7% women with an average age of 55.6 years (range: 12 - 85 years). The average DMF index was 7.9 (Std: 6.6) with an estimated prevalence of caries of 77.0%, that of periodontitis of 53.1%. The proportion of subjects with good glycemic control was estimated at 41.3%. The characteristics associated with glycemic control were: dry mouth (p = 0.005), type of diabetes treatment (p < 0.00014) and duration of diabetes (p = 0.039) and periodontal status assessed with the CPITN (p = 0.014). The results of this study confirmed the link between periodontitis and glycemic control of diabetes described in the literature but didn’t find significant association between dental caries and diabetes.

Sickle Cell Patient and Addiction to Tramadol: Case Management in Senegal

Introduction: Tramadol* is a synthetic opioid agonist used as an analgesic. Despite its minor potential of addiction, it created recently a lot of concern. Its misuse may be motivated by a need to control the pain or by looking for her euphoric character. The aim of our study was to proceed a psychopathological reading of a case of addiction to Tramadol in a sickle cell patient with histrionic personality traits. Methods: We report the case’s management of a patient with addiction to Tramadol in a sickle cell patient with histrionic personality traits. The diagnosis was based on the DSM IV criteria. The review was done in the model of semi-structured interviews. Observation: Miss DP is 23 years old, sickle cell patient (AS profile). It is addressed to us for the management of an excessive consumption of Tramadol* started 4 years ago. Because of the difficulties of care in outpatient, we conducted hospitalization. The interviews highlight the histrionic personality traits and disruption of family dynamics. Discussion: Painful chronic diseases require a long analgesic treatment. It is a factor of dependence specially when the patient has a personality disorder. Multidisciplinary treatment between psychiatrist and addiction specialist and medical doctor is a guarantee of success.

Radiotherapy of Oligoprogressive Lesions in Castration-Resistant Prostate Cancer: Impact on Second-Generation Hormone Therapy

Background: The therapeutic standard for oligoprogressive prostate cancer resistant to castration is second-generation hormone therapy. This systemic treatment is expensive. There are oligoprogressive lesions accessible to radiotherapy. Objectives: To study the impact of radiotherapy of oligoprogressive lesions on the implementation of second generation hormone therapy. Patients and Methods: A retrospective study from 2012 to 2020 was carried out. All patients with oligoprogressive prostate cancer who had received radiotherapy on one or more lesions in progression were collated. Survival was calculated using the Kaplan-Meier method. Results: 8 patients were treated with stereotactic and conformational radiotherapy between August 2012 and August 2020 in the context of oligoprogressive prostate cancer resistant to castration. The median age at diagnosis of oligoprogression was 73 years with a median PSA level of 3.11 ng/ml. Nine lesions were diagnosed with PET scan PSMA. All the lesions were treated by radiotherapy with different regimens. After a median follow-up of 12.5 months, 7 patients showed a biochemical response to treatment with a median decrease in PSA of 67%. The median survival without clinical or biochemical progression was 7 months. The median survival without the need for further systemic treatment was 9 months. Inveterate Posterior Shoulder Dislocation Treated by Sub-Scapular Transfer, about One Case

Glena-humeral dislocation is rare. More than the half of those dislocations is unremarked because the diagnostic is not an emergency. A delayed diagnostic alters the functional prognostic and sometimes forces to do a complex surgery with complications whose success rate remains uncertain. We report a case of inveterate posterior shoulder dislocation on a 42-year-old adult occurred during a sport accident and treated by sub-scapulars transfer (modified Mac Laughlin’s technical). The aim of this article is to highlight this pathology to ameliorate its diagnostic rate indeed prognostic. Our diagnostic approach and injury, noticed during surgery and therapeutic propositions, are described at the light of literature data on this subject.

Introduction: The assessment of health care needs must be reproducible, independent of the operator, hence the use of indexes like the Index of Orthodontic Treatment Need (IOTN). The aim of this study was to assess both, the prevalence of malocclusions among patients received in the Dental Surgery Unitat Yalgado Ouédraogo Teaching Hospital and their care needs according to IOTN index. Materials and Methods: It is a prospective, cross-cutting diagnostic study of malocclusions at Yalgado Ouédraogo Teaching Hospital (CHUYO), conducted from January 1 to December 31, 2016. The examiner with an examination tray records on the 12- to 16-year-old patients’ medical recorded, the variables based on the polychromatic pictures series, the IOTN rule, and the dental health component table. Results: On the whole, 81 patients were included, with 60.5% of girls and 39.5% of boys. 16- and 12-year-old patients were dominant. The average age was 14.05 years old. Various types of malocclusions were encountered. The need for orthodontic care was 56.8%. For 14 patients, 17.28%, their aesthetic component score is higher than the assessment made by the orthodontist. Conclusion: Aesthetics was an important factor in the need for orthodontic care. The aesthetic deficit entails a psychological impact that should be taken into account in the assessment of care needs.
