Multidrug Resistant <i>Shigella</i>Associated with Class 1 Integrase and Other Virulence Genes as a Cause of Diarrhea in Pediatric Patients

Background: Shigella is one of the most serious pathogens associated with bloody diarrhea in children. The empiric antibiotic therapy of enteric illness with blood streaked stool leads to emergence of multi drug resistant (MDR) Shigella. The condition gets exacerbated by presence of integrons that facilitate the horizontal spread. Virulence genes associated with MDR Shigella modulate the patient outcome, particularly in children. Objectives: The present study was aiming at isolation of MDR Shigella from children with diarrheal sickness and characterization of those isolates as regarding presence of class 1 integrase and other virulence genes. Methods: Four hundred and ninety patients under the age of five suffering from diarrheal illness were examined for presence of Shigella in their stool specimens. MDR Shigella was determined using the antibiotic susceptibility testing by disc diffusion method;those isolates were tested for presence of class 1 integrase by PCR. Multiplex PCR assay was used to determine the presence of virulence genes, virA, ial, sen, set1A, set1B, sat, ipaBCD, ipaH and stx in the MDR Shigella isolates. Results: The isolation rate of Shigella from pediatric patients was 5.3%. Most of the isolated Shigella (57.7%) were from infants between 12 and 23 month. 73.1% of the identified Shigella were MDR. intI1 gene was present in 78.9% of MDR isolates. Muliplex PCR revealed that ipaH and ipaBCD, virA, sat, ial, set1A and set1B, sen were detected in 94.7%, 78.9%, 73.7%, 68.4%, 42.1%, 36.8% of the MDR Shigella isolates respectively. Conclusion: The MDR isolates represented a considerable percentage of Shigella detected in pediatric patients. Presence of intI1 gene in most of MDR Shigella reflects the higher possibility of resistant strains spread. Existence of a variety of virulence genes in those isolates is an important indicator of serious disease outcome.

Urine YKL-40 versus Urine NGAL as Potential Markers for Diagnosis of Urinary Tract Infection in Febrile Pediatric Patients

Early diagnosis of urinary tract infection (UTI) in pediatric patients is a prob-lem due to the absence of specific symptoms and difficulty in obtaining the proper urine sample. These difficulties increase the need for effective biomarker for UTI diagnosis in young patients. Neutrophil gelatinase-associated lipocalin (NGAL) is proved to be a marker for UTI diagnosis. YKL-40 is a glycosyl hydrolase which is produced locally at the sites of inflammation. The objective of our study is to assess the value of YKL-40 as a possible marker for UTI diagnosis in febrile children and to compare its value versus NGAL. Urine culture was used as a gold standard for UTI diagnosis. The study enrolled three groups;febrile children with positive urine culture, febrile children with negative urine culture, and controls without fever and with negative urine culture. Each group included 50 children from patients attended outpatients clinics department of Mansoura University Children Hospital. For each patient enrolled in the study, complete blood count, C-reactive protein, serum creatinine, urine creatinine, routine urine analysis and urine culture were assessed. Using elISA test, urine values of NGAL (uNGAL) and YKL-40 (uYKL-40) were measured and normalized to urine creatinine (uNGAL/uCr) and (uYKL40/uCr) respectively. The values of uNGAL, uYKL-40, uNGAL/Cr and uYKL-40/Cr were significantly higher in febrile UTI group. The receiver operating curve (ROC) show the optimum cut off value for urine YKL-40 (171.5 pg) with 84% senstivity, 82% specificity and the area under the ROC curve (AUC) 0.95. The optimum cutoff value of uYKL-40/Cr was (159.2 pg/mg) with 72% sensitivity, 71% specificity and 0.81 AUC. Higher sensitivity and specificity of uYKL-40 and uYKL40/Cr compared to uNGAL and uNGAL/Cr respectively were observed. In conclusion, the diagnostic value of uYKL-40 is superior to that of uNGAL. Urine YKL-40 could be a good marker for diagnosis of UTI in febrile pediatric patients.

MBL2 Gene Polymorphism and the Association with Neonatal Sepsis in Egyptian Neonates, a Case Control Study

Mannose binding lectin (MBL) is an important component of innate immunity particularly in neonates whose adaptive immunity is not fully developed. Polymorphism in MBL2 gene promoter and exon1 determines MBL serum level and function. The aim of this study was to investigate the frequency of different MBL2 genotypes in neonatal sepsis among patients of neonatal intensive care unit (NICU). Two hundred and forty-five neonates were enrolled in this study (127 infected and 118 uninfected controls). Multiplex PCR and double amplification refractory mutation system (dARMS) were used for typing of MBL2 exon1 and promoter respectively. Klebsiella species were the most frequently isolated organisms (22.8%). There is no statistical significance difference in the distribution of different expression genotypes between infected group and controls (P = 0.11). However, prevalence of low MBL2 expression genotypes (XA/O and O/O) was higher in infected patients compared to control group (patients 25.2% and controls 15.3%). Low and medium MBL2 expression genotypes were mostly associated with Gram-negative bacterial infections (18.9% and 22.8%) respectively. A statistically significant association of Gram-negative bacterial infections with low MBL2 expression genotypes was found (P = 0.02). Higher frequency of AB and BB genotypes was observed (31.5% and 7.9%) in patients group compared to control, but without statistical significant difference.

Charcoal Nanoparticles as a Delivery System for Doxorubicin and Sorafenib in Treatment of Hepatocellular Carcinoma

Background: Hepatocellular carcinoma (HCC) is the most common type of liver cancer and one of the leading causes of cancer-related death worldwide. Advanced HCC displays strong resistance to chemotherapy, and traditional chemotherapy drugs do not achieve satisfactory therapeutic efficacy. The delivery of therapeutic compounds to the target site is a major challenge in the treatment of many diseases. Objective: This study aims to evaluate activated charcoal nanoparticles as a drug delivery system for anticancer agents (Sorafenib and Doxorubicin) in Hepatocellular Cancer Stem Cells. Method: The percent efficiency of entrapment (% EE) of the doxorubicin and sorafenib entrapped onto the activated charcoal was obtained by determining the free doxorubicin and sorafenib concentration in the supernatant-prepared solutions. Then the characterizations of nanoparticles were formed by determination of the particle size distribution, zeta potential, and polydispersity index (PDI). The anticancer activity of activated Charcoal, Doxorubicin-ACNP, sorafenib-ACNP, free doxorubicin, and free sorafenib solutions was measured based on cell viability percentage in HepG2 cell lines (ATCC-CCL 75). In vitro RBC’s toxicity of Doxorubicin/sorafenib loaded charcoal was estimated by hemolysis percentage. Results: The synthesized Doxorubicin-ACNP and Sorafenib-ACNP were evaluated and their physiochemical properties were also examined. Essentially, the percent Efficiency of Entrapment (EE %) was found to be 87.5% and 82.66% for Doxorubicin-ACNP and Sorafenib-ACNP, respectively. The loading capacity was 34.78% and 24.31% for Doxorubicin-ACNP and Sorafenib-ACNP. Using the Dynamic Light scattering [DLS] for the determination of the hydrodynamic size and surface zeta potential, a narrow sample size distribution was obtained of (18, 68, and 190 nm for charcoal, 105, 255, and 712 nm for doxorubicin, and 91, 295, and 955 nm for sorafenib), respectively. In recent years, Digital Twin (DT) has gained significant interestfrom academia and industry due to the advanced in information technology,communication systems, Artificial Intelligence (AI), Cloud Computing (CC),and Industrial Internet of Things (IIoT). The main concept of the DT isto provide a comprehensive tangible, and operational explanation of anyelement, asset, or system. However, it is an extremely dynamic taxonomydeveloping in complexity during the life cycle that produces a massive amountof engendered data and information. Likewise, with the development of AI,digital twins can be redefined and could be a crucial approach to aid theInternet of Things (IoT)-based DT applications for transferring the data andvalue onto the Internet with better decision-making. Therefore, this paperintroduces an efficient DT-based fault diagnosis model based on machinelearning (ML) tools. In this framework, the DT model of the machine isconstructed by creating the simulation model. In the proposed framework,the Genetic algorithm (GA) is used for the optimization task to improvethe classification accuracy. Furthermore, we evaluate the proposed faultdiagnosis framework using performance metrics such as precision, accuracy,F-measure, and recall. The proposed framework is comprehensively examinedusing the triplex pump fault diagnosis. The experimental results demonstratedthat the hybrid GA-ML method gives outstanding results compared to MLmethods like LogisticRegression (LR), Na飗e Bayes (NB), and SupportVectorMachine (SVM). The suggested framework achieves the highest accuracyof 95% for the employed hybrid GA-SVM. The proposed framework willeffectively help industrial operators make an appropriate decision concerningthe fault analysis for IIoT applications in the context of Industry 4.0.

Sodium-glucose cotransporter-2 inhibitors (SGLT-2 inhibitors) have transformed diabetes management by targeting renal glucose reabsorption. Designed initially as antidiabetic agents, their ability to lower blood glucose levels independently of insulin is well-documented. Beyond glycemic control, emerging research has unveiled their profound cardiorenal benefits. By inhibiting SGLT-2 protein, these drugs enhance glucose excretion in urine, reducing blood glucose levels. This mechanism has translated into significant cardiovascular and renal protection, establishing SGLT-2 inhibitors as pivotal in managing not only diabetes but also cardiovascular and renal diseases. Recent studies have illuminated the broader therapeutic potential of SGLT-2 inhibitors beyond diabetes. Evidence indicates their efficacy in managing heart failure, chronic kidney disease (CKD), and cardiovascular complications in individuals with or without diabetes. This expanded therapeutic landscape has catalyzed a paradigm shift in SGLT-2 inhibitor use, positioning them as key agents in the cardiorenal metabolic continuum. Moreover, their role in the secondary prevention of cardiovascular events and slowing CKD progression in T2DM patients has garnered considerable attention. This consensus-based review aims to offer practical guidance in an algorithmic approach to primary care healthcare professionals to optimize SGLT-2 inhibitors utilization and maximize their benefits. The review seeks to empower clinicians to effectively manage patients who may benefit from SGLT-2 inhibitor therapy by addressing common initiation barriers and optimizing treatment strategies. Additionally, it aims to raise awareness among primary care physicians regarding the multifaceted benefits of these medications and overcome clinical inertia in their adoption into routine clinical practice.
