Evaluation of Appropriateness of Proton Pump Inhibitors (PPIs) Use in the Region of Sharjah, United Arab Emirates (UAE)

Background: Proton pump inhibitors (PPIs) are drugs that reduce the production of acid in the stomach. Recently, the use of PPI has been increasing among communities, whether with or without prescription. As a part of the healthcare team, the pharmacist should not only dispense medications but also ensure the appropriate use of these medications. Studies conducted within 16 countries showed substantial variation in the appropriateness of the use of PPI drugs. Aim: To evaluate the appropriateness of PPIs use in Sharjah, UAE based on surveys answered by pharmacists, physicians, and patients. Methods: A cross-sectional survey study was conducted on December 2017 at Sharjah, UAE as an example of information obtained from the Middle East. Two different surveys were conducted on physicians and pharmacists. Both Physicians and Pharmacists were chosen randomly from Yellow Pages. The results obtained from both studies were used to develop a patient’s survey, which was distributed among University of Sharjah students, their families and random people at Shopping Malls and Clinics. Results: The results obtained from the patients’ survey showed that ~39% of PPI users from the region of Sharjah are 25 - 44 years old. Approximately 79% are using PPI according to physicians’ prescriptions. Prescriptions’ duration is varied between 1 month (39%) and 6 months (22%), where 52% of PPI users ask their physicians to prescribe PPIs when needed. Suggested reasons for the use of PPI included inappropriate food habits (52%), use of other medications (16%) or bacterial infection (13%). Around 52% of the patients did not receive any recommendations regarding the deprescribing of PPIs. According to the pharmacists’ surveys, an average sale of 5 - 10 PPI packages is reported per day, and around 50% are sold without a prescription. Accessory or ectopic breast tissue is an anomaly in the development of the breast. It is a rare condition that occurs along the embryological mammary line. In less than 1% of all breast cancers, supernumerary breast cancer is reported, with the axillary location being the most common in 60% to 90% of cases. Cancerous degeneration of this supernumerary breast tissue can pose a dual diagnostic and therapeutic problem. We report the case of locally advanced adenocarcinoma in a right supernumerary breast. This is a 75-year-old, grand-multiparous, postmenopausal, and known hypertensive patient on treatment. Family history was remarkable for brain cancer in her sister and oesophageal cancer in her mother. She consulted for a mass in the right axillary cavity on supernumerary breast evolving for a year. Clinical examination revealed a large, fixed, budding and haemorrhagic-ulcerated mass of the right axilla, with long axis measuring about 15 cm. There was as wella supernumerary breast on the left, but without particularity. A soft tissue ultrasound showed a large hypoechoicmass in the right axillary region of 116 mm with areas of central necrobiosis. Morphologically, the breasts were normal. A breast MRI revealed a subcutaneous mass in the right axillary cavity with skin ulceration and satellite lymphadenopathy. The extension assessment revealed liver metastases, and a biopsy of the mass revealed a breast adenocarcinoma. The case was the subject of a multidisciplinary consultation meeting following which a wide excision of the mass was indicated. The histo-pathology analysis results of the surgical specimen were in favour of a triple negative papillary adenocarcinoma. After a post-operative multidisciplinary consultation meeting, adjuvant chemotherapy was indicated. The development of supernumerary breasts depends on hormones, just like normal breasts. Background: Since 2021, high-risk Human Papilloma Virus (HR-HPV) testing has been the recommended screening test for cervical cancer for all settings;either used alone in a “test and treat” strategy, or with a triage test, with or without biopsy, before treatment. Cameroon has rolled out immunization against HPV 16 and 18, but studies show a higher prevalence of non-16/18 HR-HPV types. Objectives: Determine the prevalence of precancerous lesions, in women with HR-HPV infection and evaluate association of digital cervicography (DC) VIA/VILI positivity with HPV serotype, as a measure of their contribution to precancer and cancer incidence. Methodology: The study was cross-sectional, descriptive, and analytic. It took place at the Etoug-Ebe and Ekoudoum Baptist Hospitals in Yaoundé, during the period April-September 2022. We reviewed the records of women screened for cervical cancer between February 2020 and December 2021 and evaluated the prevalence of lesions on digital cervicography (DC) with VIA/VILI for women positive for HR-HPV serotypes. The data were analyzed using SPSS version 20.0 for Windows. P values Results: We identified 315 cases with a positive HR-HPV deoxyribonucleic acid (DNA) test, 224 (71.1%) had a DC VIA/VILI triage test done. Of these, 30 (13.4%) women had a positive DC VIA/VILI, with five women (2.2%) having lesions suggestive of cancer. Out of 11 cases positive for HPV 16 alone, 05 (45.5%) had a positive DC VIA/VILI test. Of the 14 cases positive for HPV 18 alone, 03 (21.4%) had a positive VIA/VILI, meanwhile only 19 (10.7%) of the 177 cases positive for non-16/18 HPV had a positive VIA/VILI test. Conclusion: A high proportion of women (13.4%) with HR HPV had a positive DC VIA/VILI, with a significant proportion (2.2%) having lesions suggestive of invasive cervical cancer HR-HPV serotype was associated with DC VIA/VILI positivity;HPV 16 had the strongest association (45.5%), followed by HPV 18 (21.4%), and non-16/18 HR-HPV (10.7%), suggesting a

Introduction: Growth is a reflection of a child’s health and nutritional status. Children with sickle cell disease often have slower statural and weight development. The aim of this study was to evaluate the nutritional profile of children with sickle cell disease (SCD) registered in the CEMECO centre database. Methodology: This was a cross-sectional study with simple random sampling of children aged 1 to 16 years registered in the clinic database. Results: We collected information on 208 children, 121 of whom had sickle cell disease and 87 of whom were normal, with a sex ratio of 1.02. The mean age of the sickle cell patients was 8.7 ± 4.4 years, while that of the non-sickle cell patients was 9.5 ± 4 years. Haemoglobin electrophoresis revealed 103 homozygous (SS), 18 double heterozygous (SC, SBetaThal, SE) and 87 normal (AA) and/or sickle cell trait (AS) sickle cell cases. We observed a significant difference in the height/age ratio (P ¥). Conclusion: The results of our study revealed stunted growth in children with sickle cell disease.

The plurality effect of topographical irregularities on site seismic response

Topography can have signifi cant eff ects on seismic ground response during an earthquake because topographic irregularities cause considerable diff erences between the seismic waves emitted by the source and the waves reaching the ground surface. When a seismic motion happens in a topographically irregular area, seismic waves are trapped and refl ected between the topographic features. Therefore, the interaction between topographies can amplify seismic ground response. In order to reveal how interaction between topographies infl uences seismic response, several numerical fi nite element studies have been performed by using the ABAQUS program. The results show that topographic features a greater distance between the seismic source and the site would cause greater seismic motion amplifi cation and is perceptible for the hills far away from the source and the ridges. Also, site acceleration response is impacted by surrounding topography further than site velocity and displacement response.

Shallow Marine Cenomanian-Turonian Benthic Foraminifera and Kerogen Type from Mangoule, Logbadjeck Formation, Douala/Kribi-Campo Basin, Cameroon, West Africa

An integrated benthic foraminiferal and organic matter analysis of samples obtained from sedimentary sections exposed in the Mangoule-Bonepoupa area, revealed a very shallow marine paleo-depositional environment for the sediments studied, with considerable influx of continental organic matter that were accumulated during the Cenomanian-Turonian age. With reference to the lithostratigraphic profile of the Douala/Kribi-Campo Basin, the age obtained reveals that the sedimentary sections studied belong particularly to the lower section of the Logbadjeck/Mungo River Formation, based on the following benthic foraminifera assemblage: Ammobaculites jessensis, Ammobaculites benuensis, Ammobaculites coprolithiformis, Ammotium cf nkalagum, Ammotium nwalum, Haplophragmoides cf beuchensis, Trochamina taylorana. A particularly very poor palynomorph assemblage was recovered and included no age diagnostic species: Acrostichum aureum, Lycopodiumsporites sp., Fungal spore, Lavigatosporites discordatus, Longapertites sp., Verrucosisporites sp. An organic petrographic analysis performed on these samples revealed Kerogen Type-II and Type-III. The entire data sets obtained from this area is remarkably poor and we suspect poor preservation as the cause viewing the extent of weathering. The paleo-depositional environment of these sediments was suggested both from the foraminifera and palynomorphs present and confirmed from the organic matter types and macrofossil contents. The studied sections are composed generally of dark to dark-gray shale beds. As one of the petroleum producing basins within the Gulf of Guinea in which exploration/exploitation activities are on-going, the current research is aim at providing more data useful in the continuous search for more potential source rocks in this basin.

Factors influencing the effectiveness of using flash glucose monitoring on glycemic control for type 1 diabetes in Saudi Arabia

Energy use pattern and optimization of energy required for broiler production using data envelopment analysis

A literature review shows that energy consumption in agricultural production in Iran is not efficient and a high degree of inefficiency in broiler production exists in *** consumption of broiler production in Ardabil province of Iran was studied and the nonparametric method of data envelopment analysis(DEA)was used to analyze energy efficiency,separate efficient from inefficient broiler producers,and calculate wasteful use of energy to optimize *** was collected using face-to-face questionnaires from 70 broiler farmers in the study *** returns to scale(CCR)and variable returns to scale(BCC)models of DEA were applied to assess the technical efficiency of broiler *** results indicated that total energy use was 154,283 MJ(1000 bird)^-1 and the share of fuel at 61.4%was the highest of all *** indices of energy efficiency,energy productivity,specific energy,and net energy were found to be 0.18,0.02 kg MJ^-1,59.56 MJ kg^-1,and^-126,836 MJ(1000 bird)^-1,*** DEA results revealed that 40%and 22.86%of total units were efficient based on the CCR and BCC models,*** average technical,pure technical,and scale efficiency of broiler farmers was 0.88,0.93,and 0.95,*** results showed that 14.53%of total energy use could be saved by converting the present units to optimal *** contribution of fuel input to total energy savings was 72%and was the largest share,followed by feed and electricity energy *** results of this study indicate that there is good potential for increasing energy efficiency of broiler production in Iran by following the recommendations for efficient energy use.
