BACKGROUND Endoscopic drainage remains the treatment of choice for unresectable or inoperable malignant distal biliary obstruction(MDBO).AIM To compare the safety and efficacy of plastic stent(PS)vs self-expanding metal stent(SEMS)placement for treatment of *** This meta-analysis was developed according to Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines.A comprehensive search was performed in MEDLINE,Cochrane,Embase,Latin American and Caribbean Health Sciences Literature,and grey literature to identify randomized clinical trials(RCTs)comparing clinical success,adverse events,stent dysfunction rate,reintervention rate,duration of stent patency,and mean *** difference(RD)and mean difference(MD)were calculated and heterogeneity was assessed with I2 *** analyses were performed by SEMS *** Twelve RCTs were included in this study,totaling 1005 *** was no difference in clinical success(RD=-0.03,95%confidence interval[CI]:-0.01,0.07;I 2=0%),rate of adverse events(RD=-0.03,95%CI:-0.10,0.03;I2=57%),and mean patient survival(MD=-0.63,95%CI:-18.07,19.33;I2=54%)between SEMS vs PS ***,SEMS placement was associated with a lower rate of reintervention(RD=-0.34,95%CI:-0.46,-0.22;I2=57%)and longer duration of stent patency(MD=125.77 d,95%CI:77.5,174.01).Subgroup analyses revealed both covered and uncovered SEMS improved stent patency compared to PS(RD=152.25,95%CI:37.42,267.07;I2=98%and RD=101.5,95%CI:38.91,164.09;I2=98%;respectively).Stent dysfunction was higher in the covered SEMS group(RD=-0.21,95%CI:-0.32,-0.1;I²=205%),with no difference in the uncovered SEMS group(RD=-0.08,95%CI:-0.56,0.39;I²=87%).CONCLUSION While both stent types possessed a similar clinical success rate,complication rate,and patient-associated mean survival for treatment of MDBO,SEMS were associated with a longer duration of stent patency compared to PS.

Variation for Health-Enhancing Compounds and Traits in Onion (Allium cepa L.) Germplasm

Consumption of onion has been associated with reduced incidence of chronic diseases. Phenolic, organosulfur and carbohydrate compounds present are largely responsible for these effects. This study examined compositional variation for health-enhancing compounds in a genetically diverse collection of onion cultivars. Total antioxidant activity and aroma profiles were characterized. Significant variation in bulb concentration for total and individual phenolic compounds, thiosulfinates, carbohydrates, and total and soluble solids was found. The range of variation was particularly large (>50-fold difference between the cultivars with the highest and lowest content) for fructo-oligosaccharides (FOS) and the polyphenols quercetin, epigallocatechin gallate and epicatechin gallate. Amino acid profiles varied significantly as well with substantial variation (~10 fold) observed in both total and essential amino acids. Total antioxidant activity was positively correlated with polyphenols content, and quercetin in particular (r = 0.83), suggesting a major contribution from phenolic compounds to onion antioxidant properties. Significant positive correlation was also found between solids and thiosulfinates content (r = 0.74) and between solids and FOS (r = 0.81), suggesting a dilution/concentration effect for organosulfur compounds and FOS in onion bulbs. The present study revealed broad variation for health-enhancing compounds content in onion germplasm, which can be exploited in breeding programs aiming at increasing onion nutraceutical value.

Quantum Chemical Studies of Some Hydrazone Derivatives

By the functional B3LYP and M05-2X of DFT and in two bases set, more and more extended (6-311G and 6-311G (d, p)), theoretical study of antioxidant properties of four hydrazones was carried out. The calculations made concern the geometrical, spectroscopic and electronic parameters of the molecules. Analysis of the results relating to the geometrical parameters was carried out by calculating interatomic distances, relative errors between calculated values and those obtained experimentally by X-ray diffraction found in the literature. The 13C NMR spectra were calculated by GIAO (Gauge Including Atomic Orbitals) methods, and the results were subjected to statistical analysis by calculating Mean Absolute Deviation (MAD), Root Mean. Square (RMS) and the correlation coefficient (R2), in comparison with experimental spectra. The analysis of the results of calculations of various electronic parameters (hardness (η), softness (S), electronegativity (χ), electrophile index (ω), energy gap (HOMO-LUMO)) reveals that, overall, the methods M05-2X/6-311G (d, p) and B3LYP/6-311G (d, p) found that (R) - (?) - carvone salicylhydrazone (N2) is the most antioxidant molecule of the four molecules and also classify them according to their stability. This confirms the results obtained on the antitrypanosomal activity, the toxicity, the cytotoxicity and the selectivity of the synthesized compounds. ?

High Sugar-Fat Diet Induces Metabolic-Inflammatory Disorders Independent of Obesity Development

Background: The modern dietary habit, which is rich in refined carbohydrates and saturated fats, increases the risk of chronic diseases due to the proinflammatory effect of these nutrients. Aim: To evaluate the impact of high sugar-fat diet in the development of metabolic-inflammatory disorders in non-obese animals. Methods: Male Wistar rats were distributed into two groups according to the diet: control and high sugar-fat for 30 weeks. It was analyzed: dietary efficiency;chow, water and caloric intake;metabolic and hormonal profile in plasma and inflammatory cytokines in epididymal adipose tissue. Data were compared by Student’s t test or by Mann-Whitney U test with p Results: HSF presented lower chow intake, higher water consumption and dietary efficiency with no difference in the caloric intake. The final body weight (FBW) and weight gain (WG) were lower in the HSF group and there was no difference in the adiposity index (AI). HSF diet-induced hyperglycemia and hyperinsulinemia with no difference for Homeostatic Model Assessment for Insulin Resistance (HOMA-IR). Triglycerides, uric acid, adiponectin and leptin levels were higher in the HSF group. The HSF group showed increased interleukin-6 (IL-6) and tumoral necrosis factor-alpha (TNF-α) levels in epidydimal adipose tissue. The urinary protein-creatinine ratio and albuminuria were higher in the HSF group. Conclusion: HSF diet intake is directly involved in the development of metabolic-inflammatory disorders independent of obesity, dissociating the view that increased adiposity is the major risk factor for complications commonly found in obese individuals.

An Assessment of Healthcare Workers Knowledge about COVID-19

Introduction: Coronavirus disease 2019 (COVID-19) is caused by a viral infection and considered the third coronavirus emerging among human beings over the past two decades. Healthcare Workers (HCWs) are at high risk of acquiring this serious infection during providing care to patients. Therefore, it’s crucial to assess the knowledge of the HCWs about COVID-19. Methods: A multicenter cross-sectional study was conducted on HCWs working at public hospitals and Primary Healthcare (PHC) centers in the Najran region, KSA to evaluate the staff’s knowledge toward COVID-19. The questionnaire consisted of two parts, the first part included the demographic data, and the second part involved questions related to the COVID-19. Results: 451 HCWs participated in this study and the median of overall knowledge score was 67%. Most HCWs properly identified symptoms (82.9%), mode of transmission (78.5%), the incubation period (96.4%), the way of preventing the infection (91.5%), the COVID-19 is not same as MERS-CoV (74.3%) and availability of a vaccine against the COVID-19 (82%). However, HCWs were less likely to identify the source of COVID-19 when it was discovered in China (22.5%), the mortality rate (44.6%), and the presence of treatment (32.1%). Overall knowledge score was statistically significantly associated with profession (P = 0.034), educational level (P = 0.033), and availability of the infection control in the workplace (P = 0.006). Conclusion: The findings of this study demonstrated an intermediate level of knowledge of HCWs about COVID-19. Intervention programs are urgently needed to raise the knowledge of HCWs about this global public health issue.

Somatic Embryogenesis in Iberian Grapevine (Vitis vinifera) Cultivars Using Carpels as Initial Explants: Protocol Establishment and Histological Evaluation

Trincadeira and Aragonez are two important grapevine cultivars in the Iberian Peninsula, used for high quality red wines production. Both cultivars are strongly affected by fungal diseases, with consequent high loses on plant productivity and fruit quality. A successful protocol for plant regeneration via somatic embryogenesis (SE) was established for both cultivars allowing further plant improvement based on gene transfer technology. Several factors were evaluated during the three different phases which characterize an SE plant regeneration protocol. The culture room temperature during the induction phase, a parameter usually accepted as standard by most researchers, proved in these trials to significantly affect the embryo induction rates. Concerning embryo conversion, it was specially affected by the embryo developmental stage, by the intensity and duration of the chilling treatment and by the supplementation of conversion culture medium with activated charcoal. The responses obtained, both for induction and conversion, proved to be highly genotype dependent. Calli structure, as well as embryo integrity, was histologically observed, allowing to characterize embryonic and non-embryonic masses and to identify abnormalities on embryo development.
