Introduction: Birth spacing to prevent closely-spaced pregnancies, which are a source of maternal and neonatal morbidity and mortality, is not always respected by our women, as recommended by the World Health Organization (WHO). The objective was to study the factors associated with closely-spaced pregnancies among women followed at the Departmental Hospital Center du Zou et du Colline (DHC/Z-C) in 2022. Study Method: This was a descriptive, analytical cross-sectional study with prospective data collection from 20 May to 30 June 2022. Results: At the end of the study, 83 of the 117 women surveyed had close pregnancies, a frequency of 70.94%. They had an average age of 27.0 ± 6.3 years and were of Fon ethnicity and related in 91.57% of cases. The majority were married (96.60%). The factors significantly associated with these closely spaced pregnancies were age between 25 and 30 years, household occupation, age at first birth, age at marriage, women’s perception of contraceptive method (CM), women’s knowledge of the existence of a center providing family planning services and non-use of MC. Conclusion: Close pregnancies are still very common in the commune of Abomey. It is therefore important to increase the use of contraceptive methods in this part of the country.

Associated Factors and Consequences of Uterine Fibroids in Hospitals in the Borgou Department of Benin in 2022

Introduction: Uterine fibroids are the most common gynecological tumors in women of childbearing age and have multiple clinical manifestations. Objective: To determine factors associated with uterine fibroids in the Borgou department of Benin in 2022. Method: Descriptive, analytical cross-sectional study from August 22 to September 22, 2022. The sample consisted of women consulting for gynecological problems in 6 hospitals in the Borgou department in the North of Benin. Results: Of the 744 women surveyed, 10.1% were diagnosed with uterine fibroids. Women with uterine fibroids were often married, paucigesture, and nulliparous. Their main reasons for consultation were the desire for motherhood (50.7%), pelvic pain (46.7%), and genital hemorrhage (14.7%). Age, history of miscarriage, high blood pressure, and diabetes were significantly associated with the presence of uterine fibroids (Conclusion: Uterine fibroids are common in hospitals in northern Benin. There were factors associated with them that should be taken into account in management strategies.

Therapeutic and Prognostic Aspects of Gynecological and Breast Cancers in Northern Benin from 2010 to 2020

Introduction: Gynaecological and Breast Cancers (GBCs) are a public health problem and management continues to be a concern. Objective: To study the therapeutic and prognostic aspects of GBC. Method: This was a cross-sectional and descriptive study from January 1, 2010, to December 31, 2020. It included cases where the diagnosis of GBC was made and confirmed on anatomopathological examination and where treatment was instituted. Male breast cancer cases were excluded. Data were entered and processed by Epi data 3.1.1. and Epi Info 7.***.0.1. Results: A total of 230 cases of GBC were collected. The mean age was 48.4 ± 11.14 years with extremes of 15 and 80 years. The cancers were of the breast (55.2%), cervix (28.8%), corpus uteri (7.8%), ovary (5.2%), vulva (1.7%) and vagina (1.3%). Of the 127 breast cancers, 107 (70.10%) women had undergone total or partial mastectomy with 89 cases of lymph node dissection (83.20%). Hormone therapy was instituted in 89 women (71.8%), chemotherapy in 87 women (81.3%), 27 (21.25%) had undergone immunohistochemistry (IHC). Histopronostic grades were specified in 96 cases, including 12 (12.5%) grade I, 60 (62.5%) grades II and 24 (25%) grade III. Of the 103 women with gynaecological cancer, 94.2% had received treatment, which was specific for 69 women (71.1%). Radiotherapy was not performed in any of the women. Among the 69 women, 8.7% of deaths were recorded. Conclusion: GBC is common in northern Benin. Their management is inadequate and mortality remains high. Priority should be given to prevention.

Introduction: Breast cancer mortality remains high in most developing countries. Benin Republic does not yet have a technical platform required for the diagnosis and effective management of breast cancer “according to the recommendations”. Objective: To study the epidemiological, diagnostic and therapeutic aspects of locally advanced and/or metastatic breast cancers at the Centre Hospitalier Universitaire-de la Mère et de l’Enfant Lagune (CHU-MEL) of Cotonou from 2018 to 2021. Study Methods: This was a retrospective cross-sectional study with a descriptive purpose, carried out from January 1, 2018 to September 30, 2021 in the Gynaecology-Obstetrics Section of the CHU-MEL of Cotonou. The study population was patients diagnosed with locally advanced and/or metastatic breast cancers. Results: At the end of the study, 63 cases of locally advanced and/or metastatic breast cancer were recorded in 80 cases of breast cancer;a 74.11% prevalence of locally advanced and metastatic breast cancer. The mean age of the patients was 42.6 years. The main reason for consultation was the discovery of a mass in the breast (65.1%). Clinically, an orange peel was noted in 39.6%. Mammography (47.6%) and breast ultrasonography (42.9%) were the reference imaging workups for breast cancer diagnosis. Histopathologically, infiltrating ductal carcinoma (94.1%) was the predominant type with Scarff-Bloom and Richardson (SBR) grade II found in 35.3%. Progesterone and estrogen receptors were present in 42.9%. Mastectomy with axillary node dissection was the surgical treatment performed (80%) after neoadjuvant chemotherapy (39.6%). Only one patient had received radiotherapy. No patient received hormone therapy. Conclusion: Locally advanced and metastatic cancers remain a real public health problem. It is essential that major investments be made in order to improve both their diagnosis and management.

Determinants of the Frequency of Prenatal Consultations among Women Giving Birth in the Maternity Wards of the Commune of Agbangnizoun (Zou), Benin

Antenatal care is a safe way to prevent pregnancy-related health problems, difficult deliveries and even further to anticipate low birth weight [1]. But pregnant women, especially in Africa, do not attend antenatal clinics as recommended by the WHO. The general objective of this study was to study the norm in terms of Antenatal Consultation (ANC) frequency as well as the determinants of the frequency among women who gave birth in the maternity hospitals of Agbangnizoun. Our research was a cross-sectional, descriptive, retrospective and quantitative type, which was carried out among 335 women in Agbangnizoun, Benin from 1 February to 3 April 2020 (3 months), in order to find out the general or specific characteristics of women that we associate with the frequency of prenatal consultation. At the end of the study, we found that 60%, 68% and 15% of the surveyed population had received ANC in the first, second and third trimesters, respectively. Ethnicity, income level of spouses over the three trimesters as well as the level of their knowledge and overall practice of ANC are determinants of the frequency of prenatal consultation.

Background: Maternal death is a major public health problem worldwide, particularly in sub-Saharan Africa. Objective: This study sought to investigate dysfunctions in the management of patients whose outcome was classified as “maternal death” in the Gynaecology-Obstetrics section of the Departmental University Teaching Hospital of Borgou Alibori (CHUD-BA) from 2017 to 2021. Method: This was a retrospective cross-sectional study with descriptive and analytical purposes. The study population consisted of pregnant women, parturients and puerperas admitted into the CHUD-BA maternity ward from 2017 to 2021. Result: A total of 2011 patients were included in this study. The in-hospital maternal mortality ratio was 1526 per 100,000 live births. The dysfunctions identified were the amount of time spent in the referring center (more than 48 hours) (p = 0.001), delay of more than 2 hours between referral and admission into the referral center (p < 0.001), means of transport (motorcycle or public transport) (p compliance to the protocol for emergency obstetric and neonatal care (SONU) (p < 0.001) and delay of more than 2 hours in the etiological management of pregnant women (p Conclusion: Particular attention should be paid to the management of pregnant women in our healthcare system if we are looking forward to reducing maternal mortality.

Background: Obstetric Fistula (OF) is a breach of the birth canal due to obstructed labor. It is a major public health problem in developing countries. Objective: Study the epidemiology and treatment of obstetric fistulas at the Departmental University Hospital Centre of Borgou and Alibori (CHUD-B/A) in Parakou, Benin. Study Method: This was a descriptive and analytical cross-sectional study with prospective data collection on women treated during OF care missions from February 2020 to April 2021 in CHUD-B/A in Parakou, Benin. The data concerned their socio-demographic characteristics, obstetric history, the clinic and therapeutic characteristics of their obstetric fistulas. Results: In total 97 patients with OF were treated during the 14-month period. The average age of the patients was 36.42 ± 11.75 years old. The patients were married (51.54%), without professional occupation (54.64%) and had no formal education (73.20%). The vesico-vaginal variety was predominating (54.64%). The fistulas were of Type I (46.39%), Type II (20.62%) and Type III (32.99%) according to the classification of Waaldjik Kees. The patients were operated on under spinal anesthesia (94.79%), and vaginally (55.67%) according to the principle of Chassar Moir (68, 60%). The associated procedures were lengthening urethroplasty (17.53%), and interposition of the Martius flap (1.03%). The overall success rate without Stress Urinary Incontinence was 71.13%. The factors associated with the failure of the fistula repair were: the nutritional status of the patients (p = 0.004), the previous repair failure (p = 0.001), the high size of the fistula (p = 0.007), the fistula severity (p and a urethral reconstruction (p Conclusion: OF is a relatively frequent pathology in Benin. Vesico-vaginal fistula is the most common form. The success rate of the surgical treatment is satisfactory, but depends on some factors.

Introduction: Maternal mortality is a robust indicator of human development. Most of it occurs in developing countries. Objectives: This study aims to determine the factors associated with maternal mortality in Gynecology and Obstetrics Department of Borgou Departmental University Hospital Center (CHUD-B). Study method: Our study took place at Gynecology and Obstetrics Department in Borgou Departmental University Hospital Center. This was a descriptive and analytical case-control study, covering a period of 7 years from January 1, 2012 to December 31, 2018. Background: Women well-informed regarding breast cancer are often most conscious about their breast health and would therefore seek medical attention when detecting the initial symptoms. Objective: This study sought to identify factors associated with a good level of knowledge regarding breast cancer among women in the commune of Parakou in 2021. Method: This was a prospective cross-sectional study with descriptive and analytical purposes, carried out from June 5 to August 5, 2022, among women residing in the first district of the commune of Parakou. A pre-designed questionnaire set up with the Kobotoolbox server was used for data collection through a cluster survey sampling technique. Pearson’s chi-square test was implemented to determine associations between variables. Result: A total of 630 women were included in this study. The prevalence of women well-informed about breast cancer was 21.75% (95% CI = [18.70 - 25.13]). Factors associated with good knowledge regarding breast cancer were age group (p-value < 0.001), school level (p-value < 0.001), occupation (p-value < 0.001), and regular attendance at a health facility (p-value Conclusion: Several women remain unaware of breast cancer in the commune of Parakou. A huge effort is still needed to increase the level of coverage of information regarding breast cancer and its prevention methods.

Introduction: Obstetric Fistula (OF) is a tragedy and a public health problem (physical, social, moral and psychological). Objective: To determine the clinical, therapeutic and prognostic aspects of obstetric fistula at CHUD/B-A maternity ward in 2016. Study Method: This is a descriptive, analytic study with prospective data collection from March 07 to December 31, 2016. Patients with obstetric fistula were the study population. Results: A total of 37 patients were treated including 35 operated, the other two not operated for pyuria and bladder calculus after examination under spinal anesthesia. The average age was 36.6 ± 10.5 years with the extremes of 21 years and 65 years. The mean age of onset of Obstetric Fistula (OF) was 28.9 ± 6.5 years. Patients came from southern Benin (51.4%), were married (67.6%), peasant (35.1%) and out of school (81.1%). The types of OF were vesico-vaginal fistula (VVF) (62.2%), urethro-vaginal fistula (24.3%), vesico-uterine fistula (VUF) (5.4%), and uretero-vaginal fistula (2.7%). The fistula site was supra trigonal (54.1%), urethral (18.9%), trigonal (10.8%), vesico-uterine (10.8%), cervical urethral (2.7%), ureteroid vaginal (2.7%). Fistulas were complex (70.3%), complicated (24.3%) and simple (5.4%). The vaginal route was one of the first routes (64.9%). The operative techniques used were CHASSAR MOIR hysterorrhaphy (74.3%). The cure with continence was 68.6%. Conclusion: OF is observed in young women of childbearing age. VVF is the most common. The management is surgical with several operating techniques. Healing is possible. Hence the importance of paying special attention to these patients for their care.
