Experience of Menopause for Women in the Niamey Region, Republic of Niger

Introduction: Menopause is poorly documented in Africa. For most of the women living there, the experience of this period does not seem to be very problematic. For women living in rural areas, other concerns are more prominent. Methods: The study was prospective, quantitative and qualitative about 500 cases collected over a period of 6 months. The study involved postmenopausal women in the Niamey region of public services, in certain residential areas, or accompanying people in health structures. Parameters taken into account were sociocultural, antecedents, experience, knowledge, and management of menopause. The data was entered and analyzed using Microsoft Office Word 2010 and Microsoft Office Excel 2010 software. Results: The average age was 56.4 years with extremes of 40 and 80 years;57% were still living with their husbands and 39% widows;60.2% housewives;53% uneducated, 17% have higher education;60.2% had had at least 6 children and 39.6% had used contraception. The comorbidities observed were arterial hypertension (24.4%) and diabetes (7%). As for the representation that women made of menopause, for all of them, this stage is the beginning of old age. The most frequent signs were asthenia (86.2%) and mood disorders (70.6%). Almost all of the patients (99.8%) had not used hormonal treatment for the menopause and among them 27% the management of the menopause was with traditional means. Conclusion: Menopausal disorders are certainly a reality in our socio-cultural context. They are not a primary concern either through ignorance or resignation of the women concerned.

Adnexal torsion is defined by the existence of a partial or complete torsion of the ovary and a tubal portion around its vascular pedicle. It is a rare gynecological emergency. We report the case of adnexal torsion on a large postpartum ovarian cyst in a 40-year-old patient. The diagnosis was suspected based on clinical and ultrasound signs and confirmed by exploratory laparotomy. The appearance of the cyst was suggestive of a mucinous cystadenoma of the ovary. The treatment was radical by performing an adnexectomy. The surgical piece weighed 5.2 kg. The postoperative course was simple. Anatomopathological examination had not been carried out;the parents had buried the operating piece.

Emergency Obstetric Hysterectomy in a Low Resources Country: An Eight-Year Retrospective Cohort Study from a Secondary Care Hospital in Niger

Introduction: Emergency obstetric hysterectomy (EOH) is a lifesaving procedure that is performed as a last resort in cases of severe postpartum haemorrhage. Objective: The objective of this study was to determine the incidence, socio-demographic profile of patients, indications, management and maternal-fetal outcomes of EOH in a maternity hospital with limited resources in Niger. Methodology: This was an eight-year retrospective cohort study involving the analysis of medical records from patients who underwent emergency obstetric hysterectomies between 1 January 2015 and 31 december 2022 at the Mother and Child Health Centre (MCHC) in Maradi, Niger. The epidemiological data, indications, and outcomes of EOH were collated and subjected to analysis using the statistical software package SPSS 21.0. Comparisons were made using the Chi-squared test. A p-value of less than 0.05 was considered statistically significant. Results: during the study period, 239 cases of emergency obstetric hysterectomy were recorded out of 269,710 deliveries, representing a frequency of 0.89%. The mean age of the patients was 32.41 years (range: 17 - 50 years). The patients were identified as married (239 cases, 100%), unemployed (228 cases, 95.4%), and not attending school (215 cases, 90%). The largest number of cases were observed in large multiparous women (i.e., those with more than five children), representing 58.6% of the total number of cases (140 patients). The average parity among this group was 6.15 children. The majority of patients (229 patients, 95.82%) had undergone in utero transfer, with 169 patients (70.71%), originating from peripheral maternity units in the Maradi region. Upon admission, 116 patients (48.53%) exhibited active genital haemorrhage, while 58 patients (24.26%) were in shock. Pathological and Etiological Aspects of Nephrotic Syndrome at the Niamey General Reference Hospital

Introduction: Studies have been conducted on nephrotic syndrome in Niger. The study aimed to determine the histological and etiological aspects of nephrotic syndrome. Patients and Method: This was a retrospective study from February 1st, 2018 to January 31st, 2024. All patients with nephrotic syndrome who underwent renal biopsy were included. Samples were analyzed at the anatomy-cytology pathology laboratory of the Faculty of Medicine in dakar (Senegal). The variables studied included clinical, biological, histological and etiological characteristics. data were analyzed using Excel 2013 and Epi-info 7.2.0 software. Results: The study included 119 patients with nephrotic syndrome. Prevalence of nephrotic syndrome was 11.24%. The male-to-female ratio was 2.25:1. The mean age at diagnosis was between 34.5 ± 18.84 years. Edema was the reason for admission in 40.34% of cases. The nephrotic syndrome was impure in 63.86% of cases. Nine histological lesions were identified. Focal and segmental glomerulosclerosis (40.09%), minimal change disease (23.53%), membranous nephropathy (13.45%), diabetic nephropathy (10.92%), membranous proliferative glomerulonephritis (3.36%), acute glomerulonephritis (3.36%), glomerular thrombotic microangiopathy (2.52%), non-IgA mesengial proliferative glomerulonephritis (1.68%) and amyloidosis (0.84%). Nephrotic syndrome was primary in 57.98% of cases. Secondary etiologies were dominated by diabetes (11.76%), followed by hepatitis B virus (9.24%), lupus, lymphoma, malaria, syphilis, cryoglobulinemia, sickle cell disease and HIV. Conclusion: Future studies should investigate the causes of glomerulopathy secondary to chronic tubulointerstitial lesions.

Socio-Spatial dynamics of Land in Southwest Niger: The Case of the Commune of Gothèye

In south-west Niger, ecosystems are losing several hectares of their surface area every year due to internally displaced persons and refugees. The commune of Gothèye is not immune to this situation. The aim of this research is to assess the impact of displaced persons and refugees on socio-spatio-temporal dynamics of ecosystems using Landsat images. To achieve this, Landsat TM, Landsat ETM+ and OLI 8 satellite images from September and March were used (2010 to 2024). Operations on Envi 5.3, field validation output and finally mapping on ArcGIS were the steps involved. discrimination is significant, with kappa coefficients of 0.97, 0.96, 0.86 and 0.85. The results obtained indicate a degradation of natural ecosystems, reflected in a change in landscape structure, with a marked reduction in the quantity and quality of ecosystem goods. Analysis of the evolution of land use showed that 31% of the land remained in its initial state (unchanged), 69% underwent modifications, and 11% was converted to cropland. Over these fourteen years, the study area has undergone changes in land use patterns, which have resulted in a modification of landscape structure, with a marked decline in the quantity and quality of ecosystem services.

Assessment of domestic Wastewater Management Practices in the Communal district I of Maradi City, Niger Republic

Wastewater treatment is a problem with much acuity in the city of Maradi in general, and particularly in the Communal district I of Maradi. It is for this reason that the present study was conducted which deals with wastewater sanitation. The main objective of this study conducted through a survey is to investigate in the domestic wastewater sanitation in the Communal district I of Maradi city. Specifically, the study aims to estimate the amount of wastewater produced by households and to analyze the sanitation system at household and at district levels. The wastewater management practices in this district were assessed through a descriptive cross sectional study in which a total of 129 households were selected by sample distribution technique in the district neighborhoods and studied using interviewer and administered questionnaires at communal and household levels. The results of the study showed that the amount of domestic wastewater generated is quite large, about 32.27 m3 per person per day, and this comes from several sources including showers, laundry, dishwashing and income generating activities. The results also revealed that the unhealthy state of the Communal district I of Maradi is related to the absence of a wastewater treatment system, the methods and practices used in its management, and the population is also largely responsible for this unhealthy state and not willing to participate to a sanitation project.

Impacts of Market Gardening Practices on Environmental Resources: The Case of Irrigable Lands in Bonkoukou (Imanan Rural Municipality, SW Niger Republic)

due to increasingly extreme climatic conditions in the Sahel, irrigated crops are emerging as imperative innovations for agricultural transformation and resilience. This study is designed to assess the impact of market gardening on soil, water and plant cover in the western Niger Republic. Semi-structured survey data were collected from 78 site operators in the Imanan rural municipality (Filingué, Tillabéry). In addition to the survey, a photographic collection illustrated certain observable facts in the field. The survey shows that the wooden enclosures (70.65% of respondents) around the fields are renewed every year. The impacts are then substantial: deforestation of plateaus, clearing of valleys for new extensions, loss of wildlife habitat, changes in the water flow regime resulting from a geomorphological change in the commune. Itou, the preparation of the planks consumes a lot of water. Thus, the new sinking of water points and the load pressure increase the drawdowns of wells and boreholes in the sites (75.46% of respondents). The frequent spills of waste oil on the sites, as well as the numerous potato storage pits, are among the factors and vectors most likely to contaminate the soils and aquifers of the study area. Market gardening destabilizes the soils and induces regressive erosion as well as area runoff of rainwater on all sites and on the slopes. On the other hand, slash-and-burn practices, drying of branches intended for fences, as well as rain-fed cowpea crops, promote improved physical properties and soil fertilization. To reduce the risks of disintegration and especially of silting up of sites, the study suggests a policy of live hedges and wire fences so that common spaces can be imposed on operators.

Agro-Morphological Variability in Eight Bambara Groundnut (Vigna subterranea (L.) Verdc.) Morphotypes from Zinder (Niger)

Bambara groundnut (Vigna subterranea (L.) Verdc.) has economic importance for the producers and the traders as cash crop since it generates substantial income for households in Niger. The objective of this study was to assess eight (8) Bambara groundnut morphotype from Université André Salifou de Zinder in Niger Republic by their morphological and agronomic characters. The experiment was conducted in a randomized complete block design with three replications during the rainy season of 2020-2021. data were collected on the Number of leaves, Plant height (cm), Growth habit (cm), Terminal leaflet length, Terminal leaflet width, Number of nodes/stem, Number of stems/plant, Number of pods/plant, Weight of 100 pods (g), Weight of 100 seeds (g), Length of seeds (mm), Width of seeds (mm), Weight of seeds/Plot (kg). There were significant differences for all characters, except Number of leaves, Terminal leaflet length, Terminal leaflet width, Length of seeds (mm) Weight of 100 pods (g), Weight of 100 seeds (g). The morphotypes UZ-VZ-04, UZ-VZ-03, UZ-VZ-06, UZ-VZ-02 and UZ-VZ-05 have demonstrated good performance for grain weight per plot and can be useful for a breeding program.

Prognosis of Acute Renal Failure of the Child during Severe Malaria in Niamey-Niger

Introduction: In malaria-endemic areas, acute renal failure (ARF) is one of the most serious complications, it occurs in 40% of severe forms of malaria in adults and is linked to 75% of deaths, especially when extra-renal cleaning is not available. In children, studies of ARF during malaria are limited. We have no published studies on this topic in Niger. The main objective of our study is to evaluate the prognosis of ARF during severe malaria in children. Patients and Method: This is a one-year prospective study (January 1, 2016 to december 31, 2016) conducted in the resuscitation unit of the pediatric department of the National Lamordé Hospital of Niamey (Niger). We included in the study children aged 0 to 15 years hospitalized for severe malaria with impaired renal function. Patients who had chronic renal failure or who had acute renal failure with a thick negative drop were excluded from the study. Acute renal failure is defined according to Kidney disease Improving Global Outcomes (KdIGO) criteria basing on creatinine clearance. Results: The incidence of ARF was 12.60%. The mean age of the patients was 4.25 ± 1.3 years [8 months - 15 years]. The mean hemoglobin level was 8.2 ± 2.7 g/dl. In 54.02% of cases, the hemoglobin level is ≤5 g/dl. Mean serum creatinine was 543.7 ± 69.5 μmol/l [107 - 2500 μmol/l] and mean azotemia was 27.5 ± 3.5 mmol/l. Severe anemia (54.02%) were more related to the occurrence of ARF(with p = 0.014). According to the RIFLE classification, 55 patients (63.22%) were in the Risk stage, 18 patients (20.69%) were in the injury stage and 14 patients (16.09%) in the failure stage. All patients were placed on injectable Artesunate. The average length of hospital stay was 8.6 ± 4.5 days [5 to 22 days]. dialysis was reported in 15/87 (17.24%). For technical and financial reasons only 8 patients were hemodialysed.
