Role of Vascular Endothelial Growth Factor-C during Stem Cell Therapy Using Autologous Bone Marrow Mononuclear Cells in Patients with Lower Limb Lymphedema

Introduction: Vascular endothelial growth factor-C (VEGF-C) is the primary lymphangiogenic factor that stimulates lymphangiogenesis by signaling via specific receptor, vascular endothelial growth factor receptor 3 (VEGFR3). This study was conducted to evaluate the change in the level of VEGF-C before and after autologous bone marrow mononuclear cell transplantation for treatment of Lower limb lymphedema. Patient and methods: Forty patients with lower limb lymphedema were divided into two groups. Group I included 20 patients with chronic lower limb lymphedema who underwent autologous bone marrow mononuclear cell transplantation. Group II included 20 patients with chronic lower limb lymphedema who were exposed only to compression therapy as a control group. VEGF-C level in the diseased limbs was measured in both groups at the beginning of the study then 3 and 6 months respectively. Results: Group I included 20 patients, 8 patients were male (40%) and 12 patients were females (60%) with mean age 29.5 ± 12.15 while group II included 20, 10 patients were male (50%) and 10 patients were females (50%) with mean age 39.5 ± 11.5. In group I, the specimens were taken at 3 and 6 months after transplantation showed a marked decrease in the VEGF-C level with statistically significant p value, 0.02 and 0.001 respectively. In group II the level of VEGF-C after compression therapy alone at 3 and 6 months interval showed fluctuation with statistically non-significant p value, 0.64 and 0.55 respectively. Conclusion: VEGF-C is essential for regulation of lymphangiogenesis. The level of VEGF-C was found elevated in patients with lymphedema and decrease after autologous mononuclear bone marrow cells, however these results were statically non-significant.

We aim to determine the baseline factors associated with partial and cEVR by analyzing the data of 1861 Egyptian patients treated for 12 weeks with a course of Peg-IFN plus RBV. Base line data of 1861 Egyptian patients with chronic hepatitis C coming at Cairo-Fatemic Hospital for HCV treatment were studied including full clinical, Ultrasonographic examination, laboratory evaluation and liver biopsy. The most significant variables in relation to complete early virological response were low Hb level (13 gm/dl) with p 0.01, the stage of fibrosis p value eline before initiating treatment is mandatory to predict which patient will be more expected to achieve a cEVR and thus reducing the side-effects and healthcare costs associated with interferon therapies.

The Allelochemicals Effect of Zygophyllum album on Control of Bromus tectorum

The aim of the present study was to investigate the potential allelopathic effects of different concentrations of Zygophyllum album (donor species) aqueous extract (2.5, 5, 7.5 and 10%) on germination percentage, plumule and radicle length of Bromus tectorum (weed) and Triticum aestivum (crop species) under laboratory conditions to have the greatest inhibitory allelopathic effect on the recipient species in mixed culture compared to that pure culture. The germination percentage, plumule and radicle length of B. tectorum in mixed culture was completely inhibited at the highest concentration of aqueous extracts of the donor species level the exerted weak measures as affected by the highest concentration level of donor in pure culture. This inhibition was markedly in obvious B. tectorum that is more sensitive to tested donor. The domineering effect of aqueous extract of the donor was more prominent on weeds than crop species. The variant response to the allelopathic substance could be related to the species specific growth regulatory effect of allelochemicals and concentration dependent. There is possibility of using these allelochemicals directly or as structural leads for the discovery and development of environment friendly herbicides to control weeds.

Objective:Gene expression profiling of breast cancer has identified five molecularly distinct subtypes of breast cancer that have different biological behavior and clinical outcomes. These subtypes are termed luminal A, luminal B, luminal HER2, HER2-enriched and triple negative breast cancers (TNBC). We aimed at identification of breast cancer subtypes among Egyptian population and their clinicopathologic features using ER, PR and HER2, Ki-67 and CK5/6. Methods:Tumors from 100 patients with invasive duct carcinoma were subtyped by immunohistochemistry using ER, PR, HER2, Ki-67 and CK5/6. The prognostic value of the immunohistochemical assignment for breast cancer disease-specific survival was investigated by using Kaplan-Meier curves. Results:Immunohistochemical profiling classified 22 cases as luminal A, 33 cases as luminal B, 9 cases as luminal HER2, 26 cases as HER2-enriched and 10 cases as TNBC. Tumors that measured more than 3.5 cm, showed predominance of HER2-enriched subtype. HER2-enriched and luminal B subtypes dominated the node positive cases (35.4% and 33.8%; respectively). Large tumor size (> 3.5 cm), hormone receptor negative state and HER2 positive state were associated with poor prognosis. Disease free survivals (DFSs) were significantly different (Pely. Conclusion:ER, PR, HER2 and Ki-67 constituted a strong surrogate for molecular breast cancer subtypes and can be easily applied. HER2-enriched subtype carries worse features being associated with large tumor size, nodal metastasis and is associated with poor outcome. Luminal A is a heterogeneous subtype with underlying several factors that can turn its prognosis adversely. TNBC subtype may behave unexpected in a favorable way.

Urinary Continence after Orthotopic Detubularized Ileocaecal Pouch Versus Ileal Neobladder:Does it Differ?

Objective:To compare both techniques of detubularized ileocaecal and ileal neobladder post radical cystectomy with regards to functional results in the form of continence(day and night)with the use of urodynamics(for assessment of voiding volume and pouch capacity).Methods:This is a prospective randomized clinical trial that was done at the National Cancer Institute,Cairo University from August 2016 and January 2018.Sixty-two patients presented with muscle invasive urinary bladder cancer and no history of urinary incontinence or urethral stricture were included in this study.All patients underwent preoperative cystoscopic examination.Radical cystectomy was done,and patients were divided into two groups according to the type of diversion.Group A included 31 patients with orthotopoic detubularized ileal neobladder and group B included 31 patients with orthotopic detubularized ileocaecal neobladder.Urodynamic studies were done to 27 patients in group A and 27 patients in group B at just 6 and 12 months postoperatively.Results:The mean age in group A was 59.2±8.0 while in group B was 57.8±6.6.In each group,we compared day and night continence in a period of 3 and 6 months.Postoperatively in group A,day continence after 3 months was 81.5% which improved to 96.3% after 6 months postoperatively with significant P-value(0.025).Night continence after 3 months was fair in 66.7%,unsatisfactory in 33.3% and 0% for good state which improved to 70% for fair state,18% unsatisfactory and 11% for good state after 6 months with significant P-value(0.035)while in group B,day continence after 3 months and 6 months postoperatively was 100% and night continence after 3 months was good in 0.0%,fair in 78.6% and unsatisfactory in 21.4% which improved to 10.7% for good state,85.7% for fair state and 3.6% for unsatisfactory after 6 months with significant P-value(0.005).Conclusion:There are no significant differences between detubularized ileocaecal and ileal neobladder in relation to continence.However,there is significant change in each

In this paper, we have presented the numerical investigation of the geometric phase and field entropy squeezing for a two-level system interacting with coherent field under decoherence effect during the time evolution. The effects of the initial state setting and atomic dissipation damping parameter on the evolution of the geometric phase and entropy squeezing have been examined. We have reported some new results related to the periodicity and regularity of geometric phase and entropy squeezing.

Biodiesel Production from Oils Extracted from Date Pits

Biodiesel production had received a considerable attention as a green, non-toxic and renewable alternative to petroleum diesel. To avoid using vegetable oils, which are expensive and compete with food, as feedstock, waste oils have been proposed. However, these waste materials contain a large amount of free fatty acids that complicates the production process. In this work, biodiesel production using an alternative feedstock;namely oils from date-pits, has been investigated. These oils have the same favorable features of straight oils, and at the same time are considered waste, since they are extracted from a waste material. The yield of oils extracted using n-hexane in a Soxhlet apparatus was compared to that of oils extracted using methanol-chloroform solvent mixture. The extracted oils were then converted to biodiesel via transesterification with methanol in presence of Novozym𪥣 or Eversa®Transform. The highest oils extraction yield of 11.7%, per dry weight sample, was obtained using Soxhlet extraction apparatus compared to 8.9% using methanol-chloroform mixture. The highest biodiesel production yield was 30% of the oil used, achieved after 6 hours using Novozym𪥣 at 40°C, 5:1 methanol to oil molar ratio and 10wt% enzyme loading. By using a chemical alkaline catalyst, NaOH, at the same conditions, the yield was 27%.

AIM:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).METHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using STATA.RESULTS:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. Our results support the importance of including self-reported visual functions as part of routine ophthalmologic examination.

AIM:To assess the utility of an autologous CD34 + and CD133 + stem cells infusion as a possible therapeutic modality in patients with end-stage liver diseases.METHODS:One hundred and forty patients with endstage liver diseases were randomized into two groups.Group 1,comprising 90 patients,received granulocyte colony stimulating factor for five days followed by autologous CD34 + and CD133 + stem cell infusion in the portal vein.Group 2,comprising 50 patients,received regular liver treatment only and served as a control group.RESULTS:Near normalization of liver enzymes and improvement in synthetic function were observed in 54.5% of the group 1 patients;13.6% of the patients showed stable states in the infused group.None of the patients in the control group showed improvement.No adverse effects were noted.CONCLUSION:Our data showed that a CD34 + and CD133 + stem cells infusion can be used as supportive treatment for end-stage liver disease with satisfactory tolerability.
