Cervical Length Estimation and Cervicovaginal Fluid for Placental <i>α</i>-Microglobulin 1 Testing to Screen Women Had Threatened Preterm Labor for Time till Spontaneous Labor

Objectives: Evaluation of diagnostic performance of rapid testing of cervico-vaginal fluid (CVF) for fetal fibronectin (FFN) and placental α-microglobulin 1 (PAMG-1) as screening of women presented by threatened preterm labor (TPTL) with intact membranes for duration till getting spontaneous labor (SL). Patients & Methods: 37 women presenting with TPTL underwent CVF samplings before digital examination and then underwent transvaginal ultrasonography for estimation of cervical length (CL). All women received fluid and anxiolytic therapy and if uterine contractions persisted, all had received tocolytic therapy with oral nifedipine or intravenous magnesium sulphate according to requirements. Incidence of SL within Results: Incidence of SL was 13.5%, 35.2% and 51.3% within 48-hr, 2 - 7 and 7 - 14 days, respectively. Duration till labor after sampling was positively correlated with CL, while was negatively correlated with positive FFN and PAMG-1 tests. Positive FFN test had high specificity, while positive PAMG-1 test had high sensitivity for labor within 7 days. Regression analysis defined short CL and positive PAMG-1 test as significant predictors for short duration till SL. ROC curve analysis defined short cervix and positive PAMG-1 test as significant predictors for labor within 48-hr and within 2 - 7 days respectively and combined negative PAMG-1 test and CL of 20 - 25 mm were significant predictors for labor within 7 - 14 days. Conclusion: PAMG-1 test had high specificity, if positive, for predicting SL and high NPP, if negative, for excluding labor within 7 days, so it can be used as rapid adjuvant to clinical evaluation to help management decision-making. Moreover, PAMG-1 test is recommended screening test for being easy-to-use bedside test, provides rapid results, can be used after vaginal exam and coitus and does not require a speculum examination or specialized equipment to analyze results.

Assessment of the Energy Conversion on the Thermal Balance and Atmospheric Emissions in Ceramic Tile Product Industry in Tunisia: A Case Study

This work aims to assess the effect of energy conversion (Thermal oil, Natural gas and cogeneration system) on atmospheric emission and energy consumption in ceramic tile product sector in Tunisia. Two tile manufactures were selected. The first plant has two production lines: The first line (FF1) operates with thermal oil with a lower calorific value (LHV) of 9811 cal/g and the second line (FG1) operating with natural gas has a lower calorific value (HHV) of 10,520 cal/g, ensuring a daily output of 300 tons each one. The second manufacture (SC2) operates with natural gas with the same LHV value. The thermal oil energy balance showed a specific consumption of 0.0481 toe/ton tile product for the FF1 manufacture line, 0.0198 toe/ton of tile product for the FG1 manufacture line and 0.0143 toe/ton of tile product for the SC2 manufactory. The electrical energy consumption was 0.0121 toe/ton of tile product for the FF1 line, 0.0108 toe/ton of tile product for the FG1 line and a production of energy (exergy) of 0.014 toe/ton for the SC2 production line. The specific consumption was split into 40% for dryer and 60% for tunnel kilns. The conversion allow to record a dryer reduction rate of 80% for nitrogen oxides (NOx), 56% for sulfur oxides (SOx), 56% for fluorinated compounds, 52% for chlorinated compounds and 52% for volatile organic compound. Whereas, the kiln reduction rate was 36% for nitrogen oxides, 51% for sulfur oxides, 36% for chlorinated compounds and 55% for fluorinated and 50% for volatile organic compounds (VCOs). Compared to natural gas line, the use of cogeneration system in kiln process shows a decrease of 67% for NOx emissions, 80% for SOx emissions, 89% for fluorinated compounds, 58% for chlorinated emissions and 64% for volatiles organic compounds. Compared to thermal oil, the use of cogeneration system reduces the thermal energy consumption by 70% and allowed to save 30% of electric energy by generate 20% of needed electric energy. Ongoing Mutations in Polytreated Metastatic Cancer Patients May Create a New Chance of Treatment with Unexpected Drugs

Objectives: In this study molecular/genomic characteristics were done on new tissue biopsies taken from Egyptian patients with refractory metastatic solid tumors aiming for two end points: To figure out a personalized treatment and to find the percent of discrepancy between the elaborated drugs of potential benefit and that stated in the guidelines. Methods: 22 eligible patients joined the study. (breast = 5, colon = 3, liver = 2, kidney = 2, ovary = 2, sarcoma = 2, metastasis of unknown origin = 2, Tongue = 1, Adrenal cancer = 1, gastric = 1 and lung cancer = 1). Biopsies were subjected to one or more of the following tests;Immunohistochemistry, Chromogenic/Fluorescence in situ Hybridization, Next Generation Sequencing, Sanger Sequencing. Results: Biomarkers and their corresponding drugs with associated potential benefits were detected as following;TUBB3, PGP and TLE3 (indicating potentiality of paclitaxel) in 22% of cases, TS (Antifolates) 18%, TOPO1 (Irinotecan) 14%, RRM1 (Gemcitabine) 13%, MGMT (Temozolomide) 7%, TOPO2 (Doxorubcin) 7%, ERCC1 (Platinum) 6%, BRAF (Vemurafenib) 2%, KRAS and NRAS (anti EGFR) 2%, C-KIT (TKIs potentiality) 1%, hormonal receptors in 5% of cases (Antihormonal potentiality), monoSPARK and polySPARK in 3% of cases indicating nabpaclitaxel potentiality. Potentiality of some drugs (Based on their corresponding biomarkers) was unexpectedly detected as following;Pemetrexed, irinotecan, dacarbazine and temozolomide in breast cancer patients, platinums and taxanes in liver, Taxanes, gemcitabine, fluoropyramidines, pemetrexed, dacarbazine and temozolomide in kidney cancer, Taxanes, gemcitabine, pemetrexed, dacarbazine and temozolomide in cancer colon, irinotecan in cancer tongue, Pemetrexed and irinotecan in adrenal gland cancer. Role of Ischemia Modified Albumin in Diagnosis of Pulmonary Embolism

Pulmonary embolism is a common and potentially lethal condition. Clinical signs and symptoms for pulmonary embolism are nonspecific. New and simple tests are therefore needed in order to help in early diagnosis of pulmonary embolism. The aim of this work is to elucidate the role of IMA in the diagnosis of pulmonary embolism. Subjects and Methods: 75 patients with suspected pulmonary embolism and 20 control healthy subjects were included in this study. Measurement of IMA was done in all subjects. Results: The mean values of IMA were statistically significantly higher among the PE patient group (0.43 ± 0.104 ABSU) in comparison with non PE patient group (0.27 ± 0.053 ABSU) and healthy control subjects (0.21 ± 0.080 ABSU). At cut-off value of 0.305 ABSU, IMA had 97.5% sensitivity and 71.42% specificity. The area under the curve was 0.952. The positive predictive value of this cut-off value was 79.59% while the negative predictive value was 96.15%. Conclusions: IMA is a good alternative to D-dimer in the diagnosis and exclusion of PE. Larger studies are needed to augment our results.

Objective:To investigate the prevalence of container breeding mosquitoes with emphasis on the seasonality and larval habitats of Aedes aegypti(***) in Makkah City,adjoining an environmental monitoring and dengue ***:Monthly visits were performed between April 2008 and March 2009 to randomly selected *** each visit,mosquito larvae were collected from indoors and outdoors containers by either dipping or pipetting. Results:The city was warmer in dry season(DS) than wet season(WS).No rain occurred at all during DS and even precipitations did fall,wetting events were much greaterduring *** survey revealed the co-breeding of Aedes,Culex and Anopheles in a variety of artificial containers in and around homes.32109 larvae representing 1st,2nd,3rd,and 4th stages were collected from 22618 container *** was far the commonest and Aedes genus was as numerous as the Culex *** larval abundance exhibited marked temporal variations,overall, being usually more abundant during *** types of artificial containers were found with developing larvae.70%of these habitats were located indoors.71.42%of indoor containers were permanent and 28.58* was semi-permanent during *** tanks was the only container type permanent during *** larval indices(CI,HI,Bl) recorded were greater during WS. Conclusions:Taken together,these results indicate a high risk of dengue transmission in the holy city.

This paper aims to analyze the geographic distribution of air pollutant concentrations in Basra Province, Southern Iraq, and to cartographically determine the spatial variation of air pollution levels as well as to recognize the hottest spots of air pollution within the study area, and conclude that the levels of air pollution in the study area are spatially varied, with an irregular spatial pattern and some hotspots.

Cancer is cell fleeing from death by blocking the intrinsic and extrinsic pathways of cell death programs. In the present work, the experimental formula was designed to remove these blockers. It was applied on 120 Swiss albino mice which were inoculated intraperitoneally and subcutaneously with Ehrlich Ascites Carcinoma cells;1 × (106) cell/mouse. The activity of the cell death programs of the tumor was detected by measuring the volume of Ascites fluid, counting the number of dead cancer cells, measuring the size of the tumor, detecting the positive reaction of caspase enzyme in cancer cells and presence of macrophages and apoptotic bodies in tumor tissue. The experimental formula succeeded in removing the blockers of the cell death program in cancer cells returning the cell death program to work again.
