In Frail Elderly Patients, Low-Dose Gemcitabine over 6-Hour Infusion Is Equally Effective and Less Toxic Than the Standard Gemcitabine Protocol for Advanced Pancreatic Adenocarcinoma: A Randomized Phase II Trial

Background:?Treatment of frail elderly patients with pancreatic cancer is still a major problem due to intolerance to standard chemotherapy doses. Aim:?This study aims to compare the low-dose gemcitabine over 6 hours (LD6H) to the standard gemcitabine protocol in terms of clinical benefit, survival, and safety in the frail elderly patients with advanced pancreatic adenocarcinoma. methods:?Patients enrolled in this trial were randomly assigned by in a 1:1 fashion via closed envelope method to either receive gemcitabine of 1000 mg/m2?over 30-minute infusion on days 1, 8, and 15 of every 4-week cycle (standard protocol arm) or gemcitabine as a weekly low-dose (250 mg/m2) over 6-hour infusion (LD6H arm). Results:?We enrolled eighty-two eligible frail elderly patients with advanced pancreatic cancer. The patients were randomly assigned to receive either standard gemcitabine protocol (40 patients) or low-dose (250 mg/m2) gemcitabine over 6-hour infusion, given weekly (42 patients). There was no significant difference between the standard group and low-dose group as regard of the overall response rate (p = 0.654), the disease control rate (DCR) (p = 0.845), the median progression-free survival (PFS) (p = 0.908) and the overall survival (OS) (p = 0.331). The low-dose regimen had a significantly lower incidence of adverse effects grades 3 or 4 when compared to the standard regimen: (p = 0.024 for fatigue, p = 0.027 for hypotension, p = 0.012 for each anemia as well as thrombocytopenia, and p = 0.006 for neutropenia). Conclusion:?Low-dose gemcitabine over 6-hour infusion is equally effective and less toxic when compared to standard gemcitabine protocol in frail elderly patients with advanced pancreatic adenocarcinoma. So, we recommend the low-dose gemcitabine for frail elderly patients with advanced pancreatic cancer.

Agro-biodiversity and Traditional Knowledge on Tunisian Coastal Oases

The Tunisian coastal oases constitute a model of very rich plant biodiversity, organized in different floors of cultivation. The date palm is the highest, the second is the diversified fruit trees and the smallest is composed by various other productions. The present paper is the results of a RAA diagnosis and bibliographic analysis of nearly thirty years of work on oases. The aim of this study is to focus on the plant diversity and the traditions in using different products. The survey revealed the existence of nearly 45 local date palm varieties, about 26 fruit trees, 65 vegetables and divers other species. It also raises nearly 30 uses of dates and date palm by-products. This survey shows also that oases inhabitants keep in possessing, for a long time, a knowledge to make ingenious concerning use and conservation of the products and by products of the date palm. This survey has all as much demonstrated the risks from weakening this balance which has long been maintained. Some recommendations to maintain and conserve this system are also presented in this paper.

Increasing global energy consumption has become an urgent problem as natural energy sources such as oil,gas,and uranium are rapidly running *** into renewable energy sources such as solar energy is being pursued to counter *** energy is one of the most promising renewable energy sources,as it has the potential to meet the world’s energy needs *** study aims to develop and evaluate artificial intelligence(AI)models for predicting hourly global *** hyperparameters were optimized using the Broyden-FletcherGoldfarb-Shanno(BFGS)quasi-Newton training algorithm and STATISTICA *** from two stations in Algeria with different climatic zones were used to develop the *** error measurements were used to determine the accuracy of the prediction models,including the correlation coefficient,the mean absolute error,and the root mean square error(RmSE).The optimal support vector machine(SVm)model showed exceptional efficiency during the training phase,with a high correlation coefficient(R=0.99)and a low mean absolute error(mAE=26.5741 Wh/m^(2)),as well as an RmSE of 38.7045 Wh/m^(2) across all ***,this study highlights the importance of accurate prediction models in the renewable energy,which can contribute to better energy management and planning.

The article presents a miniaturized monopole antenna dedicated to modern flexible electronic *** antenna combines three fundamental properties in a single ***,it is characterized by a compact size compared to the state-of-the-art literature with an overall size of 18×18×0.254 mm3,secondly,the proposed antenna integrates the reconfigurability function of frequency,produced by means of a Positive-IntrinsicNegative(PIN)diode introduced into the radiating ***,the antenna is able to switch between different frequencies and different modes,making it suitable to meet the ever-changing demands of communication ***,the antenna is equipped by the property of *** fact,a conformability test is performed and has demonstrated the stability of the antenna performance under normal and bending ***,in order to demonstrate the potential of the proposed antenna,a comparison between the simulated and measured results is made and turned out to be a strong agreement,making the antenna an excellent candidate for future miniaturized rigid and conformal devices.

Formulation of a Net Zero Assessment System for Healthcare Facilities

The international efforts to limit climate change are increasing, that during the COP26 convention, reaching Net Zero Greenhouse Gas emissions became part of the global goals for many countries and entities. One of the sectors that holds the responsibility of addressing the impacts of climate change is the healthcare sector, and accordingly, it is also encouraged to take a leading role in maintaining its sustainability and be a role model for the other sectors. Additionally, the buildings sector, through the World Green Building council, has taken the initiative to launch the Net Zero Carbon Buildings Commitment for the sector. Based on these practices, the work presented aims to introduce and formulate an Approaching Net Zero assessment methodology to be integrated as part of the sustainability criteria and rating system for healthcare facilities in developing countries. The methodology planned is based on four different aspects which are: 1) Approaching Net Zero Energy, 2) Approaching Net Zero Wastewater, 3) Approaching Net Zero Solid Waste, and 4) Approaching Net Zero GHG emissions. The facility should have achieved a Green Building certification through any certification body, and then apply for the aspect on which it requires to be assessed. The assessment methodology follows a systematic approach, where a baseline year of normal operation is determined for existing facilities to act as a base of assessment, and normal design practices for new facilities Based on the assessment and proof of performance enhancement, the facility will either be awarded a basic certificate for achieving continuous savings, or additionally be awarded a best performance certificate compared to other facilities in the same category.

Nanoflower ZnO thin-film grown by hydrothermal technique based Schottky diode

This paper reports the realization of planar Schottky diodes based on nanorod ZnO thin *** nanorod ZnO thin film was fabricated by hydrothermal technique on boron doped p-type Si(100)*** Ag//ZnO/Al planar diode operating with voltage bias from–3 to 3 *** I–V characteristics clearly indicate that the devices have rectifying *** thermionic emission theory governs the current across the studied Schottky *** device achieved a turn-on voltage of 0.9 V,barrier height 0.69 e V and saturation current of 1.2×10^(–6)*** diode shows a very large ideality factor(n>>2)which is attributed to high interface trap *** surface topology was investigated by scanning electron microscope(SEm).The structural properties of the nanostructured ZnO thin film were characterized by X-ray diffraction(XRD).The SEm images reveal that the ZnO nanorods grow perpendicular to the substrate with uniformity and high *** XRD pattern illustrates the dominant peak appearing at(002).This intense peak indicates the c-axis orientated phase of the wurtzite ZnO *** demonstrates that the crystals grow uniformly perpendicular to the substrate surface in good agreement with the SEm images.

Essential oils from Elaeoselinum asclepium: Chemical composition, antimicrobial and antioxidant properties

Objective: To evaluate the chemical composition of the essential oil isolated from Elaeoselinum asclepium(L.) Bertol.(E. asclepium), and test the efficiency of the essential oil as an antimicrobial and antioxidant ***: Essential oil was obtained from the aerial parts of E. asclepium by hydro distillation and analyzed by gas chromatography and gas chromatography coupled with mass spectrometry. We study for the first time the chemical composition of the essential oil of E. asclepium, followed by the in vitro antimicrobial activities, which were evaluated by agar diffusion method against six Gram-positive bacteria, five Gram-negative bacteria, and two fungi. In addition, The antioxidant activities were also investigated using assays of 1,1-diphenyl-2-picrylhydrazyl radical scavenging activity and ferricreducing ***: The analyzed essential oil of the aerial parts of E. asclepium was rich in apinene(43.9%), other compounds detected in appreciable amounts were sabinene(27.9%) and b-pinene(16.0%). The essential oil yields 1.2%, the IC50 values of essential oil in 1,1-diphenyl-2-picrylhydrazyl assay in the reducing power assay were 48.26 mg/m L and at 1 mg/m L, respectively. The absorbance value of essential oil at 700 nm was0.956. The antimicrobial effect was higher on Candida albicans ATCC 1024 strain with the inhibition zone 14.5 mm than bacteria and ***: The essential oil of E. asclepium has antimicrobial and antioxidant activities. These species may be used as an important source of natural antimicrobial and antioxidant agents.

Copper Delivery by the Copper Chaperone for Chloroplast and Cytosolic Copper/Zinc- Superoxide Dismutases： Regulation and Unexpected Phenotypes in an Arabidopsis mutant

Copper （Cu） is an important mineral nutrient found in chloroplasts as a cofactor associated with plastocyanin and Cu/Zn superoxide dismutase （Cu/ZnSOD）. Superoxide dismutases are metallo-enzymes found in most oxygenic organisms with proposed roles in reducing oxidative stress. Several recent studies in Arabidopsis have shown that microRNAs and a SQUAmOSA promoter binding protein-like7 （SPL7） transcription factor function to down-regulate the expression of many Cu-proteins, including Cu/ZnSOD in both plastids and the cytosol, during growth on low Cu. Plants contain the Cu Chaperone for SOD （CCS） that delivers Cu to Cu/ZnSODs, and, in Arabidopsis, both cytosolic and plastidic CCS versions are encoded by one gene. In this study, we demonstrate that Arabiclopsis CCS transcript levels are regulated by Cu, mediated by microRNA 398 that was not previously predicted to target CCS. The microRNA target site is conserved in CCS of Oryza sativa. The data suggest that Cu-regulated microRNAs may have more mRNA targets than was previously predicted. A CCS null mutant has no measurable SOD activity in the chloroplast and cytosol, indicating an absolute requirement for CCS. When the CCS null mutant was grown on high Cu media, it lacked both Fe superoxide dismutase （FeSOD） and Cu/ZnSOD activity. However, this did not lead to a visual phenotype and no photosynthetic deficiencies were detected, even after high light stress. These results indicate that Cu/ZnSOD is not a pivotal component of the photosynthetic anti-oxidant system during growth in laboratory conditions.
