Predictive and Prognostic Factors for the Outcome of the Patients Receiving Pegylated Liposomal Doxorubicin for Advanced Breast Cancer

Purpose:?The treatment of advanced breast cancer (ABC) is still challenging aiming mainly to improve or maintain the quality of life. The efficacy of pegylated liposomal doxorubicin (PLD) was proven in patients with ABC.?Because its expensive treatment?there is a great need to find the predictive factors for the clinical outcome of PLD.?Our purpose was to evaluate the factors which?would affect the clinical outcomes in patients receiving PLD for advanced breast cancer. Methods: Retrospectively, we studied the medical records of 60 eligible patients during the period of seven years (Jan.?2011-Dec.?2017).?All patients?were treated in Medical Oncology Department, South Egypt Cancer Institute, Assiut?University,?Egypt. We included only patients with visceral metastasis who received at least 2 cycles of PLD and had radiological assessment after that. Clinical benefit rate of PLD and survival outcome were assessed and correlated with patients and disease?characteristic. Results:?The majority of patients had a performance status grade II (81.7%), recurrent disease (86.7%), more than one metastatic site (83.3%), and chemoresistance to previous anthracycline (75%).?The clinical benefit rate (CBR) to PDL was 30%. We found statistical?significant association between higher CBR and biological subtypes (p??0.001), type of metastatic breast disease (p?=?0.003), chemosensitivity to anthracycline (p??0.001), and the number of previous lines of chemotherapy (p?=?0.041).?The median progression-free survival (PFS) was five months. There was a statistically-significant improvement of PFS among patients with anthracycline-sensitive tumors compared to those with anthracycline-resistant tumors (10 months vs. 5 months, respectively, p?=?0.004). The most common toxicity was palmar-plantar erythrodysesthesia (28% for all grade and 9% for grade 3 or more). Introduction: The treatment of metastatic breast cancer (MBC) is still challenging. Many studies documented the efficacy of pegylated liposomal doxorubicin (PLD) in patients with MBC, but there is a limited data about the cardiac safety with high cumulative dose (HCD) of PLD. Aim of the work: We conducted this trial to outline the cardiac safety of HCD of PLD in patients with MBC who previously received conventional anthracyclines. Methods: During the period of nine years (January 2011 to December 2019). We extracted the data of the patients with MBC receiving PLD at Medical Oncology Department, South Egypt Cancer Institute, Assiut University. These included patients’ demographics and therapeutic data including the full data of PLD, prior conventional anthracyclines, prior trastuzumab, and prior radiotherapy. Primary Debulking Surgery for Stage III Epithelial Ovarian Cancer Has a Better Outcome Than Neoadjuvant Chemotherapy Followed by Interval Debulking Surgery

Background:?Ovarian cancer (OC) is the most lethal gynecologic malignancy. About 70% of ovarian cancer patients have advanced disease and often not totally resectable. Previous studies of neoadjuvant chemotherapy (NACT) followed by interval debulking surgery (IDS) versus primary debulking surgery (PDS) give inconsistent results. The aim of this retrospective study is to evaluate the outcome of the neoadjuvant chemotherapy followed by IDS versus PDS followed by adjuvant chemotherapy for the International Federation of Gynecology and Obstetrics (FIGO) stage III epithelial ovarian cancer.?Methods:?This study was performed on eligible patients with ovarian cancer admitted in Surgical Oncology and Medical Oncology departments at South Egypt Cancer Institute-Assiut University in the period from January 2010 to December 2015. Patients were divided into two groups, the first group included those who have NACT and underwent IDS plus adjuvant chemotherapy, and the second group included those who underwent PDS followed by adjuvant chemotherapy.?Results:?This study included 380 cases. One hundred and fifty-four patients (40.53%) had IDS. The remaining two hundred and twenty-six patients (59.47%) underwent PDS. In this study, treatment modality was significant for both disease-free survival (DFS) and overall survival (OS). DFS was significantly reduced after IDS when compared to PDS (median DFS: 33.00 months vs. 45.00 months, respectively;p??0.001). Also, OS was significantly reduced after IDS when compared to PDS. (Median OS: 43 months vs. 46 months, respectively;p = 0.047). Moreover, this drop of the survivals mainly occurred in specific subgroups such as the elderly patients, patients with bad performance status, suboptimal cytoreduction, as well as high-grade tumors. Background:?Treatment of frail elderly patients with pancreatic cancer is still a major problem due to intolerance to standard chemotherapy doses. Aim:?This study aims to compare the low-dose gemcitabine over 6 hours (LD6H) to the standard gemcitabine protocol in terms of clinical benefit, survival, and safety in the frail elderly patients with advanced pancreatic adenocarcinoma. Methods:?Patients enrolled in this trial were randomly assigned by in a 1:1 fashion via closed envelope method to either receive gemcitabine of 1000 mg/m2?over 30-minute infusion on days 1, 8, and 15 of every 4-week cycle (standard protocol arm) or gemcitabine as a weekly low-dose (250 mg/m2) over 6-hour infusion (LD6H arm). Results:?We enrolled eighty-two eligible frail elderly patients with advanced pancreatic cancer. The patients were randomly assigned to receive either standard gemcitabine protocol (40 patients) or low-dose (250 mg/m2) gemcitabine over 6-hour infusion, given weekly (42 patients). There was no significant difference between the standard group and low-dose group as regard of the overall response rate (p = 0.654), the disease control rate (DCR) (p = 0.845), the median progression-free survival (PFS) (p = 0.908) and the overall survival (OS) (p = 0.331). The low-dose regimen had a significantly lower incidence of adverse effects grades 3 or 4 when compared to the standard regimen: (p = 0.024 for fatigue, p = 0.027 for hypotension, p = 0.012 for each anemia as well as thrombocytopenia, and p = 0.006 for neutropenia). Conclusion:?Low-dose gemcitabine over 6-hour infusion is equally effective and less toxic when compared to standard gemcitabine protocol in frail elderly patients with advanced pancreatic adenocarcinoma. So, we recommend the low-dose gemcitabine for frail elderly patients with advanced pancreatic cancer.

High moisture content wheat kernels are subject to elevated respiration rates due to enzyme activity and mould growth that reduce the dry grain and may produce suffi-cient energy which may be harmful to wheat *** aeration provides a powerful nonchemical stored grain insect ***,aeration is a suitable and economical device to overcome this *** moisture management is vital to prevent spoilage in stored *** objective of this study is to investigate the influence of using low relative humidity(RH)aeration on the wheat moisture *** numerical investigation based on heat and mass balances is developed and used to simulate the evolution of grain temperature and moisture under various air RH in a wheat storage *** show that the dehumidification of blown air had greater potential for decreasing RH of interstitial air and wheat moisture at 30◦C temperature and RH of 40%,50%and 60%.

Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Geotechnical Study of the Aptian Limestone of the Kef Region,Northwestern Tunisia:Evaluation for Industrial Use

This study aims to the initial characterization of Aptian sedimentary limestones in the Kef region located in the North-West of Tunisia in order to use in industrial fields. The limestone samples were collected from three outcrops respectively named Jebel Jerissa, Jebel Hmeima and Jebel Harraba. A geochemical characterization highlights a variation of the weight percentage (wt%) as follows: CaO (53 - 55), MgO (0.04 - 0.28), Al2O3 (0.07 - 0.51), Fe2O3 (0.41 - 2.87), and a loss on ignition (41.62 - 43.35). The other oxides (K2O, SO3, Na2O) are in trace amounts. Mineralogical analysis revealed that limestones contain more than 95% of calcite and the clay impurities are the minor phases detected. Petrographic study showed that these limestones are packestone-wakestone type. The hardness of Aptian limestones crosses the upper limit of the hard domain. Geotechnical tests reveal a Dry Micro Deval (MDS) coefficient varying from 23% to 33%, a Wet Micro Deval (MDH) coefficient with values oscillating around 26% to 36%, a Los Angeles coefficient (LA) about 25% against a value of the compressive strength ranging from 593 Kg/cm2 to 866 Kg/cm2. The gravimetric tests highlighted a flexural strength value from 106 Kg/cm2 at 208 Kg/cm2, while the ultrasonic coefficient oscillates from 4876 m/s to 5233 m/s, indicating the low porosity of these limestone (0.5% to 1%). The density recorded an average value of 2.50 g/cm3. The various properties studied have proved that the limestone studied can be used in various industrial fields such steel industry, aggregate, cement industry and marble.

Technological Characterization and Industrial Application of Tunisian Clays from Makthar Area (Central Tunisia) in the Ceramic Industry

This study focused on the geochemical, mineralogical and technological characterization of clays in Makthar area (Central of Tunisia) of Cretaceous-Paleogene. Its aims are to identify and promote use in the field of ceramics industry. The result of the mineralogical analysis of clays showed a dominance of illite with a percentage higher than 65%, of kaolinite and smectite with percentages of 15%. Geochemical analysis of the major elements of clay showed a SiO2 content exceeding 29% and a percentage of Al2O3 higher than 7.5%. The Fe2O3 percentage was ranging from 3% to 8%. The percentage of CaO was between 22.5% and 28% while that of K2O is 4%. The percentages of SO4, MgO and NaO2 were in very small fractions. Granulometric and microgranulometric analysis showed that the clay fraction (2/g to 178 m2/g reflecting illite dominance. Drying behavior indicated that clay mixture had a drying shrinkage less than 7%, while the firing shrinkage didn’t exceed 2% giving the possibility of clay using in the ceramics field.
