Early Gallbladder Cancer: Clinical, Morphological, Therapeutic and Evolutionary Aspects

Introduction: The early cancer of gallbladder is an entity which is not well recognized currently. It is a cancer which does not extend beyond the muscularis layer of the gallbladder and it is characterized in almost of cases by the absence of lymph node and visceral invasion. Patients and Method: We have conducted this retrospective study of all our cases of early gallbladder cancer treated in our surgical unit. We have studied these through clinical, morphological, therapeutical and evolutionary aspects. Results: Of 202 gallbladder carcinoma, 33 cancers were classified as early cancer. 25 were females and 8 were males. The mean age was 56.4 years (41 -?70 years). All patients were free of gallbladder cancer symptoms and all except one had normal CEA and CA19.9. 2 patients had synchronous tumors (one colonic cancer and one rectal cancer). For 16 patients, the diagnosis was done by ultrasonography and 17 by histological examination of the specimen removed for biliary lithiasis. 8 patients had PT1a tumor (confined only to mucosa) and 25 had PT1b tumor (tumor infiltration of the muscular layer). For 19 patients who benefited from extensive lymphadenectomy, only one (5.3%) had lymph node infiltration. 16 patients had a simple cholecystectomy and in two cases, the cholecystectomy was associated with bile duct resection. 17 patients had hepatectomy with extensive lymphadenectomy. 2 patients had a simultaneous right colectomy and abdominoperineal resection and another one benefited from choledocal cyst resection. 3 patients benefited from stone removal from bile duct and two had tumor removal from bile duct (ruptured tumor in the bile duct). 1 patient (3.7%) died in postoperative course (hospital mortality). In the follow-up period, 4 patients died from intercurrent causes. Two patients presented a recurrence at 14 and 36 months and died respectively at 19 and 42 months. An operator T is said to be paranormal if ｜｜T^2 x ｜｜〉｜｜Tx｜｜^2 holds for every unit vector x. Several extensions of paranormal operators are considered until now, for example absolute-k-paranormal and p-paranormal introduced in [10], [14], respectively. Yamazaki and Yanagida [38] introduced the class of absolute-（p, r）-paranormal operators as a further generalization of the classes of both absolute-k-paranormal and p-paranormal operators. An operator T ∈ B（h） is called absolute-（p, r）-paranormal operator if ｜｜｜T｜p｜T^＊｜^rx｜｜^r 〉｜｜｜T^＊｜^rx｜｜p＋r for every unit vector x ∈ h and for positive real numbers p 〉 0 and r 〉 0. The famous result of Browder, that self adjoint operators satisfy Browder＇s theorem, is extended to several classes of operators. In this paper we show that for any absolute-（p, r）- paranormal operator T, T satisfies Browder＇s theorem and a-Browder＇s theorem. It is also shown that if E is the Riesz idempotent for a nonzero isolated point μ of the spectrum of a absolute-（p, r）-paranormal operator T, then E is self-adjoint if and only if the null space of T -μ, N（T - μ） N（T^＊ - ^μ）.

Fracture resistance of posterior teeth restored with modern restorative materials

We studied the fracture resistance of maxillary premolars restored with recent restorative materials. Fifty maxillary premolars were divided into five groups： Group 1 were unprepared teeth; Group 2 were teeth prepared without restoration; Group 3 were teeth restored with tetric ceram hB; Group 4 were teeth restored with InTen S; and Group 5 were teeth restored with Admira. The samples were tested using a universal testing machine. Peak loads at fracture were recorded. The teeth restored with Admira had the highest fracture resistance followed by those restored with InTen-S and tetric ceram hB. Prepared, unrestored teeth were the weakest group. There was a significant difference between the fracture resistance of intact teeth and the prepared, unrestored teeth. There was also a significant difference among the tested restorative materials. Teeth restored with Admira showed no significant difference when compared with the unprepared teeth. It was concluded that the teeth restored with Admira exhibited the highest fracture resistance.

Complement proteins in blood recognize charged *** anionic phospholipid(aPL)cardiolipin binds both complement proteins C1q and factor h.C1q is an activator of the complement classical pathway,while factor h is an inhibitor of the alternative *** examine opposing effects of C1q and factor h on complement activation by aPL,we surveyed C1q and factor h binding,and complement activation by aPL,either coated on microtitre plates or in *** C1q and factor h bound to all aPL tested,and competed directly with each other for *** the aPL activated the complement classical pathway,but negligibly the alternative pathway,consistent with accepted roles of C1q and factor ***,in this system,factor h,by competing directly with C1q for binding to aPL,acts as a direct regulator of the complement classical *** regulatory mechanism is distinct from its action on the alternative *** of classical pathway activation by factor h was confirmed by measuring C4 activation by aPL in human sera in which the C1q:factor h molar ratio was adjusted over a wide *** factor h,which is regarded as a down-regulator only of the alternative pathway,has a distinct role in downregulating activation of the classical complement pathway by aPL.A factor h homologue,β2-glycoprotein-1,also strongly inhibits C1q binding to *** globular domains of C1q A,B and C chains bound aPL similarly to native C1q,confirming that C1q binds aPL via its globular heads.

The crystal structure of a new non-centrosymmetric microporous fluorinated iron phosphate, （h30）2[Fe4（h2O）2F4（PO4）2（hPO4）2]（h2O）, was determined by single crystal X-ray diffraction analysis and the result reveals that it belongs to the orthorhombic system with four molecules in the unit cell（space group P212121）. Thus, the complex was characterized by powder X-ray diffraction, spectroscopic techniques（Fourier transform infrared and Fourier transform Raman） and 19F MAS NMR. The elemental analysis of the sample was also carried out. The chiral inorganic sheets, which stacked along [100] showed the presence of FeF2O4 as well as FeF2O3h2O octahedra, PO4 besides hPO4 tetrahedra, hydronium ions（h3O＋） and isolated water molecules. hirshfeld surface analysis, especially dnom surface and fingerprint plots, were used for decoding the intermolecular interactions in the crystal network and the contribution of component units for the construction of the 3D architecture. From the hirshfeld surfaces and 2D fingerprint analysis, it was found that the subtle interactions, such as h...h associating the third intense interaction of all intercontacts, provide extra stabilization in addition to the presence of the strong hydrogen bonds mentioned above.
