Early Gallbladder Cancer: Clinical, Morphological, Therapeutic and Evolutionary Aspects

Introduction: The early cancer of gallbladder is an entity which is not well recognized currently. It is a cancer which does not extend beyond the muscularis layer of the gallbladder and it is characterized in almost of cases by the absence of lymph node and visceral invasion. Patients and Method: We have conducted this retrospective study of all our cases of early gallbladder cancer treated in our surgical unit. We have studied these through clinical, morphological, therapeutical and evolutionary aspects. Results: Of 202 gallbladder carcinoma, 33 cancers were classified as early cancer. 25 were females and 8 were males. The mean age was 56.4 years (41 -?70 years). All patients were free of gallbladder cancer symptoms and all except one had normal CEA and CA19.9. 2 patients had synchronous tumors (one colonic cancer and one rectal cancer). For 16 patients, the diagnosis was done by ultrasonography and 17 by histological examination of the specimen removed for biliary lithiasis. 8 patients had PT1a tumor (confined only to mucosa) and 25 had PT1b tumor (tumor infiltration of the muscular layer). For 19 patients who benefited from extensive lymphadenectomy, only one (5.3%) had lymph node infiltration. 16 patients had a simple cholecystectomy and in two cases, the cholecystectomy was associated with bile duct resection. 17 patients had hepatectomy with extensive lymphadenectomy. 2 patients had a simultaneous right colectomy and abdominoperineal resection and another one benefited from choledocal cyst resection. 3 patients benefited from stone removal from bile duct and two had tumor removal from bile duct (ruptured tumor in the bile duct). 1 patient (3.7%) died in postoperative course (hospital mortality). In the follow-up period, 4 patients died from intercurrent causes. Two patients presented a recurrence at 14 and 36 months and died respectively at 19 and 42 months. One patient presented a bile duct cancer at 66 months. She died at 78 months after palliative treatmen

Contribution of Cytology for the Diagnosis of Gallbladder Carcinoma to Avoid Misdiagnosis in This Commonly Resected Organ

The cytological study of gallbladder bile is a diagnostic tool used very often in the treatment of gallbladder cancer. The diagnosis of gall bladder cancer is done by morphological examinations such as ultrasound, computed tomography in 80% of cases, but in some difficult cases, benign lesions associated with malignant lesions, such as pseudo-tumoral cholecystitis, adenomyosis or adenoma and cancers requiring medical treatment by chemotherapy or radiotherapy, is without the possibility of surgical treatment. Cytology can help resolve a number of difficult situations, such as the presence of an intra-gallbladder lesion in the form of a polyp or nodule. Some studies have reported very interesting results from gallbladder cytology to cancer diagnosis with results up to 85% sensitivity. The purpose of this study is to analyze the contribution of the cytological study of gallbladder bile to the positive diagnosis of gallbladder cancer.

Optimised CNN Architectures for Handwritten Arabic Character Recognition

Handwritten character recognition is considered challenging compared with machine-printed characters due to the different human writing *** is morphologically rich,and its characters have a high *** Arabic language includes 28 *** character has up to four shapes according to its location in the word(at the beginning,middle,end,and isolated).This paper proposed 12 CNN architectures for recognizing handwritten Arabic *** proposed architectures were derived from the popular CNN architectures,such as VGG,ResNet,and Inception,to make them applicable to recognizing character-size *** experimental results on three well-known datasets showed that the proposed architectures significantly enhanced the recognition rate compared to the baseline *** experiments showed that data augmentation improved the models’accuracies on all tested *** proposed model outperformed most of the existing *** best achieved results were 93.05%,98.30%,and 96.88%on the HIJJA,AHCD,and AIA9K datasets.

A Comprehensive Analysis of the Thermo-Chemical Properties of Sudanese Biomass for Sustainable Applications

The chemical composition and thermal properties of natural fibers are the most critical variables that determine the overall properties of the fibers and influence their processing and use in different sustainable applications,such as their conversion into bioenergy and *** thermal and mechanical properties can be estimated by evaluating the content of cellulose,lignin,and other extractives in the *** this research work,the chemical composition and thermal properties of three fibers,namely bagasse,kenaf bast fibers,and cotton stalks,were evaluated to assess their potential utilization in producing biocomposites and bioenergy *** chemical composition analysis followed the Technical Association of the Pulp and Paper Industry Standards(TAPPI)*** total phenol content was quantified using the Folin-Ciocalteu method,while Fourier Transform Infrared Spectroscopy(FTIR)was employed to assess the light absorption by the *** evaluate thermal stability and higher heating values,Thermogravimetric Analysis(TGA),Differential Scanning Calorimetry(DSC),and bomb calorimetry were *** chemical analysis revealed that bagasse contained 50.6%cellulose and 21.6%lignin,kenaf bast fibers had 58.5%cellulose and 10%lignin,and cotton stalks exhibited 40.3%cellulose and 21.3%*** FTIR curves demonstrated a notable similarity among the *** TGA analysis showed degradation temperatures of 321°C for bagasse,354°C for kenaf bast fibers,and 289°C for cotton *** DSC analysis revealed glass transition temperatures of 81°C for bagasse,66.3°C for cotton stalks,and 64.5°C for kenaf bast *** higher heating values were measured as 17.3,16.6 and 17.1 MJ/kg for bagasse,kenaf bast fibers,and cotton stalks,*** three fibers have a high potential for biocomposites and bioenergy material manufacturing.

Comparison between seismic analysis of twisting and regular 52-story towers considering soil-structure interaction

A dynamic analysis of both twisting and regular towers is carried out to determine the results of considering soil-structure interaction(SSI)on high-rise *** addition,the difference between the seismic performance of using twisting towers over regular ones is *** twisting tower is a simulation of the Evolution Tower(Moscow).The towers’skeletons consist of RC elements and rest on a reinforced concrete piled-raft *** soil model is considered as multi-layered with the same soil properties as the zone chosen for the analysis(New Mansoura City,Egypt).The only difference between both towers is their shape in *** whole system is modelled and analyzed in a single step as one full 3D model,which is known as the direct approach in *** analyses are carried out using finite-element software(Midas GTS NX).Dynamic output responses due to three records of seismic loads are proposed and presented in some *** on the results,it is concluded that SSI has a considerable effect on the dynamic response of tall buildings mainly because of the foundation flexibility,as it leads to lengthening the vibration period,increasing the story drift and the base shear for both cases.

Forecasting river flow is crucial for optimal planning,management,and sustainability using freshwater *** machine learning(ML)approaches have been enhanced to improve streamflow *** techniques have been viewed as a viable method for enhancing the accuracy of univariate streamflow estimation when compared to standalone *** researchers have also emphasised using hybrid models to improve forecast ***,this paper conducts an updated literature review of applications of hybrid models in estimating streamflow over the last five years,summarising data preprocessing,univariate machine learning modelling strategy,advantages and disadvantages of standalone ML techniques,hybrid models,and performance *** study focuses on two types of hybrid models:parameter optimisation-based hybrid models(OBH)and hybridisation of parameter optimisation-based and preprocessing-based hybridmodels(HOPH).Overall,this research supports the idea thatmeta-heuristic approaches precisely improveML ***’s also one of the first efforts to comprehensively examine the efficiency of various meta-heuristic approaches(classified into four primary classes)hybridised with ML *** study revealed that previous research applied swarm,evolutionary,physics,and hybrid metaheuristics with 77%,61%,12%,and 12%,***,there is still room for improving OBH and HOPH models by examining different data pre-processing techniques and metaheuristic algorithms.

BACKGROUND Autism spectrum disorder(ASD)is a neurodevelopmental condition characterized by deficits in social communication and repetitive *** profiling has emerged as a valuable tool for understanding the underlying metabolic dysregulations associated with *** To comprehensively explore metabolomic changes in children with ASD,integrating findings from various research articles,reviews,systematic reviews,meta-analyses,case reports,editorials,and a book *** A systematic search was conducted in electronic databases,including PubMed,PubMed Central,Cochrane Library,Embase,Web of Science,CINAHL,Scopus,LISA,and NLM catalog up until January *** criteria encompassed research articles(83),review articles(145),meta-analyses(6),systematic reviews(6),case reports(2),editorials(2),and a book chapter(1)related to metabolomic changes in children with *** criteria were applied to ensure the relevance and quality of included *** The systematic review identified specific metabolites and metabolic pathways showing consistent differences in children with ASD compared to typically developing *** metabolic biomarkers may serve as objective measures to support clinical assessments,improve diagnostic accuracy,and inform personalized treatment *** profiling also offers insights into the metabolic alterations associated with comorbid conditions commonly observed in individuals with *** Integration of metabolomic changes in children with ASD holds promise for enhancing diagnostic accuracy,guiding personalized treatment approaches,monitoring treatment response,and improving *** research is needed to validate findings,establish standardized protocols,and overcome technical challenges in metabolomic *** advancing our understanding of metabolic dysregulations in ASD,clinicians can improve the lives of affected individuals and their families.
