Purpose An important outcome measure of patient care is the impact on the patient’s health-related quality of life(HRQoL).Current ear-specific HRQoL instruments are designed for one diagnosis and emphasize different subdivisions such as symptoms,hearing problems,psychosocial impact,and the need for *** optimal length of the recall period has not been *** these reasons,a new survey is needed that would cover most chronic ear *** A preliminary 24-item survey(EOS-24)was *** adult patients(included n=186)with one of seven different chronic otologic conditions from all university hospitals in Finland were recruited to respond to EOS-24 and the 15D general HRQoL *** recruiting otologists evaluated the severity of the disease and the disability caused by it.A control group was *** on the patients’responses in different diagnosis groups,the items were reduced according to pre-defined *** resulting survey was validated using a thorough statistical *** the relevance and necessity of the original 24 items were thoroughly investigated,leading to the exclusion of 8 items and the modification of *** remaining 16 items were well-balanced between subdivisions and were useful in all seven diagnosis groups,thus constituting the final instrument,*** most suitable recall period was three *** EOS-16 has been created according to the HRQoL survey guidelines with a versatile nationwide patient *** survey has been validated and can be used for a wide range of chronic ear diseases as a HRQoL instrument.

Primary and revision myringoplasty in children: Long-term outcome and analysis of the factors influencing the results

Objective:Assess the long-term outcome of pediatric ***:tympanoplasty type I,myringoplasty,was performed on 85 children(91 consecutive operations,74 primary and 17 revisions)under 16 years of *** perforations were sequela either to acute or chronic inflammatory middle ear *** grafting technique was employed with temporalis *** was performed earlier on all but five *** preoperative observation period exceeded one year for all *** mean follow-up was 5.4(SD 3.6)***:the long-term graft take rate was 84%for primary myringoplasty,and 53%for revision *** re-perforations were associated with postoperative discharge,subtotal or total perforation and revision *** the cohort,sixty ears(66%)were completely healthy(intact tympanic membrane in normal position without adhesions and with good mobility)after *** healing took place in 37%after *** preoperative ventilation tube treatment did not affect the outcome and there were no differences between age *** sonotubometry or Valsalva test results did not correlate with *** ears needed a ventilation tube during the follow-up due to poor *** mean pure tone thresholds improved significantly after ***:Myringoplasty in children is a reliable procedure without age *** year of preoperative observation excludes most unstable ears,and high tendency of spontaneous healing after re-perforation suggests the need for a lengthy follow-up before revision surgery.

Evaluation of pure-tone audiometric protocols in vestibular schwannoma screening

the objective was to evaluate the pure-tone audiogram-based screening protocols in VS *** retrospectively analyzed presenting symptoms,pure tone audiometry and MRI finding from 246 VS patients and 442 controls were collected to test screening protocols(AAO-HNS,AMCLASS-A/B,Charing Cross,Cueva,DOH,Nashville,Oxford,Rule3000,Schlauch,Seattle,Sunderland)for sensitivity and *** were pooled with data from five other studies,and analysis of sensitivity,specificity and positive likelihood ratio(LR+)for each protocol was *** results show that protocols with significantly higher sensitivity(AMCLASS-A/B,Nashville)show also significantly lowest specificity,and tend to have low association(positive likelihood ratio,LR+)to the *** highest LR+was found for protocols AAO-HNS,Rule3000 and *** conclusions,knowing their properties,screening protocols are simple decision-making tools in VS *** use the advantage of the highest sensitivity,protocols AMCLASS-A+B or Nashville can be of *** more reasonable approach,applying the protocols with high LR+(AAO-HNS,Rule3000,Seattle)may reduce the overall number of MRI scans at expense of only few primarily undiagnosed VS.

Why Doesn＇t the Hong Kong Government Sell More Public Land？

Why doesn＇t the Hong Kong government sell more of its enormous land holding to lower the city＇s high housing price and increase the residents＇ small living space？ We answer the question in an overlapping generations framework. We show that while a rapid and complete privatization of government land is efficient in the absence of externalities; it is made politically difficult by a compensation gap, when the losses of current property owners are greater than the government revenue from land sales. We argue that the cross-country diversity of government land ownership owes to historical incidents in some countries （such as the U.S. in the 19th century） that allowed disposal of government land without filling the compensation gap and the absence of such incidents in others （such as Hong Kong）.
