General anesthesia and Obstetric Anesthesia is the gold standard for a cesarean section but there are some cases where general anesthesia is unavoidable. The use of general anesthesia for cesarean delivery has decreased in recent years due to the widespread use of neuraxial techniques. The choice of anesthesia techniques for cesarean delivery depends on several factors, including the patient’s psychology and the attending physician’s experience. It is chosen because of its safety profile and its benefits to the mother and fetus. It may be indicated due to emergency, maternal refusal of regional techniques, or regional contraindications. Major complications include failed intubation, gastric content aspiration, and increased bleeding risk. This study aims to evaluate the impact of a newly launched team on obstetric anesthesia practice.

沉积成因Sr/Ba指示的福建潮控海湾全新世沉积环境及海平面意义

福建沿海多发育潮控型海湾,潮滩平坦宽广,海湾水深较浅,有孔虫等微体生物较为匮乏,且其化石在地层中保存相对较差,致使沉积微相辨识存在一定难度,从而限制了深入理解该区全新世海平面变化过程及沉积环境响应。本研究拟利用不受保存环境限制的沉积成因Sr/Ba指标,探索其在福建宁德三沙湾地区沉积微相的辨识潜力,并结合AMS 14C年龄,探讨该区全新世以来的沉积环境演化及区域海平面变化。结果表明:①沉积成因Sr/Ba在三沙湾潮滩和海湾表层沉积物中平均值分别为9.06和20.43,具有显著差异。潮滩沉积物中的Sr/Ba明显高于长江等大河口地区(1~3),这是该区淡水输入量较少、潮滩盐度较高所致,这说明该指标辨识海陆过渡沉积相时需要考虑区域水文特点。②晚第四纪NDGK2钻孔沉积物Sr/Ba自下而上可分为3层(Ⅰ—Ⅲ):层Ⅰ为杂色硬黏土,Sr/Ba比值全孔最低(均值5.29),但也高于淡水环境,推测为前期海相沉积物的暴露改造;层Ⅱ—Ⅲ均为深灰色黏土,其中层Ⅱ均值为10.77,接近潮滩相;层Ⅲ均值为全孔最高(13.44),接近海湾相。结合AMS 14C年龄可知,NDGK2钻孔的河漫滩-潮滩-海湾沉积相演化受控于全新世海平面上升过程,潮滩相形成于约8.9 ***,当时该区海平面大约在−21.0±2.5 m,最大海泛面出现于约8.2 ***,之后出现了长达6 ka的低沉积速率时期,直至约2.2 ***海湾内加速沉积。③该区15个钻孔全新统底部Sr/Ba均显现出明显的潮滩相特征,这为重建区域全新世相对海平面提供了可行性。

Distribution and Influence of Iron Phases on the Physico-Chemical Properties of Phyllosilicates

Clay minerals from different Cretaceous stratigraphic successions of Egypt were investigated using XRD, DTA, dissolution analysis (DCB), IR, Mssbauer and X\|ray Electron Spin Resonance (ESR) spectroscopes. The purity of the samples and the degree of their structural order were determined by XRD. The location of Fe in the octahedral sheet is characterized by absorption bands at ～875cm\+\{-1\} assigned as Al\|OH\|Fe which persist after chemical dissolution of free iron. The Mssbauer spectra of these clays show two doublets with isomer shift and quadrupole splitting typical of octahedrally coordinated Fe\+\{3+\}, in addition to third doublet with hyperfine parameter typical of Fe\+\{2+\} in the spectra of Abu\|Had kaolinite (H) sample. Six\|lines magnetic hyperfine components which are consistent with those of hematite are confirmed in the spectra of both Isel and Rish kaolinite samples. Goethite was confirmed by both IR and DTA. Multiple nature of ESR of these clays suggested structural Fe in distorted octahedral symmetry as well as non\|structural Fe.\; Little dispersion and low swelling indices as well as incomplete activation of the investigated montmorillonite samples by NaCO\-3 appear to be due to incomplete disaggregation of montmorillonite particles. This can be explained by the ability of Fe\|gel to aggregate the montmorillonite into pseudo\|particles and retard the rigid\|gel structure. However, extraction of this ferric amorphous compound by dithonite treatment recovers the surface properties of the montmorillonite samples.\; On the other hand, the amount and site occupation of Fe associated with kaolinite samples show an inverse correlation with the parameters used to describe the degree of crystallinity perfection, color, brightness and vitrification range of these kaolinite samples.

Intermittent fasting(IF)is an intervention that involves not only dietary modific-ations but also behavioral changes with the main core being a period of fasting alternating with a period of controlled *** duration of fasting differs from one regimen to *** fasting(RF)is a religious fasting for Muslims,it lasts for only one month every one lunar *** this model of fasting,observers abstain from food and water for a period that extends from dawn to *** period of daily fasting is variable(12-18 hours)as Ramadan rotates in all seasons of the ***,longer duration of daily fasting is observed during the *** fact,RF is a peculiar type of *** is a dry IF as no water is allowed during the fasting hours,also there are no calorie restrictions during feeding hours,and the mealtime is exclusively *** three variables of the RF model are believed to have a variable impact on different liver *** was evaluated by different observational and interventional studies among patients with non-alcoholic fatty liver disease and it was associated with improve-ments in anthropometric measures,metabolic profile,and liver biochemistry regardless of the calorie restriction among lean and obese *** situation is rather different for patients with liver *** was associated with adverse events among patients with liver cirrhosis irrespective of the underlying etiology of *** patients developed new ascites,ascites were increased,had higher serum bilirubin levels after Ramadan,and frequently developed hepatic encephalopathy and acute upper gastrointestinal *** complications were higher among patients with Child class B and C cirrhosis,and some fatalities occurred due to *** transplant recipients as a special group of patients,are vulnerable to dehydration,fluctuation in blood immunosuppressive levels,likelihood of deterioration and hence observing RF without special precautions

多类混凝土损伤比强度理论及塑性-损伤模型研究进展与应用

为完善混凝土强度理论和塑性-损伤模型，通过参考岩石损伤比强度理论，根据现有普通混凝土、再生混凝土、轻骨料混凝土和纤维混凝土等多类混凝土多轴强度试验数据，推荐损伤比变量中的五经验系数取值，完善多类混凝土损伤比强度理论并揭示约束混凝土工作原理。分析表明，随着静水压力的增加，混凝土压损伤比将由单轴受压时为1左右线性递减至小于0.5，八面体剪应力先增大后减小，轴向峰值应力提升为某一定值，压损伤比取值减小引发非弹性体积膨胀减小至不变，因而导致混凝土由单轴受压脆性破坏向多轴受压塑性破坏转变，该理论为钢管混凝土柱中发挥混凝土耗能潜力提供理论依据。依据混凝土损伤比强度理论，确定多类混凝土塑性-损伤模型中的三轴强度参数，包括膨胀角、拉压子午线强度比值和二轴等压与单轴抗压强度比值，并建议常温静力荷载下多类混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，常温地震荷载下普通混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，以及火灾升温环境下普通混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，建立约束混凝土三轴塑性-损伤模型。介绍多类混凝土塑性-损伤模型在钢-混凝土组合结构有限元模型中的应用，模型中混凝土采用实体单元而钢管与钢梁采用壳单元，可模拟钢管与混凝土之间的界面滑移与约束作用，当采取增强约束拉筋强柱构造方法时可提升钢管混凝土柱及其结构的承载力、抗震与抗火性能。

Novel model of thermo-magneto-viscoelastic medium with variable thermal conductivity under effect of gravity

The basic equations for a homogeneous and isotropic thermo-magnetoviscoelastic medium are formulated based on three different theories, i.e., the GreenLindsay(G-L) theory, the coupled(CD) theory, and the Lord-Shulman(L-S) theory. The variable thermal conductivity is considered as a linear function of the temperature. Using suitable non-dimensional variables, these basic equations are solved via the eigenvalue approach. The medium is initially assumed to be stress-free and subject to a thermal *** numerical results reveal that the viscosity, the two-temperature parameter, the gravity term, and the magnetic field significantly influence the distribution of the physical quantities of the thermoelastic medium.

Effect of natural pozzolan content on the properties o engineered cementitious composites as repair material

In order to determine the effect of Natural Pozzolan （NP） content on the mechanical properties and durability characteristics on Engineered Cementitious Composites （ECC） as repair material. This study focused on the evaluation of the most factors influencing compatibility between the repair material and the base concrete including mechanicals properties such as, compressive and flexural strengths, elastic modulus, capillary absorption and drying shrinkage. The experimental results showed that natural pozzolan reduces the compressive strength and the flexuraI strength of ECC at all ages. The elastic modulus of ECC was remarkably lower than that of normal-strength concrete. This lower ~oung＇s modulus is desirable for repair concrete, because it prevents the stresses induced by restrained shrinkage. In addition, the incorporation of high-volume natural pozzolan decreases significantly the coefficient of capillary absorption at long term and increases the drying shrinkage. Generally, based on the results obtained in the present experimental investigation, ECC can be used effectively as an overlay material over existing parent concrete.

Diabetic Macroangioapathy at the Internal Medicine Department of the Abass Ndao Hospital Center (About 359 Cases)

Introduction: Macroangiopathy plays an important role, with a high prevalence of morbidity and mortality in diabetic patients. The aim was to study the epidemiological, clinical, paraclinical, therapeutic and evolutionary profile of macroangiopathy in diabetic patients in the internal medicine department of the Abass Ndao hospital. Patients and methods: This was a descriptive and analytical cross-sectional study. Our investigations were recruited over a 7-year period (January 1, 2016 to December 31, 2022). Results: Three hundred and fifty-nine (359) patients (10.22%) were enrolled. The mean age was 62.83 years, with extremes ranging from 17 to 98 years. The [60 - 69] age group was more representative (37.32%). Women accounted for 180 cases (50.1%), with a sex ratio (m/f) of 0.99. The average duration of diabetes was 11.86 years. Average consultation time was 38.07 days, with extremes ranging from 1 to 368 days. Average hospital stay was 7.65 days. Inaugural diabetes was noted in 12 cases (3.34%). Type 2 diabetes accounted for 95.82% (n = 344) of patients. Hypertension was present in 150 patients (41.8%). Patients with 2 risk factors accounted for 173 cases (48.18%). Nineteen patients had already had a stroke (5.29% of cases). Fourteen (14) patients (4.2%) were amputees. Obliterative arteriopathy of the lower limbs (AOMI) was noted in 193 patients (54%). Stroke was noted in 101 patients (28%). Ischemic heart disease (IHD) was noted in 38 patients (11%). AOMI was more common in males (110 patients, 57%) than in females (43%). Seventy-three (73) patients (20.3%) died. Predictors of death were age over 60 and the existence of more than two cardiovascular risk factors. Conclusion: Diabetic macroangiopathy is a major cause of morbidity and mortality. The development and implementation of a prevention and management program is essential.

Spatial point-pattern analysis as a powerful tool in identifying pattern-process relationships in plant ecology: an updated review

Background:Ecological processes such as seedling establishment,biotic interactions,and mortality can leave footprints on species spatial structure that can be detectable through spatial point-pattern analysis(SPPA).Being widely used in plant ecology,SPPA is increasingly carried out to describe biotic interactions and interpret patternprocess ***,some aspects are still subjected to a non-negligible debate such as required sample size(in terms of the number of points and plot area),the link between the low number of points and frequently observed random(or independent)patterns,and relating patterns to *** this paper,an overview of SPPA is given based on rich and updated literature providing guidance for ecologists(especially beginners)on summary statistics,uni-/bi-/multivariate analysis,unmarked/marked analysis,types of marks,*** ambiguities in SPPA are also ***:SPPA has a long history in plant ecology and is based on a large set of summary statistics aiming to describe species spatial *** mechanisms known to be responsible for species spatial patterns are actually investigated in different biomes and for different *** processes,plant environmental conditions,and human intervention are interrelated and are key drivers of plant spatial *** spite of being not recommended,small sample sizes are more common in *** some areas,periodic forest inventories and permanent plots are scarce although they are key tools for spatial data availability and plant dynamic ***:The spatial position of plants is an interesting source of information that helps to make hypotheses about processes responsible for plant spatial *** the continuous progress of SPPA,some ambiguities require further clarifications.
