It is largely unknown whether lower urinary tract symptoms(LUTS)or acute retention of urine(AROU)is linked to shorter life expectancy in *** conducted a multicenter,retrospective database analysis of patients undergoing transurethral resection of prostate(TURP)to study their *** Cox regression analysis and Kaplan-Meier analysis with stratification to age and indication of TURP were *** further performed an age-and sex-matched survival analysis with the general population using data from the Census and Statistics Department of the Hong Kong Special Administrative Region(Hong Kong,China).From January 2002 to December 2012,3496 patients undergoing TURP were included in our study,with 1764 patients in the LUTS group and 1732 patients in the AROU *** age,ischemic heart disease,cerebrovascular accident,and AROU were risk factors of *** aged80 *** to the general population,younger patients in the LUTS group appeared to have better survival(nger patients in the AROU group had worse survival(80 years,survival was similar with the general population in both the LUTS and AROU *** conclusion,AROU at young age was associated with mortality,while early detection and management of LUTS may improve survival.

Background:Right ventricular pacing disrupts atrioventricular synchrony and increases the risk of atrial fi brillation(AF).However,whether algorithms for minimizing ventricular pacing reduce the incidence of AF remains ***,we conducted a systematic review and meta-analysis to compare the incidence of AF between minimizing ventricular pacing and conventional pacing protocols in patients with pacemakers ***:The PubMed,Embase,and Cochrane Library databases were searched up to August 1,2017,for randomized controlled trials that reported the incidence of AF in patients with and without the use of algorithms for minimizing ventricular ***:Eleven studies comprising 5705 participants(61%males,mean age 71 years[standard deviation 11 years])were fi nally included in the *** mean follow-up duration was 24 *** of algorithms for minimizing ventricular pacing signifi cantly reduced the incidence of AF,with an odds ratio of 0.74(95%confi dence interval 0.55–1.00;Png studies(I 2=63%).Conclusions:The incidence of AF was reduced by 26%with use of algorithms for minimizing ventricular *** incorporation of such algorithms in routine clinical practice should in theory lead to a decrease in AF-related morbidity and mortality.

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

＜正＞ The shear modulus at very small strains （＜ 0.001%） is an important parameter for predicting ground movements of many geotechnical structures. Recent advances in laboratory testing enable the evaluation of shear stiffness anisotropy of soils. While most studies of stiffness anisotropy have been conducted on sedimentary soils and clean sands, the anisotropic stiffness of weathered material has not yet been fully understood. In this paper, the degree of stiffness anisotropy of a completely decomposed tuff （CDT） from Hong Kong was evaluated through muhi - directional measurements of shear wave velocity using bender elements. The CDT exhibits stiffness anisotropy, with shear modulus in the horizontal plane, Ghh , consistently higher than the shear modulus in the vertical plane, Ghh , by about 48 % .

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Material utilization is mainly concerned by industrialpractitioners since it contributes to minimize the totalproduction costs for garment manufacturing. Apart fromthe marker loss, splice loss (the fabric loss during thejoining of different fabric rolls) is one of the primarycauses of fabric loss, but it has not been considered spe-cifically in the past literatures. The amount of splice lossduring fabric spreading is governed by various factors in-cluding the marker length, the number of fabric rolls,and the number of splicing intervals for laying-up. Inour study, these three effects on splice loss during fabricspreading were computed and analysed.

Escherichia coli has seldom been reported to cause pseudobacteremia. The investigation of an outbreak of amoxycillin clavulanic acid resistant E. coli pseudobacteremia is described. Seventeen cases occurred over a five day period. The source of the E. coli was traced to the blood culture specimen of a patient (index patient) with genuine bacteremia as a result of urinary tract infection. The other 16 case patients had pseudobacteremia which was found to be the result of cross contamination during subculture of blood specimens. The E. coli strain was carried over from the culture bottle of the index patient, through the contaminated gloved hands of a technician to the culture bottles of the other 16 cases. Although the pseudobacteremia occurred over a five day period, they all resulted from cross contamination during blood culture processing within one day. An early outbreak investigation was prompted by the unusual finding of amoxycillin clavulanic acid resistance in the case E. coli isolates in a short period. The relatedness of the E. coli strains from the 17 cases was confirmed by arbitrary primed polymerase chain reaction. Clinicians should be alerted to the possibility of a blood E. coli isolate being a contaminant despite its predominant role as a true pathogen.

AIM:To explore the effectiveness of acupuncture transcutaneous electrical nerve stimulation(Acu-TENS), a non-invasive modality in reduction of rectal discomfort during barostat-induced rectal distension. METHODS:Forty healthy subjects were randomized to receive 45 min of either Acu-TENS or placebo-TENS(no electrical output)over acupuncture points Hegu(largeintestine 4),Neiguan(pericardium 6)and Zusanli(stomach 36).A balloon catheter attached to a dual-drive barostat machine was then inserted into the subjects'rectum.A step-wise(4 mmHg)increase in balloon pressure was induced until maximal tolerable or 48 *** analogue scale and a 5-point subjective discomfort scale(no perception,first per-ception of distension,urge to defecate,discomfort/ pain and extreme pain)were used to assess rectal discomfort at each distension *** beta-endorphin levels were measured before,immediately after intervention,at 24 mmHg and at maximal toler- able distension pressure. RESULTS:There was no difference in the demographic data and baseline plasma beta-endorphin levels between the two *** threshold levels were higher in the Acu-TENS group when compared to the placebo group,but the difference reached statistical significance only at the sensations"urge to defecate"and"pain".The distension pressures recorded at the"urge to defecate"sensation for the Acu-TENS and placebo-TENS groups were 28.0±4.5 mmHg and 24.6±5.7 mmHg,respectively(P=0.043);and the pressures recorded for the"pain"sensation for these two groups were 36.0±4.2 mmHg and 30.5± 4.3 mmHg respectively(P=0.002).Compared to the placebo group,a higher number of participants in the Acu-TENS group tolerated higher distension pressures (>40 mmHg)(65%in Acu-TENS vs 25%in placebo, P=0.02).The plasma beta-endorphin levels of the Acu-TENS group were significantly higher than that of the placebo group at barostat inflation pressure of 24 mmHg(1.31±0.40 ng/mL vs 1.04±0.43 ng/mL,P= 0.044)and at maximal inflation p
