A Comparative Study of Effect of Intravenous Lidocaine Infusion, Gabapentin and Their Combination on Postoperative Analgesia after Thyroid Surgery

Objective: This prospective randomized study aimed to evaluate the effect of IV lidocaine infusion or gabapentin and their combination in providing efficient analgesia after thyroid surgery. Methods: Eighty-eight patients scheduled for thyroidectomy were randomized into four equal groups (n = 22). Group P (placebo) patients received placebo capsules 1 h preoperatively and intravenous (IV) saline infusion. Group L (Lidocaine): patients received placebo capsules 1 h preoperatively and IV bolus lidocaine 1.0 mg/kg followed by infusion 2 mg/kg/h. Group G (Gabapentin) patients received 600 mg gabapentin capsules 1 h preoperatively and IV saline infusion. Group LG (Lidocaine-Gabapentin) patients received 600 mg gabapentin capsules 1 h preoperatively and IV bolus of 1.0 mg/kg lidocaine followed by infusion 2 mg/kg/h. (lidocaine or saline infusion started before induction of anesthesia and continued until the end of surgery). Intraoperative fentanyl consumption and hemodynamic changes were recorded. Postoperative total tramadol consumption, time to first analgesic request, visual analog scale (VAS) of pain, sedation level, and side effects were assessed for 24 hours. Results: (LG) group had significant lower intraoperative fentanyl and lower postoperative tramadol consumption (p < 0.001) compared to (P), (L) and (G) groups, with prolonged time of first analgesic request (p < 0.001) compared to (P) and (L) groups, and lower VAS compared to other groups (p < 0.001 or p < 0.01). There was significantly lower postoperative nausea and vomiting (PONV) in G & LG groups compared to (P) group (p < 0.01). Conclusion: The combination of preoperative gabapentin and intraoperative lidocaine infusion provided more analgesic effect than either drug alone with lower and more delayed postoperative analgesic requirements and lower VAS. (PONV) was lower in groups received gabapentin.

Fixed Point Theorem of{a, b, c}Contraction and Nonexpansive Type Mappings in Weakly Cauchy Normed Spaces

Let C be a closed convex weakly Cauchy subset of a normed space X. Then we define a new {a,b,c} type nonexpansive and {a,b,c} type contraction mapping T from C into C. These types of mappings will be denoted respectively by {a,b,c}-ntype and {a,b,c}-ctype. We proved the following： 1. If T is {a,b,c}-ntype mapping, then inf{ ｜｜ T（x）-x｜｜：x C C} =0, accordingly T has a unique fixed point. Moreover, any sequence {Xn}n∈NN in C with limn→∞｜｜T（xn） - Xn｜｜ = 0 has a subsequence strongly convergent to the unique fixed point of T. 2. If T is {a,b,c}-ctype mapping, then T has a unique fixed point. Moreover, for any x∈C the sequence of iterates {Tn （x）}n∈N has subsequence strongly convergent to the unique fixed point of T. This paper extends and generalizes some of the results given in [2,4, 7] and [13].

Comparison between Four Types of Buffers for DNA Agarose Gel Electrophoresis

To detect the genome of viruses (in environmental and clinical samples), we use electrophoresis running buffer after PCR reaction. Also, electrophoresis buffers were used widely to separate any DNA molecule. In this paper, we used four types of previously known electrophoresis buffers to compare which is easy for preparation, simple in structure, low cost and good performance in agarose gel electrophoresis. For this, we used two agarose concentration (1%, 2%) and two types of DNA ladder (100 bp, 1 kb) represent both smaller and larger sizes of molecule for each type of buffers, from the result we found in first level both supper buffer and TAE buffer with good performance and in second level we found bicarbonate buffer also with good performance also. Finally, we found the tang buffer cannot pose any electrophoretic activity on DNA agarose gel electrophoresis.

Impact of topical nonsteroidal anti-inflammatory drugs in prevention of macular edema following cataract surgery in diabetic patients

AIM： To evaluate the efficacy of prophylactic administration of topical non-steroidal anti-inflammatory drugs（NSAIDs） on macular edema following cataract surgery in diabetic patients, and to compare between types of NSAIDs（ketorolac tromethamine 0.4% and nepafenac 0.1%）. METHODS： Group 1（control） received artificial tears substitute as a placebo group, group 2（nepafenac） received topical nepafenac 0.1%, and group 3（ketorolac） received topical ketorolac tromethamine 0.4%. Patients were examined postoperatively after completing one week, one month, two months and three months＇ intervals for evaluating cystoid macular edema（CME） development. The main study outcomes were achieving the best corrected visual acuity（BCVA） and change in the central macular thickness（CMT） measured with optical coherence topography（OCT）.RESULTS： Eighty eyes of 76 patients were included in this study. BCVA showed a statistically significant difference at the third month postoperative follow up between the control group and the NSAIDs groups（P=0.04）. There was an increase in the CMT in all cases starting from postoperative first week until third month. CMT showed a statistically significant difference between control group and NSAIDs groups from postoperative first month until third month（P=0.008, 0.027, 0.004）. There was no statistically significant difference between nepafenac and ketorolac groups in BCVA and OCT CMT. CONCLUSION： Prophylactic preoperative and postoperative NSAIDs may have a role in reducing the frequency and severity of CME in diabetic eyes following cataract surgery.

Finding neoepitopes in mouse models of personalized cancer immunotherapy

BACKGROUND： Cancer immunotherapy uses one＇s own immune system to fight cancerous cells. As immune system is hard- wired to distinguish self and non-self, cancer immunotherapy is predicted to target cancerous cells specifically, therefore is less toxic than chemotherapy and radiation therapy, two major treatments for cancer. Cancer immunologists have spent decades to search for the specific targets in cancerous cells. METHODS： Due to the recent advances in high throughput sequencing and bioinformatics, evidence has merged that the neoantigens in cancerous cells are probably the cancer-specific targets that lead to the destruction of cancer. We will review the transplantable murine tumor models for cancer immunotherapy and the bioinformatics tools used to navigate mouse genome to identify tumor-rejecting neoantigens. RESULTS： Several groups have independently identified point mutations that can be recognized by T cells of host immune system. It is consistent with the note that the formation ofpeptide-MHC I-TCR complex is critical to activate T cells. Both anchor residue and TCR-facing residue mutations have been reported. While TCR-facing residue mutations may directly activate specific T cells, anchor residue mutations improve the binding of peptides to MHC I molecules, which increases the presentation of peptides and the T cell activation indirectly. CONCLUSIONS： Our work indicates that the affinity of neoepitopes for MHC I is not a predictor for anti-tumor immune responses in mice. Instead differential agretopic index （DAI）, the numerical difference of epitope-MHC I affinities between the mutated and un-mutated sequences is a significant predictor. A similar bioinformatics pipeline has been developed to generate personalized vaccines to treat human ovarian cancer in a Phase I clinical trial.

The need for the bacterial proteinase is rapidly growing,urging to catch a lowcost medium for the microbial fermentation,nanoparticles can play a vital role in this *** proteinase of Talaromyces purpureogenus was produced on the tubers of Helianthus tuberosus that also operated as solid support for the fermentation *** interface amongst nitrogen sources(NH_(4)Cl and yeast extract)was investigated,applying the statistical modeling of central composite design under solid-state *** optimum medium for proteinase secretion was stimulated by 979.82 mg NH_(4)Cl and 437.68 mg yeast extract per 100 g substrate,yielding 108.15 U/g *** Plackett-Burman experimental design,the nanoparticles Co,Ni and Fe were assessed as inducers for proteinase *** nanoparticles(5 ppm)were the greatest in both proteinase production by the fungus as well as an inducer of the proteolysis process by the enzyme when using faba bean straw as a proteinaceous substrate in the reaction mixture,liberating the extreme quantity of amino acids,compared with the lack of the *** findings suggest the incorporation of Co nanoparticles in both the proteinase fabrication process and during the degradation of proteinaceous materials induce proteinase *** approach could be extended to modulate the productivity and activity of similar biomolecules.

Tsunami hazard in the shorelines of Khark island (Persian Gulf),Iran

Iran is located in one of the seismically active regions of the world. Due to the high probability of earthquakes throughout the country and the potential for tsunami inundation along the coasts and offshore, comprehensive studies on theinteraction of these natural phenomena arc necessary, hi this study, the most conservative scenarios are determined for possible earthquakes within the Khark zone （Persian Gulf） *** on experimental relations between tile fault length, magnitude and displacement, which are paralneters for deter- mining tsunamigenic sources. Subsequently, the maximum height of tstmami waves are calculated based on the specitieations of the seismic source and its distance from the shore as well as the coastal slope. A zoning map of tsunami hazard is finally presented.
