Trabeculectomy with large area mitomycin-C application as a first-line treatment in advanced glaucoma: retrospective review

AIM: To evaluate the outcomes of trabeculectomy with large area mitomycin-C(MMC) application as a first line treatment in advanced glaucoma. ·METHODS: The records of 55 patients with severe visual field defects undergoing trabeculectomy were retrospectively reviewed. The patients were classified as first-line therapy to either early trabeculectomy(initial trabeculectomy-Group 1) or long term medical therapy followed by trabeculectomy(primary trabeculectomy-Group 2). Trabeculectomy was performed with large-area MMC application. Intraocular pressure(IOP) values,visual acuities,mean deviations,morphology and function of the blebs,necessity for anti-glaucomatous medications and surgical complications were reported. ·RESULTS: There were 20 eyes of 18 patients in Group 1 and 37 eyes of 37 patients in Group 2. The mean preoperative IOPs in Groups 1 and 2 were 40.2±10.0mmHg(27-68mmHg) and 29.0±4.4mmHg(21-41mmHg),respectively(P=0.001). Average preoperative mean deviations(MD) in Groups 1 and 2 were 17.4±2.8dB(13.3-23dB) and 17.9± 2.4dB(13.7-23.2dB),respectively(P=0.441). Postoperative IOPs significantly decreased and were comparable in both Groups. The mean number of medications was significantly higher in Group 2(P =0.005). No cystic bleb formation was observed in Group 1,whereas 4 patients from Group 2(10.8%) developed cystic bleb(P =0.040). No visually devastating complication has occurred in both Groups. ·CONCLUSION: Initial trabeculectomy with large areaMMC application might be applied in patients with advanced glaucoma with low complication rates. Long-term topically applied anti-glaucomatous medications seem to increase the risk of cystic bleb formation.

Toll-like receptors in pathophysiology of liver diseases

Toll-like receptors(TLRs) are pattern recognition receptors that participate in host defense by recognizing pathogen-associated molecular patterns alongside inflammatory processes by recognizing damage associated molecular patterns. Given constant exposure to pathogens from gut, strict control of TLR-associated signaling pathways is essential in the liver, which otherwise may lead to inappropriate production of pro-inflammatory cytokines and interferons and may generate a predisposition to several autoimmune and chronic inflammatory diseases. The liver is considered to be a site of tolerance induction rather than immunity induction, with specificity in hepatic cell functions and distribution of TLR. Recent data emphasize significant contribution of TLR signaling in chronic liver diseases via complex immune responses mediating hepatocyte(i.e., hepatocellular injury and regeneration) or hepatic stellate cell(i.e., fibrosis and cirrhosis) inflammatory or immune pathologies. Herein, we review the available data on TLR signaling, hepatic expression of TLRs and associated ligands, as well as the contribution of TLRs to the pathophysiology of hepatic diseases.

Effect of Helicobacter pylori eradication on serum ghrelin and obestatin levels

AIM: To investigate changes in serum ghrelin and obestatin levels before and after Helicobacter pylori (H. pylori ) eradication. METHODS: A total of 92 patients presenting with symptoms of dyspepsia were enrolled in the study. Upper endoscopy was performed on all patients and used to diagnose H. pylori infection according to the presence of characteristic histopathological findings; seventy patients were diagnosed with H. pylori infection and the remaining 22 non-infected patients were classified as healthy controls. H. pylori eradication was accomplished by administering the classical triple therapy drug regimen, consisting of lansoprazole 30 mg bid , amoxicillin 1 g bid , and clarithromycin 500 mg tid for 14 d. The eradication of H. pylori was assessed with C14-urea breath test, which was performed at eight weeks after treatment. Levels of serum active ghrelin and obestatin were assessed at beginning of the study (prior to treatment) and after eight weeks. The levels were comparatively analyzed between the H. pylori negative control group, the H. pylori eradicated group, and the H. pylori non-eradicated group. RESULTS: A total of 92 patients, 50 females and 42 males with a mean age of 38.2 ± 11.9 years (range: 19-64), were analyzed. H. pylori eradication success was achieved in 74.3% (52/70) of H. pylori positive patients. The initial levels of ghrelin in the H. pylori positive and control cases were 63.6 ± 19.8 pg/mL and 65.1 ± 19.2 pg/mL (P=0.78), respectively, and initial obestatin levels were 771±427 pg/mL and 830 ± 296 pg/mL (P=0.19), respectively. The difference between the initial levels and the week 8 levels of ghrelin and obestatin in the control group was insignificant [4.5% (P=0.30) and -0.9% (P=0.65), respectively]. The difference between the initial and week 8 levels of ghrelin and obestatin in the H. pylori non-eradicated group were also insignificant [0.9% (P=0.64) and 5.3% (P=0.32), respectively]. Porous Silicon： Volume-Specific Surface Area Determination from AFM Measurement Data

Porous silicon layers manufactured by using (100), 1-5 ohm·cm p-type (boron doped) wafer by electrochemical etching in HF etanol solution. Photoluminescence (PL) spectra of anodically etched silicon obtained for different conditions studied and surface characteristics are investigated by AFM. This study gives a simple way to determine specific surface are of porous silicon which plays a major role with porosity for explaining the blue shift in photoluminescence peak. Properties such as specific surface area, pore size, and pore size distribution, the main surface properties of layer are investigated from AFM data which are important material characteristics in many processing applications. The "specific surface area" (Sspecific) generally defined as the area of solid surface per unit mass of material, solid volume or cross section area. From 3-D reconstructions of AFM data, the surface area and the volume of the porous layer can be estimated directly and volume-surface specific area is calculated. For porous silicon this feature can be defined as the total surface area per volume and given by the unit m2/cm3. The method is simple not need to construct a special set up for measurement and non destructive.

Enhancement of galena-potassium ethyl xanthate flotation system by low power ultrasound

The objective of this study is to investigate the improvement possibilities of the floatability of galena with ultrasonic application in the presence of potassium ethyl xanthate(KEX). For this purpose, micro-flotation experiments were carried out in addition to surface chemistry studies including zeta potential, contact angle, and bubble-particle attachment time measurements at various ultrasonic power levels and conditioning time. The results showed that, the maximum micro-flotation recovery of 77.5% was obtained with 30 W ultrasound power and 2 min conditioning time. In addition, more negative zeta potential values were obtained with ultrasound as well as higher contact angle and lower bubble-particle attachment time, which indicated the increased hydrophobicity of galena with ultrasound.

Matched-pair analysis of postoperative morbidity and mortality for pancreaticogastrostomy and pancreaticojejunostomy using mattress sutures in soft pancreatic tissue remnants

Effect of body mass index on functional outcomes following arthroplasty procedures

AIM: To evaluate the body mass index(BMI) change in arthroplasty patients and its impact on the patients' functional results. METHODS: Between October 2010 and May 2013, 606 patients who were operated due to gonarthrosis, coxarthrosis, aseptic loosening of the total knee and hip prosthesis were evaluated prospectively. Patients were operated by three surgeons in three medical centers. Patients who were between 30-90 years of age and who were underwent total knee arthroplasty, total hip arthroplasty, revision knee arthroplasty, or revision hip arthroplasty were included in the study. We excluded the patients who cannot tolerate our standard postoperative rehabilitation program. Additionally, patients who had systemic inflammatory diseases, diabetes mellitus, or endochrinopathies were excluded from the study. The remaining 513 patients comprised our study group. Preoperative functional joint scores, height, weightand BMI of all patients were recorded. We used the Knee Society Score(KSS) for knee and Harris Hip Score(HHS) for hip patients. Postoperative functional scores were measured at 1^(st), 6^(th) and 12^(th) months and recorded separately at outpatient visits. RESULTS: The mean age of the patients was 64.7(range: 30-90) years(207 males/306 females) and the mean follow-up duration was 14.3(range: 12-26) mo. We found that arthroplasty patients had weight gain and had an increase in BMI at the postoperative 1^(st), 6^(th) and 12^(th) months. The mean BMI of the patients was 27.7 preoperatively, 27.8 at the postoperative 1^(st) month, 28.1 at the 6^(th) month and 28.6 at the 12^(th) month(P < 0.01). At the last visit, the mean postoperative HHS of the hip arthroplasty patients was 82.2 ± 7.12(preoperatively, 52.3; 1st month, 78.2; 6^(th) month, 81.1; 12^(th) month, 82.2), and the mean KSS of the knee arthroplasty patients was 79.3 ± 4.31(preoperatively, 35.8; 1^(st) month, 75.2; 6^(th) month, 79.1; 12^(th) month, 79.3). On the Voltage and Frequency Distribution of Dielectric Properties and ac Electrical Conductivity in Al/SiO2/p-Si（MOS）Capacitors

An Al/SiO2/p-Si（MOS） capacitor with a thick（826 A） interfacial oxide layer（SiO2） which is formed by using the thermal oxidation method is fabricated to investigate both frequency and applied bias voltage dependences of real and imaginary parts of dielectric constant（e' and e "） and electric modulus（M' and M"）,loss tangent（tan δ）and ac electrical conductivity(σac) in a wide frequency range from 1 kHz to 1 MHz at room *** dielectric properties of the MOS capacitor are obtained using the forward and reverse bias capacitance-voltage（CV） and conductance-voltage（G/ω-V） measurements in the applied bias voltage range 1.4-5.6 *** values of ε',ε ",tan δ,M',M" and σacare found to be strong functions of frequency and applied bias voltage in the depletion region due to excess capacitance Cexand conductance Gex/ω especially at low *** experimental results show that the interfacial polarization can occur at low frequencies more easily,consequently contributing to the dispersion in ε',ε ",tan δ,M',M" and σacvalues of the MOS *** other reason for dispersion in the dielectric properties may be attributed to a particular density distribution of interface states(Nss) localized at the Si/SiO2interface,as well as space charge carriers and inhomogeneity of interfacial oxide *** increase in conductivity with increasing frequency can be attributed to the hopping type conduction *** can be concluded that the ε',ε ",tan δ,M',M" and σacvalues of the Al/SiO2/p-Si（MOS） capacitor are strongly dependent on both the frequency and applied bias voltage especially in the depletion region.

Myxoinflammatory fibroblastic sarcoma(MIFS) is a rare low-grade, malignant soft tissue tumor that is usually observed in the extremities of adult patients. Magnetic resonance imaging findings for this tumor type have rarely been reported. We report a case involving the distal left femur of a middle-aged man and tumoral invasion of the bone, which, to our knowledge, has been previously described only once. He was treated with distal femoral tumor resection and reconstruction with a modular prosthesis. Histopathologic diagnosis confirmed MIFS. We reviewed literature of the diagnostic imaging and bone invasion findings associated with this tumor type.
