Impaction of a lithotripsy basket during endoscopic lithotomy of a common bile duct stone

The treatments for common bile duct (CBD) stones are being continually developed. Impaction of the lithotripsy basket during endoscopic removal of CBD stones was seen in 5.9% patients. We report the case of a 66-yearold woman who underwent surgery for the removal of an impacted biliary basket. She was admitted to our hospital with a complaint of right upper abdominal pain. Magnetic resonance cholangiopancreatography revealed a CBD stone (20 mm × 15 mm). We diagnosed her with choledocholithiasis and performed endoscopic retrograde cholangiopancreatography to remove the stone. However, unfortunately, the retrievable basket around the stone became impacted. An endotriptor along with forceps could not be used owing to the entrapment of the basket, and thus we performed urgent surgery. The basket containing the stone was removed through a longitudinal choledochotomy. The wires leading to the basket were cut, and the basket containing the stone was removed via the incision. A T-tube was inserted,and the choledochotomy was closed. The postoperative course was uneventful. In conclusion, if the diameter of a CBD stone is more than 20 mm, then the risk of basket impaction increases, and surgery may be necessary as the initial treatment of the CBD stone.

The head mounted display （HMD） is widely used in virtual reality technology. In common HMD, however, the binocular disparity is set to an equal fixed value in the entire range of view. Such HMD systems have several shortcomings when used for wide views. In this study, in order to realize a natural stereo sensation of HMD with wide view, we measure the characteristics of binocular stereo perception and binocular light perception. Results show that both the stereoacuity and light sensitivity decrease as the retina＇s eccentricity increases from fovea to periphery. However, the decrease of the stereoacuity is more rapid than that of the light sensitivity. These results suggest that the binocular disparity at the peripheral field should be small, otherwise double images would be observed instead of a stereo view. Based on the results we develop a relative binocular stereoacuity model which can be applied for the design of HMD systems with wide view.

A variety of slow release fertilizers, controlled release (availability) fertilizers (CAFs),and stability fertilizers have been developed in response to the serious drawbacks of the conventional fertilizers since the early 1960's. Of these fertilizers, CAFs which are coated with resin are consumed in the largest quantity in the world. Selecting CAFs with higher performance, the author will discuss about: 1) Innovation of agro-technologies for various field crops including new concepts of fertilizer application, 2) high yielding of field crops, 3) enhancing quality and safety of farm products, and 4) controlling the adverse effect of intensive agriculture on the environment.

For square contingency tables with ordered categories, there may be some cases that one wants to analyze them by considering collapsed 3 × 3 tables with some adjacent categories combined in the original table. This paper considers the point-symmetry model (Wall and Lienert, 1976) for collapsed tables and proposes a measure to represent the degree of departure from point-symmetry for collapsed tables. Also it gives approximate confidence interval for the proposed measure.

The CRIEPI initiated the Radiation Hormesis Research based on the following rationale. If the Luckey's claim was true, our daily activities in the radiation management would be extremely erroneous. If the Luckey's claim was false, the concept publicated would bring us serious disturbance on the radiation management in Japan. We organized a hormesis research steering committee by the specialists in the concerned field, and started researches under cooperation with many universities, National Cancer Center Research Institute, and National Institute of Radiological Sciences. After interesting results obtained by various experiments on the health effects by low dose, we are now proceeding on the Round Robin tests program including fifteen research plans with universities all over Japan. These activities are categorized as: A. Effects of free radicals produced by the low dose radiation; B. Molecular biological responses to the low dose radiation; C. Radiation effects on the neurotransmission system; D. Stimulative effects of low dose radiation on the immune systems; and E. Epidemiological studies.

The concept of performance-based design, which mainly focuses on mechanical performance, has become the international standard, as in the case for ISO. The standardization of tunnel design has not been achieved because it requires integration of separate specialized fields, such as geotechnical engineering, structural engineering and concrete engineering. It is also required to clarify performance-based criteria for tunnel structures to suit specific use purposes (objectives), establish the concept of survey, planning, design, construction and maintenance based on such criteria, and develop proper management systems for operation and maintenance to suit specific tunnel use purposes. To this end, it is vital to develop a methodology for evaluating and verifying the performance of existing tunnels. This paper presents a new concept of performance requirements for tunnel structures and describes the method of quantitatively evaluating the total performance of existing tunnels in relation to the required performance, assuming the total performance to be based on the Analysis Hierarchy Process.

For square contingency tables with ordered categories, this article proposes new models, which are the extension of Tomizawa’s [1] diagonal exponent symmetry model. Also it gives the decomposition of proposed model, and shows the orthogonality of the test statistics for decomposed models. Examples are given and the simulation studies based on the bivariate normal distribution are also given.

The purpose of this paper is to propose a new model of asymmetry for square contingency tables with ordered categories. The new model may be appropriate for a square contingency table if it is reasonable to assume an underlying bivariate t-distribution with different marginal variances having any degrees of freedom. As the degrees of freedom becomes larger, the proposed model approaches the extended linear diagonals-parameter symmetry model, which may be appropriate for a square table if it is reasonable to assume an underlying bivariate normal distribution. The simulation study based on bivariate t-distribution is given. An example is given.
