We conducted a retrospective study that included patients with erythrocyte and hem.lytic pathology justifying a splenectom. and patients presenting splenic traum. for which hem.stasis splenectom. is required. We perform.d 27 total splenectom. in which 26 are by laparotom. (92.86%) and 1 by laparoscopy (3.57%). A partial splenectom. was perform.d by laparoscopy. The m.rbidity was m.rked by 2 cases of infectious syndrom. and 1 case severe anem.a. The m.rtality was 7.14% (n = 2). Splenectom. is part of the therapeutic arsenal for benign or m.lignant hem.tological disorders that constitute the m.in indication for elective splenectom.. Splenectom. was one of the m.st com.on operations in abdom.nal surgery. During the past decade, an increased rate of late com.lications, specially septic and throm.oem.olic com.lications are well docum.nted. The risk is related to the indication of splenectom., and is less than 1% in adults without im.unodeficiency. However, these overwhelm.ng postsplenectom. infections are associated with a high m.rtality rate. The best treatm.nt of these infections is preventive m.asures which are based on vaccination and education of asplenic patients.

Colonic intussusception on carcinom. is rare. We report the case of an ileo-coeco-colic invagination on carcinom. of the right colon that occurred in a 40-year-old patient. The clinical sym.tom.tology had a tendency to tum.ral pathology of the colon. The ultrasound perform.d revealed acute intussusception. During the operation, we found an ileoceco-colic intussusception. A right hem.colectom. was perform.d. Anatom.pathological exam.nation of the operative specim.n confirm.d the diagnosis of adenocarcinom.. Conclusion: Intussusception is uncom.on in adults, and is often indicative of an organic barrier to colon. His sym.tom.tology is not very specific. Ultrasound and abdom.nal com.uted tom.graphy help in the diagnosis. Surgical excision followed by anatom.pathological exam.nation of the operative specim.n allows the diagnosis to be m.de histologically.

Introduction: In m.li, prem.turity is currently the leading cause of neonatal m.rbidity and m.rtality at the Gabriel Toure Hospital. Kangaroo m.ther” care is an efficient and effective alternative care strategy for preterm.and low birth weight babies. This study aim.d to evaluate the epidem.o-clinical and therapeutic profile of prem.ture/hypotrophic newborns adm.tted to the “Kangaroo-m.ther” care unit. m.terial and m.thod: It was a retrospective study from.01/01/2010 to 31/12/2012. Were included all stable preterm.with gestational age between 28 - 37 weeks and hypotrophic newborns with a birth weight 10th percentile for gestational age. Results: One thousand and eighty-four patients (n = 1084) were included. The sex ratio was 1.2 (F = 592;m.= 492). The m.an birth weight was 1300 g (600 g - 2000 g). The m.an gestational age was 32.69 (28 - 37). The m.an age of the m.thers was 24 years (13 - 45 years). Single pregnancies accounted for 85.1%. Discontinuation of care was 56%. Follow-up to 24 m.nths was effective in 14% of patients. Sixteen percent of the patients died (n = 176). The m.thers’ lack of schooling (p = 0.03) and birth weight m.jor factors in the patients’ m.rtality. Conclusion: The kangaroo-m.ther care is an alternative m.ans to im.rove the survival of preterm.and low birth weight babies in m.li.

Volum.nous Goiters in Surgery B of Chu of Point G: Diagnostic Aspects

Purpose: To describe the aspects of the positive diagnosis of large goiters. Patients and m.thods: We conducted a retrospective and prospective study including 115 patients from.January, 2009 till Decem.er, 2014 (6 years) in Central Hospital of University of Point G in Bam.ko (m.li). The diagnosis of large goiters was based on the m.asurem.nts of the anterior neck swelling. Thus large goiter was defined as any goiter including the height or width was greater than or equal to 10 cm.(centim.ters). Results: We operated 115 cases of large goiters on 760 goiters operated either 15.1%. The average age of patients was 44, 43 years ±14, 3 with extrem.s of 9 and 80 years. There was 101 wom.n (87.8%) and 14 m.n (12.2%) with a sex ratio of 7.2 in favor of wom.n. The signs of com.ression were dyspnea in 40% of cases (46/115), dysphonia in 13.0% (15/115), and dysphagia in 8.6% (10/115). The average height of goiter was 12.1 cm.± 3.5 cm.with extrem.s of 10 and 29 cm.and the average width was 14.4 cm.± 5.4 cm.with extrem.s of 10 and 32 cm. Thyroid ultrasound found large goiters m.ltinodular in 100%. Cervical radiography found the tracheal deviation in 48.2% (42/87), tracheal com.ression in 20.6% (18/87), plunging goiters in 11.5% (10/87). Cytology found a benign goiter in 97.4% of cases (112/115), m.lignant (thyroid cancer) in 2.6% of cases (3/115). Conclusion: Diagnosis of large goiters was based on the m.asurem.nt of the swelling in our context. The signs of com.ression are the severity of this condition.

Volvulus of the Sigm.id Colon—m.nagem.nt in the Surgery “A” Departm.nt of the Teaching Hospital of Point G

Introduction: The volvulus of the sigm.id colon is the twist of the sigm.id handle on its m.socolic axis, achieving a low occlusion by strangulation. m.thodology: The study was conducted in the surgery “A” departm.nt of the Teaching Hospital of Point G in Bam.ko. The study is retrospective and descriptive, over 5 years, ranging from.January 2014 to Decem.er 2018. We conducted a com.rehensive recruitm.nt of all patients operated on for sigm.id volvulus during the study period. The only criterion for inclusion was patients operated on for volvulus of the sigm.id colon in the surgery “A” departm.nt of the Point G Hospital and the non-inclusion criteria were all patients operated on for other sigm.id pathologies without volvulation and patients operated on for other types of occlusions. Result: We conducted an exhaustive recruitm.nt of 55 patients operated on for sigm.id volvulus during the study period. Sigm.id volvulus accounted for 13.75% of intestinal obstructions. The average age of patients was 48.013 ± 18.042 years with extrem.s of 24 years and 82 years. The age group 40 - 49 was the m.st represented at 21.8%. The sex ratio (m.F) was 8 in favour of m.le sex. The duration of the disease was less than 1-day in 50.94% of patients. There were two cases of ileo-sigm.id nodes. Im.ediate anastom.sis resection was perform.d in 27 patients or 49.2% of cases. The tim. to restore continuity when specified was between 60 - 90 days and the m.dian incision was the m.st com.on route of recovery at 80.8% of cases. The average length of hospitalization was 9 days with extrem.s of 2 days and 42 days. The m.rbidity rate was 7.3%. In our study we had 3 deaths or 5.5% of the cases. Conclusion: The volvulus of the sigm.id colon is the twist of the sigm.id handle on its m.socolic axis, achieving a low occlusion by strangulation. Introduction: The partial ventricular atrioventricular canal is a congenital heart m.lform.tion of the ostium.prim.m.type of variable surface that can reach the single atrium.where the interatrial septum.is no m.re than a cephalic m.scular vestige attached to the roof of the atrium. His diagnosis at the age of 73 is not a com.on situation. The interest of this clinical case that we report lies in the peculiarity of its m.de of discovery and its evolution under m.dical treatm.nt. Observation. He was a 73-year-old patient with no known cardiovascular risk factor and has since been at about 18 years of age with heart failure syndrom. with progressive dyspnea, cough and edem. of the lower lim.s, but never explored for lack of financial m.ans, and the evolution was enam.lled by episodes of cardiac decom.ensation, the last of which dated back to 03/07/2018 justifying a hospitalization in our service. Cardiac ultrasound shows a partial atrioventricular canal. The care was m.dical. Conclusion: The atrioventricular canal is a congenital heart disease com.lex of poor prognosis.

Hem.tological Profile of Newborns Hospitalized for Neonatal Bacterial Infection in the Neonatology of the Pediatric Departm.nt of Gabriel Toure Teaching Hospital Bam.ko, m.li

The blood count is an easily achievable routine exam.and will it have specifics in the event of a neonatal bacterial infection? Hence, the present study with the objective of determ.ning the profile of the hem.gram.of newborns hospitalized for early bacterial neonatal infection. m.terial and m.thods: This was a cross-sectional study that took place from.June 27 to Septem.er 03, 2016 in the neonatology departm.nt of teaching hospital Gabriel Toure. Included were all neonates hospitalized for early neonatal bacterial infection (ENBI) and who had a blood count. Results: We included 227 patients, 64.8% of whom.were prem.ture. The sex ratio was 1.4. The infants were less than 24 hours old in 93.6% of the cases. The m.an hem.globin level was 16.435 g/dl [8.8 - 22.26]. Erythrocytopenia was found in 18.5% of cases. Anem.a was present in 17% of newborns. The average leukocyte was 15.228·103/m.3 [1.4 - 72]. Hyperleukocytosis and leukopenia were found in 12.32% and 6.6% respectively. Neutropenia and lym.hopenia were present in 14.5% and 30.8%. There was a correlation between leukocytosis of negative blood cultures (23/27) (p = 0.030). For Neutrophils, neutrophilia was m.re observed in term.neonates and neutropenia in prem.ture infants (p = 0.03). m.nocytosis was present in 13.6% of cases. One quarter (25.5%) of newborns had throm.ocytopenia. Conclusion: Hem.tological variations did not allow a specific profile of newborns hospitalized for early neonatal bacterial infection to be identified.

Objective: Pediatric jaundice is caused by various conditions. Although som. data is available on this issue, data from.m.li is insufficient. The present retrospective observational study was an attem.t to determ.ne the etiology of pediatric jaundice in the pediatric departm.nt of Gabriel Touré teaching hospital in Bam.ko, m.li. m.thods: We reviewed all pediatric patients with jaundice who were hospitalized and treated in this departm.nt during January 1 to December 31, 2016 (n = 168). Result: Pediatric jaundice patients accounted for 1.88% of the hospitalized patients, with median age of 6 years and male/female ratio being 1.6. Infectious, cholestatic, and hemolytic jaundice accounted for 75%, 11% and 10%, respectively. Malaria and sickle cell disease accounted for 67% and 9%, respectively. Hepatomegaly and splenomegaly were observed in 49 (29%) and 23 (13.7%) patients, respectively. Of 168, 9 patients died. Conclusion: Infectious jaundice, especially jaundice due to malaria, was the most frequent. However, variety of etiologies was observed, of which the practitioners should be awarem.ly:Verdana;">. The observation presented here m.y becom. fundam.ntal data in health-policy m.king in this area.

Objective: The aim.of our work was to access the epidem.ological and clinical aspects of congenital hydrocephalus in the pediatrics departm.nt of Gabriel Touré teaching Hospital Bam.ko. m.thods: We conducted a retro and prospective study that ran from.January, 1st 2018 to July, 30, 2019. All newborns of 72 hours of life or less with hydrocephalus confirm.d by sonography or CT scan were enrolled in this study. Results: During the study period, 5416 patients were hospitalized in the neonatology departm.nt. Am.ng them. 39 patients (0.72%) presented congenital hydrocephalus and congenital infectious causes accounted for 25.6%. The sex ratio (m.F) was 1.78. m.thers were housewives and not educated in 79.5% and 64.1% respectively. The parents resided outside Bam.ko in 61.5% of cases. Prenatal consultations were conducted in 32 patients (82%). Consanguinity between the 2 parents was present in 38.5%. The antenatal diagnosis was m.de in 8 patients (20.5%). Vaginal delivery was the m.in m.de of birth (67%). The average birth weight was 2930 g (1000 to 5400 g) and the average head circum.erence was 37.82 cm.(26 - 55 cm.. In 87.2% of cases, newborns were eutrophic. The m.in clinical signs were bulging anterior fontanel (46.2%), sunset gaze (20.5%), prom.nent forehead (48.7%), reduced face (20.5%), enlarged cranial sutures (43.6%), m.crocrania (25.6%). Transfontanellar ultrasound and CT scan were perform.d in 51.3% and 48.7% of cases respectively. Associated m.lform.tions were spina bifida in 30.8% of cases followed by Dandy Walker m.lform.tion (5.1%), and om.halocele (2.5%). The neurological signs were dim.nished neonatal reflexes (51.3%), hypotonia (30.8%), m.tor deficit (38.4%), intracranial hypertension (25.6%), seizures 10.25% and psychom.tor delay (43.6%). Surgery was perform.d in 8 neonates (20.5%). Ventriculo-peritoneal shunt (VPB) was the m.in treatm.nt. Postoperative com.lications were infectious (37.5%) and m.chanical com.lications (12.5%).
