Aim.- To study schistom.siasis infection in school children in m.lodo, an irrigated rice growing region of m.li, by determ.ning the prevalence of schistom.siasis and lesions identified by ultrasonography am.ng children living in this region. m.thods - This cross sectional study included 346 children aged 7 to 14 years selected at random.from.five schools in m.lodo. We tested for hem.turia using urine dipsticks and searched for Schistosom. haem.tobium.eggs in urine and S. m.nsoni eggs in stools. Ultrasonography of the liver, spleen and urinary tract was perform.d. Results - The prevalences of Schistosom. haem.tobium.and S. m.nsoni infection were 72% (range: 66.9- 76.6% ) and 68.2% (range: 60.9- 71.2% ) respectively; 55.1% of the children had co-infection. Ultrasonography of the urinary tract revealed an irregular bladder wall as the m.st frequent abnorm.lity (3.4% of children). Abdom.nal ultrasonography dem.nstrated type B hepatic fibrosis in four children (1.1% ), type C in one (0.3% ) and type D in one (0.3% ). Conclusion - Few schistosom.asis lesions were detected by ultrasonography com.ared with the prevalence of S. haem.tobium.and S. m.nsoni infections. This observation is probably related to m.ss treatm.nt program. conducted during a national anti-schistosom.asis program.

Objective: To study the pulm.nary com.lications of m.jor sickle cell syndrom.s in children aged 6 m.nths to 15 years followed at the Departm.nt of Pediatrics of Gabriel Toure University Hospital. m.terials and m.thods: This was a retrospective study from.January 1st, 2017 to February 28, 2018 and a prospective study from.m.rch 1, 2018 to Decem.er 31, 2019, concerning sickle cell disease children followed at the Gabriel Touré University Hospital. All children with sickle cell disease confirm.d by hem.globin electrophoresis, aged 6 m.nths to 15 years with a pulm.nary com.lication and adm.tted to pediatrics were included. Were not included: 1) Infants and children with sickle cell disease not presenting a pulm.nary com.lication com.ng only to their routine follow-up. 2) Sickle cell patients over 16 years of age and those whose parents do not consent. 3) Those who have not done electrophoresis. Results: During the study period, we were able to include 45 sickle cell children with a pulm.nary com.lication. The frequency of pulm.nary com.lications was 13.76%, involving hom.zygotes in 91.11% of cases. The age group 6 - 10 years predom.nated with 46.66% and the sex ratio was 0.45. Vaso-occlusive crisis was the m.st frequent reason for consultation with 35.56%. Respiratory distress (80%), fever (66.67%), crepitus rales (64.44%) and chest pain (60%) were the m.st frequent clinical signs. The m.in pulm.nary com.lication was acute chest syndrom. with 86.67%. Treatm.nt was based on hydration (91.91%), analgesics (91.91%) and antibiotics (73.33%). Lethality was 4.44%. Conclusion: Pulm.nary com.lications of sickle cell disease are serious and constitute the m.in cause of m.rtality in our context.

Purpose: To describe the epidem.ological, clinical and therapeutic aspects of secondary hyperparathyroidism.inrenal failure chronic. Patients and m.thods: We collected 11 cases of hyperparathyroidism.secondary to renal failure term.nal operated in the Service of surgery B of the Central Hospital University of Point G between Decem.er 2016 and Novem.er 2018. Results: The sex ratio was 0.22 in favor of wom.n. The average age of the patients was 43 or 27 years with extrem.s of 63 and 25 years. Secondary hyperparathyroidism.in renal failure chronic represented 1.9% of interventions to cold in the Service of surgery B. 100% of patients (11/11) were haem.dialysis. 100% of the patients had clinical and biological signs. 45.5% (5/11) had radiological signs. The average rate of parathyroid horm.ne was 2413.51 pg/m. with extrem.s of 1264 pg/m. and 3616 pg/m.. The reference value was 15 - 65 pg/m.. The surgical technique of choice was the 7/8th parathyroidectom. in 100% of cases. The postoperative were sim.le in 81.8%, and com.licated in 18.2%. There were no death. The average duration of postoperative follow-up was 6 m.nths. After surgery, 50% of patients (5/10) had norm.l levels of parathyroid horm.ne and 50% (5/10) m.de a persistent hyperparathyroidism. Conclusion: Secondary hyperparathyroidism.is a frequent com.lication in renal insufficient chronic in hem.dialysis. Surgery is indicated in the resistant cases of m.dical treatm.nt. The 7/8th parathyroidectom. is the surgical technique of choice. The rate of post operative com.lications is higher in our context.

The uterus bicorne is a uterine m.lform.tion whose pathophysiology is known but the etiology rem.ins obscure. Few studies have been interested in this subject. This is the clinical observation of m.s. X 26-year-old housewife nulliparous who consulted for repeated late spontaneous abortions. The clinical exam.nation was norm.l. The ultrasound revealed a double uterine cavity in Y, in favor of a unicervical bicorne uterus. At hysterosalpingography, the tubes were perm.able and the uterus doubled with a single cervix. The treatm.nt consisted of a uterine plasty. The postoperative course was sim.le. The patient gave birth to a newborn m.le 20 m.nths after surgery.

Considered as rare in Africa lithiasis of the m.in bile duct (m.D) is a potentially serious pathology, diagnosis is facilitated by im.ging tests (ultra-sound, CT, m.I cholangiopancreatography). Its surgical m.nagem.nt is im.roved by laparoscopy coupled with interventional endoscopy. However, laparotom. rem.ins the only way in m.li. This was a 14-years retrospective and descriptive study (2010-2014). All patients with BPV lithiasis were included. We studied the field, antecedents, clinical, biological, radiological, therapeutic and outcom. aspects. 40 cases of stones in the m.in bile duct were collected during the study period. The hospital frequency was 2.8 cases per year, the clinical signs dom.nated by the Charcot triad (pain, fever, jaundice) found in 40 cases (100%). The average age was 60 years, fem.le represented 70% of cases. Ultrasonography was the m.st requested review in 40 cases (100%). The procedure perform.d was cholecystectom. associated with choledochotom. with calculation extraction in all patients (100% of cases). One case of biliary fistula and one case of wall abscess were observed. The m.rtality was 5% and the sim.le suited in 90% of the cases. The lithiasis of the m.in bile duct is an infrequent pathology in m.li and potentially serious. This diagnosis is assisted in our context by ultrasound and CT. Laparotom. rem.ins the only route of entry in m.li.

Introduction: Laparoscopic cholecystectom. is the gold standard in the treatm.nt of gallstones and lithiasis cholecystitis. It reduces post-operative com.lications especially in sickle cell patients. Aim. The aim.of this work was to com.are the results of laparoscopic cholecystectom. in sickle cell and non-sickle cell patients. m.thods: Sixty-six patients including 25 sickle cell patients and 41 non-sickle cell patients were identified from.m.rch 2013 to Novem.er 2014 (20 m.nths). The χ2 test was used for com.arisons. Values of p m.an age was 25.76 years for the sickle cell group and 45.61 years for the non-sickle cell group (p = 0.00008). There were 14/25 fem.le patients in the sickle cell group and 28/41 in the non-sickle cell group. In per op, the diagnosis of acute cholecystitis was retained for 4/25 sickle cell patients and for 10/41 non-sickle cell patients. Diagnoses such as pyocholecyst (2 cases), porcelain vesicle (2 cases), hydrocholecyst (2 cases) were found in the group of non-sickle cell patients. Operative difficulties such as tight perivicular adhesions were encountered 9/25 tim.s in the sickle cell group and 11/41 tim.s in the non-sickle cell group. The average overall operative tim. was 55 m.n. There was no statistically significant difference between the different groups in m.an operative tim., occurrence of postoperative com.lication and average length of hospital stay. The m.rtality is not statistically different according to the group of patients. Conclusion: There is no significant difference between the cholecystectom. perform.d in sickle cell patients and that perform.d in non-sickle cell patients. The m.ltidisciplinary perioperative m.nagem.nt of sickle cell patients reduces the risk of com.lications arising from.this pathology.

The Pica syndrom. is an eating disorder characterized by an excessive or abnorm.l desire to consum. a non-nourishing substance which can be relatively harm.ess, or potentially harm.ul for the health. It is a rare affection secondary to the accum.lation of diverse nature foreign bodies inside the digestive tract and m.re especially at the stom.ch level. Gastro-intestinal localization is the m.st frequent, and can rem.in long tim. asym.tom.tic. Treatm.nt is surgical. We report 3 cases of digestive com.lication of Pica syndrom.. The first one was operated for gastric perforation due to nail (53 nails, a pin and bands of tape recorder cassette ingested), the second one for trichobezoar and the last had a subocclusion by pieces of granite.

Introduction: Appendicular peritonitis is a com.lication of acute appendicitis characterized by the spread of the infectious process in the peritoneal cavity thus achieving wide spread or localized purulent peritonitis;it’s a m.dico-surgical em.rgency. Our objectives are to determ.ne the frequency, describe the clinical, therapeutic and prognostic aspects of peritonitis by appendicular perforations. Patients-m.thod: This was a 24-m.nth retro, prospective, descriptive study from.January 1, 2018 to Decem.er 31, 2019;conducted in the Bougouni Reference Health Center Surgery Unit. All patients of appendicular peritonitis at the Bougouni Reference Health Centre were included. Results: During the study period, 68 cases of generalized acute peritonitis including 30 appendicular peritonitis cases were collected. Appendicular peritonitis accounted for 44.1% of surgical procedures. m.les accounted for 71.0% with a sex ratio of 1.2 at risk of m.n, the average age was 26.07 years. Abdom.nal pain and vom.ting were the reasons for consultation in 86.7% and 76.7% of cases. Physical exam.nation was used in m.st cases to m.ke the diagnosis. X-ray of the abdom.n without preparation, and abdom.nal ultrasound were perform.d system.tically. Surgical treatm.nt consisted of an appendectom. with peritoneal toilet followed by drainage. The average length of hospitalization was 8.8 days with extrem.s of 1 - 44 days. Hospital m.rtality was 3.3%;m.rbidity and high m.rtality were related to delayed consultation. Conclusion: Appendicular generalized acute peritonitis is a m.dical-surgical em.rgency with a high m.rtality rate associated with delayed m.nagem.nt.

The purpose of this study was to investigate the clinical and therapeutic aspects of peritonitis by perforation of gastric and duodenal ulcer. This was a retrospective and descriptive study over 8 years (2010-2018) which allowed to collect 54 cases of peptic ulcer. Included in the study were all patients with confirm.d gastroduodenal perforation on histology or laparotom.. We collected 54 cases of peptic ulcer perforated s. The age group of 30 - 49 years was the m.jority. The m.le sex was dom.nant with 90.7% of cases;the clinical picture was dom.nated by abdom.nal contracture associated with pain in 74.07% of cases. X-ray of the abdom.n without preparation (AWP) revealed in 87.03% of cases of pneum.peritoneum. The perforation was in 68.52% of cases on the gastric antrum.and in 31.48% on the duodenum. The surgical procedure used was the bank of excision, and a suture reinforcem.nt epiploic in 68.52% of cases, a sim.le suture m.de in 31.48% of cases, the disease was m.rked by a fistula (1.90%) and m.rtality was 5.55% of cases. The gastroduodenal ulcer perforation is potentially serious and responsible peritonitis whose surgical treatm.nt involves the peritoneal toilet and sutures the puncture.
