Feasibility of Improved Attenuation Correction for SPECT Reconstruction in the Presence of Dense Materials Using Dual-Energy Virtual Monochromatic CT: A Phantom Study

Objective: Computed tomography (CT)-based attenuation correction (CTAC) offers the clear benefit of reliable reconstruction of single-photon emission computed tomography (SPECT) images through its ability to achieve object-specific attenuation maps, but artifacts from dense materials often deteriorate CTAC performance. Therefore, we investigate the feasibility of CTAC in the presence of dense materials using dual-energy virtual monochromatic CT data. Methods: A sodium pertechnetate-filled cylindrical uniform phantom, with a pair of undiluted iodine syringes attached, is scanned with a dual-source CT scanner to obtain both single-energy (120 kVp) polychromatic and dual-energy (80 kVp/140 kVp with tin filtering) virtual monochromatic CT images. The single-energy and the dual-energy CT images are then converted to attenuation maps at 141 keV. SPECT images are reconstructed from 99mTc emission data of the phantom using each single-energy and dual-energy attenuation map and incorporating CTAC procedure. A region-of-in- terest analysis is performed to quantitatively compare the attenuation maps between the single-energy and the dual-energy techniques, each at an iodine-free position and a position adjacent to the iodine solutions. Results: At the iodine-free position, the phantom provides a uniform distribution of attenuation maps in both the single-energy and the dual-energy techniques. In the presence of adjacent iodine solutions, however, severe artifacts appeare in the single-energy CT images. These artifacts make attenuation values fluctuate, resulting in erroneous pixel values in the CTAC SPECT images. In contrast, dual-energy CT strongly suppresses the artifacts and hence improves the uniformity of the attenuation maps and the resultant SPECT images. Conclusions: Dual-energy CT with virtual monochromatic reconstruction has the potential to substantially reduce artifacts arising from dense materials. Napier grass （Pennisetum purpureum Schumach）, a C4 tropical plant, has high dry matter productivity and sustainability without the need for renovating every year due to good overwintering ability and the absence of serious pathogens or insects injury. This grass is suitable for multipurpose use, providing forage, bioethanol feedstock and phytoremediation of cadmium （Cd） contaminated soil in Kyushu, Japan. When two cultivars of Napier grass （Wruk wona, WK and Merkeron, ME） were grown in Cd-contaminated soils, dry matter yield and the concentration and uptake of herbage Cd were higher in WK than in ME, and soil Cd concentration was reduced by 14% in a single season of cultivation. A dwarf variety of late-heading type （DL）, Napier grass had superior sustainability for more than 5 years in Miyazaki, Japan, and has gradually increased in cultivation areas for both cut-and-carry and grazing herbage production in cooperation with regional communities. Through the extension in several Kyushu areas, DL Napier grass required temperatures above -6.2 ~C for sustainability in winter, based on its threshold response to the lethal minimum temperature. DL Napier grass pasture can be utilized by grazing beef cattle even in early spring with temperate Italian ryegrass inter-sown into its inter-row spaces. Organic digested animal manure or sun-dried leguminous leaves as green manure can replace chemical fertilizers to reduce energy consumption and allow comparable productivity.

Latent Autoimmune Diabetes in Adults Complicated by Persistent Isolated Glucosuria in the Absence of Hyperglycemia

Latent autoimmune diabetes in adults (LADA) is an autoimmune diabetes of adult-onset with the presence of diabetes associated autoantibodies. Familial renal glucosuria (FRG) is an inherited renal tubular disorder that causes persistent isolated glucosuria in the absence of hyperglycemia. We report a novel case of LADA and certain FRG. A 44-year-old man was admitted to our hospital for uncontrolled diabetes. Before admission, he had never suffered from diabetic coma and showed an improvement in HbA1c only with diet therapy. His HbA1c was 11.9% (107 mmol/mol), and anti-glutamic acid decarboxylase antibody was 13.0 U/mL. A glucagon stimulation test showed the decrease of insulin secretion: plasma C-peptide (CPR) 0 min, 0.69 ng/mL;CPR 6 min, 0.90 ng/mL. Analysis of genomic DNA revealed a novel heterozygous mutation in the SGLT2 coding gene, SLC5A2 (c.875G >A, ***292Tyr), which was assessed as probably damaging with a score of 0.998 (sensitivity: 0.27;specificity: 0.99) by an in silico analysis. Therefore, he was diagnosed with LADA and certain FRG. He has not shown any symptoms and his HbA1c improved to 6.4% (46 mmol/mol) three months after the introduction of insulin therapy. Our case clearly implies the clinical effectiveness of SGLT2 inhibition in patients with LADA.

INTRODUCTION: Gait asymmetry can become very pronounced in patients who have suffered a stroke. The impairment of trunk function in some stroke patients can restrict thorax mobility and cause respiratory muscle weakness. Phytoremediation Potential of Sorghum as a Biofuel Crop and the Enhancement Effects with Microbe Inoculation in Heavy Metal Contaminated Soil

Phytoremediation is an eco-friendly and low-cost biotechnology using plants to extract, contain, degrade, or immobilize pollutants from the contaminated environment. Selection of the ideal plant species and suitable enhancing measures to obtain high remediation efficiency and large valuable biomass are essential requirement for a successful phytoremdaition. Sorghum (Sorghum bicolor L.) is one of the most attractive bioenergy crops for producing biofuels with high biomass production. In this study, the phytoremediation potential of sorghum to heavy metals and the promotion effects by a lead-tolerant fungus (LTF) were investigated using a multiple heavy metal contaminated soil with Pb, Ni, and Cu. The results showed that the sorghum survived the heavy contamination, and LTF inoculation promoted the plant growth and increased the phytoextraction yields of Pb, Ni, and Cu. The phytoextraction potential (μg/plant) of the whole sorghum for Sorghum were 410 (Pb), 74 (Ni), and 73 (Cu), and for Sorghum with LTF inoculation were 590 (Pb), 120 (Ni), and 93 (Cu), respectively. The results suggested that sorghum would be one of the ideal candidates for phytoremediation of contaminated soil because of its high phytoremediation potential, large biomass production, and utilization in biofuel production.

A Literature Review of Fertility Education for High School Students

Purpose: Fertility education for high school students leads them to be able to decide when to get pregnant and give birth, and to think about their desired life plan. However, knowledge of fertility in Japan is low compared to other countries. This study aims to understand the state of fertility knowledge among high school students in Japan and to clarify issues in fertility education. Method: Ten articles published from 2013 to July 2023 were selected for review from ICHUSHI Web Ver. 5 and CiNii with the keywords “high school students,” “fertility,” and “education.” The review was limited to Japanese-language literature due to differences in fertility education and knowledge between Japan and other countries. Results/Conclusion: 60% - 90% of the high school students indicated that their source of information on fertility was school lessons. Half of the students understood egg aging and the effects of smoking and drinking on fertility, but their knowledge of menstruation, male reproductive physiology/function, and infertility was insufficient. 50% - 80% of students wanted to marry by the age of 30, and 70% - 80% wanted to have children. 80% of students who attended a lecture on preconception care considered the lesson an opportunity to think about their future life plans. 90% of school nurse teachers reported that fertility should be taught in schools, and half said that egg aging, causes of infertility, and other topics should be covered in high school. Educational caution points included the consideration of developmental stages and the relatedness of the content to what had already been taught, among other things. High school is the best time for fertility education because it gives students the opportunity to accept their own sexuality and think about their future. In addition, since many students plan to marry and have children by the age of 30, the acquisition of fertility knowledge can be an effective means of future life design.

Short-Term Drug-Drug Interaction between Sildenafil and Bosentan under Long-Term Use in Patients with Pulmonary Arterial Hypertension

Sildenafil and bosentan are often co-administered for pulmonary arterial hypertension (PAH) treatment. The plasma concentration of sildenafil can be decreased by half if co-administered with bosentan. Many patients take these agents simultaneously in the morning and the evening. The aim of this study was to examine the pharmacokinetics of sildenafil which was interfered with bosentan administration to ascertain whether these agents should be given concomitantly or separately. A two-way crossover study was conducted in 6 PAH patients with combination therapy of sildenafil and bosentan. Participants underwent the sequence of treatment phases: phase S (sildenafil administered 3 h before bosen-tan);phase B (bosentan administered 3 h before sildenafil);and phase C (administered concomitantly). Blood samples were collected on the last day of each phase. There was no significant difference in maximum plasma concentration or area under the plasma concentration-time curve (AUC0-8) between phase C and phase S (95.5 ± 24.8 vs. 72.9 ± 40.9 (p = 0.07), 209.7 ± 81.8 vs. 180.2 ± 126.4 (p = 0.24), respectively) or between phases C and B (87.8 ± 42.0 vs. 99.6 ± 33.9 (p = 0.59), 197.2 ± 88.2 vs. 240.7 ± 121.8 (p = 0.19), respectively) (ng/mL, mean ± standard deviation). Large intra-and inter-individual variability in sildenafil concentration was noted. The timing of administration of sildenafil and bosentan does not significantly influence the plasma concentration of sildenafil. Physicians do not need to be overly concerned about the timing of administration of these drugs to maximize the sildenafil concentration.
