Visual and refractive outcomes of posterior chamber phakic IOL in stable keratoconus

We evaluated the visual and refractive outcomes after phakic visian toric implantable collamer lens(ICL) insertion in stable keratoconus(KC). This retrospective study investigated toric ICL implantation in 14 eyes of 8 patients with stable KC. After 6 mo, the mean uncorrected distance visual acuity improved significantly from 0.77 to 0.15 logmAR. The mean best corrected distance visual acuity(BCDVA) improved from 0.18±0.1 to 0.15±0.1 logmAR. Fifty percent of eyes maintained their preoperative BCDVA; 42.8% gained one line. There was no statistical difference in high order or coma aberration. The mean refractive manifest spherical equivalent(mSE), mean refractive manifest spherical error, mean manifest astigmatism decreased significantly postoperatively. At 6 mo postoperatively, our achieved mean spherical equivalent was approximately 74%. No intraoperative or postoperative complications occurred. Toric ICL implantation was effective, predictable and safe to correct refractive error and improve visual acuity in patients with stable KC.

An adaptive neuro-fuzzy sliding mode controller for mImO systems with disturbance

This paper introduces the mathematical model of ammonia and urea reactors and suggested three methods for designing a special purpose controller. The first proposed method is Adaptive model predictive controller, the second is Adaptive Neural Network model Predictive Control, and the third is Adaptive neuro-fuzzy sliding mode controller. These methods are applied to a multivariable nonlinear system as an ammonia–urea reactor system. The main target of these controllers is to achieve stabilization of the outlet concentration of ammonia and urea, a stable reaction rate, an increase in the conversion of carbon monoxide(CO) into carbon dioxide(CO_2) to reduce the pollution effect, and an increase in the ammonia and urea productions, keeping the NH_3/CO_2 ratio equal to 3 to reduce the unreacted CO_2 and NH_3, and the two reactors' temperature in the suitable operating ranges due to the change in reactor parameters or external disturbance. Simulation results of the three controllers are compared. Comparative analysis proves the effectiveness of the suggested Adaptive neurofuzzy sliding mode controller than the two other controllers according to external disturbance and the change of parameters. moreover, the suggested methods when compared with other controllers in the literature show great success in overcoming the external disturbance and the change of parameters.

mismatch repair genes (hmLH1, hPmS1, hPmS2, GTBP/hmSH6,HmSH2) in the pathogenesis of hepatocellular carcinoma

AIm: DmA mismatch repair (mmR) is an important mechanism for maintaining fidelity of genomic DNA. Abnormalities in one or more mmR genes are implicated in the development of many cancers. We investigated the role of expression of mmR genes (hmLH1, hPmS1, hPmS2, GTBP/hmSH6, hmSH2) in hepatocellular carcinogenesis. mETHODS: We evaluated the expression level of mmR genes in 33 hepatocellular carcinoma (HCC) cases using the multiplex reverse transcription (RT) PCR assays, as well as in 16 cases of normal adjacent hepatic tissues. β-actin gene was used as an internal control and calibrator for quantification of gene expression. RESULTS: Out of the 33 studied cases, 25 were HCV positive and 30 (90.9%) showed reduced expression in one or more of the studied mmR genes. Reduced expression was found in hmSH2(71.9%), hmLH1 (53.3%), GTBP(51.1%), hPmS2 (33.3%) and hPmS1 (6%). A significant correlation was found between reduced expression of hPmS2(P= 0.0069) and GTBP(P= 0.0034), hPmS2 and non-cirrhosis (P= 0.0197), hmLH1 and high grade. On the other hand, 57.1%, 50%, 20%, 18.8%, and 6% of the normal tissues distant to tumors showed reduced expression of hmSH2, hmLH1, GTBP, hPmS2, and hPmS1 respectively. multivariate analysis revealed a significant correlation between the expression level of hmSH2(P= 0.008), hmLH1 (P= 0.001) and GTBP (P= 0.032) and HCC, between hPmS2, GTBP and HCV-associated HCC (PmmR genes seems to play an important role in HCV-associated HCC. hPmS2 is likely involved at an early stage of hepatocarcinogenesis since it was detected in normal adjacent tissues. Reduced expression of hPmS2 provides a growth advantage and stimulates proliferation which encourages malignant transformation in non-cirrhotic HCV-infected patients via acquisition of more genetic damages.

Assessment of heme oxygenase-1 （HO-1） activity in the cavernous tissues of sildenafil citrate-treated rats

Aim： To assess heine oxygenase-1 （HO-1） activity in the cavemous tissue of sildenafil citrate-treated rats. methods： One hundred and ninety-two Sprague-Dawley male rats, divided into four equal groups, were investigated. Group 1, the control group, received regular animal chow; group 2 received sildenafil citrate by intragastric tube; group 3 received sildenafil and HO inhibitor （zinc protoporphyrin, ZnPP）; and group 4 received sildenafil and nitric oxide synthase （NOS） inhibitor L-nitroarginine methyl ester （L-NAmE）. Twelve rats from each group were killed after 0.5 h, 1 h, 2 h and 3 h of drug administration. Then HO-1 activity, cGmP levels and NOS enzymatic activity in the cavernous tissues were estimated. Results： In cavemous tissue, HO-1 activity, NOS enzymatic activity and cGmP concentration increased significantly in sildenafil-treated rats compared to other groups throughout the experiment. Rats receiving either HO or NOS inhibitors showed a significant decrease in these parameters. HO- 1 cavemous tissue activity and NOS enzymatic activity demonstrated a positive significant correlation with cGmP levels （r = 0.646, r = 0.612 respectively; P 〈 0.001）. Conclusion： The actions of PDE5 inhibitor sildenafil citrate in the cavernous tissue are partly mediated through the interdependent relationship between both HO-1 and NOS activities.

Impact of a Clinical Pharmacist in the General Hospital: An Egyptian Trial

medication review aims at optimizing the impact of medications while minimizing their related problems. Drug-related problems have never been properly addressed in Egyptian hospitals. The purpose of this paper is to record and compare the prevalence and types of medication related problems, the interventions provided by the clinical pharmacists and how physicians responded along with making recommendations for error prevention. This prospective study was conducted between June 2012 and December 2012, at an Egyptian general hospital. Five trained pharmacists recorded patient-specific medication related recommendations and completed quality-of-care interventions. The average number of audited doses was 81% of those prescribed. The most prevalent medication problem was prescribing errors followed by administration errors. A total of 20 patients experienced adverse drug events. The greatest error rates across the seven months were observed in the ICU and cardiology units. Numbers of interventions offered by the pharmacists ranged from 241, to 519 per month. Nurses accepted all the interventions introduced by the pharmacists aimed at reducing administration errors while physicians’ resistance rates had an average of 21%. This study showed a positive influence of the pharmacist-led medication review in reducing potential drug-related problems in an Egyptian secondary care where the hospital under study implemented new measures to minimize drug related problems according to the findings of the trained pharmacists.

Feasibility Study of Solar Energy Steam Generator for Rural Electrification

In middle East region, where there are plentiful amounts of solar radiation and great desert areas, solar energy can play a potential role in replacing conventional fuel-operated electricity generation methods with a cost-effective, sustainable solution. This paper presents a feasibility study of a low-cost solar energy steam generator for rural areas electrification. The proposed system is based on the use of trough concentrator which converts solar radiation into thermal energy in its focal line (where a receiver pipe is installed with a fluid flowing in its interior). The aim of the paper is to predict the feasibility and potential for steam generation using a stand-alone solar concentrator with a small dimension for domestic and small-scale electricity generation. The study presented here is based on modelling of the system to determine the points at which the system is expected to produce sufficient steam energy at the tube outlet to drive a steam engine for producing electricity. Results are presented in graphical forms to show the operating points and the effect of changing selected input parameters on the behavior of the system in order to set some limits (boundaries) for such parameters. Results show that among the three input design parameters selected, the tube diameter is the most dominant parameter that influences steam energy, then the tube length and finally the flow rate of the water passing through the tube. The results of this paper can provide a useful guideline for future simulation and/or physical implementation of the system.

methylprednisolone（mP） is currently the only drug confirmed to exhibit a neuroprotective effect on acute spinal cord injury（SCI）. Vitamin C（VC） is a natural water-soluble antioxidant that exerts neuroprotective effects through eliminating free radical damage to nerve cells. Bone marrow mesenchymal stem cells（BmmSCs）, as multipotent stem cells, are promising candidates in SCI repair. To evaluate the therapeutic effects of mP, VC and BmmSCs on traumatic SCI, 80 adult male rats were randomly divided into seven groups： control, SCI（SCI induction by weight-drop method）, mP（SCI induction, followed by administration of 30 mg/kg mP via the tail vein, once every other 6 hours, for five times）, VC（SCI induction, followed by intraperitoneal administration of 100 mg/kg VC once a day, for 28 days）, mP ＋ VC（SCI induction, followed by administration of mP and VC as the former）, BmmSCs（SCI induction, followed by injection of 3 × 10~6 BmmSCs at the injury site）, and BmmSCs ＋ VC（SCI induction, followed by BmmSCs injection and VC administration as the former）. Locomotor recovery was assessed using the Basso mouse Scale. Injured spinal cord tissue was evaluated using hematoxylin-eosin staining and immunohistochemical staining. Expression of transforming growth factor-beta, tumor necrosis factor-alpha, and matrix metalloproteinase-2 genes was determined using real-time quantitative PCR. BmmSCs intervention better promoted recovery of nerve function of rats with SCI, mitigated nerve cell damage, and decreased expression of transforming growth factor-beta, tumor necrosis factor-alpha, and matrix metalloproteinase-2 genes than mP and/or VC. more importantly, BmmSCs in combination with VC induced more obvious improvements. These results suggest that VC can enhance the neuroprotective effects of BmmSCs against SCI.
