An unhappy triad:Hemochromatosis, porphyria cutanea tarda and hepatocellular carcinoma-A case report

Liver fibrosis and cirrhosis are predisposing factors for the development of hepatocellular carcinoma （HCC）. Hemosiderosis has also been described to trigger carcinogenesis. A significant iron overload, as found in hereditary hemochromatosis （HHC）, is a risk factor for HCC and may also promote the symptoms of porphyria cutanea tarda （PCT）. A 68-year old male patient presented to our clinic with a suspected HCC, elevated alpha-fetoprotein but normal liver function tests. He reported a 25 year-old history of vitiligo upon exposure to sunlight. The patient underwent an extended left hemihepatectomy, and the recovery was uneventful, with the exception of a persistent hyperbilirubinemia. Perfusion problems and extrahepatic cholestasis were ruled out by CT-scan with angiography and MR-cholangiopancreatography. However, MR1 showed an iron overload. Histology confirmed the HCC （pT3, pN0, G3, R0） and revealed a portal fibrosis and hemosiderosis. Based on the skin lesions we suspected a PCT that was confirmed by laboratory tests showing elevated porphyrin, uroporphyrin, coproporphyrin and porphobilinogen. Concurrently, molecular diagnostics revealed homozygosity for the C282Y mutation within the hemochromatosis HFE gene. After phlebotomy and normalization of liver function tests the patient was discharged. This is the first case ever showing the unusual combination of HCC in a fibrotic liver with HHC and PCT. This diagnosis not only warrants oncological follow-up but also symptomatic therapy to normalize iron metabolism and thereby improve liver function and alleviate the symptoms of HHC and PCT. Thus progression of fibrosis may be prevented and liver regeneration supported.

Effect of Intensive Inorganic Fertilizer Application on Microbial Properties in a Paddy Soil of Subtropical China

A field experiment with rice-rice rotation was conducted since 2002 in southeast China for investigating the response of soil microbial properties to intensive nitrogen fertilizer application. The tested soil was a subtropical paddy soil derived from Quaternary red clay. Differences between treatments existed in different application rates of urea when the experiment was designed. Urea was applied in five rates, i.e., 0, 0.5, 1, 1.5, and 2 U, equivalent to 0, 0.5, 1, 1.5, and 2 times the local average amount of urea application （900 kg urea ha-~ yr-~, equivalent to 414 kg N ha-1 yr-~）. In 2007, soil total nitrogen, available nitrogen, and soil organic carbon contents were increased by 10.2-27.9, 8.0-16.0, and 10.2-30.6%, respectively, in treatments with urea application rates of 0.5 to 2 U compared to control （0 U）. Microbial biomass carbon and nitrogen were also increased by 3.1-30.8 and 1.3-13.9%, respectively, in treatments with urea application. Basal respiration in treatments with urea input were 9.4-29.1% higher than that in control. However, changes of bacterial functional diversity had different trends. Urea fertilization enhanced bacterial functional diversity until treatment of 1 U, but re-decreased it from treatment of 1.5 U. Principal components analysis indicated that there were intimate relationships among soil organic matter, nitrogen nutrient, microbial biomass, and respiration. Nevertheless, microbial diversity was related to soil moisture contents after urea application. We conclude here that the application of N fertilizer improved soil microbial biomass and respiratory activity. But, microbial diversity was reduced when excessive urea was applied in the tested paddy soil.

Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma： a single-center experience

BACKGROUND： Reports of fiver transplantation （LT） in patients with mixed hepatocellular carcinoma/cholangiocarcinoma （HCC/CC） and intrahepatic cholangiocarcinoma （ICC） are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC. METHODS： From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS： The observed recurrence rate post-LT was 31% （4/13） and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation Network imaging guidelines. CONCLUSIONS： Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. Additionally, incorporating adjunctive perioperative therapies such as in the case of patients undergoing LT with hilar cholangiocarcinoma may improve outcomes but this warrants further investigation.

Introduction of two novel devices for investigating the influence of non-mechanical components such as therapeutic qi in acupuncture

OBjECTIVE： Acupuncture is a complex intervention consisting of specific and non-specific components. Acupuncture studies more frequently focus on collecting data from the patients＇ perspective and response, but the acupuncturist＇s role remains relatively unclear. In order to investigate potential non-mechanical active factors originating from the acupuncturist and transmitted to the patient during treatment, two novel devices for basic research in acupuncture were designed. The Acuplicator allows the researcher to insert needles without touching the needles themselves, while the Veliusator locks the needle in its place so that no mechanical movement can be transferred. METHODS： The Acuplicator was used to insert needles at Neiguan （PC6） on the right forearm of 23 volunteers. The insertion depth was measured using a depth gauge. The transfer of mechanical movements from the handle to the tip was detected with a precision length gauge with a motoric-tactile sensor. RESULTS： The mean insertion depth was （12.3 ± 1.5） mm （range 9.5 to 15.0 mm）. Even with intense manipulation of the needle handle, no movements within ± 1 μm could be detected at the tip when the needle was locked. CONCLUSION： With these two devices it will be possible to investigate the influence of non- mechanical components such as therapeutic qi in acupuncture.

Differentiation of periapical granulomas and cysts by using dental MRI: a pilot study

The purpose of this pilot study was to evaluate whether periapical granulomas can be differentiated from periapical cysts in vivo by using dental magnetic resonance imaging(MRI). Prior to apicoectomy, 11 patients with radiographically confirmed periapical lesions underwent dental MRI, including fat-saturated T2-weighted(T2 wFS) images, non-contrast-enhanced T1-weighted images with and without fat saturation(T1 w/T1 wFS), and contrast-enhanced fat-saturated T1-weighted(T1 wFS+C) images. Two independent observers performed structured image analysis of MRI datasets twice. A total of 15 diagnostic MRI criteria were evaluated, and histopathological results(6 granulomas and 5 cysts) were compared with MRI characteristics. Statistical analysis was performed using intraclass correlation coefficient(ICC), Cohen’s kappa(κ), Mann–Whitney U-test and Fisher’s exact test. Lesion identification and consecutive structured image analysis was possible on T2 wFS and T1 wFS+C MRI images. A high reproducibility was shown for MRI measurements of the maximum lesion diameter(intraobserver ICC = 0.996/0.998; interobserver ICC = 0.997), for the "peripheral rim" thickness(intraobserver ICC = 0.988/0.984; interobserver ICC = 0.970), and for all non-quantitative MRI criteria(intraobserver-κ = 0.990/0.995; interobserver-κ = 0.988). In accordance with histopathological results, six MRI criteria allowed for a clear differentiation between cysts and granulomas:(1) outer margin of lesion,(2) texture of "peripheral rim" in T1 wFS+C,(3)texture of "lesion center" in T2 wFS,(4) surrounding tissue involvement in T2 wFS,(5) surrounding tissue involvement in T1 wFS+C and(6) maximum "peripheral rim" thickness(all: P < 0.05). In conclusion, this pilot study indicates that radiation-free dental MRI enables a reliable differentiation between periapical cysts and granulomas in vivo. Thus, MRI may substantially improve treatment strategies and help to avoid unnecessary surgery in apical periodontitis.

We here present a detailed study of the ligand-receptor interactions between single and triple-helical strands of collagen and the α2A domain of integrin(α2A),providing valuable new insights into the mechanisms and dynamics of collagen-integrin binding at a sub-molecular *** occurrence of single and triple-helical strands of the collagen fragments was scrutinized with atom force microscopy(AFM)*** interactions of the triple-stranded fragments comparable to those of collagen can only be detected for the 42mer triple-helical collagen-like peptide under study(which contains 42 amino acid residues per strand)by solid phase assays as well as by surface plasmon resonance(SPR)***,changes in NMR signals during titration and characteristic saturation transfer difference(STD)NMR signals are also detectable whenα2A is added to a solution of the 21mer single-stranded collagen *** dynamics(MD)simulations employing different sets of force field parameters were applied to study the interaction between triple-helical or single-stranded collagen fragments withα*** is remarkable that even single-stranded collagen fragments can form various complexes withα2A showing significant differences in the complex stability with identical *** results of MD simulations are in agreement with the signal alterations in our NMR experiments,which are indicative of the formation of weak complexes between single-stranded collagen andα2A in *** results provide useful information concerning possible interactions ofα2A with small collagen fragments that are of relevance to the design of novel therapeutic A-domain inhibitors.
