Liver histology according to the presence of metabolic syndrome in nonalcoholic fatty liver disease cases

AIM:To investigate the histologic features of the liver in nonalcoholic fatty liver disease (NAFLD) cases according to the presence of metabolic syndrome or its individual components. METHODS:We enrolled 81 patients (40 male,41 fe-male) who were diagnosed with fatty liver by ultraso-nographic scan and fulfi lled the inclusion criteria. First anamnesis,anthropometric,clinical,laboratory and imaging features of all participants were recorded and then liver biopsy was performed after gaining consent from patients. Diagnosis of metabolic syndrome was dependent on patients having 3 or more out of 5 risk criteria defined by the WHO. Biopsy specimens were assessed according to Brunt et al's classification. RESULTS:Sixty-nine of the 81 patients had nonalco-holic steatohepatitis (NASH),11 had simple fatty liver and 1 had cirrhosis according to histologic evaluation. Comparisons were made between two groups of NASH patients,those with and without metabolic syndrome. We did not detect statistically significant differences in liver histology between NASH patients with and wit-hout metabolic syndrome. CONCLUSION:NASH can progress without metabolic risk factors or the presence of metabolic syndrome.

AIM:To investigate the aspects of liver histology in patients with non-alcoholic steatohepatitis(NASH)who had normal aminotransferase levels. METHODS:Thirty-four patients diagnosed with liver steatosis by ultrasonographic examination participated in the *** compared all nonalcoholic fatty liver disease and NASH cases,according to aminotransferase level,aspartate aminotransferase (AST)/alanine aminotransferase(ALT)ratio and presence of metabolic syndrome. RESULTS:Sixteen of 25 patients with high aminotransferase levels were diagnosed with NASH and nine with simple fatty liver according to liver histology. Among the nine patients with normal aminotransferase levels,seven had NASH and two had simple fatty liver. The patients with normal and high liver enzyme levels had almost the same prevalence of NASH and metabolic *** histology did not reveal any difference according to aminotransferase levels and AST/ALT ratio. CONCLUSION:Aminotransferase levels and AST/ALT ratio do not seem to be reliable predictors for NASH. Despite numerous non-invasive biomarkers,all patients with fatty liver should undergo liver biopsy.

AIM:To evaluate certain anthropometric,clinical and laboratory features indicating liver fibrosis in nonalcoholic steatohepatitis and to establish the noninvasive markers for liver ***:Eighty-one patients (40 male,41 female) who were diagnosed with fatty liver by ultrasonographic examination and fulfilled the inclusion criteria participated in the ***,anthropometric,clinical and laboratory features of all cases were recorded and then liver biopsy was performed after obtaining patient ***,necroinflammation and liver fibrosis were examined according to age ≥ 45,gender,body mass index,central obesity,aspartate amino-transferase (AST)/alanine aminotransferase (ALT)>1,γ-glutamyltransferase (GGT)/ALT>1,platelet count,insulin,c-peptide levels and the presence of hypertension,diabetes,hypertriglyceridemia and insulin ***:Eighty-one patients with non-alcoholic steatohepatitis (NASH) enrolled in the study.69 of 81 patients were diagnosed with NASH,11 were diagnosed with simple fatty liver and 1 was diagnosed with ***/ALT>1,GGT/ALT>11,high serum ferritin and fasting insulin levels,the presence of diabetes,hypertension,hypertriglyceridemia and insulin resistance seemed to enhance the severity of steatosis,necroinflammation and fibrosis but these results were not statistically ***:Liver steatosis and fibrosis can occur in individuals with normal *** was no significant concordance between severity of liver histology and the presence of predictors for liver fibrosis including metabolic risk factors.

Prescription of Cancer Treatment Modalities in Developing Countries:Results from a Multi-Centre Observational Study

Background: Treatment is an important component of a comprehensive cancer control approach and its outcomes strongly depend on infrastructure, equipment, human and financial resources available. Therefore it is imperative to generate evidence-based tools to assist health policy makers from low resourced countries in planning efficient and equitable treatment services for a defined population based on what it is feasible to these settings. Methods: The intended cancer spe-cific treatment planned and written in the patients’ medical record (treatment prescription) of untreated adult cancer cases (≥18 years of age), excluding non-melanoma skin cancer, was recorded in a chronological way from 1 January 2012 onwards in a group of eight comprehensive cancer centres located in middle income countries and offering the main modalities of cancer treatment (surgery, medical oncology and radiotherapy). Results: A total of 17,713 medical records were reviewed, of which 7106 (54.2%) met the eligibility criteria. Prescription of main cancer treatment modalities were distributed as follows: 57.6% for chemotherapy (n = 4093), 56.8% for surgery (n = 4038), and 46.8% for radiotherapy (n = 3327). There was a predominance of plans consisting of combined treatment modalities over monotherapy (55.2% versus 44.8%). At the time of diagnosis 54.3% of the cancer cases had disease that had spread beyond the primary site, 41.2% were considered as having local disease and in 4.5% of the cases the information on disease extension was unknown. Conclusions: The results obtained should be seen as an approximation of cancer treatment service demand based on what it is currently practiced and therefore feasible in developing countries, particularly in middle income countries.

肺癌是全球癌症相关死亡的主要原因,非小细胞肺癌(NSCLC)占所有肺癌病例的80%～85%。随着分子生物学的发展,针对驱动基因突变(如EGFR、ALK等)的靶向药物治疗已成为晚期NSCLC的标准一线治疗。但大多数NSCLC并没有可检测到的驱动基因突变,传统的铂类化疗效果不佳。免疫检查点抑制剂的出现极大地改变了这一领域的治疗格局。PD-1/PD-L1抑制剂已被证实可以显著提高NSCLC患者的疗效和生存期,特别是PD-L1表达阳性率≥50%的人群。在晚期NSCLC中,抗PD-1抗体Pembrolizumab和Atezolizumab已被证实可显著提高PD-L1高表达人群的客观缓解率、无进展生存期和总体生存期。
