Outcomes of Endovascular Treatment of Intracranial Aneurysms at Bifurcation Sites

Background and Purpose: Endovascular coiling is widely used for treatment of both ruptured and unruptured aneurysms. Intracranial bifurcation aneurysms were classically considered unsuitable for endovascular treatment because of the risk of coil protrusion into the parent vessel. The introduction stent assisted coiling has allowed the bifurcation aneurysms to be endovascularly treated. The present study aims to evaluate the efficacy of the endovascular treatment of the intracranial bifurcation aneurysms. Methods: This study was conducted on 76 patients with radiologically documented intracranial aneurysms at bifurcation sites either ruptured (12 aneurysms) or unruptured (64 aneurysms). Endovascular coiling of intracranial aneurysms was done for all patients in our study aided in some with single or Y-configuration stenting. The radiological outcome was assessed immediately postoperative, and at follow-up at 6 and 12 month with grading of the angiograms on the basis of modified 3-point Raymond scale. Results: In terms of the Raymond grading (RG), the initial angiographic outcome was complete occlusion (RG1) in 45 aneurysms (59.2%), neck remnant (RG2) in 20 aneurysms (26.3%), and body filling (RG3) in 11 aneurysms (14.5%), while the final angiographic outcome at 1 year was RG1 in 55 aneurysms (72.4%), RG2 in 13 aneurysms (17.1%), and RG3 in 8 aneurysms (10.5%). Eleven aneurysms (14.5%) showed recanalization. The aneurysm size and the neck width were the statistically significant factors affecting the initial RG (P = 0.0005, 0.001 respectively), final RG (P = 0.015, 0.012 respectively), and the recanalization rate (P = 0.012, 0.01 respectively). Conclusions: Endovascular treatment of intracranial aneurysms at bifurcation sites is safe and effective. Stent assisted coiling (SAC) has enabled us to offer a safe and effective endovascular treatment for bifurcation aneurysms by preventing coils herniation. Natural radioactivity levels and radiation hazards for gypsum materials used in Egypt

Radionuclides naturally occurring in building materials may significantly contribute to the annual doses to the public. For instance, familiar building materials such as the concrete and gypsum board have been reported to produce a dose of about 0.04 mSv per year for a typical person (NCRP 1987c). External as well as internal exposures are two pathways of radiation dose imparted to human beings from the building materials. As information on the radioactivity of such materials is lacking, the study of gypsum materials used in Egypt was carried out in order to estimate the annual dose to the Egyptian population due to natural radionuclides in building materials. During the study, 18 samples of commonly used gypsum raw materials were collected and measured. The activity concentrations were determined by gamma ray spectrometry. Their mean values were in the ranges of 499.29 ± 11.53 Bq·kg-1 for 40K, 91.97 ± 2.61 Bq·kg-1 for 226Ra, 37.62 ± 1.67 Bq·kg-1 for 238U and 42.27 ± 2.22 Bq·kg-1 for 232Th. The activity indexed Iγ for 18, different gypsum samples varied from 0.31 ± 0.03 to 2.3 ± 0.19 and the radium equivalent activity (Raeq), from 38.81 ± 1.68 to 324.7 ± 9.42. These values are lower than the limit of 370 Bq·kg-1 adopted by the Organization for Economic Cooperation and Development (OECD). The average indoor annual effective gamma dose rate (DE) in (mSv/y) for the people, caused by the building materials of dwellings, was assessed for most commonly gypsum materials. It was estimated to be in the range from 0.10 ± 0.003 mSv/y to 0.74 ± 0.08 mSv/y. The internal and external hazard indices (Hin, Hex) and the absorbed dose rate in air D in each sample were evaluated to assess the radiation hazard for people living in dwelling made of the studied materials. The absorbed dose rate of indoor air in samples G1, G2, G11, G17 and G18 is less than the international recommended value which is 55 nGyh-1. Impact of Pre-Radiotherapy and/or Chemoradiotherapy Hemoglobin Level on Response to Treatment in Laryngeal and Hypophayrngeal Squamous Cell Carcinoma

Background: Anemia is reported by many studies as an important risk factor for poor locoregional disease control and survival in head and neck carcinoma. We aimed to study the frequency and prognostic effect of low hemoglobin (Hb) level in head and neck squamous cell carcinoma (HNSCC) before radiotherapy (RT) and/or chemoradiotherapy (CRT). Material and Methods: We retrospectively studied the charts of 86 patients diagnosed with laryngeal and hypopharyngeal SCC in a university hospital in Cairo, Egypt. Based on the World Health Organization (WHO), anemia was diagnosed in males at Hb levels 13 g/dl in 44/75 (58.7%). Tumor sites were larynx in 77 (89.5%) and hypopharynx in 9 (10.5%) patients. Disease stage 3, 4 was common in 59/86 (68.6%) patients. Thirty-six (41.8%) patients had surgery. 13/86 (15.1%) patients received induction chemotherapy, while 31/86 (36.0%) had concomitant chemoradiotherapy. Radiotherapy was given in 65/86 (75.58%) patients. The mean RT dose was 66 Gy. Non-anemic patients had significant higher inci-dence of complete response (CR) to treatment (p = 0.034). 25.8% of male pa-tients with Hb 13 g/dl (p = 0.403). Female patients whether anemic or non-anemic had no recurrence (p = 0.036 and p = 0.403 respectively). The median duration of DFS and OS was 6.52 and 9.33 months respectively. In this paper, we present a novel technique to obtain approximate analytical solution of fractional physical models. The new technique is a combination of a domain decomposition method and natural transform method called a domain decomposition natural transform method (ADNTM). The fractional derivatives are considered in Caputo sense. To illustrate the power and reliability of the method some applications are provided.

Seroprevalence of brucellosis among exposed agro-pastoral communities in southern Saudi Arabia

Objective: To investigate the prevalence and risk factors of brucellosis in human and animal’s communities in southern Saudi ***: A cross-sectional sero-epidemiological study was conducted in Aseer and Jazan, Saudi Arabia(October 2017-October 2018). Human serum samples(n=339) were initially screened for Brucella antibodies and positive samples were further titrated for Brucella antibodies by immunocapture assay(titer of ≥1:320 as positive). Animal samples(n=828) were screened using the Rose Bengal test. Relationship status was dichotomized to measure and predict independent contributions to variations in human using univariate and multivariate stepwise binary logistic regression model. Results: The rate of brucellosis among the 339 human samples in the two regions was 33.9%, and the rate of acute brucellosis was 12.4%. The rate of brucellosis in animals was 4.7%. Human brucellosis among the target groups was higher in northwestern Aseer(53.3%) compared to Southeastern Aseer(25.9%) and Jazan region(20.6%). The disease was more prevalent among non-Saudi nationals(35.2%) compared to Saudis(30.5%). The rate of brucellosis among butchers and shepherds was 37.5% and 37.2%, respectively. The rate of brucellosis was 37.8% in people over 30 years of age. Our univariate analysis showed that residing in Aseer region(OR: 2.60, 95% CI: 1.50-4.40), especially residing in northwestern Aseer region(OR: 4.40, 95% CI: 2.40-7.90), frequent consumption of raw meat(OR: 2.90, 95% CI: 1.50-5.50), shepherds(OR: 2.10, 95% CI: 0.80-5.30), owning sheep(OR: 2.20, 95% CI: 1.10-4.40), daily contact with animals(OR: 2.10, 95% CI: 0.75-5.80), and those > 30-year-old(OR: 1.50, 95% CI: 1.00-2.40) were significantly associated with increased risks of brucellosis. 压舱水对于船舶的运行和安全至关重要.外来物种能通过船舶压舱水转移到其它海域,给当地的海洋生物以及海洋生态系统平衡带来不利的影响,甚至威胁人类健康.苏丹港原油码头Bashayer II位于红海,国际海运可能会影响当地浮游植物群落结构以及由船舶压舱水引入的外来浮游植物. 2013年,每月在压舱水区域(A)和非压舱水区域(B)通过拖网采集浮游植物样本,进行显微镜观察和计数.在此次研究中,发现了5门26科44属70余种浮游植物,其中包括16种有害浮游植物.夏季占主导地位的浮游植物优势种为蓝藻门束毛藻属浮游植物,而冬季为硅藻门浮游植物,甲藻的种群生物量在整个调查期间保持稳定.调查结果表明,观察到的所有浮游植物均为红海本地种,未发现外来种.此外,在所有采样时间中,两个站位的浮游植物组成之间均未发现显着差异(p>0.05).本研究首次对红海苏丹港Bashayer II浮游植物群落结构开展调查,为苏丹红海沿岸的浮游植物种类以及与船舶压舱水有关的浮游植物的长期观测提供了基础数据.

Seasonal Variations of Some Ecological Parameters in Tigris River Water at Baghdad Region, Iraq

This study aimed at using nine ecological parameters in evaluating the quality of the Tigris River for public usage, by choosing five sampling sites along the river in Baghdad area. These parameters included temperature, pH, the saturated ratio by dissolved oxygen (%sat), biological oxygen demand (BOD5), nitrate (NO3?1), phosphate (PO4?3), fecal coliform (FC) turbidity and total dissolved solids (TDS), and these parameters were used for calculating overall water quality index in the Tigris River at study area. The results showed high values of turbidity and TDS, as well as high count of FC in all study stations, while other studied parameters were within permissible limit defined by world health organization and Iraqi criteria. The results of the overall water quality index indicated that the Tigris River was in class medium, therefore the Tigris River water in study area is relatively not safe for direct domestic use in all seasons.

Prevalence and Correlates of Prediabetes and Diabetes Results-I: A Screening Plan in a Selected Military Community in Central Saudi Arabia

Background: Diabetes is a chronic disease of a genetic and environmental background. If it is not diagnosed and controlled early, it can have devastating complications. Aim: Measure and analyze the prevalence and risks of prediabetes and diabetes among recruits enlisted in the Wadi Al-Dawasir (WD) military zone, central Saudi Arabia. The influence of some demographic criteria, weight, and blood pressure upon blood glucose level would be evaluated. Methods: A cross sectional design was utilized, whereas predesigned questionnaire and clinical interview were used to screen the study population. Results: The participants’ median age was 33.7 (IQR 11.9), mean body mass index (BMI) was 27.6 ± 5.4 kg/m2. Out of 531 recruits screened, almost two-thirds were either overweight (34.7%) or obese (29.9%). The mean waist circumference (WC) was 94.7 ± 13.3 cm;and 27.9% had central obesity (WC ≥102 cm). Random plasma glucose (RPG) median accounted 106 mg/dl (IQR 21). The participants’ RPG levels significantly increased by age (U = 19697.0), by BMI [H(df 5) = 59.6], and by WC (U = 25,670.5), (p rho(df 503) = 0.241, p U 3515, p 25 to ≥40 kg/m2) more than normal subjects [46 (87%) vs. 5 (0.9%)], [χ2(df 5) = 18.8, p Conclusions: Prediabetes and uncontrolled diabetes are relatively prevalent among Saudi recruits’ community. An insufficient influence of obesity for developing diabetes symptoms in the study population may well be attributed to time factor separating the two variables. A preventive approach to revert the predisposition of prediabetes in WD recruits population is quite promising.

The Impact of Implementing the International Convention on the Control of Harmful Anti-fouling Systems in Ships （AFS Convention） on the Marine Environment

Marine fouling is the settlement and growth of a variety of marine organisms, such as bacteria, diatoms, protozoa and algae spores on structures immersed in seawater, such as ship＇s hulls, navigation buoys, and sonar equipment. Anti-fouling refers to material or systems used to prevent the accumulation of biological material on submerged surfaces. Bio-fouling results in higher fuel consumption and can also facilitate the transport of harmful NIS （Non-Indigenous Species）. Antifouling technologies incorporating biocides （e.g., Tributyltin） have been developed to prevent fouling. Their widespread use, however, raised concerns about their toxic effects on marine communities. The AFS Convention （International Convention on the Control of Harmful Anti-fouling Systems in Ships） is a 2001 IMO （International Maritime Organization） treaty, whereby states agree to ban the use of harmful anti-fouling paints and other anti-fouling systems that contain harmful substances. Particularly, the use of the organotinTributyltin is prohibited, since leaching of that chemical from the hulls of ships has been shown to cause deleterious effects on some sea creatures. Although the AFS Convention has entered into force, its full implementation has not yet been appropriately achieved. Most of the ratifying States have delegated the Classification Society to inspect their ships to ensure the implementation of the provisions of the Convention. Since painting ships takes place in dry docks, the full control falls in the hands of Classification Societies.
