A procedure for radioiodination of valsartan with iodine-125 is carried out via an electrophilic substitution of hydrogen atom with the iodonium cation I+. All reaction parameters were studied to optimize the labeling conditions of valsartan and to obtain a maximum radiochemical yield (RCY) of the 125I-Valsartan [125I-Val]. By using 3.7 mBq of Na125I, 50 μg of valsartan (0.2 mm) as substrate, 25 μg of Iodogen (0.15 mm) as oxidizing agent in ethanol at room temperature for 30 min, the radiochemical yield of 125I-Val was 98.6% The radiochemical yield was determined by electrophoresis using cellulose acetate moistened with 0.02 m phosphate buffer pH 7. The labeled compound was separated and purified by means of high-pressure liquid chromatography (HPLC). The biological distribution in normal mice indicates the suitability of radioiodinated valsartan to image any cardiovascular disorders.

AIm: Differentiation of benign biliary strictures (BBS) from malignant biliary strictures (mBS) remains difficult despite improvement in imaging and endoscopic techniques. The aim of this study was to identify the clinical, biochemical and or radiological predictors of malignant biliary ***: We retrospectively reviewed all charts of patients who had biliary strictures (BS) on endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous cholangiography (PTC) in case of unsuccessful ERCP from march 1998 to August 2002. Patient characteristics, clinical features, biochemical, radiological and biopsy results were all recorded. Stricture etiology was determined based on cytology,biopsy or clinical follow-up. A receiver operator characteristic (ROC) curve was constructed to determine the optimal laboratory diagnostic criterion threshold in predicting ***: One hundred twenty six patients with biliary strictures were enrolled, of which 72 were malignant. The mean age for BBS was 53 years compared to 62.4 years for mBS (P=0.0006). Distal bile duct stricture was mainly due to a malignant process 48.6% vs 9% (P=0.001). Alkaline phosphates and AST levels were more significantly elevated in mBS (P=0.0002). ROC curve showed that a bilirubin level of 84 μmol/L or more was the most predictive of mBS with a sensitivity of 98.6%, specificity of 59.3% and a positive likelihood ratio of 2.42 (95% CI=0.649-0.810). Proximal biliary dilatation was more frequently encountered in mBS compared to BBS, 73.8% vs39.5% (P=0.0001). majority of BBS (87%) and mBS (78%) were managed ***: A serum bilirubin level of 84 μmol/L or greater is the best predictor of mBS. Older age, proximal biliary dilatation, higher levels of bilirubin, alkaline phosphatase, ALT and AST are all associated with mBS. ERCP is necessary to diagnose and treat benign and malignant biliary strictures.

Background: As professionals, family physicians are obliged to remain current on advances and trends in medicine and health care delivery. This is usually achieved through engagement in continuing professional development. Instructional design is a systematic method of development of education and training programs for improved learner performance. ADDIE is an instructional systems design model for building effective education and training in five phases: analysis, design, development, implementation and evaluation. Purpose: The purpose of this study was to introduce a professional development program for primary care physicians using the ADDIE instructional design model. methods: Program requirements were defined using a needs assessment questionnaire and consultation observations. Interactive sessions were designed and developed based on the analysis results. The sessions were evaluated with interim and final feedback forms, a final problem-based questionnaire, a self-assessment questionnaire, and focus groups. Results: Scores on the final knowledge assessment were lower than expected. However, at least 50% of participants self-reported their learning improvement as “great” for 16 out of 23 topics. Focus group feedback was generally positive but also identified areas for improvement. Conclusion: Applying a structured instructional design model for creating professional development program for physicians is a fruitful, relevant experience in primary healthcare. 1) Continuing professional development (CPD) is an essential method to help physicians maintain and further develop knowledge and expertise;2) The ADDIE (analysis, design, development, implementation, evaluation) model provides an established and useful structure for creating effective CPD programs;3) The ADDIE process ensures that physicians’ appropriate learning needs are met effectively;4) The evaluation phase of the ADDIE process provides feedback that can lead to improvement in the CPD program’s future

Background: The aim of this study was to establish normal reference values for several growth factors in semen and provide a basis for understanding their physiological significance in fertile and infertile subjects. methods: 110 infertile men semen was evaluated according to WHO standards and by enzyme immunoassay (ELISA kit protocol) to determine the rate of growth factors (TGFβ1, IGFI, EGF). We have also listed neutrophils by staining with peroxidase to examine the association of inflammation with the levels of these factors. The determination of the epididymal (alpha-1,4-glucosidase) marker was directed according to the technical recommended by the WHO in 2010. Each patient received a hormonal balance including the determination of plasma FSH, LH and Testosterone. Results: Concentrations of TGFβ1 were significantly higher in (severe and extreme azoospermic, oligospermic) pathological groups compared to the control group (P 0.05) between control and pathological groups. The IGFI is partly testicular and/or epididymal as it is lowered in some patients with obstructive azoospermia. Positive correlations were observed between the seminal IGFI, mobility (r = 0.487) and sperm morphology (r = +0.187). Immunological sperm abnormalities and/or idiopathic do not alter the rate of EGF seminal. There is no significant correlation between seminal EGF or with sperm parameters or with plasma levels of FSH and LH. Conclusion: The growth factors are primarily peptides, handling and identification in a complex environment as the seminal plasma, but it requires a lot of precautions if we do not want to expose to the risk of bad farms/interpretations of data obtained.

Field an experiment was carried out in the farms of Agriculture and forestry academy at University in Nineveh. The research was conducted to investigate a practical study for the effect of four forward speeds (1.9, 2.6, 3.8 and 4.7 km/hr.) and three acceleration direction of axes lateral, longitudinal and vertical, and two types machines control (mower and Rotovators) on the vibrations transferred to the steering wheel horticulture tractor type (Goldoni). The vibration points on the handgrip were calculated and tested. Root mean square acceleration (RmS), given in m/sec2, was calculated. Results showed increased acceleration vibration of the three directions (longitudinal, lateral and vertical) transferred to the steering wheel tractor by increased forward speed. The mower score recorded the highest acceleration vibration for the three directions of Rotovators. The levels of vibration emitted from tractor to hand an operator during the experiment was high comparing with standard mechanical vibration. Handgrip vibration intensity in the vertical direction is bigger than the lateral and longitudinal direction. The total vibration evaluating was denoted as the square root mean of the three sum value (lateral, longitudinal and vertical) directions. The paper purpose was measuring and analyzing vibration level transferred to the steering wheel and reduces the machine vibration. This paper is helpful for design in order to increase the develop safety systems in easy and economical way.

Objective:To investigate the prevalence of container breeding mosquitoes with emphasis on the seasonality and larval habitats of Aedes aegypti(***) in makkah City,adjoining an environmental monitoring and dengue ***:monthly visits were performed between April 2008 and march 2009 to randomly selected *** each visit,mosquito larvae were collected from indoors and outdoors containers by either dipping or pipetting. mosquitoes were morphologically *** on temperature,relative humidity,rain/ precipitations during the survey period was retrieved from governmental sources and analyzed. Results:The city was warmer in dry season(DS) than wet season(WS).No rain occurred at all during DS and even precipitations did fall,wetting events were much greaterduring *** survey revealed the co-breeding of Aedes,Culex and Anopheles in a variety of artificial containers in and around homes.32109 larvae representing 1st,2nd,3rd,and 4th stages were collected from 22618 container *** was far the commonest and Aedes genus was as numerous as the Culex *** larval abundance exhibited marked temporal variations,overall, being usually more abundant during *** types of artificial containers were found with developing larvae.70%of these habitats were located indoors.71.42%of indoor containers were permanent and 28.58* was semi-permanent during *** tanks was the only container type permanent during *** larval indices(CI,HI,Bl) recorded were greater during WS. Conclusions:Taken together,these results indicate a high risk of dengue transmission in the holy city.

PVC(聚氯乙烯)基体和液膜电极直接电位法测定金(Ⅲ)已得到了发展,将膜同硝酸灵四氯金酸盐(Ⅲ)结合作为电活性物质,能迅速响应浓度范围10^(-5)—0.1m的金(Ⅲ),得到的响应斜率为52.8—55.2毫伏/组(十个一组),在许多阴阳离子存在的情况下,pH值为2—5时,电极对金(Ⅲ)有较好的选择性,PVC膜电极具有选择性较好(Cr^(3+)、mn^(2+)和ClO_4^-除外)和热稳定性较高的优点,与PVC膜电极相比,液膜电极响应斜率高、响应时间短。在2微克/毫升—2毫克/毫升的测定范围内,AuCl_4^-的平均回收率为98.5％,平均值的标准偏差为1.0％,测定某些金合金(含金58.3—99.9％)和药品制剂中的金,平均回收率为99.4％,平均值的标准偏差为0.7％,该方法的测定结果可与孔雀绿分光光度法和美国药典重量法的测定结果相媲美。
