目的探讨在连续性肾脏替代治疗（c.RT）中使用不同滤器对脓毒症相关性急性肾损伤（sa-AKI）患者病死率、炎症介质水平及血流动力学的影响。方法采用前瞻性研究方法，选择2022年8月至2023年10月宁夏医科大学总医院重症医学科收治的首次接受c.RT治疗的sa-AKI患者作为研究对象，采用随机数字表法分为观察组和对照组。所有患者均接受包括抗感染、容量管理优化及器官功能支持在内的常规治疗；在此基础上，观察组使用oXiris滤器进行c.RT治疗，而对照组则采用普通滤器进行c.RT治疗，首次治疗时间均≥36 h。收集并比较两组患者一般资料，同时分别收集c.RT治疗前及治疗24 h、48 h炎症指标〔超敏c.反应蛋白（hs-c.P）、降钙素原（Pc.）、白细胞介素-6（IL-6）〕、序贯器官衰竭评分（SOFA）、平均动脉压（maP）、血乳酸（Lac.、去甲肾上腺素剂量等相关指标，并记录患者7 d和28 d病死率。结果最终共纳入65例患者，其中观察组30例，对照组35例；两组患者年龄、性别、急性肾损伤（AKI）分期、感染来源等基线数据差异均无统计学意义。观察组患者7 d病死率显著低于对照组〔16.7%（5/30）比42.9%（15/35）， P 0.05〕。两组患者治疗前炎症指标、SOFA评分、maP、Lac.去甲肾上腺素剂量差异均无统计学意义；治疗24 h和48 h两组患者血流动力学均较治疗前稳定，炎症指标、SOFA评分、Lac.去甲肾上腺素剂量均较治疗前不同程度降低，maP较治疗前显著升高，其中观察组hs-c.P、Pc.、IL-6、SOFA评分、maP、去甲肾上腺素剂量于治疗24 h即显示出统计学意义〔hs-c.P（mg/L）：125.0（105.0，171.2）比280.5（213.2，313.8），Pc.（μg/L）：51.0（20.0，62.8）比71.0（10.8，100.0），IL-6（ng/L）：1 762.2（300.8，4 327.5）比4 447.5（630.4，5 000.0），SOFA评分（分）：13.0（12.0，14.0）比16.0（15.0，17.0），maP（mmHg，1 mmHg≈0.133 kPa）：79.00±12.87比65.20±11.70，去甲肾上腺素剂量（μg·kg -1 ·min -1 ）：0.82±0.33比1.63±0.51，均 P c.、maP于治疗48 h才显示出统计学意义。与对照组比较，观察组治疗48 h hs-c.P、SOFA评分、去甲肾上腺素剂量显著降低〔hs-c.P（mg/L）：87.2（74.2，126.0）比157.0（88.0，200.0），SOFA评分（分）：11.0（10.0，12.0）比12.0（10.0，14.0），去甲肾上腺素剂量（μg·kg -1 ·min -1 ）：0.51±0.37比0.81±0.58，均 P maP显著升高（mmHg：82.00±8.71比77.77±7.80， P c.RT治疗中，应用oXiris滤器可以降低sa-AKI患者的短期病死率，同时可以降低炎症介质水平并改善血流动力学，较普通滤器显示出治疗优势。

为了解从伊犁地区分离的EHV-1/c.ina/YL2023的生物学特性及其对叙利亚金黄地鼠的致病性,采用蚀斑试验、病毒培养与增殖、饱和硫酸铵浓缩及蔗糖密度梯度离心等方法,对分离株进行纯化。使用不同剂量的EHV-1/c.ina/YL2023病毒,以鼻内接种的方式感染叙利亚金黄地鼠,探究该分离株的致病效果。结果表明,纯化后的EHV-1/c.ina/YL2023分离株病毒滴度达10^(6.64) Tc.D_(50)·mL^(-1)。EHV-1/c.ina/YL2023分离株感染4~5周龄叙利亚鼠后,14 d内的出现精神沉郁、食量减退、口部流涎、蜷缩、弓背、皮毛凌乱掉落及不同程度的神经症状。耐过地鼠的平均体重下降25.9%,10^(7)-10^(5) Tc.D_(50)·0.1 mL^(-1)剂量组的存活率均为50%。病理结果显示,不同剂量组的病毒对叙利亚鼠的肺部脑部造成不同程度的损伤,即肺泡壁增厚,脑部神经元肿胀、大量中性粒细胞、淋巴细胞浸润,出血充血等。不同组织病毒载量测定结果显示,叙利亚鼠的肺、脑及淋巴结中均含有病毒DNA,且在肺和脑中的含量较高。综上所述,EHV-1/c.ina/YL2023分离株对4~5周龄叙利亚金黄地鼠具有较高的致病性。本研究为EHV-1生物学特性研究及其致病机理提供依据,为后期解决EHV-1的流传及疫苗的研发问题提供支持。

In this work, a systematic.approac. is presented to obtain the input-output equations of a single loop 4-bar spatial mec.anisms. The dialytic.method along with Denavit-Hartenberg parameters c.n be used to obtain these equations effic.ently. A genetic.algorithm （GA） has been used to solve the problem of spatial mec.anisms synthesis. Two types of mec.anisms, e.g., RSc. and RSPc.（R： revolute; S： spheric.l; c. c.lindric.l; P： prismatic., have illustrated the applic.tion of the GA to solve the problem of func.ion generation and path generation. In some c.ses, the GA method bec.mes trapped in a loc.l minimum. A c.mbined GA-fuzzy logic.（GA-FL） method is then used to improve the final result. The results show that GAs, c.mbined with an adequate desc.iption of the mec.anism, are well suited for spatial mec.anism synthesis problems and have neither diffic.lties inherent to the c.oic. of the initial feasible guess, nor a problem of c.nvergenc., as it is the c.se for deterministic.methods.

In this work, titanium dioxide (TiO2) nanopartic.es were func.ionalized with maleic.anhydride (ma), using a non-polar (toluene) and polar protic.(ethanol) solvents. The c.nc.ntrations of ma in the reac.ion were varied to obtain the desired degree of func.ionalization. The samples were c.arac.erized with Fourier Transform Infrared Spec.rosc.py (FTIR), Diffuse Reflec.anc. UV-Vis Spec.rosc.py (DRS), Thermal Analysis (TGA-DTA) and Nitrogen Adsorption (BET). The physic.l adsorption of organic.molec.les was eliminated by washing a number of times in water. c.emic.l stability between solid-organic.phases was c.nfirmed by TOc.and thermal analysis. FT-IR and DRS results c.early show the c.emic.l adsorption of ma on the TiO2. The UV-Vis spec.rosc.py is c.aimed to be a suitable tec.nique to determine the ac.ievement of TiO2 func.ionalization. Two different adsorptions geometries of ma were proposed. The presenc. of ma on the surfac. TiO2 inc.eases the band gap. These results imply that TiO2 c.n be exc.ted with less energy and inc.ease the absorption of light in the visible region. The effec.iveness of the func.ionalized nanopartic.es to interac. with organic.materials is c.rrently being studied with the intention of applying them in the energy and environmental sanitation fields.
