Objectives: The main objective was to evaluate antibiotic prescribing in the Department of Pediatrics at gabriel Touré teaching hospital. Methods: This was a prospective descriptive study conducted from September 1 to November 31, 2018, in the Department of Pediatrics at gabriel Touré teaching hospital. All children aged 0 to 15 years hospitalized for any pathology during the study period and having received at least one antibiotic had been included. Results: We collected 445 children’s records out of 1032 admissions during the study period, representing a hospital frequency of 43.1%. The sex ratio was 1.3%. The 2 - 5 age group accounted for 48.1%. Fever was the reason for consultation in 45.6% of cases. Patients’ general condition was altered in 60% of cases. The fathers were blue-collar workers in 65.4% and the mothers housewives in 85%, and had no education in 42.9% and 64.5% respectively. Hepatomegaly was present in 18.2%, splenomegaly in 9.6% and peripheral adenopathy in 3.1%. The site of infection was pulmonary in 37.6% and ENT in 9.2%. bacterial infection was assumed on admission in 54% of cases, and meningitis in 57.7%. The discharge diagnosis was malaria in 54.6%, severe acute malnutrition in 18.6% and meningitis in 6.7%. The death rate was 3.8%. Neutrophilic leukocytosis was present in 47.3% of patients. CRP was positive in 85% of patients. Blood cultures taken in 27.6% of patients were positive in 5. CSF analysis in 30% of patients showed elevated leukocytes in 6.5%. No cultures were positive. Antibiotic prescription was justified by infectious hypotheses in 43.1% of patients. β-lactam antibiotics were prescribed in 98.6%. Antibiotic therapy was not adapted to national/international recommendations in 68.3% of cases, and was not justified in 16.3% of cases on D5 of hospitalization. Conclusion: Antibiotic use was justified in more than half of patients, but remained inadequate in almost two-thirds of cases, in line with national and international recommend

When contributing to participatory research, farmers usually appreciate the performance of cowpea varieties using qualitative scores. The score they attribute to each variety are based on local knowledge. The specific criteria they individually use to attribute a score are not well described. The objectives of this work were to: 1) identify and describe exhaustively the local criteria used by farmers to measure the agronomic performance of cowpea;2) assess the variability and statistical structure of these farmer criteria across local contexts;3) and analyze the association between these farmer criteria and the classical agronomic measurement. To achieve these objectives, an augmented block design was implemented across fifteen locations in the regions of Maradi, Dosso and Tillabéri, representing a diversity of local contexts. From a set of 36 cowpea varieties, fifteen varieties were sown per location, including five varieties (controls) common to all locations. In each location, two replicates were sown in randomized Fisher’s blocks. After agronomic measurement and participatory evaluation (scoring of varieties by farmers), a group survey (focus group) was conducted in each location to identify the criteria considered by farmers to found their discretional scoring of varieties during the participatory evaluation. The analysis of the data identified, across locations, thirteen criteria defined by farmers to characterize the agronomic performance of cowpea. Some of these criteria were different according to location. Farmers ranked the three varieties with the best performance for each agronomical trait (Top 3 varieties). A comparison of the farmer ranking with the ranking based on agronomic measurements revealed similarity and complementary between both methods. This study highlighted the importance of considering both local and scientific knowledge in local varietal evaluations.

Cervico-facial cellulitis is a serious poly-microbial infection of the deep fascial spaces of the face and neck which may be life-threatening. The existence of an underlying ground that causes immune failure may promote or complicate this infection. The aim of this work was to describe the socio-demographic and clinical aspects of dental cervicofacial cellulitis associated with HIV in our context. It was a descriptive cross-sectional study that concerned all cases of cervico-facial cellulitis with positive HIV serology encountered at the CHU-CNOS of bamako from May 2017 to January 2018. Twenty-four cases were collected. Cellulite with positive HIV accounted for 1.14% of consultations in the service. Females accounted for 67.66% of cases with a sex ratio of 0.5. The average age was 33.58 years old. Housewives accounted for 33.33% of cases. The bride and groom represented 83% of the cases. The triad (pain, swelling, and trismus) was the most common reason for consultation with 41.7% of cases. The tooth 37 was involved in 25% of the cases. In 87% of cases, patients were screened in the service. HIV-1 was found in 75.0% of cases. Complications such as peripheral facial palsy were found in three cases. The medico-surgical treatment was performed in 23 cases with a favorable evolution. The search for an underlying ground, in particular HIV infection, must be systematic in the face of diffuse cervicofacial cellulitis.

Objectives: The main objective was to study the epidemiological, diagnostic and therapeutic aspects of medullary aplasia (MA). Methods: This was a prospective and descriptive retro study conducted from January 1, 2008 to December 31, 2018 in the pediatric oncology unit of the pediatrics department of the gabriel Touré teaching Hospital in bamako. Results: We collected 29 children’s cases out of 1632 admissions during the study period, representing a frequency of 1.8% and an incidence of 2.6 cases per year. The sex ratio was 2.6. The 11 - 15 age group accounted for 45%, with an average age of 8.93 years. The majority of fathers (55.2%) and mothers (62.1%) had received no education;they were mainly farmers (62.1%) and housewives (86.2%). The average consultation time was 92.21 days. Anemia was the reason for consultation in 69% of cases. Pallor was present on admission in 96.5%;infectious syndrome accounted for 79.3%, anemic syndrome for 51.7% and hemorrhagic syndrome for 27.6%;the three syndromes were associated in 27.6%. Malaria was associated with bone marrow aplasia in 58.6%. Anemia was present in 93.1%, neutropenia in 65.5% and thrombocytopenia in 86.2%. All had received a labile blood product (LBP) transfusion, and 24 (83%) had received antibiotics. Patients were treated with corticosteroids (58.6%), androgens (20.7%) and immunosuppressants (20.7%). The death rate was 34.6%. At last count, 15 (83%) had discontinued treatment, 2 (11%) were undergoing treatment and 1 (6%) was in remission. Conclusion: Effective treatment of MA requires improved technical facilities and better economic conditions for parents.

background: Anastatica hierochuntica L. (Brassicaceae) has been claimed to treat various conditions, including complicated childbirth. The principal objective of this research was to provide scientific evidence on the contractile effect of the aqueous and hydroethanolic extracts of the whole plant from A. hierochuntica during labor. Method: Qualitative and quantitative methods helped identify and quantify the phytoconstituents in both extracts. The antioxidant activity was studied using four standard methods (ABTS, DPPH, FRAP, and LPO). Results: The inhibitory power of the extracts on pro-inflammatory enzymes such as 15-lipoxygenase and phospholipase A2 was determined. The contractile effects of the extracts were evaluated using isolated pregnant mice uteri. Steroids, triterpenoids, flavonoids, tannins, coumarins, and reduced compounds were identified in both extracts. The hydroethanolic extract exhibited higher content of hydrolyzable tannins (15.07 ± 0.24 mg tannic acid equivalent/g) than the aqueous decoction (11.8 ± 0.69 mg TAE/g). The extracts demonstrated the ability to scavenge DPPH and ABTS radicals, reduce ferric ions, and inhibit especially lipid peroxidation. No significant difference was noted in the phospholipase inhibition between the extracts and betamethasone. The hydroethanolic extract displayed the most significant anti-lipoxygenase activity with an IC50 value of 55.82 ± 1.15 μg/mL. The lethal dose (LD50) of extracts was estimated to be superior to 2000 mg/kg body weight (bw). The ability of the samples to produce one cm amplitude of contraction at 50% effective concentrations (EC50) was 5.33 × 10−8 ± 0.32 mg/mL for oxytocin, 1.51 ± 0.08 mg/mL for aqueous extract and 3.57 ± 0.61 mg/mL for hydroethanolic extract. Conclusion: These findings could provide evidence for using Anastatica hierochuntica to facilitate childbirth.

Objective: The main objective was to study the disturbances of the Blood Count of children hospitalized in the general pediatric of the gabriel Touré teaching hospital. Methods: This was a prospective and descriptive study conducted from September 1 to November 30, 2018 in the general pediatrics department of the gabriel Touré teaching hospital in bamako. Data were collected on patient records using a survey sheet. Results: We collected 512 files of children out of 1030 admissions during the study period;the rate of completion of the blood count is 50%. The male sex was predominant with a sex ratio of 1.3. The majority of patients were under 5 years old (58%). The majority of fathers (56%) and mothers (64%) of children had no education;they are mainly farmers (61%) and housewives (88%). Pallor was the reason for consultation in 29% of patients and present in 60%. On blood count, anemia was present in 92% of patients, half of whom (50%) had severe anemia with a hemoglobin level below 7 g/dL. The anemia was mainly microcytic (72%) and hypochromic (66%). Hyperleukocytosis (62%), eosinophilia (68%) and basophilia (58%) were the abnormalities observed in the white line. Thrombocytopenia accounted for 40%. Severe malaria (53%) was the main discharge diagnosis and almost all patients (99%) were alive at discharge. Conclusion: The characteristics of anemia require a study of the complete blood count in healthy children with dosage of serum iron and ferritin for a better understanding of the phenomenon.

由于在利用蚁群算法构建差异工件(即工件有尺寸差异)单机批调度问题的解时,批的加工时间是不确定的,从而不能类似于经典调度问题的蚁群算法把批加工时间的倒数作为蚁群算法中的启发式信息,引入批的利用率和批的负载均衡率作为蚁群算法中的启发式信息,提出了JACO(ant colony optimization based a job sequence)和baCO(ant colony optimization baseda batch sequence)两种蚁群优化算法.在算法JACO中,解的编码为工件序列,它对应着用BF(best fit)分批规则生成的调度方案,信息素代表工件间的排列顺序;在算法baCO中,解的编码为批序列,信息素代表工件间的批相关性,由此信息素通过中间信息素量来构造相应的解,并引入特定的局部优化策略,提高了算法的搜索效率.实验表明,与以往文献中的sa(simula-ted annealing)、ga(genetic algorithm)算法以及FFLPT(first-fit longest processing time)、BFLPT(best-fit longest processing time)启发式规则相比,算法JACO和baCO明显优于它们,且baCO算法比JACO算法效果更好.

高速电梯存在导轨多源激励关联性和多源激励耦合作用引起轿厢水平振动的问题,针对这一问题,提出了基于导轨多激励关联的轿厢水平振动预测方法。首先,建立了轿厢的水平振动模型,分析了导轨多源激励因素,借助Dematel方法获得了导轨多源激励关联重要度;然后,以导靴等效刚度、导靴等效阻尼、上方导靴到轿厢质心的垂直距离、下方导靴到轿厢质心的垂直距离作为输入,将水平振动加速度峰值和水平振动角加速度峰值作为输出,建立了轿厢水平振动支持向量机预测模型,并利用布谷鸟算法对支持向量机惩罚因子和RBF核参数进行了寻优搜索;最后,利用拉丁超立方抽样方法得出了一系列方案,通过Simulink仿真得出了各方案的水平振动加速度峰值和角加速度峰值,并将其作为预测模型的样本数据,分别建立了CS-SVR、SVR、RBF、BP、ga-BP预测模型,进行了对比实验;采用决定系数、均方误差、平均相对误差和最大相对误差对该预测模型进行了评价。研究结果表明:与KLK2样机实验相比,CS-SVR预测结果误差约为5.43%,预测效果最优;该结果验证了所提方法的有效性。
