Objective:Benefits of adjuvant treatment in pT1 N1 gastric cancer(GC)remain ***,an effective biomarker for early GC is the need of the *** prognostic and predictive roles of single patient classifier(SPC)were validated in stageⅡ/Ⅲ*** this study,we aimed to elucidate the role of SPC as a biomarker for pT1 N1 ***:The present retrospective biomarker study(NCT03485105)enrolled patients treated for pT1 N1 GC between 1996 and 2012 from two large hospitals(the Y cohort and S cohort).For SPC,mRNA expression of four classifier genes(GZMB,WARS,SFRP4 and CDX1)were evaluated by real-time reverse transcription-polymerase chain reaction *** SPC was revised targeting pT1 stages and the prognosis was stratified as high-and lowrisk group by the expression of SFRP4,a representative epithelial-mesenchymal transition ***:SPC was evaluated in 875 patients(n=391 and 484 in the Y and S cohorts,respectively).Among 864 patients whose SPC result was available,41(4.7%)patients experience GC *** to revised SPC,254(29.4%)patients were classified as high risk[123(31.5%)and 131(27.1%)in the Y and S cohorts,respectively].The high risk was related to frequent recurrence in both Y and S cohort(log-rank P=0.023,P<0.001,respectively),while there was no difference by GZMB and WARS *** analyses of the overall-cohort confirmed the high risk of revised SPC as a significant prognostic factor[hazard ratio(HR):4.402(2.293-8.449),P<0.001]of GC.A significant difference was not detected by SPC in the prognosis of patients in the presence and absence of adjuvant treatment(log-rank P=0.670).Conclusions:The present study revealed the revised SPC as a prognostic biomarker of pT1 N1 GC and suggested the use of the revised SPC for early-stage GC as like stageⅡ/Ⅲ.

AIM:To evaluate the potential usefulness of acoustic radiation force impulse (ARFI) images for evaluation of hepatocellular carcinomas (HCC)-associated radiofrequency ablation. METHODS:From January 2010 to June 2010,a total of 38 patients with HCC including recurred HCCs after RFA underwent ARFI elastography. The brightness of tumor was checked and the shear wave velocity was measure-d for the-quantification of stiffne-ss. According to theb-rightne-ss,the-tumors we-re-classifie-d as b-righte-r,samecolor and darker compared with adjacent parenchyma. Using the same methods,8 patients with recurred HCCs after RFA state were evaluated about the brightness compared with adjacent RFA ablation area. RESULTS:In the 38 patients with HCCs,20 (52.6%)were brighter than surrounding cirrhotic parenchyma. Another 13 (34.2%) were darker. The others (5 cases,13.2%) were seen as the same color as the adjacent liver parenchyma. Post-RFA lesions were darker than previous tumor and surrounding parenchyma in all 38 cases. However,recurred HCCs were brighter than the treated site in all 8 cases. CONCLUSION:Using ARFI technique is helpful for differential diagnosis in order to detect recurred HCCs more easily in patients with confusing status.

AIM： To identify the clinical features and outcomes of infrequently reported leptomeningeal carcinomatosis （LMC） of gastric ***： We analyzed 54 cases of cytologically confirmed gastric LMC at four institutions from 1994 to ***： The male-to-female ratio was 32：22, and the patients ranged in age from 28 to 78 years （median,48.5 years）. The majority of patients had advanced disease at initial diagnosis of gastric cancer. The clini-cal or pathologic tumor, node and metastasis stage ofthe primary gastric cancer wasin 38 patients （70%）.The median interval from diagnosis of the primarymalignancy to the diagnosis of LMC was 6.3 mo, rang-ing between 0 and 73.1 mo. Of the initial endoscopic f indings for the 45 available patients, 23 （51%） of the patients were Bormann typeand 15 （33%） patientswere Bormann type. Pathologically, 94% of cases proved to be poorly differentiated adenocarcinomas. Signet ring cell component was also observed in 40% of patients. Headache （85%） and nausea/vomiting （58%） were the most common presenting symptoms of LMC. A gadolinium-enhanced magnetic resonance imaging was conducted in 51 patients. Leptomeningeal enhancement was noted in 45 cases （82%）. Intrathecal （IT） chemotherapy was administered to 36 patients-primarily methotrexate alone （61%）, but also in combi-nation with hydrocortisone/± Ara-C （39%）. The median number of IT treatments was 7 （range, 1-18）. Concomitant radiotherapy was administered to 18 patients, and concomitant chemotherapy to seven patients. Sev-enteen patients （46%） achieved cytological negative conversion. Median overall survival duration from the diagnosis of LMC was 6.7 wk （95% CI： 4.3-9.1 wk）. In the univariate analysis of survival duration, hemoglobin, IT chemotherapy, and cytological negative conversion showed superior survival duration （P = 0.038, P = 0.010, and P = 0.002, respectively）. Polyalkenoate cement(PAC)is a promising material for regenerative hard tissue *** ionically rich glass component of PAC encourages bioactive interaction *** release of essential ***,PAC bioactivity is restricted owing to(i)structurally inherent cationic network formers and(ii)surface bacterial biofilm *** two factors cause a deficiency in ion release,further complicated by secondary infections and premature therapeutic ***,a multivalent zwitterionic network modifier(mZM)is presented for upregulation of ionic exchange and bioactivity *** introducing a non-zero charged mZM into PACs,an increase in the proportion of non-bridging oxygen *** network modification promotes ion channel formation,causing a multiple-fold increase in ion release and surface deposition of hydroxy-carbonate apatite(ca.74%).Experiments ex vivo and animal models also demonstrate the efficient remineralization ability of the ***,divalent cationic interaction results in bacterial biofilm reduction(ca.68%)while also influencing a shift in the biofilm species composition,which favors commensal ***,PAC modification with mZM offers a promising solution for upregulation of bioactivity,even aiding in customization by targeting site-specific regenerative therapy in future applications.

OBJECTIVE:The interleukin(IL)-12 cytokine family is closely related to the development of T helper cells,which are responsible for autoimmune disease enhancement or ***-12 family members are generally heterodimers and share threeα-subunits(p35,p19,and p28)and twoβ-subunits(p40 and EBI3).However,aβ-sheet p40 homodimer has been shown to exist and antagonize IL-12 and IL-23 signaling ***,we assumed the existence of a p40-EBI3 heterodimer in nature and sought to investigate its role in immune ***:The presence of the p40-EBI3 heterodimer was confirmed by ELISA,immunoprecipitation,and western blotting.A p40-EBI3 vector and p40-EBI3-Fc protein were synthesized to confirm the immunological role of this protein in mice with collageninduced arthritis(CIA).The anti-inflammatory effects of p40-EBI3 were analyzed with regard to clinical,histological,and immune cell-regulating features in mice with ***:Clinical arthritis scores and the expression levels of proinflammatory cytokines(e.g.,IL-17,IL-1β,IL-6,and TNF-α)were significantly attenuated in p40-EBI3-overexpressing and p40-EBI3-Fc-treated mice with CIA compared to vehicle-treated mice with *** joint damage and vessel formation-related gene expression were also reduced by p40-EBI3 heterodimer *** vitro,the p40-EBI3-Fc protein significantly suppressed the differentiation of Th17 cells and reciprocally induced CD4+CD25+Foxp3+(regulatory T)cells.p40-EBI3 also inhibited osteoclast formation in a concentrationdependent ***:In this study,p40-EBI3 ameliorated proinflammatory conditions both in vivo and in *** propose that p40-EBI3 is a novel anti-inflammatory cytokine involved in suppressing the immune response through the expansion of Treg cells and suppression of Th17 cells and osteoclastogenesis.
