Preliminary study on a single balloon cross-midline expansion via unipedicular approach in kyphoplasty

Background Recently, bilateral transpedicular kyphoplasty with a single balloon was performed for osteoporotic vertebral body compression fractures （OVCFs） to reduce the fees of the operation, but the time of operation and radiation exposure are longer. The aim of this study was to determine the safety and effectiveness of a single balloon cross-midline expansion via unipedicular approach in kyphoplasty for OVCFs. Methods Thirty-six patients with painful OVCFs （61 vertebrae） were enrolled in this research. Unilateral transpedicular puncture was performed under the fluoroscopy monitoring of an oblique angle down the pedicle. A single balloon was introduced through unipedicular approach, The final balloon position was in the midline of the vertebral body with the balloon cross-midline expansion and bone cement filled. Clinical outcomes were determined by comparison of the preoperative and postoperative visual analogue scale （VAS） and Oswestry disability index （ODI）. Radiographic assessment included restoration of vertebral height and correction of kyphosis. Follow-up was conducted for 6-12 months （mean 9.2 months）. Results Thirty-six consecutive patients with 61 vertebrae were successfully operated on with a mean operation time of 37.4 minutes per vertebra. All patients had dramatic pain relief and functional recovery within 96 hours after the procedure with no surgery or device-related complications. VAS score improved from 7.27±1.02 preoperatively to 2.71±0.75 postoperatively （P 〈0.01）. ODI score was decreased from （71.14±10.94）% preoperatively to （26.56±6.35）% postoperatively. The average loss of anterior body height was （14.33±2.76） mm before procedure and （10.03±1.83） mm after procedure （P 〈0.01）, while the average loss of middle body height was （10.15±2.70） mm before procedure and （5.89±1.83） mm after procedure （P 〈0.01）. EYA4 gene functions as a prognostic marker and inhibits the growth of intrahepatic cholangiocarcinoma

The rubber tree,Hevea brasiliensis,produces natural rubber that serves as an essential industrial raw ***,we present a high-quality reference genome for a rubber tree cultivar GT1 using single-molecule real-time sequencing(SMRT)and Hi-C technologies to anchor the～1.47-Gb genome assembly into 18 *** chromosome-based genome analysis enabled us to establish a model of spurge chromosome evolution,since the common paleopolyploid event occurred before the split of Hevea and *** show recent and rapid bursts of the three Hevea-specific LTR-retrotransposon families during the last 10 million years,leading to the massive expansion by～65.88%(～970 Mbp)of the whole rubber tree genome since the divergence from *** identify large-scale expansion of genes associated with whole rubber biosynthesis processes,such as basal metabolic processes,ethylene biosynthesis,and the activation of polysaccharide and glycoprotein lectin,which are important properties for latex production.A map of genomic variation between the cultivated and wild rubber trees was obtained,which contains～15.7 million high-quality single-nucleotide *** identified hundreds of candidate domestication genes with drastically lowered genomic diversity in the cultivated but not wild rubber trees despite a relatively short domestication history of rubber tree,some of which are involved in rubber *** genome assembly represents key resources for future rubber tree research and breeding,providing novel targets for improving plant biotic and abiotic tolerance and rubber production.

As one of the three payloads for the Advanced Space-based Solar Observatory(ASO-S)mission,the Lyman-alpha(Lyα)Solar Telescope(LST)is composed of three instruments:a Solar Corona Imager(SCI),a LyαSolar Disk Imager(SDI)and a full-disk White-light Solar Telescope(WST).When working in-orbit,LST will simultaneously perform high-resolution imaging observations of all regions from the solar disk to the inner corona up to 2.5 R⊙(R⊙stands for the mean solar radius)with a spatial resolution of 4.8′′and 1.2′′for coronal and disk observations,respectively,and a temporal resolution of 30–120 s and 1–120 s for coronal and disk observations,*** maximum exposure time can be up to20 s due to precise pointing and image stabilization *** the three telescopes of LST,SCI is a dual-waveband coronagraph simultaneously and independently observing the inner corona in the HI Lyα(121.6±10 nm)line and white light(WL)(700±40 nm)wavebands by using a narrowband Lyαbeam splitter and has a field of view(FOV)from 1.1 to 2.5 R⊙.The stray-light suppression level can attainline and 360.0 nm wavebands,respectively,which adopt an off-axis two-mirror reflective structure with an FOV up to 1.2 R⊙,covering the inner coronal edge area and relating to coronal *** present the up-to-date design for the LST payload.

目的评价双球囊与单球囊椎体后凸成形术治疗骨质疏松性脊柱压缩骨折的临床价值。方法51例疼痛性骨质疏松脊柱压缩骨折患者，累及椎体69个。在X线监测下，经双侧椎弓根穿刺伤椎，完成球囊扩张，骨水泥填充。双球囊组患者29例（38个椎体），应用双球囊在椎体内双侧同时协调扩张；单球囊组患者22例（31个椎体），应用单球囊双侧入路在椎体内先后交替扩张。比较术前与术后椎体高度与Cobb角的差异，并分别计算两组的术后椎体高度平均恢复率和与Cobb角平均矫正度，比较两组之间差异，并随访6～12个月。结果51例患者手术顺利，术后第96小时内疼痛均明显缓解，无临床并发症。患者10分制视觉模拟疼痛评分法评分由术前平均7．7分降至术后2．6分（P〈0．01）；Oswestry功能障碍指数评分由术前平均73％降至术后26％（P〈0．01）。双球囊组伤椎椎体前缘和中部平均丢失高度由术前14．7mm和10．5mm改善至术后10．1mm和5．5mm，Cobb角由术前平均22.4°，矫正至术后12．3°，手术前后差异具有显著性意义（P〈0.01）；单球囊组伤椎椎体前缘和中部平均丢失高度由术前14．7mm和10．6mm，改善至术后10．4mm和6．5mm，Cobb角由术前平均21．2°，矫正至术后11．6°，手术前后差异具有显著性意义（P〈0．01）。双球囊组与单球囊组术后椎体平均高度恢复率分别为72．8％、70．1％，Cobb角平均矫正度分别为10．1°和9．5°，两组相比差异无统计学意义（P〉0．05）。随访期间，无疼痛加重病例出现。结论双球囊与单球囊椎体后凸成形术治疗老年骨质疏松脊柱压缩骨折均可获得满意疗效。

目的探讨经椎弓根单球囊跨中线扩张椎体后凸成形术治疗多发性骨质疏松脊柱压缩骨折的临床价值。方法36例疼痛性骨质疏松脊柱压缩骨折患者，累及椎体61个。在X线监测下，经一侧椎弓根将单球囊置入椎体的中线区，使单球囊在椎体内跨中线扩张，骨水泥在椎体内跨中线分布。术后观察患者视觉模拟疼痛评分（VAS）和Oswestry功能障碍指数（ODI）变化以及椎体高度恢复和后凸畸形矫正情况，并采用配对t检验对上述指标进行比较。随访6．0～12．0个月（平均9．2个月）。结果36例患者手术顺利，平均每个椎体手术时间为（37．4±9.6）min，术后96h疼痛明显缓解，功能改善显著，无严重临床并发症发生。患者VAS由术前平均（7．3±1．0）分降至术后（2.7±0.8）分（t=19．53，P〈0．01）；ODI由术前平均（71．1±10.9）％降至术后（26．6±6.4）％（t=18．54，P〈0．01）。术前病椎椎体前缘和中部高度丢失平均分别为（14．3±2．8）mm和（10．2±2.7）mm，术后分别为（10．0±1．8）mm和（5、9±1．8）mm，两者差异均有统计学意义（t值分别为14．68和16．44，P值均〈0．01）。Cobb角术前平均23．4°±5．0°，术后矫正至16．2°±2.8°（t=15．60，P〈0．01）。2例分别在椎体前缘和椎间隙内发现少许骨水泥渗漏，但无临床症状，其余椎体X线检查未发现骨水泥渗漏的表现。术后随访期间，患者无疼痛加重或椎体再次塌陷以及邻近椎体发生骨折的情况。结论经椎弓根单球囊跨中线扩张椎体后凸成形术治疗骨质疏松性脊柱压缩骨折疗效满意，手术时间与放射暴露时间短。

背景:椎体后凸成形技术操作时需用2个球囊在椎体内同时协调扩张,并在形成的空腔内注入骨水泥,但专用球囊价格昂贵,限制了该技术的广泛开展。采用单球囊扩张椎体后凸成形治疗骨质疏松性脊柱压缩骨折,可降低手术费用,但此干预过程中介入的骨水泥填充物生物相容性及椎体后凸成形效果需进一步观察。目的:应用单个球囊分别在椎体两侧先后扩张,并在其形成的空腔内行骨水泥填充,观察骨水泥填充物生物相容性及治疗骨质疏松性脊柱压缩骨折的效果。设计:前后对照观察。单位:解放军济南军区总医院影像科。对象:纳入2004-05/2005-05在解放军济南军区总医院医学影像科应用单球囊技术治疗的患者22例,累及椎体31个,男5例,女17例;年龄68～85岁。纳入对象均为疼痛性骨质疏松椎体压缩骨折;CT和MRI确定椎体后壁完整,脊髓无受压表现;均对治疗方案和实验知情同意。实验和治疗经医院伦理委员会批准。方法:治疗采用冠龙公司的国产椎体后凸成形专用器械[批准文号:鲁药管械(准)字2004第2150017号]包括穿刺器械和可膨胀球囊。骨水泥为国产丙烯酸树脂骨水泥Ⅲ[批准文号:国食药监械(准)字2005第3650267号]。在X射线监测下,经双侧椎弓根穿刺伤椎,使用单球囊先后置入两侧,完成球囊扩张,改善椎体高度。主要观察指标:①观察术中、术后骨水泥生物相容性。②观察术后72h内疼痛缓解状况,椎体高度恢复和脊柱后凸畸形矫正情况。结果:①骨水泥生物相容性:1例1个椎间隙发现少许骨水泥渗漏,但无临床症状,余椎体无明显骨水泥渗漏的X射线平片表现;未发现炎症和排斥反应。②疼痛缓解:22例患者术后72h内疼痛均明显缓解。③椎体高度恢复和脊柱后凸畸形矫正情况:单球囊扩张并骨水泥填充矫正前伤椎椎体前缘和中部高度平均分别丢失(14.70±4.21)mm和(10.62±4.11)mm,矫正后平均分别丢失(10.38±4.23)mm和(6.45±4.04)mm。Cobb角由矫正前平均(21.15±6.33)°,矫正后为(11.64±4.33)°。结论:临床结果证实骨水泥与宿主生物相容性好,发生渗漏时无特殊宿主不良反应。应用单球囊扩张并骨水泥填充矫正老年骨质疏松脊柱压缩骨折,可有效恢复椎体高度,缓解疼痛。

Cholangiocarcinoma refers to malignant tumors that develop in epithelial lining of biliary system, and it is divided into two categories according to tumor location, intrahepatic cholangiocarcinoma （ICC） and extrahepatic cholangiocarcinoma （ECC）. ICC occurs from the epithelial cells of the intrahepatic bile duct, its branches and interlobular biliary tree; and ECC is divided into hilar cholangiocarcinoma and distal cholangiocarcinoma by the circumscription at the confluence of cystic duct and the common hepatic duct.

Realtime trigger and localization of bursts are the key functions of GECAM,an all-sky gamma-ray monitor launched on 2020 December *** developed a multifunctional trigger and localization software operating in the CPU of the GECAM Electronic *** onboard software has the following features:high trigger efficiency for real celestial bursts with a suppression of false triggers caused by charged particle bursts and background fluctuation,dedicated localization algorithm optimized for both short and long bursts,and low time latency of the trigger information which is downlinked through the Global Short Message Communication service of the global BeiDou navigation *** paper provides a detailed description of the design and development of the trigger and localization software system for *** covers the general design,workflow,the main functions,and the algorithms used in the *** paper also includes on-ground trigger tests using simulated gamma-ray bursts generated by a dedicated X-ray tube,as well as an overview of the performance for real celestial bursts during its in-orbit operation.

球囊扩张椎体后凸成形术的技术操作及相关问题的探讨

目的讨论椎体后凸成形术的技术操作方法及相关问题。方法 51例疼痛性骨质疏松脊柱压缩骨折患者,累及椎体69个。在 X 线监测下,经双侧椎弓根穿刺病变椎体,使用球囊扩张,改善椎体高度,骨水泥填充。术后观察疼痛缓解状况及骨折复位情况。结果 51例患者手术顺利,术后72 h 内疼痛均明显缓解,无临床并发症。术前病变椎体前缘和中部平均丢失高度为(15±4)、(11±4)mm,术后矫正至(10±4)、(6±3)mm;Cobb 角术前平均22°±6°,术后为12°±4°,手术前后差异有统计学意义(t 值分别为12.12、13.33、18.97,P 值均<0.01)。术后椎体高度平均恢复率(71.50±14.16)%,Cobb 角平均矫正9.85°±4.31°。椎间隙少量渗漏1个椎体,椎体前软组织少量渗漏1个椎体,但无临床症状,其余椎体 X 线检查无明显骨水泥渗漏的表现。结论球囊椎体后凸成形术治疗老年骨质疏松脊柱压缩骨折均可获得满意疗效,掌握好椎弓根穿刺、球囊扩张技术以及把握好骨水泥的推注时机是取得手术成功,降低并发症的关键。
