Hybrid QoS model which consists of certain and uncertain expressions has strong power of semantic QoS description. For solving the hybrid QoS-aware semantic web service composition problem, this paper presents an Uncertain Multi-attribute decision making based Composition algorithm （UMC）. The UMC includes two parts. First, UMC-Core can be used to synthetically evaluate the hybrid service quality information. Second, UMC-DH （Distributed and Heuristic framework for UMC） aims at enhancing the run-time performance of UMC when the problem space increases. The simulation results show that the UMC has lower execution cost, higher approximation ratio and success ratio than other similar approaches.

Background:Gallbladder cancer(GBC)was once considered a contraindication for laparoscopic surgery,but it is becoming more common to use laparoscopic surgery for GBC *** aim of this study was to analyze the long-and short-term outcomes of patients with more advanced T-staged GBC treated with curative intent as defined by the National Comprehensive Cancer Network(NCCN)after laparoscopic resection(LR)versus open resection(OR).Methods:A multicenter database was used to select consecutive GBC patients treated with curative-intent resection as defined by the NCCN between 2016 and *** patients were divided into the LR group and the OR *** score matching(PSM)was used to eliminate selection *** endpoints were overall survival(OS),progression-free survival(PFS),and short-term *** factors that were independently associated with OS and PFS were ***:Of 626 GBC patients treated with curative-intent resection,after PSM,51 patients were in the LR group and 153 patients were in the OR *** LR group had more patients who were suitable to receive adjuvant chemotherapy(AC),a longer operation time,more harvested lymph nodes,and a lower overall morbidity *** rates of OS and PFS were not significantly different between the two *** was independently associated with better OS and ***:The overall morbidity of GBC patients after LR was lower,but the long-term outcomes between LR and OR were not significantly *** GBC patients treated with LR were more likely to receive AC,and the use of AC after curative-intent resection of GBC helped achieve better long-term survival outcomes.

目的:探讨腹腔镜下无萎缩肾切开取石术(laparoscopic nephrolithotomy,LANL)治疗复杂性肾结石的可行性。方法:回顾性分析我院2017年1月至2023年1月16例LANL患者的临床资料。所有患者术前均行CT平扫+尿路成像(CTU)以明确结石的数量、大小、形状、肾盂形态及肾积水的程度。记录患者术前肾功能情况,并与术后24 h肾功能进行比较。同时记录手术时间、术中肾蒂阻断时间、术中出血情况、术后结石残留率及术后并发症。结果:16例手术均获成功,无一例中转开放。手术平均时间(160.0±31.2)min;肾脏热缺血平均时间(24.8±2.8)min;术中平均出血量(142±55)mL;术后引流管留置时间(5.44±1.46)d,术后平均住院时间(7.50±0.89)d。术后仅2例有结石残留,结石清除率为87.5%(14/16)。所有患者均无漏尿、出血及尿源性脓毒血症发生。患者术后1月随访GFR水平较术前显著升高(P<0.05)。术后中位随访17.75个月,CTU显示患肾梗阻程度明显减轻。结论:经合理筛选适应证及丰富腹腔镜手术经验前提下,后腹腔镜LANL是一种安全、有效的微创方法,其单次清石率高,尤其适于需单次处理的复杂性肾结石。

Primary central nervous system germ cell tumors(CNS-GCTs) in children and adolescents have unique clinical features and methods of treatment compared with those in adults. There is little information about Chinese children and adolescents with CNS-GCTs. Therefore, in this study we retrospectively analyzed the clinical features and treatment outcome of Chinese children and adolescents with primary CNS-GCTs. Between January 2002 and December 2012, 57 untreated patients from a single institution were enrolled. They were diagnosed with CNS-GCTs after pathologic or clinical assessment. Of the 57 patients, 41 were males and 16 were females, with a median age of 12.8 years(range, 2.7 to 18.0 years) at diagnosis; 43(75.4%) had non-germinomatous germ cell tumors(NGGCTs) and 14(24.6%) had germinomas; 44(77.2%) had localized disease and 13(22.8%) had extensive lesions. Fifty-three patients completed the prescribed treatment, of which 18 underwent monotherapy of surgery, radiotherapy, or chemotherapy, and 35 underwent multimodality therapies that included radiotherapy combined with chemotherapy or surgery combined with chemotherapy and/or radiotherapy. PEB(cisplatin, etoposide, and bleomycin) protocol was the major chemotherapy regimen. The median follow-up time was 32.3 months(range, 1.2 to 139 months). Fourteen patients died of relapse or disease progression. The 3-year event-free survival(EFS) and overall survival rates for all patients were 72.2% and 73.8%, respectively. The 3-year EFS was 92.9% for germinomas and 64.8% for NGGCTs(P = 0.064). The 3-year EFS rates for patients with NGGCTs who underwent monotherapy and multimodality therapies were 50.6% and 73.5%, respectively(P = 0.042). Our results indicate that multimodality therapies including chemotherapy plus radiotherapy were better treatment option for children and adolescents with CNS-GCTs.

该文提出了一种分布式Web服务QoS注册系统的负载均衡方法。提出了节点负载状态划分的概念。根据负载状态采用不同的负载信息散布策略,极大地减少了网络消耗。提出了基于简单协商的负载均衡方法。负载均衡用数据复制的方式,主要特点是在进行实际负载均衡操作以前即与复制目标节点进行协商,提出合理的复制需求,然后根据对方提供的资源情况,发起复制。提高了负载均衡的效率,降低了复制的盲目性。该方法在分布式Web服务QoS注册原型系统中进行了实验,达到了较好的效果。

Hepatotoxicity is a common side effect for patients treated with gefitinib,but the related pathogenesis is unclear and lacks effective predictor and management strategies.A multi-omics approach integrating pharmacometabolomics,pharmacokinetics and pharmacogenomics was employed in non-small cell lung cancer patients to identify the effective predictor for gefitinib-induced hepatotoxicity and explore optional therapy ***,we found that patients with rs4946935 AA,located in Forkhead Box O3(FOXO3)which is a well-known autophagic regulator,had a higher risk of hepatotoxicity than those with the GA or GG variant(OR=18.020,95%CI=2.473 to 459.1784,P=0.018)in a gefitinib-concentration dependent ***,functional experiments identified that rs4946935_A impaired the expression of FOXO3 by inhibiting the promotor activity,increasing the threshold of autophagy initiation and inhibiting the autophagic activity which contributed to gefitinib-induced liver *** contrast,erlotinib-induced liver injury was independent on the variant and expression levels of *** study reveals that FOXO3 mutation,leading to autophagic imbalance,plays important role in gefitinib-induced hepatotoxicity,especially for patients with high concentration of *** conclusion,FOXO3 mutation is an effective predictor and erlotinib might be an appropriately and well-tolerated treatment option for patients carrying rs4946935 AA.

Objective： Infra-pyloric artery （IPA） is an important anatomical landmark in treatment of gastric cancer and is the key vessel for pylorus-preserving gastrectomy and subgroup of infra-pyloric lymph nodes. However, its anatomical variation is not thoroughly understood. Our study aimed to clarify the origination of the IPA. Methods： We did this prospective, multicenter, open-label, observational study at gastric surgery departments of 34 hospitals in China. Gastric cancer patients aged 18 years or older and scheduled to undergo elective total or distal gastrectomy were assigned. During the surgery, IPA dissecting and exposing the origination point with photographs or video clips were required. The primary outcome was the origination of the IPA. Analysis of variance, χ2 tests and Fisher＇s tests were used to analyze the differences between groups. The study is registered at *** （No. NCT03071237）. Results： Between May 8 and July 31, 2017, 429 patients were assigned for the study, and 419 （97.7%） patients had the IPA dissected and recorded through photograph or video and were included in the primary outcome analysis. The median age was 62 years old, and 73.7% were male. Among the patients, 78.5% received laparoscopic surgery. Single IPA origination was identified in 398 （95.0%） patients, including gastroduodenal artery （GDA） in 154 （36.8%） patients, anterior superior pancreaticoduodenal artery （ASPDA） in 130 （31.0%） patients, and right gastroepiploic artery （RGEA） in 114 （27.2%） patients. Fifteen （3.6%） patients were identified with multiple IPA and 6 （1.4%） patients were identified as IPA absence. The differences in the distribution of surgical approach （P=0.003） and geographic area （P=0.030） were statistically significant. No difference was shown in sex, age, gastrectomy type, tumor location, and clinical T, N and M stage. Objective The relationship between serum uric acid(suA)levels and glycemic indices,including plasma glucose(FPG),2-hour postload glucose(2 h-PG),and glycated hemoglobin(HbA1 c),remains *** aimed to explore the associations between glycemic indices and suA levels in the general Chinese *** The current study was a cross-sectional analysis using the first follow-up survey data from The China Cardiometabolic Disease and Cancer Cohort Study.A total of 105,922 community-dwelling adults aged≥40 years underwent the oral glucose tolerance test and uric acid *** nonlinear relationships between glycemic indices and suA levels were explored using generalized additive *** A total of 30,941 men and 62,361 women were eligible for the current *** additive models verified the inverted U-shaped association between glycemic indices and suA levels,but with different inflection points in men and *** thresholds for FPG,2 h-PG,and HbA1 c for men and women were 6.5/8.0 mmol/L,11.0/14.0 mmol/L,and 6.1/6.5,respectively(suA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices).Conclusion An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes,while the inflection points were reached earlier in men than in women.
