Malignant hyperthermia(MH)is a genetic disorder of skeletal muscle cells that affects muscle cytoplasmic calcium homeostasis,with high mortality and low morbidity.Generally,it presents with non-specific signs of a hypermetabolic response,including high fever,tachycardia,and elevated end-tidal carbon dioxide(ETCO_(2)).The successful treatment lies in the timely recognition and early use of dantrolene.[1]As an inhibitor of Ca2+release through ryanodine receptor(RYR)channels,the skeletal muscle relaxant dantrolene has proven to be both a valuable experimental probe of intracellular Ca2+signaling and a lifesaving treatment for MH.[2]Dominant mutations in the skeletal muscle RYR1 gene are well-recognized causes of both malignant hyperthermia susceptibility(MHS)and central core disease(CCD).

Antibiotic abuse now poses a grave threat to global ecology and bestirs public concerns about the residue issue in daily necessities.The traceability measurements along supply chain or logistic circulation have become increasingly essential given the labile nature of diverse synthetic residuals on site.In an attempt to answer this urgency,here a miniaturized fluorometric aptasensor prototype was contrived that catered to the point-of-care screening norm for two typical additives:chloramphenicol and enrofloxacin.The key target-indicating module worked in vitro based on the competitive binding-induced fluorescence recovery of fluorescein-labeled aptamers,which were photobleached beforehand in the format of double helix on burlike nanogold carriers.The“prickly”geometry of the latter not just enriched the capture probes at preferentially substrate-accessible spires;but also contributed to a tip-enhanced surface plasmon effect,sensitizing the signal-on during the duplex dissociation even at nanomolar threshold of the analytes.On the other hand,to encompass a full portable,a set of optical devices were mounted within a 3D-printed cartridge(adaptor)to converge the light beam and route it towards the detector,for which the smartphone camera came up in handy with a home-developed App for calibrating the emissive brightness.Enlightened by the high-dynamic-range compression,an imaging diagnostic algorithm was built in to grid and digitize each slide in the album for augmented detection performance.Thus,a novel bio-to-silico integration was invented that capable of in situ rapid reporting on the antibiotic presence with high sensitivity and selectivity.Further field practices in spiked milk on sales proved the precision and rudimentary feasibility of the well-assembled model of appliance,thus holding nice prospects in nonexpert(e.g.,family and local community)utilities for foodborne antibiotic identification.

AIM:To characterize the distribution of meibomian gland(MG)area loss(MGL)and its relationship with demographic characteristics,mites,and symptoms.METHODS:This retrospective observational study included patients who visited the Dry Eye Clinic of Shenzhen Eye Hospital between June 2020 and August 2021.General patient characteristics,ocular symptoms,Demodex test results of the eyelid edges,and the results of a comprehensive ocular surface analysis were collected.MGL was analyzed using Image J software.REsuLTS:This study enrolled 1204 outpatients aged 20-80(40.70±13.44)y,including 357 males(29.65%)and 847 females(70.35%).The patients were classified into mild(n=155;12.87%),moderate(n=795;66.03%),severe(n=206;17.11%),and extremely severe(n=48;3.99%)MGL groups.MGL was significantly larger in female than in male(P=0.006).The degree of MGL also significantly differed in age(P<0.001)and the more numbers of mites with severity(P<0.001).Multivariate disordered multinomial logistic regression analysis identified that female sex,older age,secretory symptoms,and a large number of mites were risk factors for MGL(P<0.05).CONCLUSION:Patients with MGL are more likely to be older,female,more numbers of mites,and increased secretion.

Emerging evidence of the beneficial effects of defatted rice bran (DFRB) on gut health has advanced the development of fermented defatted rice bran as a potential functional food. However， less is known about its effects and underlying mechanisms on gut health. In this study， a mouse model together with fecal microbiota transplantation (FMT) was utilized to study the effects and mechanisms of fermented DFRB (FR) on gut barrier function. We found that FR improved the intestinal morphology， gut tight junction proteins， mucin， antimicrobial peptides， and IL-22 and promoted the gut Clostridium butyricum and butyrate. Notably， correlation analysis indicated gut C. butyricum and butyrate were two FR-induced effectors that improved gut health. FMT results suggested that C. butyricum， butyrate， and fecal microbiota from the FR group all reduced PHD2 expression by activating PPAR-γ in the mouse colon. This decrease in gut PHD2 subsequently upregulated the HIF-1α expression， which in turn increased the expression of its targeted downstream tight junction proteins， mucin and antimicrobial peptides， and colonic IL-22 secretion. Overall， FR-derived C. butyricum and butyrate might improve gut barrier function through the HIF-1 signaling pathway， which provides a reference for the application of fermented DFRB as a potential functional food for improving of gut barrier function.

目的:提高木瓜成熟度检测准确率及鲁棒性。方法:提出一种基于多目标采样和改进Mask R-CNN的木瓜成熟度检测方法。通过均值平均精度、准确率、精确率—召回率曲线和计算时间等指标,验证所提方法的有效性和鲁棒性,并将其检测效果与Faster R-CNN、RetinaNet和CenterMask等方法进行对比。结果:试验方法对木瓜成熟度检测的平均精度均值、50%平均精度均值、75%平均精度均值分别为98.43%,98.67%,98.68%,对未成熟、半成熟和成熟木瓜成熟度的平均检测精度为99.38%,98.81%,99.37%。结论:该方法可用于开发木瓜成熟度检测的电子系统,提升木瓜成熟度检测和木瓜分级的性能。

BACKGROUND Acute carpal tunnel syndrome(ACTS)is commonly caused by repetitive strain,trauma,or inflammatory conditions.However,ACTS due to tophaceous gout is a clinical event that remains poorly understood and underreported.This rare manifestation necessitates prompt diagnosis and intervention to prevent irreversible complications.CASE suMMARY A 51-year-old man who had poorly controlled hyperuricemia presented with ACTS secondary to tophaceous gout.Because of rapid symptom progression symptoms and severe median nerve compression within 3 mo,the patient underwent emergency decompression surgery for both wrists at different time points.Postoperatively,he exhibited complete recovery of sensory and motor functions,with no recurrence at long-term follow-up.Favorable outcomes were achieved through immediate decompression surgery,anti-inflammatory medications,postoperative active and passive range-of-motion exercises,and intermittent wrist splinting.Prompt diagnosis and surgical intervention,when necessary,are crucial for preventing long-term complications and obtaining favorable outcomes in patients with ACTS.An optimal gout management strategy involving pharmacologic therapy and lifestyle modifications may help minimize ACTS recurrence and improve clinical outcomes.CONCLUSION Prompt surgical intervention and optimal gout management are crucial for preventing irreversible nerve damage and ACTS recurrence.

基因治疗这种堪称革命性的治疗方法,开拓了治疗癌症的新思路,其最关键性问题是实现核酸药物靶向肿瘤组织并精准治疗。核酸药物直接递送存在核酸酶降解代谢、细胞膜上的负电荷排斥现象以及稳定性差等问题,所以核酸药物需要载体协助,成功的载体递送除能使核酸药物在肿瘤区域大量富集外,还要起到药物控释作用,而天然多糖除无毒、生物相容度高、易修饰的特点外,它本身就具有免疫调节、抗肿瘤、抗炎等多种生物活性。本篇总结了最具代表性的五种多糖的结构特征及在核酸药物递送方面的应用,继而归纳了多糖的常用的纳米级载体形式,为构建天然多糖递送核酸的新型载体并将其应用到免疫抗肿瘤治疗研究中奠定基础。

BACKGROUND Appendiceal mucinous neoplasms(AMNs),although not classified as rare,are relatively uncommon tumors most often discovered incidentally during colorectal surgery.Accurate identification of AMNs is difficult due to non-specific sym-ptoms,overlapping tumor markers with other conditions,and the potential for misdiagnosis.This underscores the urgent need for precision in diagnosis to pre-vent severe complications.CASE suMMARY This case report describes the unexpected discovery and treatment of a low-grade AMN(LAMN)in a 74-year-old man undergoing laparoscopic hemicolectomy for transverse colon adenocarcinoma(AC).Preoperatively,non-specific gastroin-testinal symptoms and elevated tumor markers masked the presence of AMN.The tumor,presumed to be an AMN peritoneal cyst intraoperatively,was con-firmed as LAMN through histopathological examination.The neoplasm exhibited mucin accumulation and a distinct immunohistochemical profile:Positive for Ho-meobox protein CDX-2,Cytokeratin 20,special AT-rich sequence-binding protein 2,and Mucin 2 but negative for cytokeratin 7 and Paired box gene 8.This profile aids in distinguishing appendiceal and ovarian mucinous tumors.Postoperative recovery was uncomplicated,and the patient initiated adjuvant chemotherapy for the colon AC.CONCLUSION This case highlights the diagnostic complexity of AMNs,emphasizing the need for vigilant identification to avert potential complications,such as pseudomyxoma peritonei.

BACKGROUND With less than 90 reported cases to date,stercoral perforation of the colon is a rare occurrence.Stercoral ulceration is thought to occur due to ischemic pressure necrosis of the bowel wall,which is caused by the presence of a stercoraceous mass.To underscore this urgent surgical situation concerning clinical presentation,surgical treatment,and results,we present the case of a 66-year-old man with a stercoral perforation.CASE suMMARY A 66-year-old man with a history of hypertension,hyperlipidemia,and gout presented at the emergency department with lower abdominal pain and a low-grade fever lasting for a few hours.Abdominal computed tomography indicated a suspected bezoar(approximately 7.6 cm)in the dilated cecum,accompanied by pericolic fat stranding,mild proximal dilatation of the ileum,pneumoperitoneum,and minimal ascites.Intraoperatively,feculent peritonitis with isolated cecal perforation were observed.Consequently,a right hemicolectomy with peritoneal lavage was performed.A histopathological examination supported the intraoperative findings.CONCLUSION In stercoral perforations,a diagnosis should be diligently pursued,especially in older adults,and prompt surgical intervention should be implemented.
