Magnetic resonance imaging-guided focused ultrasound thalamotomy launch with remote telemedicine international proctorship

Background: COViD-19 limitations have hindered the implementation of new technologies by preventing proctors from coming to the site.We share our first experience of magnetic resonance imaging(MRi)-guided focused ultrasound(MRgFUS)treatment with an international remote online proctorship,and develop and evaluate the methodology of remote MRgFUS proctorship.Methods: This single-center,nonrandomized controlled prospective study included 94 patients:27 with essential tremor(ET)and 67 with tremor-dominant Parkinson's disease(PD).The coming of proctors was impossible,so we arranged for the remote participation of proctors from the United Kingdom,Spain,and israel.A total of 38 patients(40.4%)received telemedicine-proctored treatment(proctor group)and 56 received their treatment independently(solo group).We used the Clinical Rating Scale for Tremor(CRST)for ET patients and the Unified Parkinson's Disease Rating Scale(UPDRS)Part iii for PD patients.Results: in patients with ET,success rates were 81.8%(proctor group)and 100%(solo group)(p=0.22).CRST reduction on the treated side was 71.43%[65.83%;80.56%](proctor group)versus 60.87%[53.99;79.58](solo group)(p=0.19).None of the patients showed worsening of tremors within 1 year.in patients with PD,the success rates were 92.6%(proctor group)and 100%(solo group)(p=0.08).The UPDRS Part iii improvement was 30.1%(proctor group)versus 39.9%(solo group)(p=0.003).The 1-year recurrence rate was 40%(proctor group)and 17.5%(solo group)(p=0.04).No complications were observed at 6 months.Conclusions: We developed a feasible and safe methodology for telemedicine remote online-proctored MRgFUS treatment.No significant difference was observed between the solo and developed remote proctor protocols in terms of complication rate,effect,and long-term results;however,UPDRS Part iii improvement was better in the PD solo group.This study demonstrated that the MRgFUS international proctorship can be performed successfully remotely.

The efficient transmission of images,which plays a large role inwireless communication systems,poses a significant challenge in the growth of multimedia technology.High-quality images require well-tuned communication standards.The Single Carrier Frequency Division Multiple Access(SC-FDMA)is adopted for broadband wireless communications,because of its low sensitivity to carrier frequency offsets and low Peak-to-Average Power Ratio(PAPR).Data transmission through open-channel networks requires much concentration on security,reliability,and integrity.The data need a space away fromunauthorized access,modification,or deletion.These requirements are to be fulfilled by digital image watermarking and encryption.This paper ismainly concerned with secure image communication over the wireless SC-FDMA systemas an adopted communication standard.it introduces a robust image communication framework over SC-FDMA that comprises digital image watermarking and encryption to improve image security,while maintaining a high-quality reconstruction of images at the receiver side.The proposed framework allows image watermarking based on the Discrete Cosine Transform(DCT)merged with the Singular Value Decomposition(SVD)in the so-called DCT-SVD watermarking.in addition,image encryption is implemented based on chaos and DNA encoding.The encrypted watermarked images are then transmitted through the wireless SC-FDMA system.The linearMinimumMean Square Error(MMSE)equalizer is investigated in this paper to mitigate the effect of channel fading and noise on the transmitted images.Two subcarrier mapping schemes,namely localized and interleaved schemes,are compared in this paper.The study depends on different channelmodels,namely PedestrianAandVehicularA,with a modulation technique namedQuadratureAmplitude Modulation(QAM).Extensive simulation experiments are conducted and introduced in this paper for efficient transmission of encrypted watermarked images.in addition,different variants of SC-FDMA based on the Discrete Wavelet Transform(DWT),Discre

Burden of routine orthopedic implant removal a single center retrospective study

BACKGROUND Open reduction and internal fixation represent prevalent orthopedic procedures,sparking ongoing discourse over whether to retain or remove asymptomatic implants.Achieving consensus on this matter is paramount for orthopedic surgeons.This study aims to quantify the impact of routine implant removal on patients and healthcare facilities.A retrospective analysis of implant removal cases from 2016 to 2022 at King Fahad Hospital of the University(KFHU)was conducted and subjected to statistical scrutiny.Among these cases,44%necessitated hospitalization exceeding one day,while 56%required only a single day.Adults exhibited a 55%need for extended hospital stays,contrasting with 22.8%among the pediatric cohort.The complication rate was 6%,with all patients experiencing at least one complication.Notably,34.1%required sick leave and 4.8%exceeded 14 d.General anesthesia was predominant(88%).Routine implant removal introduces unwarranted complications,particularly in adults,potentially prolonging hospitalization.This procedure strains hospital resources,tying up the operating room that could otherwise accommodate critical surgeries.Clearly defined institutional guidelines are imperative to regulate this practice.AiM To measure the burden of routine implant removal on the patients and hospital.METHODS This is a retrospective analysis study of 167 routine implant removal cases treated at KFHU,a tertiary hospital in Saudi Arabia.Data were collected in the orthopedic department at KFHU from February 2016 to August 2022,which includes routine asymptomatic implant removal cases across all age categories.Nonroutine indications such as infection,pain,implant failure,malunion,nonunion,restricted range of motion,and prominent hardware were excluded.Patients who had external fixators removed or joints replaced were also excluded.RESULTS Between February 2016 and August 2022,360 implants were retrieved;however,only 167 of those who met the inclusion criteria were included in this study.The remaining implants were rejected due

introduction: HiV, the human immunodeficiency virus, is the etiological agent of acquired immunodeficiency syndrome (AiDS). The aim of this study was to assess the evolution of the viral load in patients under treatment. Methodology: This was a study carried out from July 2017 to June 2022 at the Point G University Hospital laboratory. The determination of the viral load of patients was carried out by PCR on the ABOTT M2000sp/rt platform. Results: A total of 129 patients infected with HiV-1, aged 19 to 72 years with a mean age of 40.05 years ± 10.71;all on antiretroviral chemotherapy. The female gender predominated among our patients. The most common treatment regimen was 2iNTi + 1iNNTi with 72.9% followed by 2iNTi + 1iNi with 13.2%. As for the combinations of molecules, the combination TDF + 3TC + EFV and TDF + 3TC + DTG predominated, respectively 65.1% and 13.2%. 89.9% of our patients had undetectable viremia after 12 months of treatment (p ith an average viral load which had evolved from 681315.65 copies/ml ± 1616908.484 to M0 at 5742.36 copies /ml ± 35756.883 at M12 (p Conclusion: Generally speaking, antiretroviral treatment had contributed to controlling viral loads, however the therapeutic combination TDF + 3TC + DTG had made it possible to obtain more patients with undetectable viremia instead.

The propagation of coherent light in multimode optical fibers results in a speckled output that is both complex and sensitive to environmental effects.These properties can be a powerful tool for sensing,as small perturbations lead to significant changes in the output of the fiber.However,the mechanism to encode spatially resolved sensing information into the speckle pattern and the ability to extract this information are thus far unclear.in this paper,we demonstrate that spatially dependent mode coupling is crucial to achieving spatially resolved measurements.We leverage machine learning to quantitatively extract the spatially resolved sensing information from three fiber types with dramatically different characteristics and demonstrate that the fiber with the highest degree of spatially dependent mode coupling provides the greatest accuracy.

An analysis of the sectorial structure of energy consumption shows that residential and tertiary sector buildings are the third-highest consumers,responsible for 29.5%of a city’s final energy consumption.The Building Quality Control Laboratory of the Basque Government aims to promote quality,innovation,and sustainability in buildings.To accomplish this goal,it has constructed an experimental facility with different energy generation technologies and a very versatile control system for testing different energy systems and operation modes.in this study,we tested a facility for supplying domestic hot water and heating for a multi-family house by means of a condensing boiler and an aerothermal heat pump(together with the corresponding control).This installation could reproduce the thermal demands required to be satisfied by the generation equipment through a programmed operation of the installation based on real demands.Additionally,this installation was analyzed using thermoeconomics(TE)to solve problems unable to be solved using traditional energy analyses based on the First Law of Thermodynamics.These problems include:(1)Determining the costs of the products of the installation based on physical criteria,(2)detecting the places where losses actually occur,evaluating their costs,and proposing cost-effective improvements,and(3)diagnosing issues in the installation.As a result,this paper suggests a solution to the preventive maintenance problems confronting the technical maintenance personnel for thermal installations in buildings by applying TE knowledge and using real data collected from sensors.

Vegetable oil production from oil palm(Elaeis guineensis Jacq.)is an important industry due to the rising demand every year.The somatic embryogenesis culture can propagate oil palm duplicate as parent plant,which can be selected as breeding material to produce new planting germplasm with high production or disease resistance.This study aims to evaluate the genotypic effect of somatic embryogenesis,while immature leaflets were employed as explants.The culture used embryo induction medium based on Murashige and Skoog(MS)modifications that contained 5 mg/L Naphthalene Acetic acid(NAA)and 0.5 mg/L Benzyl Amino Purine(BAP).The genotypic effect was statistically significant in the percentage of callus induction,producing somatic embryos,and germination embryos.in this study,we successfully cloned thirteen oil palm genotypes(GE-02,GE-03,GE-06,GE-07,GE-09,GE-23,GE-24,GE-27,GE-28,GE-32,GE-33,GE-34,and GE-35),with the highest number of somatic embryos formed on GE-27 with a percentage of 70.1%.The cloning was successful in accelerating the propagation of oil palm for materials breeding programs to create new varieties with high production and disease resistance.it is necessary to observation the performance of these clones in the field in terms of mantle flower appearance.

BACKGROUND Kidney transplantation leads to continuous improvement in the survival rates of kidney transplant recipients(KTRs)and has been established as the treatment of choice for patients with end-stage kidney disease.Health-related quality of life(HRQoL)has become an important outcome measure.it is highly important to develop reliable methods to evaluate HRQoL with disease-specific questionnaires.AiM To translate the disease-specific instrument Kidney Transplant Questionnaire 25(KTQ-25)to the Greek language and perform a cross-cultural adaptation.METHODS The translation and adaptation of the original English version of the KTQ-25 to the Greek language were performed based on the international Quality of Life RESULTS Eighty-four KTRs(59 males;mean age 53.5±10.7 years;mean estimated glomerular filtration rate 47.7±15.1 mL/min/1.73 m2;mean transplant vintage 100.5±83.2 months)completed the Greek version of the KTQ-25 and the 36-item Short-Form Health Survey,and the results were used to evaluate the reliability of the Greek KTQ-25.The Cronbach alpha coefficients for all the KTQ-25 dimensions were satisfactory(physical symptoms=0.639,fatigue=0.856,uncertainty/fear=0.661,appearance=0.593,emotions=0.718,total score=0.708).The statistically significant correlation coefficients among the KTQ-25 dimensions ranged from 0.226 to 0.644.The correlation coeffi-cients of the KTQ-25 dimensions with the SF-36 physical component summary(PCS)ranged from 0.196 to 0.550;the correlation coefficients of the KTQ-25 with the SF-36 mental component summary(MCS)ranged from 0.260 to 0.655;and the correlation coefficients of the KTQ-25 with the total scores with the SF-36 PCS and MCS were 0.455 and 0.613,respectively.CONCLUSiON According to the findings,the Greek version of the KTQ-25 is valid and reliable for administration among kidney transplant patients in Greece.

A Dimensional Reduction Approach Based on Essential Constraints in Linear Programming

This paper presents a new dimension reduction strategy for medium and large-scale linear programming problems. The proposed method uses a subset of the original constraints and combines two algorithms: the weighted average and the cosine simplex algorithm. The first approach identifies binding constraints by using the weighted average of each constraint, whereas the second algorithm is based on the cosine similarity between the vector of the objective function and the constraints. These two approaches are complementary, and when used together, they locate the essential subset of initial constraints required for solving medium and large-scale linear programming problems. After reducing the dimension of the linear programming problem using the subset of the essential constraints, the solution method can be chosen from any suitable method for linear programming. The proposed approach was applied to a set of well-known benchmarks as well as more than 2000 random medium and large-scale linear programming problems. The results are promising, indicating that the new approach contributes to the reduction of both the size of the problems and the total number of iterations required. A tree-based classification model also confirmed the need for combining the two approaches. A detailed numerical example, the general numerical results, and the statistical analysis for the decision tree procedure are presented.

Change in Grain-Size Composition of Lignite under Cyclic Freezing-Thawing and Wetting-Drying

The paper presents the change in grain-size composition of lignite under cyclic freezing-thawing (FTC) and wetting-drying (WDC). The article shows that in the spring and autumn periods the lignites can be subjected to repeated freezing-thawing and wetting-drying, which determines the possibility of changing their grain-size composition and structure. Experimental studies in laboratory conditions on the influence of cyclic freezing-thawing (FTC) and wetting-drying (WDC) on the quality indicators of lignites have been carried out, their granulometric (fractional) composition has been studied. Freezing-thawing cycle conditions are as follows (FTC): minimum exposure temperature: -20°C;maximum: +5°C;relative humidity: 30%;number of processing cycles: 3. Wetting-drying cycles are as follows (WDC): drying temperatures are +20, +40, +60, +80°C, drying time 90 minutes, the coals are further subjected to rain (soaking) for a period of water saturation to humidity of 30% - 40% and dry again. The number of wetting-drying cycles is 3 times. The tests have revealed the destructive effects of FTC and WDC on the samples of lower metamorphic grade coal, and the cycles of wet-dry lead to the much higher yield of fine sizes (-6+0;-13+0 mm) than the cycles of freeze-thaw. Furthermore, it is found that the increase in the yield of fines depends on the heating temperature: coal disintegration proceeds more intensively at a higher temperature of drying.
