Refining the American Urological Association and American Society for Radiation Oncology guideline for adjuvant radiotherapy after radical prostatectomy using the pathologic Gleason score

Recently, it has been suggested that the guideline for adjuvant radiotherapy （ART） following radical prostatectomy （RP） sponsored by the American Urological Association and American Society for Radiation Oncology （AUA/ASTRO） may result in a significant overtreatment. Thus, the objective of the present study was to refine the AUA/ASTRO guideline for ART in patients at risk for biochemical recurrence （BCR） after RP. To this end, we reviewed our prospectively maintained database and selected 193 patients who met the AUA/ASTRO ART criteria. With a median follow-up of 24.0 months, BCR rate was 17.6% （34/193）. When stratified by the Gleason score, BCR rate in men with Gleason score 6 was 6.8%. There was no significant association between BCR-free survival and surgical margin （P= 0.690） and pathologic stage （P= 0.353） in patients with the Gleason score 6. However, in patients with positive surgical margins （PSMs）/pathologic stage ≥T3, there was a significant difference in BCR-free survival according to Gleason score （≤7 vs8-10, P= 0.047）. Multivariate Cox regression analysis demonstrated that pathologic stage ≥T3 （HR = 2.106; P = 0.018）, PSMs （HR = 2.411; P = 0.003）, and pathologic Gleason score 8-10 （HR = 4.715; P 〈 0.001） were independent predictors of BCR after RP. Therefore, in addition to pathologic stage 〉T3 and PSMs, Gleason score 8-10 predicts 8CR after RP. In patients with Gleason score 6, observation rather than ART may be more appropriate regardless of stage and surgical margin status.

Low-molecular-weight fucoidan inhibits the proliferation of melanoma via Bcl-2 phosphorylation and PTEN/AKT pathway

Intermittent, low-dose, antiandrogen monotherapy as an alternative therapeutic option for patients with positive surgical margins after radical prostatectomy

The aim of the present study was to determine whether oncologic outcomes and adverse events associated with active on/off intermittent antiandrogen monotherapy （daily bicalutamide, 50 mg per day） are comparable with those of standard external beam radiation therapy （EBRT） or combined androgen blockade （CAB） therapy in prostate cancers with positive surgical margins after radical prostatectomy. Two hundred twenty-three patients with positive surgical margins post-radical prostatectomy who underwent active surveillance （AS, n = 32）, EBRT without hormone therapy （n = 55）, intermittent antiandrogen monotherapy without EBRT （IAAM, n= 50）, or CAB without EBRT （n = 86）, between 2007 and 2014, were reviewed retrospectively. Pathologic outcomes, biochemical recurrence rates, radiological disease progression, and adverse events were collected from medical records. Biochemical recurrence rates, biochemical recurrence-free survival rates, and radiological recurrence were not different between the groups （P = 0.225, 0.896, and 0.284, respectively）. Adverse event rates and severities were lower for IAAM compared with EBRT or CAB （both P 〈 0.05）, but were comparable to those for AS （P = 0.591 and 0.990, respectively）. Grade ：〉3 adverse events were not reported in the IAAM or AS groups. Erectile dysfunction and loss of libido rates were lower in the IAAM group compared with the EBRT and CAB groups （P = 0.032）. Gastrointestinal complications were more frequently reported in the EBRT group （P = 0.008）. Active on/off IAAM treatment might be an appropriate treatment option for patients with positive surgical margins after radical prostatectomy. Furthermore, regarding oncologic outcomes, IAAM was comparable to standard EBRT but had a milder adverse event profile.

Subsurface scattering using splat-based diffusion in point-based rendering

Point-based graphics has gained much attention as an alternative to polygon-based approaches because of its simplicity and flexibility. However, current point-based techniques do not provide a sufficient rendering quality for translucent materials such as human skin. In this paper, we propose a point-based framework with subsurface scattering of light, which is important to create the soft and semi-translucent appearance of human skin. To accurately simulate subsurface scattering in multilayered materials, we present splat-based diffusion to apply a linear combination of several Gaussian basis functions to each splat in object space. Compared to existing point-based approaches, our method offers a significantly improved visual quality in rendering human faces and provides a similar visual quality to polygon-based rendering using the texture space diffusion technique. We demonstrate the effectiveness of our approach in rendering scanned faces realistically.

In this study,we investigated the preventive effects of emamectin benzoate 9.7%SL,which was newly developed to reduce the injection volume and number of injection holes required to protect against pine wood *** of the Pinus thunbergii trees injected with emamectin benzoate 9.7%SL at 0.3 mL/cm diameter at breast height(DBH)died within 2 years of inoculation with pine wood *** benzoate 9.7%SL injected at 0.6 mL/cm DBH resulted in no tree mortality for 3 *** residue of emamectin benzoate 9.7%SL in pine twigs injected with 0.3 mL/cm DBH was 0.490μg/g at 1 year after injection and 0.303μg/g after 2 *** residues values are greater than 0.031μg/g,previously determined IC95 value for emamectin benzoate against the pine wood *** field experiment and residue analysis showed that emamectin benzoate 9.7%SL could be a substitute agent for emamectin benzoate 2.15%EC,which is widely used to prevent pine wood nematode in the field and that injection volume and number of injection holes can be greatly educed using this new formulation,which will reduce injury to the cambium,interruption of water movement,and infection of inoculation wounds by wood-decay or blue stain fungi.

How has the disease course of pediatric ulcerative colitis changed throughout the biologics era?A comparison with the IBSEN study

BACKGROUND In Korea,infliximab was approved for use in children with ulcerative colitis(UC)in October *** To compare the clinical course of UC before and after the introduction of biological agents,and to compare with the IBSEN *** Patients under 18 years of age,who were diagnosed with UC and followed from January 2003 to October 2020,were included in the *** A(n=48)was followed for at least 2 years between January 2003 and October 2012,and Group B(n=62)was followed for at least 2 years between November 2012 and October *** compared endoscopic remission,drug composition,relapse rate,steroidfree period,and the quality of life of each *** plotted the clinical course of the included patients using the pediatric UC activity index score,and compared our patients with those in the IBSEN *** After 2 years of treatment,colonoscopy evaluation revealed different outcomes in the two treatment *** was confirmed in 14 patients(29.2%)of Group A,and in 31 patients(50.0%)of Group B(Pmilar proportions to the graphs in the IBSEN ***,in Group B,the proportion of patients corresponding to curve 1(remission or mild severity after initial high activity)was high at 76%(47/62).CONCLUSION The incidence of relapse has decreased and the steroid-free period has increased after the introduction of the biological *** clinical course also showed a different pattern from that of IBSEN *** active use of biological agents may change the long-term disease course in moderate to severe pediatric UC.

Structural neural connectivity of the vestibular nuclei in the human brain:a diffusion tensor imaging study

Many animal studies have reported on the neural connectivity of the vestibular nuclei（VN）.However,little is reported on the structural neural connectivity of the VN in the human *** this study,we attempted to investigate the structural neural connectivity of the VN in 37 healthy subjects using diffusion tensor tractography.A seed region of interest was placed on the isolated VN using probabilistic diffusion tensor *** was defined as the incidence of connection between the VN and each brain *** VN showed 100% connectivity with the cerebellum,thalamus,oculomotor nucleus,trochlear nucleus,abducens nucleus,and reticular formation,irrespective of *** the threshold of 5 streamlines,the VN showed connectivity with the primary motor cortex（95.9%）,primary somatosensory cortex（90.5%）,premotor cortex（87.8%）,hypothalamus（86.5%）,posterior parietal cortex（75.7%）,lateral prefrontal cortex（70.3%）,ventromedial prefrontal cortex（51.4%）,and orbitofrontal cortex（40.5%）,*** results suggest that the VN showed high connectivity with the cerebellum,thalamus,oculomotor nucleus,trochlear nucleus,abducens nucleus,and reticular formation,which are the brain regions related to the functions of the VN,including equilibrium,control of eye movements,conscious perception of movement,and spatial orientation.

Delayed gait recovery in a stroke patient

We report on a stroke patient who showed delayed gait recovery between 8 and 11 months after the onset of intracerebral hemorrhage. This 32-year-old female patient underwent craniotomy and drainage for right intracerebral hemorrhage due to rupture of an arteriovenous malformation. Brain MR images revealed a large leukomalactic lesion in the right fronto-parietal cortex. Diffusion tensor tractography at 8 months after onset revealed that the right corticospinal tract was severely injured. At this time, the patient could not stand or walk despite undergoing rehabilitation from 2 months after onset. It was believed that severe spasticity of the left leg and right ankle was largely responsible, and thus, antispastic drugs, antispastic procedures （alcohol neurolysis of the motor branch of the tibial nerve and an intramuscular alcohol wash of both tibialis posterior muscles） and physical therapy were tried to control the spasticity. These measures relieved the severe spasticity, with the result that the patient was able to stand at 3 months. In addition, the improvements in sensorimotor function, visuospatial function, and cognition also seemed to contribute to gait recovery. As a result, she gained the ability to walk independently on even floor with a left ankle foot orthosis at 11 months after onset. This case illustrates that clinicians should attempt to find the cause of gait inability and to initiate intensive rehabilitation in stroke patients who cannot walk at 3-6 months after onset.
