Nowadays, the sleeper, railroad bed, welding technology and materials of the high-speed & heavy-load Continuous Welded Rail (CWR) have been improved greatly. However, the orientation relationship between welding surface and rail is still orientation structure, which appeared 185 years ago in UK, namely, the welding surface is normal to the longitudinal direction of the rail. Thus rail welds are subject to vertical pure shear stress, lateral pure shear stress and axial pure tensile stress. Pure shear stress and pure tensile stress are both dangerous stresses, which result in failure of welds under high-speed & heavy-load condition for a long time. When the wheels pass rail welds, it is difficult to avoid vertical bumping and lateral vibration. In order to solve these problems, in this paper we put forward the single inclined welding surface having dip angle α with the lateral rail or dip angle β with the vertical rail and analyze the position geometry and stress state. Thus the vertical bumping and lateral vibration are basically eliminated, then the bearing capacity of the welding surface is significantly improved, and the stress of axial pure tensile and lateral pure shear of the welding surface is reduced.

背景与目的:针对人表皮生长因子受体(human epidermal growth factor receptor 2,HER2)阳性转移性乳腺癌患者,曲妥珠单抗治疗能够延长患者总生存期,显著改善患者预后,但是原研曲妥珠单抗价格较高。生物类似药理论上具有相当的疗效和安全性。本临床试验旨在评估曲妥珠单抗生物类似药AK-HER2与原研曲妥珠单抗在HER2阳性转移性乳腺癌患者中的疗效、药代动力学、安全性和免疫原性。方法:这项多中心、随机、双盲Ⅲ期临床试验在中国43个分中心开展。本研究遵从研究方案、赫尔辛基宣言阐明的伦理学原则和药物临床试验质量管理规范,获得医院医学伦理委员会批准,临床试验注册机构为国家药品监督管理局(临床试验批件号为2015L04224,临床试验登记号为CTR20170516)。在入组前获得了受试者的书面知情同意书。入组患者随机分配至AK-HER2组与对照组,分别接受AK-HER2或原研曲妥珠单抗(赫赛汀®,初始负荷剂量8 mg/kg,维持剂量6 mg/kg,每3周为1个治疗周期,总治疗时间为16个周期)与多西他赛(剂量75 mg/m2,治疗持续至少9个周期)联合治疗。本临床试验主要研究终点是第9个周期AK-HER2组与对照组的客观缓解率(objective response rate,ORR)。次要疗效终点包括ORR16、疾病控制率(disease control rate,DCR)、临床获益率(clinical benefit rate,CBR)、无进展生存期(progression-free survival,PFS)和1年生存率。本研究在第6个周期用药后,随机选择100例受试者(AK-HER2组∶对照组=1∶1)进行血样采集,采集时间点分别为输注45 min时(即给药结束)、给药结束后第4、8、24、72、120、168、336、504 h。采集后血样进行PK参数(PK parameter set,PKPS)分析。其他评估指标包括安全性和免疫原性评估。结果:2017年9月—2021年3月期间共有550例HER2阳性转移性乳腺癌患者入组该临床试验。AK-HER2组(n=275)和对照组(n=272)的ORR9分别为试验组受试者(n=237)达CR或PR的有129例,ORR9为54.4%,对照组受试者(n=241)达CR或PR的有134例,ORR9为55.6%。AK-HER2组与对照组的ORR9比率为97.9%[90%置信区间(confidence interval,CI):85.4%~112.2%,P=0.784]差异无统计学意义。在所有次要疗效终点中,两组均未观察到差异有统计学意义。本研究进行了AK-HER2组和对照组药代动力学(pharmacokinetics,PK)参数的均值比值分析,结果显示,两种药物的药代动力学特征相似。原研曲妥珠单抗治疗导致药物减量或暂停的治疗期间出现的不良事件(treatment emergent adverse event,TEAE)发生率,AK-HER2组为3.6%(10例),对照组为8.1%(22例),两组差异有统计学意义(P=0.027)。AK-HER2组发生率较对照组明显减少,其余组间差异均无统计学意义。抗药抗体(anti-drug antibody,ADA)与中和抗体(neutralizing antibody,NAB)阳性率组间差异均无统计学意义(P=0.385和P=0.752)。结论:在HER2阳性转移性乳腺癌患者中,AK-HER2与参照药原研曲妥珠单抗的疗效、药代动力学、安全性和免疫原性相当。

Background:The current deep learning diagnosis of breast masses is mainly reflected by the diagnosis of benign and malignant *** china,breast masses are divided into four categories according to the treatment method:inflammatory masses,adenosis,benign tumors,and malignant *** categorizations are important for guiding clinical *** this study,we aimed to develop a convolutional neural network(CNN)for classification of these four breast mass types using ultrasound(US)***:Taking breast biopsy or pathological examinations as the reference standard,CNNs were used to establish models for the four-way classification of 3623 breast cancer patients from 13 *** patients were randomly divided into training and test groups(n=1810 vs.n=1813).Separate models were created for two-dimensional(2D)images only,2D and color Doppler flow imaging(2D-CDFI),and 2D-CDFI and pulsed wave Doppler(2D-CDFI-PW)*** performance of these three models was compared using sensitivity,specificity,area under receiver operating characteristic curve(AUC),positive(PPV)and negative predictive values(NPV),positive(LR+)and negative likelihood ratios(LR-),and the performance of the 2D model was further compared between masses of different sizes with above statistical indicators,between images from different hospitals with AUC,and with the performance of 37 ***:The accuracies of the 2D,2D-CDFI,and 2D-CDFI-PW models on the test set were 87.9%,89.2%,and 88.7%,*** AUCs for classification of benign tumors,malignant tumors,inflammatory masses,and adenosis were 0.90,0.91,0.90,and 0.89,respectively(95%confidence intervals[CIs],0.87-0.91,0.89-0.92,0.87-0.91,and 0.86-0.90).The 2D-CDFI model showed better accuracy(89.2%)on the test set than the 2D(87.9%)and 2D-CDFI-PW(88.7%)*** 2D model showed accuracy of 81.7%on breast masses≤1 cm and 82.3%on breast masses>1 cm;there was a significant difference between the two groups(P1).The accu

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

Purpose Malnutrition is severe among gastric cancer patients in china. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in china. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P 1). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P 1) were significant in the majority of tests. Conclusion The Chinese version of the PG-SGA is a valid tool to access the patients’ nutritional status for Chinese gastric patients.

The prevalence of malnutrition is high among oncology patients in china. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in china. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P 1). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P 1) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity.
