In this study,we synthesized an organic material with near-infrared emission capabilities:4-(2-(4-(9-(4-(diphenylamino)phenyl)naphtho[2,3-c][1,2,5]thiadiazol-4-yl)phenyl)-1H-phenol-1-ylidene)malononitrile(TPA).Furthermore,TPA-PEG2000 fluorescent nanoparticles were prepared via coating the shells with ***-PEG2000 exhibited strong near-infrared emission near 700 nm,with a photoluminescence quantum yield of 15.09%,indicating a high emission *** biology experiments have confirmed its low toxicity and excellent *** cholesterol and phospholipid levels in liver cancer cell membranes with low sensitivity or high drug resistance lead to increased rigidity,reduced membrane fluidity,reduced endocytic efficiency,and reduced ***,the uptake of TPA-PEG2000 nanoparticles into cells and the near-infrared fluorescence intensity can be used to evaluate the sensitivity of systemic liver cancer treatment in a simple and efficient manner.

混合型肝细胞-胆管细胞癌(cHCC-CC)是原发性肝癌的一种特殊分型,具有肝细胞癌和肝内胆管细胞癌双表型的分化特点,因其缺乏特异性的诊断标志物和治疗靶点,术前诊断困难,治疗不易且预后较差。常规的血清学、影像学检查以及组织学活检是cHCC-CC初步筛查及诊断的主要手段。可切除cHCC-CC患者宜尽可能接受外科手术治疗,但关于是否接受肝移植治疗仍需慎重选择。晚期患者的局部治疗及系统治疗尚缺乏统一的标准方案,其疗效仍存在争议,包含铂类药物的系统治疗可使部分患者获益。靶向治疗及免疫治疗在晚期cHCC-CC患者治疗中的潜在价值不断凸显。多学科诊疗模式的引入为患者的综合治疗提供了有力的保障,异种模型及多组学技术的发展为患者真正实现个体化治疗提供了可观的前景。本文对cHCC-CC诊断、治疗现状及发展进行探讨,以期为临床提供参考。

We derive the potential energy of gravity waves(GWs)in the upper troposphere and stratosphere at 45°S-45°N from December 2019 to November 2022 by using temperature profiles retrieved from the Constellation Observing System for Meteorology,Ionosphere,and Climate-2(COSMIC-2)*** to the dense sampling of COSMIC-2,in addition to the strong peaks of gravity wave potential energy(GWPE)above the Andes and Tibetan Plateau,we found weak peaks above the Rocky,Atlas,Caucasus,and Tianshan *** land-sea contrast is responsible for the longitudinal variations of the GWPE in the lower and upper *** 40°N/S,the peaks were mainly above the topographic regions during the *** 20°N/S,the peaks were a slight distance away from the topographic regions and might be the combined effect of nontopographic GWs and mountain *** the Equator,the peaks were mainly above the regions with the lowest sea level altitude and may have resulted from *** results indicate that even above the local regions with lower sea level altitudes compared with the Andes and Tibetan Plateau,the GWPE also exhibits fine structures in geographic *** found that dissipation layers above the tropopause jet provide the body force to generate secondary waves in the upper stratosphere,especially during the winter months of each hemisphere and at latitudes of greater than 20°N/S.

目的探讨基于美国国家癌症研究所监测、流行病学和最终结果(SEER)数据库混合型肝癌(CHC)预后危险因素及列线图预测模型构建,并探讨其应用价值。方法采用回顾性队列研究方法。收集2004年1月至2019年12月SEER数据库208例CHC患者的临床病理资料;男150例,女58例;年龄≥60岁121例,<60岁87例。208例CHC患者按照随机数字表法按7∶3分为训练集145例和验证集63例。使用COX比例风险模型构建列线图预测模型,验证并比较列线图预测模型和第8版美国癌症联合委员会(AJCC)分期预测模型的预测效能。观察指标:(1)CHC患者生存情况。(2)CHC患者预后影响因素分析。(3)列线图预测模型的构建和效能评价。正态分布的计量资料以■±s表示,偏态分布的计量资料以M(范围)表示。计数资料以绝对数表示,组间比较采用χ^(2)检验。等级资料比较采用秩和检验。采用COX回归模型进行单因素及多因素分析。采用Kaplan⁃Meier法计算生存率,采用Log⁃Rank检验进行生存分析。采用一致性指数(C⁃index)、受试者工作特征曲线下面积评价预测模型的预测效能;校准曲线验证预测模型的准确性;决策曲线评价预测模型的临床效能。结果(1)CHC患者生存情况。145例训练集CHC患者生存时间为15.0(1.0~166.0)个月,1年总生存率为52.7%。63例测试集患者生存时间为11.5(1.0~176.0)个月,1年总生存率为48.3%。(2)CHC患者预后影响因素分析。多因素分析结果显示:T分期、手术治疗、肿瘤分化程度、化疗是CHC患者预后的独立影响因素(P<0.05)。(3)列线图预测模型的构建和效能评价。根据多因素分析结果,构建列线图预测模型。训练集和验证集列线图预测模型的C⁃index分别为0.758和0.742;训练集和验证集AJCC分期预测模型的C⁃index分别为0.622和0.662。训练集和验证集CHC患者列线图预测模型的1、3、5年总生存率受试者工作特征曲线下面积分别为0.830、0.861、0.887和0.798、0.813、0.844;训练集和验证集CHC患者AJCC分期预测模型的1、3、5年总生存率受试者工作特征曲线下面积分别为0.668、0.699、0.747和0.719、0.760、0.796。校准曲线结果显示:1年总生存率校准曲线与理想的斜率为1的直线拟合良好,2种预测模型的预测结果与实际结果具有较高的一致性。决策曲线结果显示:列线图预测模型比AJCC分期预测模型具有更好的预测效能。结论T分期、手术治疗、肿瘤分化程度、化疗是CHC患者预后的独立影响因素,基于SEER数据库构建的列线图预测模型可更精准地预测和评估CHC患者的预后。

Effects of Crosswise Appendage Plate Flexibility on the Drag Reduction Characteristics of Cylinder

In engineering applications (Like an ocean riser), fluid flow around bluff bodies generates substantial resistance, which can jeopardize structural integrity, lifespan, and escalate resource consumption. Therefore, employing drag reduction measures becomes particularly crucial. This paper employs the immersed boundary method to investigate the impact of transversely oriented appendage plate flexibility on the drag of cylinders under different Reynolds numbers and distances. The results indicate that flexible appendage plate exerts drag reduction effects on the downstream cylinder, with this effect gradually diminishing as Reynolds numbers increase. At identical Reynolds numbers, the drag reduction effect initially increases and then decreases with distance, with the optimal drag reduction distance observed at D = 2.5. Compared to cylinders without appendage plate, the maximum drag reduction achieved is 30.551%. Addressing the drag reduction issue in cylinders holds significant importance for ensuring engineering structural integrity, enhancing engineering efficiency, and developing novel underwater towing systems.

胰十二指肠切除术作为普通外科最复杂、最具挑战性的手术,也是治疗胰头、壶腹周围肿瘤的经典术式。随着医疗技术及医疗器械的不断改进,微创技术在胰十二指肠切除术中应用愈发广泛,但也存在着诸多争议。本文主要结合笔者团队经验及国内外文献对开腹、腹腔镜及机器人胰十二指肠切除术3种不同手术方式的临床应用现状及研究进展作一综述,从而为把握手术适应证、制定个体化治疗策略提供参考。
