Rotational alignment in total knee arthroplasty： nonimage-based navigation system versus conventional technique

Background Proper rotational alignment during total knee arthroplasty （TKA） is important for adequate postoperative patellofemoral and tibiofemoral kinematics,as well as for achieving balanced flexion space at 90°.The effects of computer navigation-assisted total knee replacement and conventional total knee arthroplasty on rotational alignment,mechanical axis,component position and clinical outcomes were compared.Methods Two methods were used in 82 patients and the rotation of the femoral and tibial components in the transverse plane,the combined rotation of the two components,the mismatch between them,and the mechanical axis of the lower limb were analyzed.All of these parameters were measured from postoperative radiographs and computed tomography images.Functional outcomes were compared at 6 weeks and 6 months postoperatively.Results Significant differences were found between the two techniques （P 〈0.05） in the following parameters：average rotation of the femoral component （（1.51±3.55）° vs.（-0.63±3.04）°）; combined rotation of the femoral and tibial components （2.85±4.07）° vs.（0.28±3.43）°）; and mismatch between the femoral and tibial components （（1.44±4.55）° vs.（-0.43±2.86）°）.Differences in the rotation of the tibial component were not statistically significant.The prevalence of outliers （malalignment 〉±3° intemal/external rotation） of the femoral component （31.7% vs.12.5%） and the tibial component （36.6% vs.15%） were significantly reduced when the navigation system was used （P〈0.05）.In addition,while patients in the navigation group had significantly better mechanical axis and functional outcomes at 6 weeks after surgery （P 〈0.05）,there was no significant difference between the two groups （P 〉0.05） with respect to functional outcomes at 6 months.Conclusion The navigation system exhibited higher accuracy than the conventional technique in the transverse and coronal plane,and provided better early functional outcomes.

Background This study was designed to analyze three tibial axis reference lines including the anterior tibial cortex （ATC） line, the fibular line （FL）, and the anatomical axis of tibia （AAT） line, to determine which line most closely parallels the mechanical axis （MA） of the tibia in the sagittal plane. The clinical relevance of the study is that through finding a reliable landmark on the leg, a surgeon may minimize posterior tibial slope measurement errors thereby and improving the technique for assuring proper alignment of total knee arthroplasty.

Objective Exposure to microgravity results in postflight cardiovascular deconditioning in astronauts.Vascular oxidative stress injury and mitochondrial dysfunction have been reported during this process.To elucidate the mechanism for this condition,we investigated whether mitochondrial oxidative stress regulates calcium homeostasis and vasoconstriction in hindlimb unweighted(HU)rat cerebral arteries.Methods Three-week HU was used to simulate microgravity in rats.The contractile responses to vasoconstrictors,mitochondrial fission/fusion,Ca^(2+) distribution,inositol 1,4,5-trisphosphate receptor(IP3 R)abundance,and the activities of voltage-gated K+channels(KV)and Ca^(2+)-activated K+channels(BKCa)were examined in rat cerebral vascular smooth muscle cells(VSMCs).Results An increase of cytoplasmic Ca^(2+) and a decrease of mitochondrial/sarcoplasmic reticulum(SR)Ca^(2+) were observed in HU rat cerebral VSMCs.The abundance of fusion proteins(mitofusin 1/2[MFN1/2])and fission proteins(dynamin-related protein 1[DRP1]and fission-mitochondrial 1[FIS1])was significantly downregulated and upregulated,respectively in HU rat cerebral VSMCs.The cerebrovascular contractile responses to vasoconstrictors were enhanced in HU rats compared to control rats,and IP3 R protein/mRNA levels were significantly upregulated.The current densities and open probabilities of KV and BKCa decreased and increased,respectively.Treatment with the mitochondrial-targeted antioxidant mitoTEMPO attenuated mitochondrial fission by upregulating MFN1/2 and downregulating DRP1/FIS1.It also decreased IP3 R expression levels and restored the activities of the KV and BKCa channels.MitoTEMPO restored the Ca^(2+) distribution in VSMCs and attenuated the enhanced vasoconstriction in HU rat cerebral arteries.Conclusion The present results suggest that mitochondrial oxidative stress enhances cerebral vasoconstriction by regulating calcium homeostasis during simulated microgravity.

目的了解血培养分离出耐甲氧西林金黄色葡萄球菌的患者临床特征,探究血培养分离出耐甲氧西林金黄色葡萄球菌的危险因素。方法收集2017—2022年血培养分离出耐甲氧西林金黄色葡萄球菌的住院患者的临床病史资料和实验室数据,分析血培养分离出耐甲氧西林金黄色葡萄球菌的独立危险因素。结果2017—2022年血培养标本共分离金黄色葡萄球菌181株,其中耐甲氧西林金黄色葡萄球菌49株,总检出率27.07%,各年检出率变化无明显趋势。分离率科室构成比前三位为肾内科(20.41%)、骨科(10.20%)、儿科(10.20%)。患者以老年、男性为主,其感染部位以血流感染(83.67%)和肺部感染(48.98%)为主。49株耐甲氧西林金黄色葡萄球菌对青霉素(100%)、红霉素(87.76%)和克林霉素(73.47%)耐药率较高;132株对甲氧西林敏感的金黄色葡萄球菌仅对青霉素(91.67%)耐药率较高。使用头孢菌素过往史和年龄为血培养分离出耐甲氧西林金黄色葡萄球菌的独立危险因素。结论血培养分离出的耐甲氧西林金黄色葡萄球菌对多种临床常用抗菌药物耐药,使用头孢菌素过往史和年龄的因素在耐甲氧西林金黄色葡萄球菌感染中起着重要作用,临床应注意相关危险因素,合理防治。

Long non-coding RNA GATA6-AS1 is mediated by N6-methyladenosine methylation and inhibits the proliferation and metastasis of gastric cancer

BACKGROUND Through experimental research on the biological function of GATA6-AS1,it was confirmed that GATA6-AS1 can inhibit the proliferation,invasion,and migration of gastric cancer cells,suggesting that GATA6-AS1 plays a role as an anti-oncogene in the occurrence and development of gastric cancer.Further experi-ments confirmed that the overexpression of fat mass and obesity-associated protein(FTO)inhibited the expression of GATA6-AS1,thereby promoting the occurrence and development of gastric cancer.AIM To investigate the effects of GATA6-AS1 on the proliferation,invasion and migration of gastric cancer cells and its mechanism of action.METHODS We used bioinformatics methods to analyze the Cancer Genome Atlas(https://portal.***.cancer.gov/.The Cancer Genome Atlas)and download expression data for GATA6-AS1 in gastric cancer tissue and normal tissue.We also constructed a GATA6-AS1 lentivirus overexpression vector which was transfected into gastric cancer cells to investigate its effects on proliferation,migration and invasion,and thereby clarify the expression of GATA6-AS1 in gastric cancer and its biological role in the genesis and development of gastric cancer.Next,we used a database(http://***/starbase2/)to analysis GATA6-AS1 whether by m6A methylation modify regulation and predict the methyltransferases that may methylate GATA6-AS1.Furthermore,RNA immunoprecipitation experiments confirmed that GATA6-AS1 was able to bind to the m6A methylation modification enzyme.These data allowed us to clarify the ability of m6A methylase to influence the action of GATA6-AS1 and its role in the occurrence and development of gastric cancer.RESULTS Low expression levels of GATA6-AS1 were detected in gastric cancer.We also determined the effects of GATA6-AS1 overexpression on the biological function of gastric cancer cells.GATA6-AS1 had strong binding ability with the m6A demethylase FTO,which was expressed at high levels in gastric cancer and negatively correlated with the expression of GATA6-AS1.Following transfection with s

Distribution of oxide inclusions in H13 castings under super-gravity field with multi-stage rotation speeds

The 42 kg industrial H13 castings were prepared by different super-gravity fields with multi-rotation speeds,and the distribution of oxide inclusions in the castings was studied.In addition,the inward movement Reynolds number and inward movement time of oxide inclusions as well as the solidification time of molten steel at different positions in the castings were calculated to clarify the removal mechanism of oxide inclusions in super-gravity field.The results show that the large size(i.e.,greater than 10μm)oxide inclusions are mainly concentrated in the inner and outer parts of the super-gravity castings with constant rotation speed(500 r min^(-1))and five-stage rotation speeds(500,600,750,850,and 950 r min^(-1)),respectively,while there are no large oxide inclusions in the super-gravity castings with three-stage rotation speeds(500,600,and 750 r min^(-1)).Although an increase in the particle size of inclusion and the rotation speed in super-gravity field is conducive to the increase in the inward movement Reynolds number of oxide inclusions and reduction in the inward movement time of oxide inclusions,it will reduce the local solidification time of molten steel.In the range of the rotation speed studied,the super-gravity field with three-stage rotation speeds has the best effect on the removal of inclusions in H13 molten steel.

为缓解因大量投喂冰鲜鱼造成的大口黑鲈养殖水体污染、病害频发和生产效益降低等问题,加快推进优质配合饲料的开发,开展投喂配合饲料和冰鲜鱼对大口黑鲈营养价值和健康状况影响的研究至关重要。为此,本研究将360尾大口黑鲈分为3个试验组,分别投喂配合饲料、冰鲜鱼和混合饲料(配合饲料和冰鲜鱼的质量各占饲料总质量的50%),在饲喂90 d后对3组大口黑鲈肌肉中矿物元素含量、氨基酸的组成和含量、脂肪酸的组成和含量以及血清生化指标进行分析。结果表明,配合饲料组大口黑鲈肌肉中的Na、Ca、Zn含量显著高于冰鲜鱼组(P<0.05)。与配合饲料组和混合饲料组相比,冰鲜鱼组大口黑鲈肌肉中必需氨基酸总量占氨基酸总量的百分比以及必需氨基酸总量占非必需氨基酸总量的百分比更高。冰鲜鱼组大口黑鲈肌肉中各必需氨基酸的氨基酸评分和化学评分以及必需氨基酸指数均高于配合饲料组。配合饲料组大口黑鲈肌肉中饱和脂肪酸总量显著高于混合饲料组和冰鲜鱼组(P<0.05),但冰鲜鱼组的多不饱和脂肪酸(PUFA)总量、二十碳五烯酸含量和二十二碳六烯酸含量显著高于配合饲料组(P<0.05);与配合饲料组相比,冰鲜鱼组的n-6 PUFA含量更低,n-3 PUFA含量更高(P<0.05),并且脂肪酸品质更优。冰鲜鱼组和混合饲料组血清总蛋白质含量和碱性磷酸酶活性显著高于配合饲料组(P<0.05)。此外,冰鲜鱼组大口黑鲈血清中葡萄糖含量和谷草转氨酶活性最低(P<0.05)。因此,在本研究条件下,配合饲料影响大口黑鲈肌肉的营养价值和肝脏健康,建议参考冰鲜鱼的营养组成,进一步优化配合饲料配方。

With the development of new experimental techniques(e.g.,single-cell RNA sequencing(scRNA-seq),light sheet microscopy,and clustered regularly interspaced palindromic repeats(CRISPR)/CRISPR-associated protein 9(Cas9),emerging studies have begun to uncover the interactions between the nervous system and the immune system,both in health and diseases.For example,in mucosal tissues such as the lung and the intestine,neurotransmitters including acetylcholine[1]and adrenaline[2]can directly regulate the immune cell functions thus affect the results of allergy,asthma,and anti-pathogen host defense[3].

Total knee arthroplasty （TKA） is an effective treatment for severe knee arthritis. However, TKA is more challenging when that arthritis is associated with femoral deformity that has resulted in irregularity of the canal and variation in anatomic landmarks. In such cases, it is difficult to restore the mechanical alignment of the lower limb using intramedullary instruments.
